SCHEDULE H, PART V, SECTION B, LINE 5PERSONS WHO REPRESENT THE COMMUNITY: A COMMITTEE WAS FORMED THAT COMPRISED INDIVIDUALS WHO ENCOMPASSED THE DIVERSITY OF STEPHENS COUNTY. THE HOSPITAL PARTICIPATED IN THE COMMUNITY HEALTH NEEDS ASSESSMENT CONDUCTED BY THE PATHWAYS TO A HEALTHY STEPHENS COUNTY COMMITTEE. THE INPUT CAME FROM SURVEYS, FOCUS GROUPS, AND COMMUNITY INFORMANT INTERVIEWS.
SCHEDULE H, PART V, SECTION B, LINE 7A&DWEBSITE WHERE CHNA CAN BE FOUND: http://duncanregional.com/community-health-needs-assessment-chna THE CHNA REPORT IS PRINTED AND PUT IN WAITING ROOMS AND OTHER PLACES IN THE HOSPITAL.
SCHEDULE H, PART V, SECTION B, LINE 11NEEDS IDENTIFIED IN CHNA: From the data and analysis, the county at large had four key areas of health concern: 1. Healthy living and the lack of exercise opportunities, fruit and vegetable consumption, increasing obesity rate, increasing cardiovascular disease, diabetes, sexual health and the dental health needs of the community. 2. Mental Health/Substance Abuse Services and the lack of trained, qualified, mental health/substance abuse programs and access in Stephens County. 3. Safety/Injury prevention education and opportunities for providing better shelter, safety and options during inclement weather, after tornado effects, safety at local schools and other needs of the community during adverse situations. 4. Prevention, education, and needs of the community around cancer and the need to decrease the number of preventable cancer deaths in our county. Pathways to a Healthy Stephens County discussed the four areas and selected two (2) main areas of focus for the next three (3) years. Pathways, as a whole, selected to concentrate on the two heaviest areas of concern reported by our citizens: (1) Healthy Living and (2) Mental Health/Substance Abuse. The Pathways group approved tackling these two areas instead of all four areas so the organizations and community could focus on the two greatest areas of concern. The plan has been to work on our key areas and as momentum increases and the community sees sustainable results, come back to the other two areas for re-evaluation within our county. The areas identified in the Community Health Needs Assessment that will not be tackled are (3) Safety/Injury prevention and (4) Cancer. While we feel both areas need to be worked on, the limitations available to us as a community and within Duncan Regional Hospital (DRH) limit the number of items we can effectively tackle at one time. DRH and the Pathways group both plan to work together in support of these two areas at a later date but cannot effectively manage these within the scope of what is available to us at this time. As part of the Pathways group, Duncan Regional Hospital has been extremely supportive of taking on the two key areas of citizen concern only. Duncan Regional Hospital has committed resources, both human and financial, meeting space, food, advertising, and numerous other items to the Pathways group in support of programs to improve the two key areas of focus. At this time, DRH is one of the main partners in the Pathways group and has two of our top executives serving as committee chairs over working task forces over both major areas of focus. Duncan Regional Hospital is committed to working with the Pathways group to help formalize, fund, and monitor results in both major areas for the improvement of health in Stephens County. Actions taken in fiscal year 2015 include the following: The hospital increased the number of certified health businesses, schools, etc. The goal was to increase the number from 5 to 15, and the hospital had 23 organizations certified healthy for 2015. The hospital implemented a county wide media campaign using the Pathways website to launch a "Veg Up" campaign site. The development of the Farmer's market in Stephens County was completed. The hospital partnered with multiple organizations to promote and sponsor family friendly activities that promote physical activity. A website was set up to have an activity to help increase the awareness of opportunities for families to be active. The hospital partnered with the Oklahoma State Department of Health and the Stephens County Health Department for a Wellness in All Policies program. The hospital developed a Stephens County Mental Health Task Force to increase community involvement. A community based referral system was implemented. The hospital partnered with the Cancer Center of Southwest Oklahoma for a media campaign.

SCHEDULE H, PART III, SECTION A, LINE 4BAD DEBT FOOTNOTE: ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN EVALUATING THE COLLECTIBILITY OF ACCOUNTS RECEIVABLE, THE HOSPITAL ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYER SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR UNCOLLECTIBLE ACCOUNTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYER SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE HOSPITAL ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR UNCOLLECTIBLE ACCOUNTS, IF NECESSARY (FOR EXAMPLE, FOR EXPECTED UNCOLLECTIBLE DEDUCTIBLES AND COPAYMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYER HAS NOT YET PAID, OR FOR PAYERS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY). FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS (WHICH INCLUDE BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLE AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL), THE HOSPITAL RECORDS A SIGNIFICANT PROVISION FOR UNCOLLECTIBLE ACCOUNTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE STANDARD RATES (OR THE DISCOUNTED RATES IF NEGOTIATED OR PROVIDED BY POLICY) AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN 2015, THE HOSPITAL'S ALLOWANCE FOR DOUBTFUL ACCOUNTS INCREASED BY APPROXIMATELY $759,000 OR 5.8% PRIMARILY DUE TO AN INCREASE IN OVERALL GROSS PATIENT ACCOUNTS RECEIVABLE OF APPROXIMATELY $6,730,000 OR 14.5% AT JUNE 30, 2015, AS COMPARED TO JUNE 30, 2014.
SCHEDULE H, PART III, SECTION B, LINE 8COSTING METHODOLOGY: THE ORGANIZATION UTILIZES MEDICARE COST REPORT METHODOLOGY, WHICH APPORTIONS ROUTINE COSTS (ROOM AND BOARD) BASED UPON MEDICAID OR MEDICARE DAYS TO TOTAL DAYS, AND THE ANCILLARY PORTION IS APPORTIONED BASED UPON PROGRAM CHARGES TO TOTAL CHARGES. THE ORGANIZATION HAD A MEDICARE SHORTFALL OF $903,634. THE HOSPITAL TREATS MEDICARE SHORTFALLS AS A COMMUNITY BENEFIT.
SCHEDULE H, PART III, SECTION B, LINE 9BCOLLECTION POLICY: IN SUPPORT OF ITS MISSION, THE HOSPITAL VOLUNTARILY PROVIDES FREE CARE TO PATIENTS WHO LACK FINANCIAL RESOURCES AND ARE DEEMED MEDICALLY INDIGENT IN ACCORDANCE WITH THE HOSPITAL'S WRITTEN CHARITY CARE CRITERIA. NO FURTHER COLLECTION EFFORTS ARE MADE.
SCHEDULE H, PART VI, LINE 2NEEDS ASSESSMENT: THE HOSPITAL ASSESSES COMMUNITY NEEDS ON AN ONGOING BASIS, THROUGH INVOLVEMENT IN COMMUNITY PROGRAMS AND SURVEYS WITH OUR PHYSICIANS.
