PART V SEC B LINE 5QUALITATIVE DATA WAS GATHERED DURING INDIVIDUAL INTERVIEWS WITH A TOTAL OF 9 STAKEHOLDERS OF ST. ANTHONY'S HOSPITAL SERVICE AREA. THESE STAKEHOLDERS WERE IDENTIFIED BY A CHNA OVERSIGHT COMMITTEE INCLUDING HOSPITAL LEADERSHIP AND COLLABORATING AREAS OF OTHER HOSPITAL FACILITIES IN BAYCARE HEALTH SYSTEM. EACH STAKEHOLDER WAS INTERVIEWED BY TRIPP UMBACH, A CONSULTING FIRM REPRESENTING THE HOSPITALS, AND LASTED APPROXIMATELY 60 MINUTES. ALL RESPONDENTS WERE ASKED THE SAME SET OF QUESTIONS (QUESTIONS AND RESPONSES CAN BE FOUND IN THE CHNA REPORT STARTING ON PAGE 54) PREVIOUSLY DEVELOPED BY TRIPP UMBACH AND REVIEWED BY THE INDIVIDUAL HOSPITALS FACILITY LEADERSHIP. 1. FAITH COMMUNITY NURSE 2. EXECUTIVE DIRECTOR OF A FREE CLINIC 3. PRESIDENT/CEO OF THE YMCA OF GREATER ST. PETERSBURG 4. SUBSTANCE ABUSE LIAISON AND ADVOCATE 5. COO OF CATHOLIC CHARITIES 6. CONCURRENT REVIEW NURSE 7. DIRECTOR PINELLAS COUNTY HEALTH DEPARTMENT 8. DIRECTOR OF BUSINESS DEVELOPMENT FOR COMMUNITY HEALTH CENTERS OF PINELLAS COUNTY 9. PROJECT MANAGER FOR THE ONE BAY HEALTHY COMMUNITIES INITIATIVE IN ADDITION TO THE SPECIFIC STAKEHOLDER'S IDENTIFIED, ST. ANTHONY'S HOSPITAL GATHERED QUALITATIVE DATA FROM RESIDENTS OF THE COMMUNITY BY CONDUCTING FACILITATED FOCUS GROUPS. THE PARTICIPANTS WERE ABLE TO PROVIDE DIRECT INPUT RELATED TO TOP COMMUNITY NEEDS OF THEMSELVES, THEIR FAMILIES, AND COMMUNITIES. THE GOAL OF THE FOCUS GROUP PROCESS IS THAT EACH PARTICIPANT FEELS COMFORTABLE AND SPEAKS OPENLY SO THAT THEY CONTRIBUTE TO THE DISCUSSION. IT WAS EXPLAINED TO PARTICIPANTS THAT THERE ARE NO WRONG ANSWERS, JUST DIFFERENT EXPERIENCES AND POINTS OF VIEW. THIS PROCESS ENSURES THAT EACH PARTICIPANT SHARES THEIR EXPERIENCES FROM THEIR POINT OF VIEW, EVEN IF IT IS DIFFERENT FROM WHAT OTHERS HAVE SAID. SPECIFICALLY, FOCUS GROUP PARTICIPANTS WERE ASKED TO IDENTIFY AND DISCUSS WHAT THEY PERCEIVED TO BE THE TOP HEALTH ISSUES AND/OR CONCERNS IN THEIR COMMUNITIES. FOCUS GROUP INPUT IS SUBJECT TO THE LIMITATIONS OF THE IDENTIFIED TARGET POPULATIONS (I.E., VOCABULARY, PERSPECTIVE, KNOWLEDGE, ETC.), AND THEREFORE, IS NOT FACTUAL AND INHERENTLY SUBJECTIVE IN NATURE. THE FOCUS GROUP AUDIENCE PARTICIPANTS WERE: 1. RESIDENTS EARNING A LOW INCOME THAT ARE MEDICAID-INELIGIBLE 2. PRIVATE BEHAVIORAL HEALTH PRACTITIONERS SERVING RESIDENTS WITH BEHAVIORAL HEALTH NEEDS 3. AFRICAN AMERICAN RESIDENTS 4. PROFESSIONALS SERVING HOMELESS RESIDENTS

Efforts were specifically dedicated to the following action step categories with highlighted activities as noted below: Improve access to and coordination of mental health services. o BayCare is actively recruiting psychiatrists in an effort to expand access. o Ongoing initiatives continue focused to increase the community's awareness to recognize when someone is in mental health distress. 'Mental Health First Aid' adult and pediatric-specific classes are provided throughout the community to increase awareness. o Funding for a pilot project to bring mobile behavioral health services to Pinellas County homeless shelters has been approved. This program was scheduled to launch in May 2016. o The Pathways program is now operational and is designed to provide education and navigation services during crisis. o Collaborative efforts continue with community organizations including Suncoast, PAR, Boley and HEP to improve mental health services in Pinellas County. Continue advocacy outreach efforts to expand access o Navigators are in place across BayCare to facilitate enrollment in marketplace plans. Outreach activities continue (by navigators) to encourage enrollment in health insurance plans across various community locations. Team members are also available to interview uninsured patients in the hospitals and assist insured patients needing assistance for copays/deductibles (by phone or by appointment). The number of people served continues to increase. Continue to implement the Medical Home Model through BayCare Medical Group (BMG) and the hospital division o Patients at high-risk for hospital readmission are not always able to visit their primary care physician (PCP) within 7 days of discharge. St. Anthony's Hospital plans to implement a Chronic Care Clinic to assist these patients. By offering and scheduling a Chronic Care Clinic visit(s) between inpatient discharge and the PCP follow-up visit, patients are provided more of an opportunity to stay well in their own homes and to avoid a readmission to the hospital. Chronic Care Clinic visits may include reinforcement of education, medication management and support in identifying signs and symptoms to prompt treatment prior to conditions worsening. Enhance cancer prevention and treatment initiatives o Community outreach efforts continue across the health system to encourage the community to understand risk factors and to promote early detection of various types of cancer. o BayCare sites have signed the '80% by 2018' pledge with the American Cancer Society to promote appropriate colorectal cancer screening in 80% of age-appropriate community members by 2018. o The St. Anthony's Hospital lung cancer screening program was established and continues to provide low-dose lung cancer screenings. Additional St. Anthony's Hospital resources are being committed to follow the screened patients to insure that they follow through with recommendations. o The St. Anthony's Hospital campus remains tobacco-free (and maintain the incentives offered to team members for not smoking). Various smoking cessation programs are offered throughout BayCare. o BayCare has a representative serving on the HPV vaccination stakeholder task force addressing the low vaccination rates in Pinellas County & working to reduce HPV-associated cancers in the future. o Collaborative efforts continue across BayCare to provide mammography services to the under-served. St. Anthony's Hospital's Mammography Voucher program provided >125 screening mammograms which resulted in 4 women diagnosed with breast cancer in 2015. St. Anthony's Hospital has renewed the Mammography Voucher Program contract for 2016 and will continue/expand the Oncology Financial Navigator function to assist mammography voucher program patients to receive proper services. Improve care coordination and outcomes for heart failure (CHF) patients o Efforts to provide consistent care to treat heart failure across BayCare are ongoing. o Efforts are underway to provide consistent patient education materials as well as to develop clinical scorecards and evidence-based measure order sets across locations. o Various initiatives are underway to decrease readmissions across BayCare including daily inpatient rounds; post-discharge phone calls; utilization of faith community nurses; increased referrals to palliative care; establishment of heart failure support groups and nurse practitioner-led transitions of care programs. Sites continue to monitor, analyze and report CHF readmission and length of stay (LOS) metrics. Enhance stroke prevention and recognition initiatives o Hospitals are making significant progress to increase the percentage of patients appropriately receiving thrombolytic therapy (for eligible patients). o The first meeting of the newly formed "BayCare Stroke Collaborative" took place on 8/15/2015 with defined goals to identify best practices; standardize education and increase BayCare's community presence. o In August 2015, a system-wide neuro-focused Facebook campaign took place which included the online stroke risk assessment; warning signs of a stroke; and the importance of a primary stroke center. o Efforts are underway to launch consumer-focused marketing tactics to promote stroke awareness across the system in May 2016. Evaluate and align services offered to patients with diabetes to improve health outcomes for diabetic patients o Collaborative relationships with the YMCA are in place to refer patients diagnosed with pre-diabetes and to possibly help the YMCA with pre-diabetes nutrition classes. o Through the Jesse DuPont grant, the St. Anthony's Hospital team has begun teaching diabetes education management classes (at no charge to the patient) to the uninsured at the Pinellas County Health Department, Free Clinic and Pinellas Hope. Uninsured patients discharged from the hospital (with diabetes) are enrolled. o The outpatient diabetes management collaborative continues to work together to standardize outpatient education and services across BayCare. o In building on a BayCare-wide initiative in 2014 to implement a diabetic ketoacidosis protocol, a 'Hyperglycemia Power Plan' is integrated within the Electronic Health Record. It is now rolling out across BayCare. This will provide for a single standard of care for patients with hyperglycemia across BayCare. Improve care coordination and support initiatives to reduce readmissions o All hospitals continue to strive to reduce readmissions (specific efforts are underway to decrease readmissions from heart failure, diabetes, COPD and other pulmonary diseases). o Medication assistance was provided to patients through the BayCare Indigent Drug Reimbursement Program at St. Anthony's Hospital. Facilitate access to transportation services for those in need o St. Anthony's Hospital continues to facilitate and/or directly provide transportation to patients who are not able to afford it post-discharge. Increase access to (and coordination of) educational services and preventive/wellness care o BayCare continues to offer a variety of outreach educational events and screenings to increase awareness about important health issues and to improve the health of the community. o Faith Community Nurses continue to contribute far-reaching positive impact on the communities served by St. Anthony's Hospital. The Faith Community Nursing program continues to expand. o Educational opportunities for Faith Community Nurses include Sleep Disorders, Stroke Prevention and Treatment, Domestic Violence Prevention, Health Literacy and Pastoral Care Skills. o The St. Anthony's Hospital Faith Community Nursing program is now providing a designated Faith Community Nurse on a weekly basis to CASA (which houses 100 clients). CASA provides various services to women, men and children living with domestic violence (in Southern Pinellas County). o The St. Anthony's Hospital team connected with a governmental agency in Pinellas County to p

PART I, LINE 3CPatients who are uninsured or underinsured and cannot pay for hospital services are eligible for charity consideration. These patients are screened by designated team members in our Financial Assistance Department. The Agency for Health Care Administration (AHCA) defines charity eligibility at 200 percent of the federal poverty guidelines, unless the total hospital bill is more than 25 percent of the patient's annual income. Medicaid recipients who have exceeded their coverage limits are also considered for charity care. St. Anthony's Hospital, Inc goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days.
PART I, LINE 6AThe Community Benefit Report is available to the public and was prepared by BayCare Health System Inc. (EIN 59-2796965), a related organization.
PART I, LINE 7Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients, including emergency room patients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program. Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits. PART I, LINE 7F Bad Debt expense of $24,585,870 was included on form 990, Part IX, Line 25, Column (a), but subtracted for purposes of calculating the percentage in this column.
part iiSt. Anthony's Hospital supports the health of its community by providing community support, coalition building, and workforce development in coordination with a number of external organizations. It provides rent forgiveness for several charitable organizations: -Alpha House of Pinellas County which provides housing for pregnant women and their families who are in crisis. Boley Centers which aides members of the community with mental disabilities, the homeless, Veterans, and at-risk youth. -Junior League who supports a number of initiatives in its local community through volunteerism and fund-raising which support a healthy community (such as, CASA/Center Against Spouse Abuse, YWCA emergency shelter, Ronald McDonald House, etc.). In addition to supporting these charitable organizations with the use of free space, St. Anthony's Leadership provides volunteer hours and serves on Community Boards, such as, the Florida Catholic Conference and West Central Florida Stroke Council.
PART III, SECTION A, LINES 2, 3, & 4Lines 2 & 3 Bad debt expense is reported as total bad debt for the facility. The amount of bad debt expense attributable to patients eligible for financial assistance is calculated as a charge ratio, derived from data sampling. The resulting charge ratio is then applied to total bad debt accounts of the organization, which calculates the bad debt attributable to finance assistance. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid. Line 4 The organization's financial statements include a footnote that describes bad debt expense on page 9 and 10 of the Trinity Health BayCare Participants and Their Affiliated Organizations Notes to Consolidated Financial Statements.
PART III, SECTION B, LINE 8Cost reports were used to report Medicare allowable costs. Medicare defines allowable costs as those appropriate and helpful in developing and maintaining the operation of patient care facilities and activities. It specifically excludes certain costs that are not directly related to patient care. The hospital incurs additional expense related to the provision of care to Medicare patients that Medicare has deemed non-allowable. This additional expense includes costs of physician services (emergency on-call fees, hospitalist program, recruitment, etc.), advertising costs, cafeteria costs for meals sold to visitors, etc. The hospital attempts to collect coinsurance and deductibles from Medicare beneficiaries. To the extent collection efforts are unsuccessful, Medicare reimburses the hospital at 70% of unpaid amounts. The following table reconciles the surplus or shortfall from Line 7 to the actual surplus or shortfall. The additional costs were allocated to Medicare based upon Medicare's percentage of total allowable costs. The unpaid coinsurance/deductibles were estimated using historical collection results. Any shortfall amounts have not been treated as community benefit. -Line 7 Surplus or (Shortfall) ($12,596,320) -Additional non-allowable costs and unpaid/non-reimbursed coinsurance/deductibles ($8,295,185) -Total Surplus or (Shortfall) ($20,891,505)
PART III, SECTION C, LINE 9BPatients who are unable to pay are encouraged by BayCare Health System representatives, via personal interviews, signage, on patient billing statements, brochures or Customer Service phone calls, to submit financial information to the Financial Assistance Department to determine eligibility for programs, such as County, Medicaid, Disability, Victims of Crime, Charity, etc. For those patients who provide all the necessary documentation and qualify for charity according to the Financial Assistance policy, (defined in Part I, line 3c), patients' account balance would be written off completely to charity and not billed to the patients.

PART VI, LINE 5 - PROMOTION OF COMMUNITY HEALTHSt. Anthony's Hospital was founded in 1931 by the Franciscan Sisters of Allegany, NY with a mission to serve the needs of the people and to build a hospital evoking the Franciscan Sisters' faith and mission of treating everyone with the utmost kindness and consideration, regardless of circumstances. St. Anthonys Hospital is dedicated to providing community-owned health care services that set the standard for high-quality, compassionate care. The mission is evident as more than 2,000 team members and 510 highly qualified physicians work daily to achieve St. Anthony's mission of improving the health of the community. Community Benefit, Outreach and Partnerships Community benefit is an essential expression of St. Anthonys Hospitals commitment to improving the health of the communities it serves. Whether providing charity care to the uninsured or underinsured, donating resources to local free clinics or hosting health education classes and support groups, St. Anthonys generously contributes to the wellbeing of the St. Petersburg and south Pinellas County community. Mission Integration/Community Affairs Committee Made up of community members who represent diverse community organizations to keep St. Anthony's aware of community needs and facilitate collaboration, this committee of the St. Anthony's Hospital Board of Trustees includes leaders from the Historic Uptown Neighborhood Association, the City of St. Petersburg, the St. Petersburg Police Department, Catholic Charities, Office of Aging, Family Services, St. Petersburg Free Clinic, physicians, a high school principal, a rabbi, a long-term care facility and other business and social community leaders. Homeless Task Force Committee St. Anthony's has always had a strong commitment to the poor as evidenced by its Homeless Task Force Committee. This task force monitors and responds to the needs of the homeless in our area and those we serve. Pinellas Hope Since 2007, St. Anthonys has collaborated with Catholic Charities of the Diocese of St. Petersburg on a pilot project called Pinellas Hope. St. Anthony's team members have traditionally been generous and quick to respond when asked to help a worthy cause, and hospital administration and team members volunteered resources to help make the program a success. The program continues to operate successfully through strong community support and oversight by Catholic Charities. Today Pinellas Hope offers an emergency transitional shelter that provides meals to hundreds of homeless residents who seek temporary refuge at the shelter. The project helps homeless adults by giving them a place to live and providing assistance with social services, case management, food, and job placement. In 2015, St. Anthony's donated $10,888 in food for residents at Pinellas Hope. Team members volunteered their time, twice a month, serving meals to more than 3,350 persons. St. Anthony's team members also provided other assistance with donations of clothing, blankets and various supplies. Pinellas Hope Medical Respite Center Seeing the homeless population's need for continued recuperative care after hospitalization, St. Anthony's, in collaboration with Catholic Charities, Morton Plant Mease Health Care and BayCare Health System, opened a 10-unit Medical Respite Center within Pinellas Hope in 2009. Each Medical Respite client resides in a casita, a small one-room housing unit, and receives daily care and support coordinated by a case manager. Some clients simply need a stable place to regain their strength while others need continued medical treatment. Clients who need such services as wound care, IV antibiotics or other invasive medical treatment receive visits from BayCare's Home Health Services. In addition, before a client is discharged from the program, a case manager establishes an appointment for the client with a primary care provider and ensures access to prescribed medications. Each client moves into some type of housing, including the Pinellas Hope camp if needed, rather than return to the streets. The program has assisted more than 270 clients. St. Anthony's has oversight of the Medical Respite program at Pinellas Hope. In 2014, St. Anthony's Hospital was honored with an Angel Award from Catholic Charities for its work at the Medical Respite Program at Pinellas Hope. Pinellas Care Clinic The Pinellas Care Clinic provides outpatient medical care to adults living with HIV. Patients receive care in an environment that maintains their optimal physical, emotional and spiritual well-being. The clinic also serves the community as a scientific resource through education and research. The program is complemented by other HIV services available through the BayCare Health System, including St. Anthony's Hospital. Community Screenings & Special Events St. Anthony's Hospital provided free health screenings for community members on many diseases and conditions, including diabetes, heart disease, memory disorders, and hypertension. part vi, lines 15a & 15bProcess used for Compensation Review and Approval The organization uses an independent compensation committee, appointed by the Board of Directors. The Compensation Committee's purpose is to provide oversight for the organization's executive compensation program, review and approve compensation and benefits for all "disqualified persons" subject to the Intermediate Sanctions regulations issued under Section 4958 of the Internal Revenue Code (including the Chief Executive Officer, Chief Operating Officer & Chief Financial Officer, other system and entity executives, and other disqualified persons as defined in the Intermediate Sanctions regulations (i.e., voting members of the governing body, family members, former officers)), and establish the compensation philosophy for all other executives. This committee engages nationally recognized compensation consultants to assist them in review of executive compensation. The compensation consultants provide a review of each vice president and above in the system to determine if that employee's compensation is reasonable when compared against market standards. The data reviewed comes from compensation studies that include comparable compensation for similarly qualified persons in functionally comparable positions at similarly situated organizations. The organization keeps contemporaneous minutes of the compensation committees meetings and decisions. External consultants review compensation every other year with the last review occurring in 2015 but the compensation committee regularly monitors compensation and all other procedures are followed annually. Part VI, Line 16b - Procedure to evaluate Joint Venture Arrangements The organization has a joint venture committee of subject matter experts who review potential arrangements with taxable joint ventures.
