THE COMMUNITY HEALTH NEEDS ASSESSMENT WAS CONDUCTED TO INCLUDE ALL FOUR (4) FACILITIES WHICH COMPRISE MEMORIAL HEALTH SYSTEM OF EAST TEXAS: (1) MEMORIAL MEDICAL CENTER OF EAST TEXAS (LUFKIN), (2) MEMORIAL MEDICAL CENTER - LIVINGSTON, (3) MEMORIAL MEDICAL CENTER - SAN AUGUSTINE, AND (4) MEMORIAL SPECIALTY HOSPITAL.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Memorial Medical Center -Lufkin. Form 990, Schedule H, Part V, Section B - Community Health Needs Assessment - Line 11 Memorial Medical Center -Lufkin Implementation Plan Measurable Objectives and Implementation Activities A comprehensive Community Health Needs Assessment (CHNA) was conducted for Memorial Health System of East Texas (MHSET) from August to December of 2012. Memorial Health System of East Texas includes four hospital facilities: Memorial Medical Center - Lufkin, Memorial Medical Center -Livingston, Memorial Medical Center -San Augustine and Memorial Specialty Hospital. As a system, the study area was defined as Angelina, Polk and San Augustine Counties. The analysis included a careful review of the most current health data available and input from numerous community representatives with special knowledge of public health. Findings indicated that there were eight main needs in the communities served by MHSET. The system prioritized those eight needs and decided to address the top seven. MHSET decided not to specifically address the need for "increased access to affordable dental care," largely due to its position (last) on the prioritized list and the system's capacity to address that need. Dental services are not a core business function of the health system and the leadership team felt that resources and efforts would be better spent addressing the first seven prioritized needs. The final list of prioritized needs, in descending order, is listed below: 1. There is a need for prevention, education and early detection for heart disease, cerebrovascular disease, diabetes and cancer 2. The community needs increased access to affordable primary care 3. The community needs additional healthcare providers Primary care physicians, specialists, mental health providers, and physicians accepting Medicare and Medicaid HMO products 4. There is a need to address unhealthy lifestyles, such as smoking and obesity 5. There is a lack of a mental health and behavioral health continuum of care 6. There is a need to decrease health disparities by targeting specific populations, including: The high prevalence of diabetes among Native Americans in Polk County Low income, uninsured, underinsured and elderly A safe place for treatment for undocumented persons A culturally sensitive, language appropriate healthcare environment for minority populations 7. There is a need to increase access to transportation for healthcare services for those without access (such as elderly and low income residents)
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - Memorial Medical Center -Lufkin. Memorial Medical Center - Lufkin leadership has developed the following Implementation Plan to identify specific activities and services which directly address these seven priorities. The objectives were identified by studying the prioritized health needs, within the context of the hospital's overall strategic plan and the availability of finite resources. The Plan includes a rationale for each objective categorized under each health priority, followed by specific implementation activities. PRIORITY 1: There is a need for prevention, education and early detection for heart disease, cerebrovascular disease, diabetes and cancer. Objective 1: Memorial Medical Center - Lufkin will focus financial and staff resources on educating the community about diabetes prevalence, prevention and disease management. Rationale: As of 2009, diabetes is the seventh leading cause of death in Texas and the state also ranks 15th in country, with 1 being the highest prevalence and 51 being the lowest, in terms of diabetes among adult residents. Excluding 2005, self-reported diabetes prevalence rates have been higher in Region 4-5N than Texas and the United States since 2002. Education regarding diabetes and available resources will allow community members to better cope with or prevent diabetes. Implementation Activities: Memorial Medical Center - Lufkin will host the Diabetes Expo, which offers community members a comprehensive look at preventing and managing diabetes, in November during Diabetes Awareness Month. The hospital will work to provide a vendor fair for products and services, discounted glucose screenings, educational presentations by local physicians and cooking demonstrations on how to prepare healthy meals. Previously this event has been held biannually, but planning has begun to work toward an annual event beginning in 2013. Memorial Health System of East Texas will produce a monthly cooking show, Memorial Cooking Innovations, which is a 30 minute cooking show featuring a registered dietitian and a certified chef. The show features diabetic friendly foods that are delicious and easy to prepare and is broadcast on the local government access television channels daily and in 49 cities across the United States. Additionally, the broadcast is made available to view at any time online at www.memorialhealth.us.
Schedule H, Part V, Section B, Line 11 Facility , 3Facility , 3 - Memorial Medical Center -Lufkin. Diabetes classes will be offered at Memorial Medical Center - Lufkin. UPDATE: The Diabetes Expo was changed to an annual event beginning in 2013. The Cooking Show and the Diabetes Classes were offered as outlined above. The Memorial Cooking Show is now available in 62 cities across the country. Additionally, a monthly editorial accompanied by healthy cooking recipes are published in 12 East Texas newspapers. In addition, the Diabetes Education Center hired a Registered Dietitian and Certified Diabetes Educator to compliment the existing staff and provide education and nutrition education and counseling for our community and industry Health and Wellness team during events as well as marketer for speaking engagements for community and employer groups. Objective 2: Memorial Medical Center - Lufkin will organize and conduct a variety of cancer related events, such as campaigns and luncheons, to increase awareness and promote early cancer detection through appropriate screenings. Rationale: As of 2009, cancer is the second leading cause of death in Angelina County, Polk County, San Augustine County and Texas. There are much higher incidence rates of particular cancers, such as prostate cancer, in various counties in the study area than in Texas. Self-reported rates of Digital Rectal Exams at the appropriate times also suggest a potential disparity among residents in Region 4-5N compared to Texas. Increased awareness and opportunities for appropriate screenings will help community members improve their prognoses by detecting cancer in its earliest stages Implementation Activities: Memorial Medical Center - Lufkin will participate in the Know Your Stats Prostate Awareness Campaign. In September, the hospital hosts several men's breakfasts to educate men on the importance of prostate screenings. Local urologists participate to offer information on prostate cancer, signs and symptoms, risk factors and treatment methods. Prostate Specific Antigens are also offered at a discounted rate. Memorial Medical Center - Lufkin will organize and conduct its annual breast cancer awareness luncheon, Power of Pink. The hospital works with the American Cancer Society to supply vendors and sign women up to get mammograms. In 2012, the Power of Pink luncheon marked its 20th year milestone and reached more than 800 women in the Lufkin community. Memorial Medical Center - Lufkin will actively participate in Mammography Month. The hospital will run an educational media campaign on understanding the signs and symptoms of breast cancer and encourage women to get mammograms. A free gift is offered to those scheduling a mammogram at one of MHSET's facilities.
Schedule H, Part V, Section B, Line 11 Facility , 4Facility , 4 - Memorial Medical Center -Lufkin. Memorial Health System of East Texas' Cancer Committee will evaluate the possibility of offering lung cancer screenings at each of its facilities. UPDATE: The Prostate Awareness Campaign, Power of Pink, and educational campaigns on screening mammography have been completed. Imaging Services and the Cancer Center in Lufkin began offering lung cancer screening starting in October 2015 following the Medicare guidelines. In addition, an educator to provide cancer prevention, education and early detection information has been added to our community and industry Health & Wellness team during events. Objective 3: Memorial Medical Center - Lufkin will engage in collaborative initiatives to educate the community on a variety of health topics. Rationale: Many interviewees, which all had special knowledge of public health in Angelina, Polk and San Augustine Counties, indicated that education about resources in the community and the appropriate use of those resources are critical to improving the health of the community. It was also mentioned that education efforts should focus on prevention, education and early disease intervention. Providing an opportunity for community members to actively learn about important health topics will help increase appropriate healthcare utilization. Implementation Activities: Memorial Health System of East Texas will participate in the monthly Speakers' Bureau at each of its facilities. At this event, MHSET physicians speak to civic clubs on a variety of health care topics. Memorial Health System of East Texas will conduct the monthly Women's Power Lunch, which features local physicians speaking on pertinent health topics, at Memorial Medical Center-Lufkin. This event yields about 130 attendees per month in Lufkin. For the last three years Memorial Health System of East Texas has partnered with Methodist Hospital System in Houston to educate thousands of East Texans on the signs and symptoms of stroke. Memorial Medical Center - Lufkin will continue to collaborate on this program, which has expanded into the East Texas Heart and Vascular Initiative, where public education forums are held at industries, schools, assisted living centers, retirement centers, and civic clubs. Rationale: There is a high prevalence of both smoking and obesity in Region 4-5N, Texas and the United States. Data suggests that there are much higher rates of physical inactivity and smoking in Region 4-5N than in Texas or the United States, but obesity appears to be a critical health problem nationwide. More than 50 percent of survey respondents indicated that services for nutrition and weight management programs, as well as tobacco cessation programs are either inadequate or very inadequate in the community. Many interviewees also identified pressing lifestyle issues that need additional attention or promotion of available services. These topics included diabetes, obesity, smoking cessation, heart disease, parenting, fitness and substance abuse. By engaging in initiatives that promote healthy lifestyles, the hospital will contribute to improving healthy lifestyles in the community.

Objective 1: Memorial Medical Center- Lufkin will provide financial support and volunteers to organizations that either offer services or strive to address mental health and substance abuse concerns in the community. Rationale: Data suggests that an increasing percentage of residents in Region 4-5N report experiencing poor mental health days. More than 50 percent of survey respondents indicate that mental health screenings are either inadequate or very inadequate in the community and more than half of respondents also indicated that the needs of persons experiencing mental illness and persons with chemical dependency were not being adequately met. The interviews suggested a crucial need to address the gaps among the continuum of care for patients with mental or behavioral health issues, with substance abusers specifically at risk. By providing financial support and volunteers to organizations that work to meet these identified needs, the hospital can help address mental health and substance abuse concerns in the community. Implementation Activities: Memorial Medical Center -Lufkin will provide financial support and volunteers to organizations that offer prevention and treatment for alcohol and drug abuse, underage drinking, etc. Some of these organizations include: Alcohol Drug Abuse Council of Deep East Texas Alzheimer's Association Memorial Health System of East Texas works collaboratively with and provides funding to the Burke Center, which founded in 1974 and provides a variety of quality behavioral healthcare services to people with mental illness and intellectual and developmental disabilities through the provision of substance abuse prevention services. Veronica Sjolander works another job full time and is available after 5pm and Norma Sanford works another job full time and is available after 3pm. Each year Memorial sponsors fund raising events hosted by The Alcohol and Drug Abuse Council, including their annual golf tournament that raises funds for ADAC prevention projects. Additionally, Memorial offers financial and volunteer support annually to the Alzheimer's Association's Memory Walk. Objective 2: As a part of the larger health system, Memorial Medical Center -Lufkin will provide staff and leadership to organizations in the community that work to improve the continuum of care between mental health and behavioral health services. Rationale: Data suggests that there are various health disparities among specific populations in Region 4-5N. There are mental health disparities with regard to gender and income and physical inactivity disparities by education and income. There are also disparities among residents who cannot see a doctor because of medical cost. Women more than men, Black and Hispanic populations more than the White population, persons making less than $50,000 compared to those making more than $50,000 and those with no high school diploma compared to those with college degrees were more likely to report that they were unable to see a doctor because of medical cost. Interviewees noted over and over again that there was a specific need to reach targeted populations. Some mentioned the Native American population in Polk County, others mentioned the elderly. Nearly everyone spoke about the needs of under or uninsured residents in the community and many mentioned the needs of undocumented persons and other minority populations. The hospital can work to address these needs through collaboration with the larger system. Implementation Activities: Memorial Health System of East Texas, including Memorial Medical Center-Lufkin, will continue to be a presenting sponsor for the Senior Expo. Memorial Medical Center -Lufkin will engage in outreach in public schools, such as infection and behavior related activities.

Implementation Activities: Memorial Medical Center -Lufkin will assist with the annual Dr. Bill Shelton Totally Awesome Fishing Adventure. Part of the proceeds raised at this event will go to resources for indigent cancer patients. The pharmacy at Memorial Medical Center -Lufkin works with indigent cancer patients to obtain expensive chemotherapy needed for treatment. In 2012, the hospital has obtained about $42,000 in medications for 3 cancer patients. Part of the proceeds from Power of Pink goes to the Cancer Center for indigent care and transportation. UPDATE: The hospital continues to provide support to cancer patients who need assistance with cancer care. The hospital partnered with David Conciene starting in 2015 to organize the Fishing Adventure fundraising event. The health district offers obstetrics services for non-residents. PRIORITY 7: There is a need to increase access to transportation for healthcare services for those without access (such as elderly and low income residents). Objective 1: Memorial Medical Center - Lufkin will work to provide access to transportation and transportation vouchers to patients at the hospital. Rationale: Transportation was cited by multiple interviewees as a barrier to accessing health services. By providing access to transportation for those without access, Memorial Medical Center -Lufkin can increase access to healthcare for community members who would otherwise go without care. Implementation Activities: Memorial Medical Center - Lufkin will offer a shuttle service that transports patients from the Joe W. Elliott House to the hospital. The hospital also pays for the maintenance of that facility. Part of the proceeds that are raised through Power of Pink and the Fishing Tournament will go to provide gas vouchers for cancer treatment and prescription medication for qualified patients. Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE HOSPITAL USED THE COSTING METHODOLOGY DERIVED FROM IRS WORKSHEET 2 - "RATIO OF PATIENT CARE COST-TO-CHARGES" TO CALCULATE THE AMOUNTS SHOWN ON LINES 7(a) & 7 (b). IN ACCORDANCE WITH THE INSTRUCTIONS, COMMUNITY BENEFITS EXPENSED TOTALLING $152,336 INCLUDED IN PART IX, LINE 25, COLUMN (A) AND $1,092,257 IN MISCELLANEOUS INCOME INCLUDED IN PART VIII, LINE 12, COLUMN (A) WERE SUBTRACTED FROM EXPENSES REPORTED ON PART IX, LINE 25, COLUMN (A) IN ORDER TO DETERMINE ALLOWABLE COSTS TO BE USED IN THE CALCULATION OF THE RATIO OF COST-TO-CHARGES. BAD DEBTS EXPENSE IS NOT INCLUDED IN THE EXPENSES REPORTED ON PART IX, LINE 25 COLUMN (A),
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE HOSPITAL USED THE COSTING METHODOLOGY DERIVED FROM IRS WORKSHEET 2 - "RATIO OF PATIENT CARE COST-TO CHARGES" TO CALCULATE THE AMOUNT SHOWN ON LINE 2. THE HOSPITAL ACCOUNTS FOR DISCOUNTS AS DEDUCTIONS FROM GROSS PATIENT REVENUE AND WHERE APPLICABLE, PAYMENTS ON PATIENTS' ACCOUNTS AS DEDUCTIONS FROM UNCOLLECTIBLE ACCOUNTS (BAD DEBTS EXPENSE).
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyTHE AMOUNT SHOWN ON LINE 3 IS BASED ON THE HOSPITAL'S ESIMATE THAT 10% OF THE ACCOUNTS RECORDED AS BAD DEBT EXPENSE WOULD HAVE BEEN CLASSIFIED AS CHARITY (COMMUNITY BENEFIT) HAD THE APPROPRIATE INFORMATION BEEN OBTAINED. THIS ESTIMATE IS BASED PRIMARILY ON THE POPULATION OF ANGELINA COUNTY'S MEDIAN HOUSEHOLD INCOME ($41,354), THAT 19.3% OF COUNTY RESIDENTS HAD INCOMES BELOW THE FEDERAL POVERTY LEVEL, THAT 53.33% OF GROSS PATIENT REVENUE WAS FROM OUTPATIENTS. REASONS WHY ADEQUATE INFORMATION WAS NOT OBTAINED INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING: LANGUAGE BARRIERS WITH THE SPANISH SPEAKING POPULATION A PATIENT'S LACK OF UNDERSTANDING ABOUT THE PROCESS AND WHERE TO FIND THE INFORMATION REQUESTED SOME PATIENT'S ARE UNABLE OR UNWILLING TO SUPPLY SUPPORTING DOCUMENTATION LACK OF UNDERSTANDING BY THE PATIENT OF HOW A CHARITY CLASSIFICATION RATHER THAN A BAD DEBT CLASSIFICATION MAY BENEFIT THEM.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE HOSPITAL DOES NOT REPORT BAD DEBTS EXPENSES (UNCOLLECTIBLE ACCOUNTS) IN ACCORDANCE WITH HFMA STATEMENT NO. 15. THE HOSPITAL ACCOUNTS FOR ANY LOSSES ON ACCOUNTS RECEIVABLE USING THE ALLOWANCE METHOD. THE HOSPITAL IS A MEMBER OF A SYSTEM WITH CONSOLIDATED FINANCIAL STATEMENTS. THE AUDITED CONSOLIDATED FINANCIAL STATEMENTS FOOTNOTE TO NET PATIENT ACCOUNTS RECEIVABLE FOR THE YEAR ENDED JUNE 30, 2015, INCLUDES THE FOLLOWING: "THE PROVISION FOR BAD DEBTS IS BASED ON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS, TAKING INTO CONSIDERATION HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS." ALSO, SEE PAGE 9 OF THE ATTACHED CONSOLIDATED FINANCIAL STATEMENTS OF CATHOLIC HEALTH INITIATIVES FOR THE REMAINDER OF THE FOOTNOTE.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE HOSPITAL BELIEVES THAT THE SHORTFALL ON LINE 7 SHOULD BE TREATED AS COMMUNITY BENEFIT. IN THE HOSPITAL'S ANNUAL COMMUNITY BENEFIT REPORT FILLED WITH THE STATE OF TEXAS, THE UNREIMBURSED COST APPLICABLE TO MEDICARE PATIENTS IS A PART OF THE DETERMINATION THAT A HOSPITAL MEETS THE STATE'S REQUIREMENT THAT THE COST OF PROVIDING CHARITY CARE IS EQUAL TO AT LEAST 5% OF NET PATIENT SERVICE REVENUE. IRS REVENUE RULING 69-545, WHICH ESTABLISHED THE COMMUNITY BENEFIT STANDARD NON-PROFIT HOSPITALS, STATES THAT IF A HOSPITAL SERVICES PATIENTS WITH GOVERNMENTAL HEALTH BENEFITS, INCLUDING MEDICARE, THEN THIS IS AN INDICATION THAT THE HOSPITAL OPERATES TO PROMOTE THE HEALTH OF THE COMMUNITY. AS NO "COST REPORT" WAS PREPARED FOR THE YEAR ENDED JUNE 30, 2015 (A SHORT-YEAR REPORT WAS FILED FOR THE SIX MONTHS ENDED JUNE 30, 2015), THE AMOUNT OF THE SHORTFALL WAS DETERMINED BY MULTIPLYING THE GROSS REVENUE FROM MEDICARE PATIENTS BY THE HOSPITAL'S ESTIMATED CONTRACTUAL ADJUSTMENT PERCENTAGES TO DETERMINE THE ESTIMATED NET REVENUE AMOUNT. THE GROSS REVENUE FROM MEDICARE PATIENTS WAS THEN MULTIPLIED BY THE RATIO OF COST TO CHARGES PERCENTAGE TO DETERMINE THE ESTIMATED COST OF THE SERVICES PROVIDED.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceAS SOON AS SUFFICIENT INFORMATION IS AVAILABLE CONCERNING THE PATIENT'S FINANCIAL RESOURCES AND ELIGIBILITY FOR GOVERNMENT ASSISTANCE, A DETERMINATION IS MADE CONCERNING THE PATIENT'S ELIGIBILITY FOR CHARITY.
