Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationSchedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. Their related income is exempt from federal income tax under Section 501(A). Some of the subsidiaries are taxable entities, and some of the income of the tax-exempt entities is subject to taxation as unrelated business taxable income. The Company and its subsidiaries file U.S. federal income tax returns, and they also file in various state and foreign jurisdictions. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist for the years ended December 31, 2024 and 2023.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - St. Francis Hospital, Inc.. Bon Secours St. Francis Health System, including Bon Secours St. Francis Downtown, Bon Secours St. Francis Eastside, Bon Secours St. Francis Millennium, and Bon Secours St. Francis Medical Campus at Simpsonville completed a joint CHNA report which reflects the collaborative efforts to conduct an assessment of the health needs of the community they serve. The CHNA was created utilizing quantitative data which was collected at national, state, regional, and local levels as well as qualitative data that was collected through community input. Key data sources included the United States Census Bureau; Healthy People 2030; County Health Rankings and Roadmaps; Centers for Disease Control; and South Carolina DHEC biostatistics, vital records, county health profiles and hospital discharge data. Dates of data collected ranged from 2000 - 2021. For each indicator, data was pulled for the most recent year available. To assess trends and progress, data points were compared to data pulls from the previous CHNA study when updated data points were available. Collection and analysis of qualitative data was completed through a series of town hall meetings and the implementation of a community survey. Qualitative data was collected by the Johnson Group, the consulting firm that compiled that data in the previous CHNA studies. Three Town Hall meetings were conducted in January 2022. Originally set to be completed in person, due to a community spike in COVID cases, all three Town Halls were completed by Zoom. The Town Halls included over 70 community residents, faith-based representative, and some health and human service representatives. These groups represented the interests of urban and suburban residents, mostly from less affluent communities that are home to lower-income families, minority families and the elderly. The community-wide survey reached 775 randomly selected Greenville County residents who completed a lengthy questionnaire about community health issues. Both the secondary data and the town hall results were used to inform survey question selection and the prioritization process of identified needs. A 21-member Advisory team of local public health professionals, health and human service agencies, and Bon Secours St. Francis Health System leaders assisted in guiding the assessment, providing additional insight and feedback into community issues and priorities. The advisory committee met three times between January - April 2022. Additional detail can be found on the Bon Secours website at https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment

PATIENTS WHO LIVE IN THE COMMUNITY SERVED BY A BON SECOURS MERCY HEALTH HOSPITAL WILL BE OFFERED HEALTHCARE FINANCIAL ASSISTANCE. FOR THOSE PATIENTS LIVING OUTSIDE OF THE COMMUNITY, EXTENUATING CIRCUMSTANCES MUST BE DOCUMENTED AND APPROVED BY THE PFS MANAGER AND BE MEDICALLY NECESSARY OR EMERGENT IN NATURE. A LIST OF THE ZIP CODES OF THE COMMUNITY SERVED FOR EACH BON SECOURS MERCY HEALTH HOSPITAL IS MAINTAINED IN A SEPARATE DOCUMENT AND READILY AVAILABLE VIA THE CONTACT LIST AT THE END OF THE POLICY LOCATED AT https://www.bonsecours.com/patient-resources/financial-assistance. BON SECOURS MERCY HEALTH'S FINANCIAL ASSISTANCE POLICY REQUIRES A PATIENT TO APPLY FOR HEALTH INSURANCE COVERAGE AND/OR ENTER THE MARKETPLACE/EXCHANGE BEFORE FINANCIAL ASSISTANCE MAY BE EXTENDED. EXCEPTIONS TO THIS POLICY INCLUDE PATIENTS DISCHARGED TO A SKILLED NURSING FACILITY, PATIENTS WHO ARE DECEASED WITH NO ESTATE, AND PATIENTS WHO HAVE DOCUMENTED HOMELESSNESS.
Schedule H, Part V, Section B, Line 20 Facility A, 1Facility A, 1 - BON SECOURS MERCY HEALTH HOSPITALS. Bon Secours Mercy Health hospitals take all reasonable efforts to notify an individual about its Financial Assistance Policy before initiating Extraordinary Collection Actions (ECAs) pursuant to IRC Section 501(r)-6(c)(4), including but not limited to, providing the individual with a written notice that indicates financial assistance is available for eligible individuals, identifies the ECAs that the hospital (or other authorized party) intends to initiate to obtain payment, and states a deadline after which such ECA may be initiated that is no earlier than 30 days after the date that the written notice is provided. During the annual internal audit of Bon Secours Mercy Health (BSMH) hospitals which was completed on October 28, 2024, the internal audit team identified an error to properly notify the patient about the extraordinary collection actions (ECAs) that the hospital intends to initiate to obtain payment and provide the patient with no less than 30 days for payment before initiating such ECAs. The error was the result of a systematic change made by a third party engaged to generate patient account statements. The change was made in January 2024 and inadvertently removed the ECA notification on the statements that contained more than one episode of care. Any statements with only one episode of care were not impacted. The statements were corrected in October 2024, and all statements after that date include the proper ECA notification language. At the time the error was identified, BSMH identified 317 accounts during the period of January 23 - October 2, 2024, that received adverse credit reporting and did not receive the proper written notification required under IRC Section 501(r)-6(c)(4). In March 2025, BSMH notified the applicable collection agencies to reverse the adverse credit reporting and put a hold on any further collection actions. BSMH will not pursue any additional collections on the open balances totaling $354,089 for the identified dates of service for those patients, and any open balances will be written-off. BSMH issued the letter notifying the patients of the corrective action taken, no additional actions are needed by the patient, and the balances for those identified episodes of care have been resolved. This letter was mailed to the respective recipients in June 2025. BSMH will continue to perform an internal audit on a regular and continuous basis at least once per year to ensure compliance. We do not believe the error to be considered "willful or egregious BSMH has performed internal research to identify how the minor oversight occurred and taken additional steps to ensure compliance going forward.

Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation60017830
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCost of financial assistance at cost was calculated with a cost to charge ratio using worksheet 2. The cost related to Medicaid patients was determined using Bon Secours Mercy Health's cost accounting system and included both inpatients and outpatients for traditional Medicaid and Medicaid managed care plans. For subsidized services Bon Secours Mercy Health's cost accounting system was used to determine costs related to the specific service excluding traditional Medicaid and Medicaid managed care patients. Costs for charity and bad debt accounts are deducted using a ratio of cost to charge specific to that subsidized service. Costs for other programs reflect the direct and indirect costs of providing those programs.
Schedule H, Part II Community Building ActivitiesBon Secours Mercy Health (BSMH) addresses various community concerns including health improvement, poverty, workforce development, and access to health care. BSMH hospitals conduct community health education and support groups, health fairs and screenings for the communities served. BSMH hospitals work with state and local leadership to address community needs and provide healthcare services to the poor and underserved. BSMH is committed to addressing the social determinants of health (SDOH) and social needs for patients and communities throughout our footprint. With a deep understanding of both areas, the team is working systematically across the ministry and in our local communities to ensure we respond holistically and impactfully. BSMH addresses SDOH through its Community Health Needs Assessment (CHNA), Community, investment strategy, advocacy/public policy partnerships, and cross sector solutions. BSMH addresses social needs through SDOH patient assessment, health education/promotion programs, capacity building of local community resources, and closed loop referral system. Additional detail regarding BSMH's community building activities and the promotion of health of its communities can be found in its 2024 Community Health Annual Report available at https://www.bonsecours.com/about-us/community-commitment/additional-reports and https://www.mercy.com/about-us/mission/giving-back.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. NET PATIENT ACCOUNTS ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL RECEIVABLES BASED UPON BON SECOURS MERCY HEALTH'S (BSMH) HISTORICAL COLLECTION EXPERIENCE ADJUSTED FOR CURRENT ENVIRONMENTAL RISKS AND TRENDS FOR EACH MAJOR PAYOR SOURCE. SIGNIFICANT PROVISION IS MADE FOR SELF-PAY PATIENT ACCOUNTS IN THE PERIOD OF SERVICE BASED ON PAST COLLECTION EXPERIENCE. BSMH'S CONCENTRATION OF CREDIT RISK RELATED TO NET PATIENT ACCOUNTS IS LIMITED DUE TO THE DIVERSITY OF PATIENTS AND PAYORS. NET PATIENT ACCOUNTS CONSIST OF AMOUNTS DUE FROM GOVERNMENTAL PROGRAMS (PRIMARILY MEDICARE AND MEDICAID), PRIVATE INSURANCE COMPANIES, MANAGED CARE PROGRAMS AND PATIENTS THEMSELVES. NET PATIENT SERVICE REVENUE FOR SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY PAYOR COVERAGE IS RECOGNIZED BASED ON CONTRACTUAL RATES FOR SERVICES RENDERED. BSMH RECOGNIZES A SIGNIFICANT AMOUNT OF PATIENT SERVICE REVENUE AT THE TIME SERVICES ARE RENDERED EVEN THOUGH IT DOES NOT ASSESS THE PATIENT'S ABILITY TO PAY. AS A RESULT, THE PROVISION FOR BAD DEBTS IS PRESENTED AS A DEDUCTION FROM PATIENT SERVICE REVENUE (NET OF CONTRACTUAL PROVISIONS AND DISCOUNTS). AMOUNTS RECOGNIZED ARE SUBJECT TO ADJUSTMENT UPON REVIEW BY THIRD-PARTY PAYORS. FOR UNINSURED PATIENTS THAT DO NOT QUALIFY FOR CHARITY CARE, BSMH RECOGNIZES REVENUE WHEN SERVICES ARE PROVIDED. BASED ON HISTORICAL EXPERIENCE, A SIGNIFICANT PORTION OF BSMH'S UNINSURED PATIENTS WILL BE UNABLE OR UNWILLING TO PAY FOR SERVICES PROVIDED. THUS, BSMH RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS RELATED TO UNINSURED PATIENTS IN THE PERIOD THE SERVICES ARE PROVIDED. ANY DISCOUNTS APPLIED TO SELF-PAY PATIENTS WOULD BE DEEMED EITHER CHARITY OR A CONTRACTUAL ADJUSTMENT. BAD DEBT WOULD BE BASED ON THE BALANCE AFTER THE CHARITY OR CONTRACTUAL ADJUSTMENT THAT IS DEEMED UNCOLLECTABLE FOLLOWING A REASONABLE COLLECTION EFFORT.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBON SECOURS MERCY HEALTH'S (BSMH) FINANCIAL ASSISTANCE POLICY DOES NOT PERMIT THE COST OF PATIENTS WHO ARE UNCOOPERATIVE OR UNABLE TO BE LOCATED TO BE RECLASSIFIED FROM FINANCIAL ASSISTANCE TO BAD DEBT. BSMH's Financial Assistance Policy Requires an application and supporting documentation. BSMH also utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain such information as credit score to assist in determining whether a patient is presumed eligible for financial assistance pursuant to the BSMH Financial Assistance Policy. THEREFORE, ZERO DOLLARS ARE BEING REPORTED ON PART III, LINE 3 AS AMOUNTS INCLUDED IN BAD DEBT THAT COULD BE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER BSMH'S FINANCIAL ASSISTANCE POLICY. THE HOSPITAL FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE BAD DEBT EXPENSE AS COMMUNITY BENEFIT.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteBON SECOURS MERCY HEALTH'S (BSMH) AUDITED FINANCIAL STATEMENTS DO NOT CONTAIN A FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE. BAD DEBT EXPENSE IS REFLECTED AS A DEDUCTION FROM REVENUE RATHER THAN AS AN OPERATING EXPENSE. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS, 2. SIGNIFICANT ACCOUNTING POLICIES, (d) NET PATIENT ACCOUNTS AND NET PATIENT SERVICE REVENUE (PAGES 9 & 10) STATES Patient receivables are recorded at net realizable value based on certain assumptions determined by payor class. For third party payors including Medicare, Medicaid, and commercial insurance, the net realizable value is based on the estimated contractual reimbursement percentage, which is based on current contract prices or historical paid claims data by payor. For self-pay receivables, which includes patients who are uninsured and the patient responsibility portion for patients with insurance, the net realizable value is determined using estimates of historical collection experience. These estimates are adjusted for estimated conversions of patient responsibility portions, expected recoveries and any anticipated changes in trends.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsA cost to charge ratio is applied to the organization's Medicare gross charges to calculate Medicare costs, which are then compared to Medicare payments received, to determine a Medicare Gain or Loss. BON SECOURS MERCY HEALTH AND ITS MEMBERS follow the Catholic Health Association (CHA) Guidelines for determining community benefit. CHA Community Benefit reporting guidelines suggest that a Medicare shortfall (Loss) is not treated as community benefit, even though the hospital has incurred losses in providing care to Medicare patients. Therefore, none of the amount on Line 7 is treated as community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS KNOWN TO QUALIFY FOR CHARITY CARE OR FINANCIAL ASSISTANCE ARE NOT SENT TO A COLLECTION AGENCY. THE ORGANIZATION REPEATEDLY OFFERS PATIENTS ACCESS TO FINANCIAL HELP DURING THEIR HOSPITAL STAYS AND AFTER, AS WELL AS WITH EACH BILLING NOTICE. BILLS ARE SENT TO A COLLECTION AGENCY AS A LAST RESORT AND ONLY: WHEN PATIENTS HAVE THE ABILITY TO PAY SOME PORTION OF THEIR HEALTHCARE EXPENSES BUT REFUSE TO DO SO; WHEN PATIENTS REFUSE TO WORK WITH THE ORGANIZATION TO DETERMINE IF THEY QUALIFY FOR FREE OR DISCOUNTED CARE VIA FEDERAL, STATE, LOCAL OR HOSPITAL ASSISTANCE PROGRAMS; WHEN THE ORGANIZATION IS UNABLE TO LOCATE THE PATIENT OR PERSON RESPONSIBLE FOR THE BILL. Patients that are presumed to be eligible for financial assistance based on individual life circumstances will be provided 100% financial assistance. Patients determined to have presumptive financial assistance eligibility will not be required to meet income criteria, asset eligibility criteria, or fill out a financial assistance application. BSMH utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain information such as credit scores to assist in determining a patient's presumed eligibility.

Schedule I, Part III, Column (b) Estimated Number Of RecipientsMedical Device Assistance : Estimated based on number of grant payments
Schedule I, Part II SCHEDULE I, PART IIDescription of the Bon Secours Mercy Health Mission Fund: Bon Secours Mercy Health performs its philanthropic work through its mission department. This initiative, called the Bon Secours Health System Mission Fund ("Mission Fund"), was developed to promote the Catholic Health Ministry and the Bon Secours Mercy Health, Inc. Mission. This purpose is realized through the funding of initiatives that improve the health and well-being of communities, particularly for disenfranchised and marginalized people, served by Bon Secours Mercy Health Local Systems ("Local Systems"), Cosponsors and the Congregation of the Sisters of Bon Secours. The scope of its purpose and use of funds would be to: -promote healthy community coalition initiatives in conjunction with local system efforts, -develop local system and community excellence for a specific health condition and preventive need, and -improve access for uninsured populations and reduce health disparities among populations in the community. The Strategic Quality Plan of the health system calls for focused efforts to Build Healthier Communities. The health system understands "health" to include social and communal dimensions and has adopted the following articulation of a healthy community. The conditions of communities and individuals served by Bon Secours Mercy Health reflect the interaction of significant factors and complex behaviors at the individual, communal, and societal level. It is not likely that interventions by any one organization will result in substantial improvement or benefit to the community. Rather, increased participation by stakeholders and greater cooperation among entities with appropriate skills and resources is necessary for systemic change and improved outcomes. Mission Fund grants place emphasis on increased collaboration among community based members (Healthy Community Initiative), public health officials and other providers of services. The Mission Fund anticipates that most endeavors that seek to bring meaningful improvement require time and commitment. Consequently, local system grant recipients may expect continuity of support (several years) to establish and track outcomes. At the same time, grant applicants need to cultivate and achieve a wide array of financial resources necessary to sustain promising projects and service programs.
Schedule I, Part I, Line 2 Procedures for monitoring use of grant fundsBon Secours Mercy Health (BSMH) has a long-standing history of providing philanthropic support to community-based organizations that advance our Mission and support our health care priorities. Preference is given to donations and sponsorships for health and wellness for the community, supporting a community service or program, supporting programs that advance the mission of BSMH, and support for organizations with good standing and positive community image. The application process and requirements vary by BSMH hospital The leadership team and/or Sponsorship Committee of each BSMH hospital will review all requests for sponsorship and donations on a monthly or quarterly basis. Awardees may be required to report back on how the funds were used in their subsequent application for funding. BSMH has a financial assistance program that includes charity care, which provides free or discounted medically necessary care to eligible patients who cannot afford to pay. Eligibility is typically based on factors like income and lack of insurance, with specific criteria often found on their billing or patient financial assistance website. Based on the aforementioned eligibility criteria, some services such as medical transportation, durable medical equipment and outpatient prescriptions are separate from hospital charges and may be eligible for financial assistance.
