| Schedule H, Part V, Section B, Line 3E | THE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND WERE IDENTIFIED THROUGH THE CHNA. |
| Schedule H, Part V, Section B, Line 5 Facility , 1 | Facility , 1 - GARNET HEALTH MEDICAL CENTER. Community input for the Community Health Needs Assessment (CHNA) in Orange County was collected between March 14, 2022, and May 22, 2022, through a regional survey conducted by the Siena College Research Institute (SCRI), in partnership with Garnet Health and the Local Health Department Prevention Agenda Collaborative. This collaborative includes seven local health departments and sixteen hospital facilities, representing a broad cross-section of public health expertise and community stakeholders. The survey engaged 5,699 adult residents aged 18 and older across the region, including 906 respondents from Orange County. Responses were weighted by age, gender, race/ethnicity, and socioeconomic status to ensure accurate representation. Among Orange County's 2024 demographic projections show approximately 3.41% Asian and Pacific Islander Non-Hispanic, 11.64% Black Non-Hispanic, 26.33% Hispanic, 52.02% White Non-Hispanic, and 6.60% other ethnicities. Age groups included 24.59% aged 0-17 years, 35.25% aged 18-44, 24.25% aged 45-65, and 15.71% aged 65 and older. Socioeconomic data indicate that 8.12% of households earn less than $15,000 annually, and 14.81% earn between $25,000 and $50,000. The median household income is $95,298, which is higher than some surrounding areas; however, certain communities remain economically vulnerable, particularly in rural or underserved regions. Education data reveal that 10.67% of residents have not completed high school, including 3.57% with less than high school and 7.10% with some high school education, highlighting potential health literacy and access barriers. To capture voices from medically underserved and vulnerable populations, The Orange County Department of Health led two focus groups with service providers representing low-income individuals, veterans, seniors, people with disabilities, and members of the LGBTQ community. These focus groups provided qualitative insights into barriers to care and health disparities experienced by these groups. Organizations that actively provided input during the CHNA process include Bon Secours Charity Health System (Good Samaritan Hospital), Bon Secours Community Hospital, St. Anthony Community Hospital, Garnet Health Medical Center (formerly Orange Regional Medical Center), HealthAlliance Hospital, Nuvance Health, Montefiore Hudson Valley Collaborative, Montefiore Nyack Hospital, Montefiore St. Luke's Cornwall, Putnam Hospital Center, Vassar Brothers Medical Center, St. Joseph's Medical Center, as well as local health departments, federally qualified health centers (FQHCs), offices of aging, veterans' affairs, school districts, and food banks. Additional key collaborators that provided input or were invited to participate include Siena College Research Institute, The Foundation for Community Health, Dutchess County Department of Behavioral & Community Health, Orange County Department of Health, Putnam County Department of Health, Rockland County Department of Health, Sullivan County Public Health Services, Ulster County Department of Health and Mental Health, and Westchester County Department of Health. The organizations that provided input represent specific vulnerable populations as follows. The Orange County Department of Health and local federally qualified health centers represent low-income, uninsured and medically underserved populations, contributing data and perspectives on access barriers faced by households earning less than $15,000 annually (approximately 8.12% of the households) and those limited health literacies. Veterans affairs offices; provided insights into health and social needs or veterans, who comprise a significant portion of the county's medically underserved population. Offices of the aging focused on senior (15.71% of the population aged 65 and older), emphasizing challenges related to chronic disease management and healthcare access. Disability service providers represented individuals with physical and cognitive disabilities, highlighting mobility and communication barriers. LGBTQ advocacy groups contributed perspectives on health disparities affecting sexual and gender minorities. Food banks and social service organizations emphasized the impact of food insecurity and poverty, which affects approximately 10.4% of residents living below the Federal Poverty Guidelines (FPG). This diverse and inclusive input process ensured that the CHNA reflects the health needs and disparities experiences by medically underserved, low-income, minority, veteran, senior, disabled and LGBTQ populations across Orange County. |

| Schedule H, Part V, Section B, Line 11 Facility , 1 | Facility , 1 - GARNET HEALTH MEDICAL CENTER. In 2024, Garnet Health Medical Center (GHMC) continued its commitment to addressing significant health needs identified in the CHNA, including prevention of chronic disease, improved mental health, substance abuse prevention, and increased access to care. As a multi-campus health system serving the Mid-Hudson region, GHMC and GHMCC made substantial progress on initiatives that directly supported vulnerable populations through targeted, measurable programs. Middletown Campus: In 2024, net community benefits totaled $76.3 million (11.32% of hospital expenses). This includes: - $30.6 million (4.55%) in financial assistance and means tested government programs - $45.7 million (6.78%) in other community benefits These efforts supported initiatives such as: i) Diabetes education, school wellness programming, and chronic disease screenings, including the Know Your Numbers campaign, which screened 600+ participants for key cardiovascular indicators between 2023-2025. ii) Behavioral health services and youth mental health training, with new investments in group support and outreach. iii)Food insecurity interventions, including: -- Food Farmacy, which screened 4,000+ patients and distributed 500+ diet-friendly food bags in 2024. -- FreshRx Produce Prescription Program, an ongoing partnership with Cornell Cooperative Extension serving both Sullivan and Orange counties. New and expanded 2024 programs include: i) "Just Breathtaking" Lung Disease Support Group: A free educational and peer support group launched to help patients cope with chronic lung disease and associated mental health concerns. ii) Warrior Kids: In partnership with Snap-Ed and Eat Smart NY - Cornell Cooperative Extension, this program engaged 379 children from 2023-2025 in healthy lifestyle education and physical activity. iii) Free prostate and breast cancer screenings: Ongoing outreach events targeted to uninsured and underinsured populations. iv) Speakers Bureau: Provided expert-led health education throughout the year on priority topics such as substance abuse, mental wellness, nutrition, and chronic disease prevention. v) Cardiovascular Care Expansion: GHMC launched a new open-heart surgery program and unveiled a state-of-the-art hybrid surgical suite at its Middletown campus, advancing care access for complex cardiac conditions. While workforce shortages remain a challenge, GHMC continued to expand its telehealth services and pursue provider recruitment to maintain access to care across rural and underserved communities. These 2024-specific actions reflect GHMC's sustained implementation of its CHIP and its dedication to transparency, accountability, and meaningful community health impact. In addition, other identified health needs including Access to Medical Providers, Cancer, Obesity, Preventative Care, Women's and Children's Health have not been specifically addressed through the development of the Implementation Strategy. However, many of these needs are covered through Garnet Health's provision of comprehensive services or through the above-mentioned priority areas to be focused on over the next three years. Finally, identified health needs including Affordable Housing, Domestic Violence, Lack of Affordable/Reliable Transportation are Outside the scope of services provided by Garnet Health Medical Center, but opportunities may exist for Garnet Health to support organizations addressing these needs. |

| Schedule H, Part I, Line 5a Budgeted amounts for free or discounted care | GARNET HEALTH MEDICAL CENTER (GHMC) does not maintain a separate, formal budget line specifically designated for financial assistance under its financial assistance policy. Although a discrete budget line is not maintained, Garnet Health forecasts and tracks financial assistance expenditures to ensure adequate resources are available throughout the year, demonstrating that it remains fully committed to providing significant financial assistance and charity care to patients in need. Financial assistance is incorporated within the broader operational and community benefit planning processes, enabling flexibility to respond to patient needs as they arise. In 2024, GHMC delivered substantial financial assistance through a variety of means-tested programs, uncompensated care, and sliding-scale services. This approach ensures that patients who qualify for assistance receive appropriate care without the constraints of a rigid budget. This operational model supports the hospital's mission of serving the community and maintaining compliance with regulatory requirements. By embedding financial assistance into overall community benefit efforts rather than isolating it as a separate budget item, the hospital efficiently addresses patient needs while demonstrating transparency and accountability. |
| Schedule H, Part I, Line 7g Subsidized Health Services | LINE 7G DOES NOT INCLUDE ANY COSTS ATTRIBUTABLE TO A PHYSICIAN CLINIC. |
| Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | FOR FINANCIAL STATEMENT PURPOSES, GHMC HAS ADOPTED ACCOUNTING STANDARDS UPDATE NO. 2014-09 (TOPIC 606). IMPLICIT PRICE CONCESSIONS INCLUDES BAD DEBTS. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE IN ACCORDANCE WITH HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION STATEMENT NO. 15 AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE. |
| Schedule H, Part III, Line 3 Bad Debt Expense Methodology | GHMC follows FASB ASC Topic 606 for revenue recognition, which requires that implicit price concessions be recorded as a reduction of patient service revenue rather than as bad debt expense. As a result, GHMC does not separately report bad debt expense in its financial statements. Therefore, the estimated amount of bad debt attributable to patients who may have been eligible under the financial assistance policy is considered to be zero. |
| Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote | Per Note 2 of the audited financials statements: Subsequent changes to the estimate of the transaction price (determined on a portfolio basis when applicable) are generally recorded as adjustments to net patient service revenue in the period of the change. Portfolio collection estimates are updated monthly based on collection trends. Subsequent changes that are determined to be the result of an adverse change in the patient's ability to pay (determined on a portfolio basis when applicable) are recorded as bad debt expense. Bad debt expense for the periods ended December 31, 2024 and 2023, was not significant. |
| Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs | THE MEDICARE SHORTFALL OF $25,644,163 COULD BE CONSIDERED A COMMUNITY BENEFIT EXPENSE TO THE EXTENT THAT IT IS IN EFFECT A SUBSIDY OF A GOVERNMENTAL HEALTHCARE PLAN, WHICH ENABLES THE RECIPIENTS OF THESE BENEFITS TO RECEIVE HEALTHCARE. THE AMOUNT INDICATED ON LINE 6, AS MEDICARE ALLOWABLE COSTS RELATED TO MEDICARE PAYMENTS, HAS BEEN CALCULATED BY APPLYING THE RATIO OF PATIENT CARE COST TO CHARGES TO GROSS CHARGES FOR MEDICARE AND MEDICARE ADVANTAGE PLAN, LESS PAYMENTS. THIS RATIO IS CONSISTENT WITH THAT USED FOR MEDICAID AND OTHER PROVIDERS. |
| Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance | GHMC'S COLLECTION POLICY INDICATES THAT IF A PATIENT IS NOT ABLE TO PAY, THEY WILL BE REFERRED FOR MEDICAID ELIGIBILITY OR FINANCIAL AID. ALSO, CHARITY CARE ELIGIBILITY IS EXTENDED TO 350% OF THE FEDERAL POVERTY GUIDELINES (FPG) FOR THOSE PATIENTS WHO ARE INDIGENT BUT HAVE SOME INCOME, AND DO NOT QUALIFY FOR A 100% WRITE-OFF. DURING THE FINANCIAL ASSISTANCE DETERMINATION PROCESS, COLLECTION EFFORTS WILL BE PLACED ON HOLD, AND THE ACCOUNT WILL BE ASSIGNED TO A CHARITY FINANCIAL STATUS. ALSO, ACCOUNTS PENDING PAYMENT FROM MEDICAID FOR ELIGIBLE PATIENTS WILL NOT BE REFERRED TO COLLECTIONS. |

| Schedule H, Part VI, Line 2 Needs assessment | THE MAJORITY OF GHMC'S BOARD OF DIRECTORS CONSISTS OF LOCAL HEALTH CONSUMERS WHO PROVIDE EXPERTISE IN IDENTIFYING THE HEALTHCARE NEEDS AND PRIORITIES OF THE COMMUNITIES SERVED. IN ADDITION, GHMC CONDUCTS, COORDINATES, AND PARTICIPATES IN A BROAD RANGE OF COMMUNITY OUTREACH PROGRAMS THAT ALLOW COMMUNITY MEMBERS THE OPPORTUNITY TO INTERACT DIRECTLY WITH REPRESENTATIVES FROM THE HOSPITAL. |
| Schedule H, Part VI, Line 3 Patient education of eligibility for assistance | TO INFORM AND EDUCATE OUR PATIENTS REGARDING THEIR ELIGIBILITY FOR FINANCIAL ASSISTANCE, GARNET HEALTH MEDICAL CENTER UTILIZES SEVERAL METHODS, INCLUDING PROVIDING A FINANCIAL AID SUMMARY AT REGISTRATION, POSTING SIGNS IN PATIENT AREAS, DETAILING THE INFORMATION ON THE ORGANIZATION'S WEBSITE, AND INCLUDING IT ON PATIENT STATEMENTS. |
| Schedule H, Part VI, Line 4 Community information | Garnet Health Medical Center (GHMC) defines its community as its broader service area, which includes portions of Orange, Sullivan, and Ulster Counties in New York State. Approximately 87% of GHMC's patients reside within this service area, and approximately 70% of patients reside specifically in Orange County. For purposes of this Community Health Needs Assessment (CHNA), GHMC has placed emphasis on Orange County to more effectively focus its resources on addressing significant health needs, targeting areas of greatest need and health disparities within its primary patient population. Comparative data from Sullivan and Ulster Counties is included in the CHNA report when available, to provide regional context. As of 2024, the population of Orange County is projected to be approximately 400,016 residents. The racial and ethnic composition reflects significant diversity, with 26.3% of the population identifying as Hispanic, 11.6% as Black Non-Hispanic, 3.4% as Asian and Pacific Islander Non-Hispanic, and 52.0% as White Non-Hispanic. The age distribution in Orange County includes 24.6% of residents under age 18, 35.3% aged 18 to 44, 24.5% aged 45 to 64, and 15.7% aged 65 and older. This distribution indicates a need for health services across all age groups. Household income data highlights socioeconomic diversity. As of 2024, 8.1% of Orange County households earn less than $15,000 annually, and 14.8% earn between $25,000 and $50,000. The median household income in Orange County is $95,298, which is higher than some surrounding areas; however, certain communities remain economically vulnerable, particularly in rural or underserved regions. Approximately 10.4% of Orange County residents live below the Federal Poverty Guidelines, with higher concentrations of poverty in specific census tracts that may experience limited access to healthcare and other services. Regarding education, 10.7% of Orange County residents have not completed high school, which may contribute to barriers in health literacy, employment, and access to healthcare services. In addition, approximately 4.0% of Orange County residents are uninsured, which presents ongoing barriers to accessing preventive services, managing chronic conditions, and receiving timely care. While this rate is lower than national averages, pockets of underinsurance and lack of coverage persist, particularly in rural areas and among low-income populations. Poverty data indicates that multiple census tracts in Orange County exceed state benchmarks for the percentage of residents living below the Federal Poverty Guidelines. This comprehensive demographic and geographic data are essential to Garnet Health's efforts to reduce health disparities, identify service gaps, and align resources with the evolving needs of the communities it serves, with an emphasis on Orange County. |
| Schedule H, Part VI, Line 5 Promotion of community health | GARNET HEALTH MEDICAL CENTER (GHMC) HAS SHOWN ITS ONGOING COMMITMENT TOWARDS ENHANCING THE HEALTH STATUS OF THE COMMUNITY IN SEVERAL WAYS. THE MAJORITY OF GHMC'S GOVERNING BODY CONSISTS OF PERSONS WHO RESIDE IN GHMC'S PRIMARY SERVICE AREA WHO ARE NEITHER EMPLOYEES NOR INDEPENDENT CONTRACTORS OF THE ORGANIZATION, NOR FAMILY MEMBERS THEREOF. AS LOCAL HEALTH CONSUMERS, THEY PROVIDE EXPERTISE IN IDENTIFYING THE HEALTHCARE NEEDS AND PRIORITIES OF THE COMMUNITIES SERVED. ALSO, GHMC CONDUCTS, COORDINATES, AND PARTICIPATES IN A BROAD RANGE OF COMMUNITY OUTREACH PROGRAMS THAT ALLOW COMMUNITY MEMBERS THE OPPORTUNITY TO INTERACT DIRECTLY WITH REPRESENTATIVES FROM THE HOSPITAL. GHMC'S MEDICAL STAFF IS AN OPEN MEDICAL STAFF WHEREBY ANY JUSTIFIED PHYSICIAN IS ENCOURAGED TO APPLY FOR MEDICAL STAFF MEMBERSHIP. During the 2024 tax year, GHMC allocated surplus funds totaling over $4.9 million to initiatives aligned with the health priorities identified in its most recent Community Health Needs Assessment (CHNA). These investments were not limited to routine capital improvements but were directed toward addressing gaps in care, expanding access, and supporting medically underserved populations throughout the service region. More specifically, approximately $1.6 million was utilized to complete targeted updates to the pharmacy and laboratory service lines. These upgrades improved diagnostic accuracy and timely access to medications, both of which are essential for managing chronic conditions such as diabetes and cardiovascular disease. These needs were directly identified in the CHNA, particularly among low-income and rural populations with limited access to primary and specialty care. Enhanced lab and pharmacy capacity support early detection and treatment adherence, improving outcomes for patients who may face transportation, affordability, or health literacy barriers. An additional $3.3 million was invested in routine equipment replacements, including imaging systems, surgical tools, and patient monitoring equipment. While some of these expenditures are part of standard hospital operations, many were strategically selected to reduce wait times and support continuity of care in key service lines identified as high-need in the CHNA, such as maternal health, behavioral health, and emergency services. These upgrades enable Garnet Health to provide more timely and effective care to vulnerable populations, including seniors, individuals with disabilities, and those experiencing housing or economic insecurity. Surplus funds were also used to strengthen telehealth infrastructure and care coordination, particularly benefiting rural patients and those with transportation barriers in Sullivan and Orange Counties.
