DescMilford Regional Medical Center serves the healthcare needs of the residents of 19 communities in Central Massachusetts, a population now approaching 250,000. Guided by its mission "to provide quality healthcare services to our community with dignity, compassion and respect," MRMC strives to be responsive to diverse constituencies - individuals and families of all ages, employees and employers of large and small businesses, physicians and other healthcare providers, our school systems, our local and area officials and safety personnel, churches, community organizations, our insurers and payers and our own employees and volunteers. As a nonprofit hospital, we acknowledge the needs of the underserved, especially low-income and minority populations. A full-service community and regional teaching hospital, Milford Regional is a 145-bed, acute-care facility. There are approximately 200 physicians on the active medical staff and an additional 100 courtesy or consulting physicians. MRMC is an affiliate of UMASS Memorial as a teaching hospital. It is also affiliated with Children's Hospital, Dana Faber Cancer Institute, Brigham and Woman's Hospital, and Brigham and Women's Physician Organization. In addition to providing basic acute care services (medical, surgical, emergency medicine, family-centered obstetrics and pediatrics), MRMC has eight state-of-the-art operating suites, consolidated surgical services (including admitting and pre-admission testing) and a medical/surgical floor many with advanced patient monitoring capabilities. The Medical Center treated 5,265 medical and surgical inpatients in 2015. In 2015 Milford Regional built a 78,000 square foot facility doubling the size of the current ED, increasing the number of rooms in the ICU and creating all private patient rooms where currently up to four patients currently share a room. The $54 million new facility opening was planned for October 2015. In Northbridge, a brand new 24,000 square foot facility called Milford Regional in the Blackstone Valley opened in October 2014. Milford Regional and its physician practice group, Tri-County Medical Associates, share space inside this building which houses family medicine and OB/GYN along with Milford Regional rehabilitation, laboratory and radiology services. Our physician base also grew as Tri-County Medical Associates, welcomed 24 new physicians to our active medical staff. Family Medical Associates in Medway became Tri-County's first practice to receive Patient Center Medical Home (PCMH) certification. Four more practices are preparing for certification this coming year. The Milford Regional Emergency Department (ED) saw 55,570 patients in 2015. The Medical Center was awarded with a three year grant to support a bilingual Community Health Worker to direct patients without a primary care provider to the Edward M. Kennedy Community Health Center in Milford. 2,009 patients were referred to the health center in 2015. In addition, a six month grant from MA EOHHS supports a multidisciplinary team of providers to expand behavioral health care in the ED. Milford Regional was nationally recognized by HIMSS Analytics for achieving stage 6 on the Electronic Medical Records Adoption Model. Seven of our maternity nurses received the International Board Certified Lactation Care Award. In June 2015 we received the American Heart Association /American Stroke Association's Get with the Guidelines- Stroke Gold Plus Qualify Achievement Award. We were also awarded $1.3 million for CHART Phase 2 to decrease hospital readmissions over the next two years. All of these advancements at Milford Regional in 2015 and the high standard of patient care the hospital sets for itself resulted in local, regional and national recognitions. Milford Regional was chosen for the seventh straight year as the region's choice for healthcare and the number one choice locally by the annual Community Newspapers' survey. In addition, Milford Regional earned a patient safety grade of A from the Leapfrog Group in the spring and fall of 2015.Desc

Schedule H, Part V, Section B, Line 5-Milford Regional Medical Center IncMilford Regional Medical Center completed our Community Health Needs Assessment (CHA) in September 2015. The process was led by Community Benefits Steering Committee with support from HRiA and under the leadership of MRMC CEO Frank Saba. The CHA included quantitative data analysis, a community survey with over 1000 respondents and interviews with key stakeholders. Interviewees represented health care, public safety, government, education and the clergy. A Strategic Implementation Plan for FY 2014-2016 developed by the steering committee was reviewed and evaluated addressing goals and objectives of the priority areas addressing behavioral health, healthy weight for youth, access to care, and chronic disease prevention. Community Benefits Steering Committee has expanded to include community leaders outside the hospital walls who are instrumental in furthering goals and increasing visibility of hospital mission and mandate. With community partnerships, support from hospital departments, and grant funding through the Executive Office of Health and Human Services and the Metrowest Health Foundation, Milford Regional has addressed many of these issues in a substantive way. Milford Regional plays a leadership role in convening groups to improve health outcomes and works closely with the other agencies and institutions including Metrowest Health Foundation, MA Department of Public Health, Health Care for All, MA Executive Office of Health and Human Services, local elected officials, and coordinating 70 partner organizations as the chair of CHNA 6. Given the scope of issues identified in the assessment, this work will be ongoing.
Schedule H, Part V, Section B, Line 11-Milford Regional Medical Center IncThe Community Needs Assessment was presented to community leaders, CEOs, School Superintendents and Boards of Health Directors at a Leadership Roundtable hosted by Milford Regional. The focus was on Access to Care with a panel representing primary care, behavioral health care and specialty care.

Prepared and distributed printed Community Benefits Annual Report and made available on the hospital website. In addition, the Annual Report filed with the Massachusetts Attorney General is published online and available to the public.
Schedule H, Part I, Line 7Lines 7 a/b/c were calculated using worksheets 1,2,3 provided in the instructions.
Schedule H, Part I, Line 7, Column fLine 7F was determined by the cost and reimbursement information on the Medicare Cost Report. It was calculated by dividing Column E, Net Community Benefit Expense by total of Part IX Line 25 of Form 990 Core Form of $192,231,159.

Schedule H, Part III, Section B, Line 5Milford Regional Medical Center completed our Community Health Needs Assessment (CHA) in September 2015. The process was led by Community Benefits Steering Committee with support from HRiA and under the leadership of MRMC CEO Frank Saba. Given the scope of issues identified in the assessment, this work will be ongoing.
Schedule H, Part III, Section B, Line 8The Hospital's Medicare Cost Report was used to determine the amount reported in Line 6.
Schedule H, Part III, Section C, Line 9bOnce it has been determined that a patient qualifies for full or partial free care, all collection procedures cease and the full balance, or amount that qualifies under partial free care, is written off. Massachusetts hospitals are required to adopt a credit and collections policy under the Health Safety Net (HSN) regulations (114.6 CMR 13.08 (1) (c)) that outlines the specific process for determining eligibility for public assistance programs and hospital collection practices.
Schedule H, Part VI, Line 2Milford Regional stays current with studies by other agencies, particularly the Metrowest Adolescent Health Survey conducted by the MetroWest Health Foundation and state and local data. The Office of Community Benefits is actively engaged with the Community Health Network Area (CHNA) and other community coalitions/committees focusing on substance abuse, access to services, interpersonal violence prevention, cancer prevention and nutrition /physical activity/wellness to address emerging health needs.
Schedule H, Part VI, Line 3Patients are informed of the availability of financial assistance by way of payment statements printed with financial assistance information. At the time of discharge from the Emergency department, uninsured patients are encouraged to meet with an on-site Financial Counselor. If unable to meet with a Financial Counselor at the time of discharge, the patients are given a screening letter that gives contact information for our financial assistance department. The day following any emergency room visit, a form is generated on each uninsured patient and sent to the Financial Counselors. At that time, the Financial Counselors contact the patient to coordinate a meeting to apply for assistance. Scheduled self-pay Surgical Day Care (SDC) and inpatients are reviewed prior to admission to determine payment arrangements or to coordinate a meeting with a Financial Counselor. Insured SDC and inpatients who are unable to leave a deposit or make payment arrangements toward a large out of pocket expense are also encouraged to meet with a Financial Counselor.
