DescNewYork-Presbyterian/Queens, located in the Borough of Queens in New York City, is a 535 bed licensed, not-for-profit medical center. NYH/Q has received a determination letter from the Internal Revenue Service that it is exempt from federal income taxes as an organization described in Section 501(c)(3) of the Internal Revenue Code. The Medical Center is a voluntary, not-for-profit acute care community teaching hospital located in the Flushing section of Queens, New York. Presently, the Medical Center provides medical and surgical services and has a certified bed capacity of 535. It was incorporated in 1974 as Booth Memorial Medical Center and changed its name to The New York Hospital Medical Center of Queens in 1992 in conjunction with its affiliation with The Society of the New York Hospital (now, The New York and Presbyterian Hospital). On July 1, 2015 NYP Community Programs, Inc. became the active parent of NewYork-Presbyterian/Queens and its subsidiaries. Services and Programs: The Medical Center offers diagnostic and therapeutic services on an inpatient and outpatient basis in approximately 64 medical specialties and sub-specialties. NYP/Q has advanced capabilities in areas such as cardiac, general, and minimally invasive surgery. It receives tertiary inpatient referrals in infectious disease, nephrology/dialysis, radiation oncology, interventional radiology, and gastroenterology. It operates a Level III Neonatal Intensive Care Unit and a Level I Trauma Center (both designed to handle the most severe cases). The Medical Center provides the following programs as Centers of Excellence: The Neuroscience Institute, Cancer Center, Breast Center, Eye Center, Pediatric Asthma Center, and Obesity Surgery. NYPH/Q has been designated by the New York State Department of Health ("DOH") as an AIDS Designated Center. The Ambulatory Care division provides adult and pediatric primary/preventive care at 20 sites throughout Queens County. Cardiac Services: The Medical Center offers a variety of cardiac services including diagnostic and interventional cardiology, cardiac surgery, congestive heart failure and rehabilitation centers and cardiac research, education and prevention programs. The Cardiac Catheterization Laboratory at NYP/Q is one of only two interventional cardiology programs in Queens County, New York. The three catheterization laboratories at NYH/Q performed a combined 2,722 procedures in 2015. Now, physicians in the Electrophysiology Laboratory can implant new MRI-safe pacemakers for our patients. NYH/Q, in collaboration with The New York and Presbyterian Hospital ("NYPH"), operates a cardiac surgery program at the NYP/Q campus. The Cardiac Health Center directs the Medical Center's education and prevention programs. Lectures and special events are regularly presented to the general public at the site and at community centers and health fairs throughout Queens County. The Cardiac Health Center provides primary and secondary cardiac rehabilitation programs, including nutrition services, monitored fitness exercise, and psycho-social counseling. Surgical Services: In 2015, NYP/Q performed 15,963 inpatient and ambulatory surgical cases. These operations ranged from minimally invasive surgical procedures such as laparoscopic cholecystectomy and arthroscopic orthopedics to major open procedures such as aortic aneurysm repair and ablative and reconstructive surgery. NYP/Q's Department of Surgery has encouraged use of inpatient and outpatient minimally invasive surgery, which reduces hospital stays and recovery times. Primary care and community outreach: NYP/Q places particular emphasis on community health programs and outreach initiatives. The Hospital team reaches out to Queenss residents to support health education and awareness - offering free health fairs, health lectures, screening, support groups and blood drives. A current calendar of events can be found at nyhq.org on the website home page. This commitment is expressed in its continued strategic expansion of community-based sites to enhance access throughout the Medical Center's service area, and includes: The Trude Weishaupt Memorial Satellite Dialysis Center (the "Dialysis Center") The Dialysis Center provides hemodialysis, chronic ambulatory peritoneal dialysis, and patient and family education. NYP/Q's renal hypertension division also provides outpatient ambulatory consultative services including the diagnosis and prevention of kidney stones, hypertension treatment and evaluation, and the management of kidney diseases. The Dialysis Center provided 40,200 treatments in 2015. Medical and dental staff: As of December 31, 2015, there were approximately 1,517 physician and dentist members of the medical and dental staff of the Medical Center holding appointments in two categories: attending staff and emeritus staff. The medical and dental staff is organized into eleven clinical departments: anesthesiology, cardiothoracic surgery, emergency medicine, medicine, obstetrics and gynecology, orthopedics and rehabilitation, pathology, pediatrics, radiation therapy, radiology and surgery. The clinical departments include approximately 15 major services, subdivided further into 64 medical specialties and sub-specialties. Graduate Medical Education: The Medical Center serves as an academic medical center through the continuing education of residents, medical students, nursing students and professional and technical students. In addition to the programs listed above, the Medical Center participates in training residents in obstetrics/gynecology, dental medicine, orthopedics, pediatrics, radiation oncology and general surgery, through rotating residencies affiliated with various facilities including NYPH (Cornell Medical Center), and The Hospital for Special Surgery. For The Year Ending December 31, 2015, NYP/Q reported the following statistics: Laboratory Procedures (Excl. Blood Bank & CDL) 1,781,610 Radiology Procedures 272,974 Blood Bank 161,279 Surgical Procedures 15,963 Deliveries 4,330 Cardiac Catheterizations & PCI 3,063 EEG/EKG 56,222 EPS Ablation Procedure 128 Pacemakers 301 Event Monitor 104 Defibrillator (ICDs) 125 Cardio Version 148 Ambulatory Surgery/Endoscopy OPD 14,892 Emergency Encounters (Admits and T&R) 120,638 Ambulatory Care Visits 137,162 Patient Discharges 31,439 Average Length of Stay 5.71 Full-Time Employees 3,354 The Medical Center provides quality medical care regardless of race, creed, sex, sexual orientation, national origin, handicap, age, or ability to pay. Although reimbursement for services rendered is critical to the operations and stability of the Medical Center, it is recognized that not all individuals possess the ability to pay for essential medical services and, furthermore, the Medical Center's mission is to serve the community with respect to health care and health care education. Therefore, in keeping with the Medical Center's commitment to serve members of the community, the Medical Center provides the following: charity care to the indigent; care to persons covered by governmental programs at below cost; and health care activities and programs to support the community. These activities include wellness programs, community education programs, health screenings, and a broad variety of community support services.Desc

PART V, LINE 5THE HOSPITAL ESTABLISHED A WORKGROUP CONSISTING OF MEMBERS OF THE NYPQ'S ESTABLISHED COMMUNITY ADVISORY COUNCIL (CAC), WHICH BY CHARTER IS DIRECTLY RESPONSIBLE TO THE BOARD OF TRUSTEES, TO ENSURE GOVERNING BODY INVOLVEMENT. KEY INTERNAL AND EXTERNAL CONSTITUENTS AND RELEVANT PARTNERS WERE ENLISTED TO ADVISE AND/OR PARTICIPATE IN THE NEEDS ASSESSMENT AND TO CONTRIBUTE TO THE DEVELOPMENT, IMPLEMENTATION, AND MONITORING OF PRIORITY INITIATIVES AS A NEWLY FORMED COMMUNITY BENEFITS COMMITTEE OF THE CAC.
PART V, LINE 11The needs assessment revealed the most significant public health opportunity in Chronic Disease prevention and Healthy Women Infants and Children. The Hospital, in collaboration with its community partners chose two strategies in these two prevention priorities to focus on for the next three years: Childhood obesity among Hispanic population, and exclusive in-hospital breastfeeding of Chinese infants. Specific initiatives regarding nutrition education and exercise for a select population of school children in Pre-K through grade 2 is in process to reduce prevalence of overweight and obesity, and a multi-pronged program for early education and support of Chinese prenatal women to enhance potential success of exclusive breast was implemented. Two-hundred fifty children and their parents have been actively participating in the childhood obesity program to date. The goal is to increase awareness and to effect behavior changes that promotes healthy lifestyle. THE HOSPITAL DID NOT ADDRESS ALL OF THE NEEDS IDENTIFIED IN THE MOST RECENT CHNA. THE HOSPITAL DOES NOT HAVE APPROPRIATE EXPERTISE OR EXPERIENCE TO MAKE A SIGNIFICANT IMPACT ON THE FOLLOWING NEEDS: " LOW INCOME ACCESS TO SUPERMARKETS " ALTERNATE TRANSPORTATION USE " FLUORIDATED WATER
PART V, LINE 22D:THE HOSPITAL USED ITS HIGHEST VOLUME PAYER WHEN CALCULATING THE MAXIMUM AMOUNT THAT CAN BE CHARGED TO OUTPATIENTS AND THE MEDICAID RATE WHEN CALCULATING THE MAXIMUM AMOUNT THAT CAN BE CHARGED TO INPATIENTS.

PART VI, Line 2:New York Presbyterian/Queens conducted a Community Health Needs assessment for its service area, Queens County, NY, one of the five boroughs of New York City and is recognized as the most culturally diverse county in the United States. New York Presbyterian/Queens(NYP/Q) has an established Community Advisory Council that assists, and is responsible to, the Board of Trustees and Hospital administration making recommendations for program, facilities and other NYP/Q community related purposes. The thirty-four members of the Council represent a broad constituency of the Queens population including businesses, community boards, local civic associations, publishing, community development and the academic community. The Council nominated members to serve on a newly formed Community Service/Benefits Workgroup to coordinate the selection and prioritization of health needs. The workgroup represented, and was assisted by, several organizations to ensure input from key service area populations. The workgroup participants also represented major racial and ethnic minorities from the community who are more likely to experience health disparities, i.e., African American, Hispanics, Chinese, Korean, and Asian Indian populations. In addition to the focused efforts of the Community Service/Benefits Workgroup, New York Presbyterian/Queens collaborates with many organizations and community groups through multiple clinical and administrative relationships. These ongoing discussions and analyses of community health need and resource availability provide critical input needed to establish service and program priorities. Three major public health expert sources of data were utilized to frame the potential list of critical health needs in Queens County: Healthy People 2020, Prevention Agenda 2013-2017 and Take Care New York 2016. The Hospital solicited input from the community beginning in March of 2013. Invitations and directions on how to participate in prioritizing health needs were posted in the Hospitals main lobby, on video monitors throughout the main campus, and distributed at off-site ambulatory care facilities. Information about the Health Needs Assessment and participation in the survey were also posted on the Hospitals website and published in seven local news outlets/publications throughout the County. The survey was conducted through face to face distribution in the Hospital lobby and at off-site ambulatory care centers between the months of March and July of 2013. It was also distributed to Community Board District Managers throughout the borough and at various churches, community organizations and health fairs. In addition, the Hospital engaged an external consultant to solicit health need priorities through a telephone survey including English, Spanish, Chinese and Korean representation. After reviewing all sources of information, the Hospital chose to focus on prevention of chronic disease and promotion of healthy women, infants and children. These two areas were chosen because they met the Hospitals selection criteria; i.e. they were scored as the two most critical health need categories by the Community Health Needs Assessment Survey respondents and the Hospital has the ability to positively impact performance in each. PART VI, LINE 3: NYP/Queens conducts its charity care and financial aid program in full compliance with the policy and procedures set forth in the Financial Aid Law 2087. NYP/Queens utilizes Federal Poverty Guidelines (FPG) to determine eligibility for providing free or discounted care. The FPG family income limit for eligibility for charity care is 400%. The Hospital provides quality medical care regardless of race, creed, sex, sexual orientation, national origin, handicap, age or ability to pay. In keeping with the Hospitals commitment to serve members of the community, the Hospital provides the following: charity care to the indigent, care to persons covered by governmental programs at below cost, health care activities and programs to support the community. The Hospital makes available free care programs for qualifying patients under its charity care and financial aid policy. During the registration, billing and collection process, a patients eligibility for charity care is determined under the Hospitals charity care and financial aid policy. Further information regarding financial assistance can be obtained on the New York Hospital Queens website at: http://www.nyhq.org/Frequently_Asked_Questions_about_Financial_Assistance. In addition, appropriate signage will advise patients residing in the hospitals service area that we have a charity care policy. The signage is posted at all relevant access points; admitting, emergency department, ambulatory care etc. and is posted in six languages. ExplanationCertain officers and key employees of the New York Presbyterian Hospital that are identified in part VII as officers or trustees of the New York Hospital Medical Center of Queens are responsible for executing the mission and management of the New York Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2015 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individuals listed in part VII that are compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York Presbyterian Hospital and other related organizations in the aggregate. This amount included $20,140,759.04 attributable to a fund maintained by NYPQ as a condition of its mortgage loan insured by the Secretary of Housing and Urban Development ("HUD"). Based on reconsideration of the requirements of Part II, Line 4 and the purposes of the fund maintained in accordance with NYPQ's HUD-insured loan, we believe that this fund should not be included in Part II, Line 4 and, accordingly, have not included the current fund balance of $22,354,758 in the 2015 IRS Form 990 Schedule K, Part II, Line 4. Part II, Line 9: Consistent with prior years, NYPQ is reporting $2,063,376 as working capital expenditures from proceeds based on use of earnings on bond funds to pay interest on bonds after the date the project was placed in service. NYPQ's information about use of earnings on bond funds is limited since such funds and earnings are controlled by the issuer and the trustee.
