DescEL PASO CHILDREN'S HOSPITAL IS THE ONLY SEPARATELY LICENSED, NON-TAXING, INDEPENDENT, 501(C)(3) NOT-FOR-PROFIT CHILDRENS HOSPITAL IN THE EL PASO, TEXAS REGION AND THE ONLY DEDICATED PEDIATRIC HOSPITAL WITHIN A 200-MILE RADIUS. THE HOSPITAL PROVIDES PEDIATRIC INPATIENT, OUTPATIENT AND EMERGENCY CARE SERVICES TO CHILDREN FROM BIRTH THROUGH ADOLESCENCE. IT RECEIVED THE JOINT COMMISSION ACCREDITATION ON MARCH 9, 2012, ITS MEDICAID PROVIDER NUMBER IN JUNE 2012, AND OPENED FEBRUARY 14, 2012. THROUGH THE HOSPITAL'S AFFILIATION WITH TEXAS TECH UNIVERSITY HEALTH SCIENCE CENTER, THE HOSPITAL CONDUCTS PEDIATRIC RESEARCH AND EDUCATION, AND SERVES AS A TEACHING HOSPITAL FOR THE TEXAS TECH RESIDENCY PROGRAM. THE HOSPITAL HAS FOUR FULL-TIME TEXAS MEDICAID HEALTHCARE PARTNERSHIP (TMHP) PERSONNEL LOCATED IN THE BUILDING TO ENSURE THAT EVERY ELIGIBLE PATIENT IS ENROLLED IN CHIP-TEXAS which IS SPECIFICALLY LEGISLATED TO PROVIDE MEDICAID-LIKE COVERAGE TO CHILDREN EVEN IF THEIR PARENTS DO NOT QUALIFY. AS A RESULT, THE FEDERALLY DEFINED AND REPORTED 1.8% OF TOTAL EXPENSE ON FORM 990 FOR TEXAS BASED PEDIATRIC HOSPITALS IS GENERALLY LOW. THIS INDICATES THAT THE LARGE MAJORITY OF CHILDREN CAN BE ENROLLED IN SOME TYPE OF GOVERNMENT PROGRAM. AS A RESULT, ONLY A VERY FEW CASES REMAIN IN THE TRADITIONAL CATEGORY OF CHARITY. THOSE PATIENTS REMAINING IN A CHARITY CLASSIFICATION ARE PROVIDED FREE OR REDUCED CHARGES PER WRITTEN EL PASO CHILDREN'S HOSPITAL POLICY. MORE CHARITY CARE INFORMATION IS Available ON PAGE 17 OF THE ORGANIZATION'S AUDITED FINANCIAL STATEMENTS. See ATTACHED.Desc

ExplanationLiability for uncertain tax positions: The El Paso Childrens Hospital and Physician Group are nonprofit corporations, exempt from Federal income tax under Section 501(c)(3) of the Internal Revenue Code. Accordingly, no provision has been made for income taxes in the financial statements. The Hospital recognizes the tax benefit from uncertain tax positions only if it is more likely than not that the tax positions will be sustained on examination by the tax authorities, based on the technical merits of the position. The tax benefit is measured based on the largest benefit that has a greater than 50% likelihood of being realized upon ultimate settlement. The Hospital had no unrecognized tax benefits at September 30, 2014 and 2013. The Hospital files exempt organization returns and applicable unrelated business income tax returns in the U.S. Federal jurisdiction and Texas Department of Revenue. The Hospital is no longer subject to income tax examinations by taxing authorities for years before fiscal year 2008 for its federal and state filings.ExplanationTxt

SCHEDULE H, PART V, SECTION B, LINE 5CHNA INPUT FROM REPRESENTATIVES OF THE COMMUNITY: PRIMARY DATA WAS COLLECTED FROM INTERVIEWS WITH COMMUNITY LEADERS AND STAKEHOLDERS. EL PASO CHILDREN'S HOSPITAL COMMISSIONED 10 IN-DEPTH INTERVIEWS WITH COMMUNITY MEMBERS WHO REPRESENT THE BROAD INTERESTS AS WELL AS THE SPECIFIC POPULATIONS IN EL PASO COUNTY. COMMUNITY STAKEHOLDERS INTERVIEWED REPRESENTED THOSE WITH PERSPECTIVES ON THE HEALTH OF CHILDREN WITHIN THE HOSPITAL COMMUNITY, INCLUDING HEALTHCARE AND SOCIAL SERVICE PROVIDERS, COMMUNITY OFFICIALS, THE LOW-INCOME POPULATION, THE WORKING CLASS AND GENERAL POPULATION, AND THE LATINO POPULATION. SECONDARY DATA WAS COLLECTED USING DATABASES CREATED BY OTHER AGENCIES OR ORGANIZATIONS. TYPES OF DATA COLLECTED INCLUDED DEMOGRAPHIC INFORMATION, CHRONIC DISEASE AND MORBIDITY/MORTALITY INFORMATION, HEALTH STATUS INDICATORS AND HEALTH BEHAVIORS, FAMILY PLANNING AND BIRTHS, GENERAL COMMUNITY/ENVIRONMENT INFORMATION, AND LOCAL, STATE AND NATIONAL BENCHMARKS. EL PASO CHILDREN'S HOSPITAL ALSO REQUESTED THE COLLECTION OF SECONDARY DATA ON A BROAD ARRAY OF HEALTH INDICATORS AND DEMOGRAPHIC INFORMATION.
SCHEDULE H, PART V, SECTION B, LINEs 7 & 10CHNA AND IMPLEMENTATION STRATEGY: THE CHNA AND IMPLEMENTATION STRATEGY CAN BE FOUND ON THE FOLLOWING WEBSITE: HTTP://ELPASOCHILDRENS.ORG/COMMUNITY-HEALTH-NEEDS/
Schedule H, Part V, Section B, Line 11NEEDS ADDRESSED AND NOT ADDRESSED ON THE IMPLEMENTATION STRATEGY: Through utilizing the prioritized needs list and group discussion, El Paso Children's Hospital elected to address issues from all five need areas identified in the CHNA. 1. LIMITED HEALTH CARE PROVIDERS AND ACCESS POINTS - In El Paso County, it is difficult for many residents to find a primary care or specialty care provider. This challenge is particularly acute in specific regions and rural locations of the area. The Hospital is working to Retain existing pediatric subspecialists within the community, Increase the number of pediatric subspecialists in the hospital community, and Increase the variety of pediatric subspecialists in the hospital community. 2. PREVENTIONS AND WELLNESS - Issues related to disease prevention and maintaining health affect the overall wellness of children within the hospital community. The Hospital will Continue to provide kid-friendly health education within the community at various events. The number of community events and attendance will be tracked. The Hospital will Continue existing provider education, and investigate new opportunities, including a Diabetes Conference for School Nurses and a Non-Medical Conference designed for parents. There are also plans to Expand asthma outpatient class offerings by hospital respiratory team, and increase utilization. The number of classes and participants will be tracked. More diabetes educational event offerings by hospital endocrinology team will also be held. The number of events and participants will be tracked. 3. POVERTY, LOW INCOME, NON-INSURANCE, LOW EDUCATIONAL STATUS - Poverty and non-insurance are large barriers to care within the El Paso Children's Hospital community. To combat this need, the Hospital will continue providing discounts for non-insured patients seeking care in the El Paso Children's Hospital system. The number of discounts given and the total monetary amount of the program will be tracked. Charity assistance will still be provided to those who qualify. The number of individuals given charity care assistance through the program will be tracked. The Hospital will also Work to create partnerships with local medical, nursing, and public health schools to develop resources for the low income and noninsured populations. Progress in the execution of this effort will be tracked. 4. CULTURAL BARRIERS - A family's culture can affect their health. The decision to or not to access health care services can be influenced by the cultural background of a family. Over 80 percent of the community residents are Hispanic, creating a vibrant and diverse culture in the area. In other areas of the United States where there is an emerging Hispanic community, the language barrier is the most significant issue in accessing appropriate health care for families. To combat this unmet need of the community, the Hospital will continue to provide translator services and employ bilingual providers. The hospital recognizes there are many factors that influence cultural barriers to healthcare outside of language. El Paso Children's Hospital will strive to work with strategic community partners to learn more about existing cultural barrier issues within the community in an effort to develop targeted strategies to address disparities in this area. Specifically, El Paso Children's Hospital will look into partnering with other community organizations in developing cultural competency training. 5. TRANSPORTATION - The ability to access health care in a physical sense represents a large barrier to care, especially for at-risk and vulnerable populations. The Hospital will meet this need by enhancing the initiative of offering subsidized transportation vouchers to low-income individuals and families for transportation to and from El Paso Children's Hospital facilities. The number of vouchers utilized and the monetary contribution to this initiative will be tracked. The hospital's CHNA did not identify any additional needs within the community that were not addressed through the implementation strategy.
Schedule H, Part V, Section B, Line 13hEligibility Criteria Explained in the FAP: The hospital also uses Medicaid and Medicare to determine eligibility for financial assistance.
SCHEDULE H, PART V, SECTION B, LINE 18, 19 & 20BILLING AND COLLECTIONS: NEITHER EL PASO CHILDREN'S HOSPITAL, NOR THIRD PARTIES AUTHORIZED BY EL PASO CHILDREN'S HOSPITAL, TAKE ANY ACTIONS UPON NON-PAYMENT FROM A PATIENT BEFORE MAKING A REASONABLE EFFORT TO DETERMINE IF THE PATIENT IS ELIGIBLE FOR THE FACILITY'S FINANCIAL ASSISTANCE POLICY.

SCHEDULE H, PART VI, LINE 4Community Information: The City of El Paso is located in far west Texas, along the US/Mexico border. On the Mexican side of the border, the population of the Ciudad Juarez exceeds that of El Paso, Texas. Most of El Paso County's population is centered around the City of El Paso. The El Paso City and County unemployment rates in June 2014 were significantly higher than the State of Texas and national average. The median age of people in the El Paso Children's Hospital community is varied, with some areas in the community below 21.0 and other areas as high as 40.7. This likely translates to a broad variety of health needs across the community, based on age population, and proportionately more children in certain zip codes over others. As a comparison, the median age in Texas is 33.6 years, and the national median age is 36.8 years. The racial makeup of the hospital community is predominately White, and African American as a very distant second. El Paso County is less racially diverse than the State of Texas overall. At 81 percent of the total population in the hospital community, the Hispanic population in El Paso County is very large, relative to 38 percent in Texas. El Paso County has a significantly lower median household income than the State of Texas. Not surprisingly, the poverty level in El Paso County is also significantly higher than that of Texas. The child poverty rate in El Paso County is also higher than the Texas state average. Several of the zip codes in the hospital community have particularly high child poverty rates. Zip codes 79901, 79916, and 79838 are nearly twice the rate of El Paso County and three times that of the State of Texas. In the 2010-2011 school year, 68.8% of El Paso County children were eligible for free or reduced price school lunches, compared to 50.1% of children in Texas. Schedule H, Part VI, Line 5 Promotion of Community Health: El Paso Childrens Hospital elevates the quality and scope of medical care in the El Paso region by providing unmatched excellence in specialized pediatric patient care as well as research and education opportunities. OUTSIDE INTERESTS SHALL BE DISCLOSED AS A POTENTIAL CONFLICT OF INTEREST WHEN EPCH LEADERS RECEIVE MONIES FOR DUTIES PERFORMED AT ENTITIES WITH THE SAME OR SIMILAR SERVICE OFFERED AND IN A GEOGRAPHIC LOCATION SERVING THE SAME COMMUNITY. THE COMPLIANCE OFFICER REVIEWS WITH THE CEO AND GENERAL COUNSEL ANY CONFLICT OF INTEREST CERTIFICATION FORMS THAT IDENTIFY POTENTIAL CONFLICTS. THE COMPLIANCE OFFICER MAY ASK THE INTERESTED PERSON TO DISCUSS THE TRANSACTION IN ORDER TO OBTAIN FURTHER DETAILS. THE COMPLIANCE OFFICER MAY, IF APPROPRIATE, APPOINT A DISINTERESTED PERSON OR COMMITTEE TO INVESTIGATE ALTERNATIVES TO ANY TRANSACTION OR ARRANGEMENT AND DETERMINES WHETHER THE HOSPITAL CAN OBTAIN A MORE ADVANTAGEOUS TRANSACTION OR ARRANGEMENT WITH REASONABLE EFFORTS FROM A PERSON OR ENTITY THAT WOULD NOT GIVE RISE TO A CONFLICT OF INTEREST. THE COMPLIANCE OFFICER PROVIDES QUARTERLY REPORTS TO THE BOARD OF DIRECTORS DETAILING CONFLICTS OF INTEREST AND HOW THEY WERE ADDRESSED WITH RESPECT TO RESEARCH. EPCH REQUIRES THAT A RESEARCH INVESTIGATOR DISCLOSE TO EPCH OUTSIDE FINANCIAL INTERESTS ABOVE SPECIFIED THRESHOLDS WHEN RESEARCH WOULD LIKELY BE AFFECTED BY THOSE INTERESTS. EPCH REVIEWS THE DISCLOSURES, DETERMINES WHETHER THERE IS A CONFLICT AND, IF SO, MANAGES, REDUCES, OR ELIMINATES IT, BEFORE ALLOWING PARTICIPATION IN THE PROPOSED RESEARCH PROJECT. UPON DISCOVERY OF A POTENTIAL CONFLICT OF INTEREST THAT HAS NOT BEEN DISCLOSED, THE EPCH LEADERS' MEDICAL STAFF MEMBER IS NOTIFIED BY THE COMPLIANCE OFFICER AND AFFORDED THE OPPORTUNITY TO EXPLAIN THE ALLEGED FAILURE TO DISCLOSE IF, AFTER REVIEWING THE RESPONSE AND MAKING SUCH FURTHER INVESTIGATION AS APPROPRIATE THE COMPLIANCE OFFICER SHALL INFORM THE CEO OF THE DETERMINATION WHETHER THE EPCH LEADER/MEDICAL STAFF MEMBER FAILED TO DISCLOSE AN ACTUAL OR POTENTIAL CONFLICT OF INTEREST. ANY VIOLATIONS OF THIS POLICY MAY BE GROUNDS FOR DISCIPLINARY ACTION INCLUDING TERMINATION OF EMPLOYMENT OR HOSPITAL CREDENTIALS, AND FOR CIVIL OR CRIMINAL PROSECUTION.
