Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule D, Part V, Line 4 Intended uses of endowment fundsEndowment fund earnings are used toward research, education, and to fund department chairs, as well as general operations and uncompensated care offered to patients who meet the criteria established under Endeavor Health's financial assistance policy. The Board of Directors designated a portion of net assets without donor restrictions as a board-designated endowment for the purpose of funding community benefit activities to meaningfully advance the health and well-being of the communities served. Areas of focus include social determinants of health, health equity, education, career development, and supply chain diversity.
Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteThe affiliates and subsidiaries of the System are primarily tax-exempt organizations under Internal Revenue Code Section 501(c)(3), and their related income is exempt from federal income tax. Accordingly, there is no material provision for income tax for these entities. Some of the income received by exempt entities is subject to taxation as unrelated business income. The System files federal income tax returns and returns for various states in the U.S. ASC 740-10, Income Taxes, requires that realization of an uncertain income tax position is more likely than not (i.e., greater than 50% likelihood of receiving a benefit) before it can be recognized in the consolidated financial statements. Furthermore, this interpretation prescribes the benefit to be recorded in the consolidated financial statements as the amount most likely to be realized assuming a review by tax authorities having all relevant information and applying current conventions. This interpretation also clarifies the financial statement classification of tax-related penalties and interest and sets forth new disclosures regarding unrecognized tax benefits. No amount was recorded for the year ended December 31, 2024. At year-end December 31, 2024, the System has a net operating loss carryforward of $416,494, which generated deferred tax assets of $120,665. These assets are offset by a valuation allowance of $112,623.

Schedule H, Part V, Section B, Line 3EThe significant health needs of the hospital's patients and community are identified through prioritized descriptions reflected in the Community Health Needs Assessment (CHNA).
Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - Facility Reporting Group A. Facility Reporting Group A: Reporting Group A consists of the following hospital facilities: 1 - Evanston Hospital 2 - Highland Park Hospital 3 - Glenbrook Hospital 4 - Skokie Hospital Community Health Survey Endeavor Health Clinical Operations (EHCO) developed a comprehensive online community health survey to accept input from people who represent the broad interests of the community served by the hospital. The survey, available in English and Spanish, asked participants to share their beliefs and perceptions about access to care, behavioral health, chronic disease/health issues, modifiable risk factors/behaviors and other concerns. It also asked the participants to rank their top health concern for the community. Community members who live in the hospital's service area were invited to participate in the online survey through social media, a community leader newsletter, and various emails to EHCO donors, volunteers, employees and patients. The survey link, located on Endeavor Health's website (endeavorhealth.org/survey), was open March 25 - April 8, 2024 and 4,981 people in EHCO's service area responded. Key Informant Survey An additional online survey was implemented as part of the CHNA process specifically to solicit input from key informants. Key informants are defined as individuals who are considered experts in public health. Potential participants were chosen because of their ability to identify primary concerns of the community, including the medically underserved, low-income and minority populations served by the hospital. On March 4, 2024, the EHCO hospital presidents sent an email to the key informants to explain the purpose of the survey and invite them to participate. A direct link to the survey, which remained open until March 18, 2024, was also included. A reminder email was sent on March 14, 2024 to encourage participation. The key informants were asked the same questions as the community, focused on access to care, behavioral health, chronic disease/health issues, modifiable risk factors/behaviors and Social Determinants of Health and other concerns. It also asked them to rank their "top health concern" for the community. A total of 104 surveys were completed at EHCO's invitation. The Key Informant Survey was modeled off of the Community Survey, with minimal variations. Through this process, input was gathered from individuals whose organizations work with low-income populations, minority populations or other medically underserved populations (including the disabled, the elderly, the homeless, Medicaid/Medicare beneficiaries, the mentally ill, pregnant teens, substance abusers, undocumented individuals, veterans and uninsured/underinsured residents). The following organizations were invited to participate in the key informant survey: - Second Baptist Church - NorthShore Pastoral Care - Evanston Department of Health and Human Services - Lake County Health Department - Skokie Health Department - NorthShore Emergency Medicine - NorthShore Immediate Care Centers - Erie Evanston/Skokie Health Center - Rosalind Franklin University - Fenix Health Center - The Josselyn Center - Heartland Health Centers - Turning Point Behavioral Health Center - Family Service Center - Family Services of Glencoe - Northwestern University - New Trier Township High School - Evanston Chamber of Commerce - NorthShore Market - The Aux - Terry Performance Group - Evanston Community Foundation - Glenbrook South High School - School District 34 - School District 31 - Glenview Park District - Glenview Police Department - NorthShore Police Department - North Shore School District 113 - Highland Park Community Foundation - Highland Park Library - Highwood Library and Community Center - Moraine Township - Niles Township - Northfield Township - Skokie Library - Niles Township High School - Skokie Fire Department - Oakton Community College - University of Lake County - School District 69 - Skokie Community Foundation - Erie Family Health - American Academy of Dermatology - Evanston Pride - Youth Job Center - Peer Services - Cradle to Career - NAMI CCNS - Childcare Network of Evanston - McGaw YMCA - Connections to the Homeless - North Shore Senior Center - Presbyterian Homes - Frisbie Senior Center - Wesley Childcare Center - Faith in Action - Tri-Con Child Care - United Way of Lake County - Mano a Mano - PADS of Lake County - Catholic Charities - Middle Eastern Immigrant and Refugee Alliance - TASC - CJE Senior Life Focus Groups EHCO recognized the importance of gathering opinions and feedback from under-resourced populations in the community who have some of the greatest healthcare needs. Independent moderators were identified to conduct four focus groups, one with Mongolian-speaking under-resourced community members, another with Spanish-speaking under-resourced community members and two with English-speaking under-resourced community members. These focus groups were held in Evanston, Northbrook, Skokie and Waukegan, all located within EHCO's primary service area. Potential participants were given a short screening questionnaire to verify they lived in the hospitals' service area and to ensure that participants selected would vary in age, gender, insurance status, income and educational levels, to the extent possible. The focus groups followed a guideline which mirrored questions included in the community and key informant surveys so that the information gathered could be used to compare with the survey results. Each moderator summarized the results of the focus groups which were used as one of the tools in identifying the most predominant community needs. The focus groups included the following organizations: - Second Baptist Church - Niles TWP Food Pantry - Northbrook Public Library (Mongolian) - Roberti Care (Spanish)
Schedule H, Part V, Section B, Line 6a Facility A, 1Facility A, 1 - Facility Reporting Group A. Facility Reporting Group A: Reporting Group A consists of the following hospital facilities: 1 - Evanston Hospital 2 - Highland Park Hospital 3 - Glenbrook Hospital 4 - Skokie Hospital The Community Health Needs Assessment (CHNA) conducted by Endeavor Health Clinical Operations (EHCO) included all four EHCO hospitals: Evanston Hospital, Glenbrook Hospital, Highland Park Hospital, and Skokie Hospital.
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - Facility Reporting Group A. Facility Reporting Group A: Reporting Group A consists of the following hospital facilities: 1 - Evanston Hospital 2 - Highland Park Hospital 3 - Glenbrook Hospital 4 - Skokie Hospital In consideration of the top health priorities identified through the CHNA process - and taking into account hospital resources and overall alignment with the hospital's mission, goals and strategic priorities - it was determined that EHCO would focus the majority of its efforts on developing and/or supporting strategies and initiatives that address the following priority areas (in no particular order), with health equity and SDOH woven throughout: 1. Behavioral Health (includes mental health, substance abuse and access to care) 2. Community Health and Wellness (includes nutrition, physical activity, healthy body weight and food security) 3. Chronic Diseases (includes heart disease/high blood pressure/stroke and diabetes) 4. Cancer (includes smoking/tobacco cessation) 5. Access to Care (includes primary, specialty, screenings, diagnostics and prescription medication) Based on relationships with community partners, clinical expertise, strategic priorities and an ongoing commitment to community engagement, EHCO believes it is best equipped to make an impact in the above priority needs, including focused attention within communities of greatest need. The corresponding Implementation Strategies describe programs EHCO is undertaking over the coming years to address the prioritized health needs within the surrounding communities, as follows: Behavioral Health - Provide standardized behavioral health inpatient placement for individuals in crisis via the ED Crisis Teams and Care Management Center. - Offer free, confidential 24/7 telephone support to individuals needing behavioral health support and referrals (1-847-HEALING). - Provide school-based mental health assessments, medication management and psychotherapy for students (ex. Bridges Program). - Provide psychiatric services in person and virtually via Evanston Hospital's Community Health Center (CHC) for community members living within the CHC boundary area (ex. CHC, Phoenix Program). Community Health and Wellness - Offer free wellness webinars focused on health education and living a healthy lifestyle. - Provide digital health literacy education (including Endeavor MyChart portal) to community members via Simbulance outreach team. - Offer education to providers on inclusive language to address the topic of obesity with patients. - Provide outpatient nutrition counseling by a registered dietitian. Chronic Diseases - Support controlled hypertension (HTN) levels among Endeavor Health Medical Group (MG) patients. - Support controlled levels of diabetes/A1C among MG patients. - Use the Lens of Equity Tool to identify populations and develop targeted interventions around chronic disease management. - Provide health education and screenings at community events (Evanston Hospital Community Health Center staff). - Provide diabetes and nutrition education for community members and healthcare professionals. Cancer - Utilize FIT Tests (fecal immunochemical test) for Endeavor Health Medical Group (MG) patients who have been recommended a colonoscopy screening and declined. - Use Lens of Equity Tool to identify populations and develop targeted interventions around cancer screenings. - Offer smoking cessation program to Kellogg oncology patients who use tobacco. - Offer oncology support groups for patients, family members and caregivers. - Provide community-based cancer prevention, awareness and screening programs (ex. presentations, health fairs). Access to Care - Deploy a team of Community Health Workers (CHW) to provide patient support which may include finding medical homes, scheduling appointments and screenings, addressing social determinants of health and referrals to other community resources. - Provide primary and specialty care to adults who lack private medical insurance via Evanston Hospital's Community Health Center. - Provide oral healthcare services for medically underserved adult patients via the Dental Center at Evanston Hospital. - Provide healthcare services, education and support to Evanston Township High School students via onsite Health Center. EHCO focused on three priority health issues as a result of the previous 2022 CHNA: 1. Access to Health Services 2. Mental Health 3. Chronic Health Conditions In acknowledging the wide range of priority health issues that emerged from the CHNA process, EHCO determined that it could only effectively focus on those which it deemed most pressing, most under-addressed and most within its ability to influence. EHCO worked with key stakeholders to develop strategies, tactics and metrics for the majority of the top 50% prioritized needs identified in the CHNA. The remaining needs in the top 50% are addressed as noted below. - Homelessness and Housing (adequate, affordable, safe) - In an effort to address affordable housing issues, EHCO is a member of the Evanston Anchor Institutions Project. Additionally, EHCO is currently providing funding through its Community Investment Fund to Connections for the Homeless and Lutheran Social Services of Illinois. Both of these organizations have extensive experience and expertise with homelessness and are located within the hospitals' service area. - Transportation - Transportation has been identified as a regional issue impacting economic development, as well as access to care. To mitigate the challenges, EHCO provides financial support to social service organizations, senior living facilities, townships and local villages to subsidize ride share as part of their existing resources. Additionally, parking, taxi or Lyft vouchers are provided for patients identified with transportation needs. - Dementia/Alzheimer's - EHCO recognizes that the magnitude of this health condition requires action and collaboration on numerous levels to have measurable outcomes. Besides the clinical and health impacts, Dementia/Alzheimer's also provides caregiver, financial, transportation and aging in place challenges. Therefore, NorthShore Research Institute actively engages in research and clinical trials for Dementia & Alzheimer's to benefit patients as well as the community at large impacted by this degenerative disease. - Community Violence - Endeavor is a member of the Northwell Collaborative Gun Violence Prevention Learning Collaborative. At a health system level, EHCO's Government Advocacy Department engages with elected officials on violence prevention issues. EHCO also provides funding through its Community Investment Fund to organizations such as TASC (treatment alternatives to street crime) and ONE Northside to address root causes of violence in the community. Within the hospitals, EHCO has trained Sexual Assault Nurse Examiners (SANE) to provide comprehensive care to sexual assault victims entering through the emergency departments, as well as an LCSW to provide resources to help victims. Additionally, EHCO employs a Pathways Program coordinator who provides crucial support for patients experiencing domestic violence, human trafficking, and sexual assault. - Adults Aging in Place - EHCO recognizes that the magnitude of this identified health need requires action and collaboration on numerous levels to have measurable outcomes. EHCO provides financial support to social service organizations, senior living facilities, townships and local villages to subsidize ride share as part of their existing resources. During 2024, EHCO took the following actions to address the key priority areas identified in the 2022 Community Health Needs Assessment (programs and services by hospital facility):
Schedule H, Part V, Section B, Line 11 Facility A, 2Facility A, 2 - A, 1 Evanston Hospital. During 2024, 1,066 students made 1,661 visits to the Health Center. Additionally, expanded therapy and medication management at ETHS, adding an LCSW and psychiatrist/fellow. In 2024, 78 students utilized mental health services at the on-site clinic, totaling 379 visits. Promoted mental health awareness through an online monthly webinar series for parents and teens. Additionally, partnered with Josselyn to train community members how to identify and respond to mental health emergencies through Mental Health First Aid Training. Also provided MHFA Instructor Training, including training instructors from underrepresented and under-resourced populations, providing wraparound support to enable them to train other community responders. Communities included Evanston, Skokie, Des Plaines, Waukegan, Libertyville, Vernon Hills, Glencoe and Chicago. Through a partnership with Erie Evanston/Skokie Health Center EHCO provided primary, mental and dental care services to under/uninsured patients, including 3,946 Erie Evanston/Skokie Health Center clients who received specialty care services at EHCO. SDOH screenings were conducted for 16,713 hospital inpatients with 2,636 screened as medium-to-high risk. EHCO hired nine Community Health Workers (CHWs) to advance community health priorities through patient navigation and community outreach/engagement. These CHWs have been supporting breast cancer outreach, social needs follow-up-including interpersonal safety-and broader efforts to address health gaps. Addressed through targeted interventions and outreach for high blood pressure and diabetes, focusing on populations with health gaps. Launched the Diabetes System Quality Collaborative and Phase II of the Hypertension System Quality Collaborative, adding two full-time employees to support this work. Achieved overall A1c control of 73% and overall BP control of 75%. Improved rates for breast, cervical and colorectal cancer screening through targeted outreach and the use of a data analysis tool to understand screening rate disparities. Closed the disparity gap in breast screening rates among lower median family income patients.

Schedule H, Part I, Line 6a Community benefit report prepared by related organizationA single community benefit report was prepared for Edward Hospital (36-3297173), Elmhurst Memorial Hospital (36-2167784), Naperville Psychiatric Ventures (d/b/a Linden Oaks Hospital) (36-3965251), Evanston Hospital, Skokie Hospital, Highland Park Hospital, Glenbrook Hospital (36-2167060), Northwest Community Hospital (36-2340313), and Swedish Hospital (36-2179813).
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceEndeavor Health Clinical Operations calculated the cost of charity care and means-tested government programs using the cost-to-charge ratio derived from Schedule H, Worksheet 2, Ratio of Patient Care Cost-to-Charges. Amounts in the other benefits section of Line 7 were calculated using costs charged directly to the individual programs from the financial accounting system. An indirect cost allocation factor for hospital shared services was also calculated and included for each program indicated in other benefits.
Schedule H, Part II Community Building ActivitiesEconomic Development: Endeavor Health supports economic development through its partnerships with Chambers of Commerce in Evanston, Skokie, Glenview, Northbrook, Deerfield and Lake County Partners. Endeavor Health team members serve on the Board of Directors or the Evanston, Skokie, Highland Park, Glenview and Northbrook Chambers. Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBad debt expense was reported at cost using a cost-to-charge methodology. The Schedule H, Worksheet 2, Ratio of Patient Care Cost-to-Charges calculated to report charity care on Part I, Line 7a was multiplied by the bad debt expense to determine the bad debt expense at cost. Discounts and payments on patient accounts are excluded from bad debt expense in order to reflect the expected amount to be paid.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThere is no bad debt expense included in community benefits.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteSee pages 9 and 14-18 of the audited financial statements for footnotes describing accounts receivable and bad debt expense.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsThe Medicare allowable costs were determined using the data from the Medicare Cost Report. Endeavor Health Clinical Operations (EHCO) believes that all of the Medicare shortfall should be considered as community benefit. The Internal Revenue Service Community Benefit Standard includes the provision of care to the elderly and Medicare patients. Medicare shortfalls must be absorbed by EHCO in order to continue treating the elderly in our community. The hospital provides care regardless of this shortfall and thereby relieves the government of the burden of paying the full cost of care for Medicare beneficiaries.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceEndeavor Health Clinical Operations (EHCO) is included in the billing and collections policy for Endeavor Health. The policy applies to all patients. Patients are screened for eligibility for financial assistance before collection procedures begin. Financial counseling will be provided to help patients identify available federal or state healthcare coverage programs which may be available to them, as well as to determine eligibility under the financial assistance policy. Collection activity will be placed on hold pending the outcome of these determinations. After a patient meets the qualifications for financial assistance, the account balance is either partially or entirely written off. Any remaining balance would be collected under the existing billing and collections policy.

Schedule H, Part VI, Line 2 Needs assessmentEndeavor Health screens adult patients in many care settings for social drivers of health (SDOH) to identify patients in need and connect them to resources. Collecting SDOH information deepens our understanding of the challenges patients face and guides navigation to critical services. EndeavorHealth screens for five key areas: food insecurity, housing instability, transportation barriers, interpersonal safety and utility difficulties. This data helps us understand patient needs and connect them to essential resources like food pantries. We have launched a Community Health Worker Program to address gaps in cancer screening rates, 30-day readmissions, and chronic disease outcomes, while also strengthening SDOH navigation and connections to community resources. Endeavor Health has a dedicated Community Impact and Engagement team made up of professionals across the system who are committed to strengthening our community's health and wellbeing. Strategic priorities and commitments include: - Community Health Needs Assessment - Identifying and addressing community health needs. - Community Engagement - Convening and connecting people for collective impact. - Community Health and Wellness - Promoting health and wellness through education and outreach. - Community Investment Fund - Investing in local organizations committed to community health and wellbeing. - Anchor Strategy - Leveraging organizational resources to support community health and economic growth. - Community Benefit - Demonstrating community impact. The hospitals also convene a semiannual Community Advisory Committees (CAC) at each hospital, to ensure accountability to the local communities. These committees advise hospital administration on services and initiatives from a community perspective and identify community resources to improve the overall health of families across the service area. The CACs are primarily comprised of community members, along with hospital administrative and professional staff. The hospitals strive for diverse membership to include business, faith community, social services, civic organizations, government and elected officials, as well as interested citizens. The CACs help to identify gaps in healthcare services within the community and seek opportunities for partnerships between the hospitals and community organizations. In addition, the CACs assist EHCO with the community health needs assessment process.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceEndeavor Health Clinical Operations (EHCO) informs and educates low-income patients of our Financial Assistance program across many points within the system. We have posted signs--both English and Spanish--in all our facilities' Emergency Departments and at our Central Registration areas as well as in the following languages upon request or on the EHCO website: Arabic, Assyrian, German, Japanese, Korean, Mongolian, Polish, Russian, and Vietnamese. The EHCO patient information television channel is available to all patients upon admission and a section describes how to obtain Financial Assistance. The EHCO billing statements describes the Financial Assistance program and necessary contact information. Our collection agencies and billing offices also coordinate charity care to potentially eligible patients. Finally, the EHCO website (endeavorhealth.org) is another useful resource that informs patients about the Financial Assistance program with links to the application process and the actual financial application. EHCO also has approximately 16 full-time financial counselors, 45 External Customer Service agents, and 8 internal agents representing our hospitals, clinics, and physician offices. These counselors and representatives are responsible for helping patients understand their bills and coordinating financial assistance and insurance eligibility when appropriate. Any uninsured patient admitted to our hospitals or those identified at registration will have their case reviewed by a financial counselor. EHCO has also earned Certified Application Counselor Organization (CACO) status with the Centers for Medicare and Medicaid Services (CMS). Our counselors are certified and capable of helping patients navigate Medicaid and Insurance Exchange options. ECHO partners with Great Lakes Medicaid with the goal to provide under insured and uninsured patients with insurance coverage through various Medicaid programs. The service is offered to patients receiving care as an inpatient, in the emergency department and in various outpatient departments, and is tailored to guide the patient through a complex application process for applicable federal, state, and community benefit programs. Any patient having difficulty paying their portion of the bill or wanting to know if they are eligible for financial assistance can have their case reviewed by calling our centralized Customer Service billing department, completing paper application or applying using NorthShore Connect patient portal. Our Customer Service department responds to approximately 1,400 calls per day from patients with questions about their bill or that are unable to pay. In addition to the formal application process for Financial Assistance, EHCO may also grant Financial Assistance on a presumptive basis. Presumptive Financial Assistance approvals are based upon individual life circumstances such as enrollment in low-income government programs with income eligibility below 200 percent of federal poverty level income guidelines or verification of financial indigence through a credit rating agency and their calculated healthcare credit score.

Schedule H, Part VI, Line 6 Affiliated health care systemEndeavor Health Clinical Operations (EHCO) is part of Endeavor Health. The core mission of Endeavor Health is to "help everyone in our communities be their best." Endeavor Health is a Chicagoland-based integrated health system driven by our mission to help everyone in our communities be their best. As Illinois' third-largest health system and third-largest medical group, we proudly serve an area of more than 4.2 million residents across seven northeast Illinois counties. Our more than 27,600 team members, including more than 1,700 employed physicians, are the heart of our organization, delivering seamless access to personalized, pioneering, world-class patient care across more than 300 ambulatory locations and nine hospitals, including eight Magnet-recognized acute care hospitals - Edward (Naperville), Elmhurst, Evanston, Glenbrook (Glenview), Highland Park, Northwest Community (Arlington Heights), Skokie and NorthShore (Chicago) and Linden Oaks Behavioral Health Hospital (Naperville). The Endeavor Health service area is composed of 171 ZIP codes across nearly 75 miles, with a total population of nearly 4.2 million. In 2024, EHCO conducted a comprehensive Community Health Needs Assessment (CHNA) to identify and prioritize key health issues of the surrounding communities. Endeavor Health conducted this CHNA as part of a simultaneous, system-wide approach to our four CHNAs across the Endeavor Health service area. The insights gained through this assessment guide the development of an implementation strategy to address priority concerns from 2025-2027. The 2024 CHNA serves as a tool toward reaching three basic goals: - To improve residents' health status, increase their life spans and elevate their overall quality of life. A healthy community is not only one where its residents suffer little from physical and mental illness, but also one where its residents enjoy a high quality of life. - To reduce the health disparities among residents. By gathering demographic information along with health status and behavior data, it will be possible to identify population segments that are most at-risk for various diseases and injuries. Intervention plans aimed at prioritizing these individuals may then be developed to address some of the socio-economic factors which have historically had a negative impact on residents' health. - To increase accessibility to preventive services for all community residents. More accessible preventive services will prove beneficial in accomplishing the first goal (improving health status, increasing life spans, and elevating the quality of life), as well as lowering the costs associated with caring for late-stage diseases resulting from a lack of preventive care.

ExplanationEndeavor Health Clinical Operations (EHCO) provides cash and non-cash assistance to organizations that help support EHCO's mission to preserve and improve human life. Requests to support an outside organization must be reviewed and approved by the Community Relations department and EHCO management to ensure the organization is aligned with EHCO's mission and charitable purpose. EHCO also provides cash grants to sub-recipients of grant awards. EHCO requires all sub-recipients to sign a consortium agreement stating that the sub-recipient understands and agrees to all applicable compliance requirements. NorthShore meets with the sub-recipients on a regular basis and reviews the sub-recipients' A-133 reports. In addition, EHCO provides cash support to related organizations. Support to related organizations is monitored through the EHCO budget and reporting procedures for the consolidated financial statements. EHCO also provides nursing scholarships to individuals. The scholarship amount depends on the degree sought, and the number of scholarships awarded depends on the available funds for each year. Scholarships are awarded on an annual basis to applicants that meet the eligibility criteria. Applicants must be current employees of EHCO as a staff nurse or other related clinical position. Applicants must also be currently enrolled in a nursing degree program, and enrollment must be maintained for the year the scholarship is received. In addition, applicants must sign a Scholarship Agreement to work for NorthShore as a staff nurse or in a related clinical position for one year following receipt of the scholarship. Finally, EHCO has an Employee Assistance Fund which helps employees who are experiencing economic hardship and are unable to afford housing, utilities, and other basic living needs due to a qualifying incident. Applicants must be current employees of EHCO working 20 hours or more per week and not at the Assistant Vice President level or above. The recipients are selected based on an objective determination of need by an independent Application Review Committee with guidelines for making distributions so that such distributions are made consistently across varying circumstances. The applicants must provide a narrative describing their circumstances and documentation to substantiate the amount of any payments.ExplanationTxt

Form 990, Part III, Line 4c PROGRAM SERVICE DESCRIPTION - RESEARCHThe Endeavor Health Research Institute (Research Institute) was organized in 1996 to provide a means for integrating leading-edge research into improved clinical care. The Research Institute also creates an environment to recruit and retain externally-funded research leadership in order to achieve the mission and goals of the organization. The Research Institute now houses more than 299 principal investigators who currently occupy 95,000 net square feet of research space and garner nearly $24 million in external research support, and in 2024, secured five new NIH awards. Our investigative teams work on more than 1,043 active research studies, of which 550 are clinical trials that include more than 2,280 participating patients. In 2024, EHCO's contribution to the internal support for the Research Institute exceeded $12 million. The Research Institute builds on EHCO's promise to deliver excellence in all aspects of patient care. Efforts are directed to research initiatives that make a difference across multiple disciplines. From oncology and neurology to orthopedics and cardiovascular care, the Research Institute focuses on areas of inquiry that have the potential to make the greatest impact on improving and saving lives across the communities we serve. EHCO offers more than 550 active clinical trials of various types, from innovative surgical procedures to the latest drug therapies. EHCO's clinical research portfolio focuses on a range of conditions, including cancer, neurologic disorders and stroke, cardiovascular disorders, and a wide range of pediatric and gynecologic conditions. Additionally, major clinical trial programs in advanced imaging and medical genetics are also offered. The Research Institute is also a member of the Illinois Precision Medicine Consortium (IPMC), which is part of a national landmark longitudinal All of Us Research Program (AoURP) cohort program to improve the ability to prevent and treat disease based on individual lifestyle, environment, and genetics. EHCO has enrolled over 2,280 EHCO patients in the study, and participants in EHCO's AoURP are being asked to share a wide-range of health, environmental, and lifestyle information. The Endeavor Health Program for Personalized Cancer Care (PPCC) continues to pioneer new strategies in cancer care. The PPCC uniquely focuses on the genetic pattern of an individual's hereditary DNA to derive a personalized cancer risk assessment profile. Based on the inherited risk of developing a given cancer, the PPCC is beginning to implement personalized cancer care strategies that encompass the entire spectrum of disease. The PPCC believes this approach will lead to more efficient use of health care resources by targeting prevention and screenings toward individuals at greater risk of developing cancer, earlier cancer detection, and, ultimately and most importantly, reduced cancer deaths and suffering. The Precision Medicine Program is expanding investigations to identify hereditary risks of other chronic diseases such as cardiovascular disease and diabetes. Identification and testing of polygenic risk scores to quantify disease risk has now accelerated efforts to integrate these discoveries into individualized patient care for disease prevention and/or improved patient outcomes.
