PART V.B., LINE 5:Beebe Healthcare began the comprehensive Community Health Needs Assessment (CHNA) process in the summer of 2015, capturing data from a variety of primary and secondary sources. Utilizing guidelines set forth by the Catholic Health Association of the United States (CHA) in collaboration with Vizient Incorporated, and Healthy Communities Institute. The Beebe CHNA process followed the following steps: Step 1: Plan and Prepare for the Assessment Step 2: Determine the Purpose and Scope of the CHNA Step 3: Identify Data that Describes the Health and Needs of the Community Step 4: Understand and Interpret the Data Step 5: Define and Validate Priorities Step 6: Document and Communicate Results The organization felt it was imperative to collect primary data due to the limitation of servicing a relatively rural population not well characterized by solely secondary data sources. According to the Delaware Health Tracker, inaccurate data on the population at risk in small geographic areas continues to complicate epidemiologic studies in community settings. Census data are less accurate for cities or counties than for states. Therefore, an ideal approach incorporates both primary and secondary data. In order to fully incorporate the primary data elements, the CHNA team conducted focus groups, town hall meetings, stakeholder interviews, and online surveys. By gathering information from local residents and area organizations, the organization hopes to gain a better insight to the priority needs of those living in the service area. Additional data was also collected through completion of a community health survey by area residents, community based organizations and health care providers. In 2015, with the help of the Delaware Division of Social Services Mobile Crisis Unit, a portion of the Sussex county homeless population was surveyed at the local homeless shelters. Lastly, key informant interviews were completed with community stake holders. Beebes Population Health Department staff organized and analyzed the data, and was responsible for collecting current demographic information from the Delaware Population Consortium, U.S. Census Bureau, the Delaware Hospital Association, and the Delaware Health Tracker. Staff also conducted a community assets and resource analysis, which was included in the overall assessment and rationale for prioritized community needs. The final product of the Community Health Needs Assessment reflects the primary data collected via surveys and analyzed for significance, in conjunction with the secondary data obtained from resources such as the Delaware Health Tracker which houses all of the available national data sources. Once this information was synthesized, the Beebe Population and Community Health Committee evaluated the results of the data collection and analysis using the principles of the Robert Wood Johnson Foundation County Health Rankings Model, emphasizing the varied and vast determinants of health. Ultimately, priorities were established by evaluating all of the identified needs in the context of the feasibility, strengths and resources of the Beebe Healthcare system.

PART V.B, LINE 11:The health needs identified through the 2016 Community Health Needs Assessment were prioritized on the basis of multiple dynamics including primary and secondary data, along with current resources, collaborations and feasibility of effective programming. The prioritization was agreed upon by Beebe Healthcares Population and Community Health Committee and the Healthier Sussex County Task Force. The three highest ranking needs, in order are: (1) Mental Health and Behavioral Health, (2) Obesity / Nutrition / Chronic Disease, (3) Cancer / Prevention & Screening. Note: Access and Availability as well as Education and Information have been identified as Priority Areas in the Community Health Needs Assessment. Beebe Healthcare's Implementation Plan integrates and addresses these needs, as relevant, throughout the 3 Priority Areas: Mental and Behavioral Health, Obesity/Nutrition/Chronic Disease, and Cancer/Prevention & Screening. Beebe Healthcare has implemented the following programs to address the significant needs identified in the CHNA. They are as follows: Priority Area: Mental and Behavioral Health -1) Mental and Behavioral Health Nurse Navigation Services - The Mental and Behavioral Health Nurse Navigator completes behavioral health screenings and assessments to identify tailored care for each individual. The nurse navigator also collaborates with our Telehealth psychiatrists, the patient, family, and any outpatient providers to develop and implement a plan of care. 2) Telepsychiatry Services - Inpatient/Emergency Department/Ambulatory - In 2014 Beebe Health Care introduced Telepsychiatry services for our patients. With the use of a mobile video camera the Telehealth service can be accessed at any time for professional screening of our patients 3) Neonatal Abstinence Syndrome Programming, 4) Women's Health Nurse Navigation services - In an effort to further enhance support provided post- hospital discharge, Beebe Healthcare has hired a nurse practitioner to function as a Nurse Navigator for the Women's Health and Orthopedics Service Lines. This provider serves as a liaison between Beebe and the community to help community members navigate our services. She is available to answer questions and help patients understand what screenings they may need or which physicians they should contact. 5) High School Wellness Centers - Wellness Centers provide free health education, nutritional help, emotional support, and, when appropriate, referrals to students' personal physicians for follow-up care. Priority Area: Obesity, Nutrition, and Chronic Disease - 1) Community Outreach Programs which provide free health screenings and education to the Sussex County community by means of health fairs and through connections with civic organizations and local businesses. 2) Food Prescription Program - This is a partnership between the Food Bank of Delaware and Beebe Healthcare to provide food prescription vouchers to inpatient and outpatient care coordination clients. 3) Beebe Wellness Program which provides a collection of services including exercise, workshops and mind/body relaxation techniques, and one-on-one coaching. 4) Wellness Works Program which is a 4-week seminar covering the basics of healthy living: healthy eating, exercise and movement, stress management and a plan for living well. 5) Health Coaching Program - The Beebe health coach works 1:1 or in small groups, partnering with individuals to help them make lifestyle changes. Working together, the health coach and individual (s) work together to create a personalized health plan to help take a person from where he/she currently is to where he/she wants to be with overall health and wellness goals. This may include healthy eating, weight management/weight loss, movement and exercise or stress management. 6) Diabetes Management & Medical Nutrition Therapy Program - Since 1998, Beebe Healthcares Diabetes Management team has provided outpatient diabetes education for the community. This comprehensive program received national recognition by the American Diabetes Association (ADA) for providing education that meets the national standard for diabetes self-management education programs. 7) Ornish Reversal Program - The program teaches participants to adopt a healthy lifestyle based on the four key elements: a low-fat, whole foods, plant-based diet; at least 30 minutes of exercise each day; a practice of stress management techniques; and active engagement in supportive relationships. This program has been proven to undo heart disease by dealing with the root causes and not just its effects. Priority Area: Cancer, Prevention and Screening - 1) Tunnell Cancer Center Cancer Screening, Education, and Outreach Program - Beebe Healthcares Tunnell Cancer Center launched the Lung Cancer Screening using LowDose Computed Tomography (LDCT) in April 2015. From the onset of the program in April 2015 through May 31, 2016, the Cancer Screening Nurse Navigator pre-screened all patients with provider orders for LDCT. Beginning June 1, 2016, physicians assumed the responsibility of screening their patients for appropriateness for screening. The program also provides education to community residents about smoking cessation and lung cancer screening.

PART VI, LINE 4:THE MEDICAL CENTER IS LOCATED IN SUSSEX COUNTY, THE SOUTHERN-MOST OF THREE DELAWARE COUNTIES, WHICH CONSISTS OF 938 SQUARE MILES, OR 48% OF DELAWARE'S ENTIRE LAND AREA. BECAUSE OF ITS CLIMATE AND LOW POPERTY TAXES, SUSSEX COUNTY HAS BECOME AN ATTRACTIVE RETIREMENT COMMUNITY. ALL OF THE COUNTY'S GROWTH OVER THE NEXT DECADE IS PROJECTED TO COME FROM PERSONS MOVING INTO THE COUNTY. TRADITIONALLY, SUSSEX COUNTY HAS BEEN REGARDED AS A RURAL AGRICULTURAL AREA. HOWEVER, THE EASTERN EDGE OF SUSSEX COUNTY THAT FRONTS ON THE DELAWARE BAY AND ATLANTIC OCEAN HAS BECOME A MAJOR TOURIST RESORT AND, MOST RECENTLY, A FAVORED RETIREMENT AREA, RETAIL, AND HOSPITALITY CENTER. EASTERN AND SOUTHEASTERN SUSSEX COUNTY, WHICH IS BEEBE MEDICAL CENTER'S PRIMARY SERVICE AREA, IS PARTICULARLY ATTRACTIVE DUE TO ITS PROXIMITY TO THE ATLANTIC OCEAN AND DELAWARE BAY. AS A RESULT, THE MEDICAL CENTER GENERALLY EXPERIENCES A SIGINIFICANT INCREASE IN PATIENT VISITS DURING THE SUMMER PERIODS. ACCORDING TO THE 2010 CENSUS, DELAWARES POPULATION WAS 897,934, WITH 61% LIVING IN NEW CASTLE COUNTY, 21% IN SUSSEX, AND 18% IN KENT COUNTY (DELAWARE HEALTH TRACKER). THERE HAS BEEN NO CENSUS SINCE 2010, BUT THE ESTIMATED POPULATION OF DELAWARE IN 2014 WAS 935,614 (DELAWARE HEALTH TRACKER). THE ESTIMATED POPULATION FOR SUSSEX COUNTY IN 2010 WAS 197,145 WITH A POPULATION OF 41,073 (20.8%) OVER THE AGE OF 65. SUSSEX ALSO HAS A VERY HIGH PERCENTAGE OF MEDICARE RECIPIENTS AT 23.3%. THE PERCENTAGE IN POVERTY IS LISTED AT 13.9%. THE SERVICE AREA POPULATION IS ESTIMATED AT 199,118 WITH A 3-YEAR GROWTH RATE OF 4.6%, APPROXIMATELY 9,197 RESIDENTS. IT IS NOTED THAT OUR SERVICE AREA HAS A 21% POPULATION AGE 65 OR OLDER WHERE THE U.S AVERAGE IS 13%. (BEEBE MEDICAL STAFF DEVELOPMENT PLAN, NOVEMBER 18, 2012). Median household income in Sussex county is $53,751, per capita it is $28,558. In Sussex in 2015, 11.6% of adults and children were uninsured. 3 hospitals serve the area, BayHealth Milford, Nanticoke in Seaford and Beebe Medical Center in Lewes. Sussex is considered a medically underserved area/population and designated as a health professional shortage area (HPSA) for primary care, mental health and dental care.
Part VI, Line 5COMMUNITY BUILDING ACTIVITIES INCLUDE INTERPRETATION SERVICES TO SUPPORT INCREASING HISPANIC POPULATION. PROVIDING MENTORS IN CONJUNCTION WITH COLLEGES AND HIGH SCHOOLS IN THE AREA FOR STUDENTS PURSUING NURSING/HEALTHCARE CAREERS. RECURITMENT AND RETENTION OF PHYSICIANS IN UDERSERVED AREAS RELATIVE TO COMMUNITY NEEDS ASSESSMENT.
Part VI, Line 6BEEBE MEDICAL CENTER (BMC) IS A MEMBER OF AN AFFILIATED HEALTH CARE SYSTEM THAT INCLUDES BEEBE PHYSICIAN NETWORK (BPN), A NOT-FOR-PROFIT TAX-EXEMPT CORPORATION THAT OPERATES A PHYSICIAN NETWORK PROVIDING INTEGRATED PHYSICIAN SERVICES IN COORDINATION WITH THE MEDICAL CENTER. BMC IS LOCATED IN A COMMUNITY THAT HAS BEEN DESIGNATED AS A PHYSICIAN UNDER-SERVED AREA SINCE 1976 AND BPN WAS ORGANIZED TO ENCOURAGE PHYSICIANS TO PRACTICE IN THE AREA TO ENSURE ACCESS TO CARE FOR THE SURROUNDING COMMUNITIES. IN OCTOBER 2013 BMC'S HEALTH CARE SYSTEM ADDED BEEBE HEALTHYBACK (BHB), A FOR-PROFIT LIMITED LIABILITY COMPANY, THAT OPERATED A SPECIALIZED CHRONIC BACK AND NECK TREATMENT PROGRAM LOCATED IN CENTRAL SUSSEX COUNTY. AS OF DECEMBER 31, 2015, THIS PARTNERSHIP WAS DISSOLVED AND BMC ABSORBED THE OPERATIONS. IN MAY 2013 BMC'S HEALTH CARE SYSTEM ADDED DELMARVA HEALTHCARE NETWORK, LLC (DHN). DHN IS AN ACCOUNTABLE CARE ORGANIZATION (ACO) WITH BEEBE AS THE SOLE MEMBER.
