| Desc | Owensboro Health, Inc. ("OH") is the parent organization of a diversified system of health care organizations that provides a broad range of inpatient and outpatient services and other complementary health care services. OH employs healthcare professionals, including physicians, nurses, advance practice registered nurses, physician assistants and others licensed professionals who provide direct patient care in the organization's main hospital, Owensboro Health Regional Hospital. In addition, the organization operates several wholly owned and controlled subsidiaries, including Owensboro Health Medical Group, Inc., Foundation for Health, Inc., One Health Network, LLC, One Health Solutions, LLC, and The Health Network of Western Kentucky, LLC. OH's primary activity consists of providing medical and patient care services in its main hospital, Owensboro Health Regional Hospital, serving 11 counties in western Kentucky and southern Indiana. Our medical services are delivered by highly skilled physicians along with a caring and compassionate nursing staff. OH supports our care teams with state-of-the art equipment to provide our patients with advanced medical treatments and procedures. In the fiscal year ended May 31, 2015, OH had total admissions of 16,914, patient days of 81,252, and total ER visits of 68,718. Cost of Participating in Government Programs: OH is committed to serving all persons in need, regardless of race, creed, sex, nationality, religion, disability, age or ability to pay. To promote access to care, OH participates in the following public health programs: Medicaid, Medicare, Tricare and local health departments. In general, payments from these programs frequently do not cover the costs OH incurs to serve program beneficiaries. Uncompensated Care and Financial Assistance: OH provides medical care without charge, or at reduced cost, to residents of the communities that it serves. OH's Financial Assistance Program was established to assist patients who do not qualify for medical assistance programs, like Medicaid, and whose annual incomes are at or below certain percentages of the Federal Poverty Guidelines. During the reporting period, OH provided $3,581,125 in financial assistance to low-income and/or uninsured patients. OH does not include in that amount $23,271,246 of bad debt expense, which are amounts written off for providing services to persons who may be able, but are unwilling, to pay for the services they receive. As described elsewhere, OH believes a portion of its bad debt expense derives from patients who might have qualified for financial assistance had they submitted assistance applications. OH is committed to expanding its programs to improve access to health care in its primary and secondary service areas, which include rural and economically depressed areas and areas that lack adequate numbers of primary care and specialty providers. | Desc |

| SCHEDULE C, PART II-B, LINE 1I | POLITICAL CAMPAIGN AND LOBBYING ACTIVITIES PERCENTAGE OF LOBBYING ACTIVITIES FROM KENTUCKY HOSPITAL ASSOCIATION DUES. |
| Schedule C, Part II-B, Line 1G | IRS INSUBSTANTIAL LOBBYING within THE CONTEXT OF GOVERNMENTAL, COMMUNITY AND LEGISLATIVE AFFAIRS, OH HAS ONE EMPLOYEE THAT ENGAGES IN LOBBYING ACTIVITIES OR ATTEMPTS TO INFLUENCE LEGISLATION. HOWEVER, UNDER NO CIRCUMSTANCES IS THERE ANY ENGAGEMENT IN POLITICAL ACTIVITIES. LOBBYING ACTIVITIES INCLUDE BOTH DIRECT LOBBYING AND GRASS ROOTS LOBBYING. FROM A DIRECT LOBBYING PERSPECTIVE, THE EXECUTIVE DIRECTOR OF GOVERNMENTAL, COMMUNITY AND LEGISLATIVE AFFAIRS ENGAGES IN LOBBYING ACTIVITIES AT THE FEDERAL, STATE AND LOCAL LEVELS. THE EXECUTIVE DIRECTOR DOES MEET WITH MEMBERS OF CONGRESS ON OCCASION DURING THE YEAR EITHER IN WASHINGTON OR IN OWENSBORO. AT THE STATE LEVEL, THE EXECUTIVE DIRECTOR IS REGISTERED AS A LEGISLATIVE AGENT WITH THE KENTUCKY GENERAL ASSEMBLY. LOBBYING EFFORTS ARE GENERALLY LIMITED TO THAT PERIOD OF TIME IN WHICH THE GENERAL ASSEMBLY IS IN SESSION. THIS PERIOD INCLUDES A 30-DAY LEGISLATIVE SESSION IN ODD NUMBERED YEARS AND A 60-DAY LEGISLATIVE SESSION IN EVEN NUMBERed YEARS. IN ADDITION, LOBBYING AT THE LOCAL LEVEL IS GENERALLY CONFINED TO REGULATORY MATTERS AND IS NOT ONGOING. FROM A GRASS ROOTS LOBBYING PERSPECTIVE, THE EXECUTIVE DIRECTOR OVERSEES THE HEALTH IN ACTION NETWORK. HEALTH IN ACTION IS AN ELECTRONIC EMAIL SYSTEM THAT PROVIDES OH EMPLOYEES WITH UPDATES ON LEGISLATION AND 'CALLS TO ACTION' WHEN APPROPRIATE. JOINING THE NETWORK AND CHOOSING TO RESPOND ARE VOLUNTARY. THIS NETWORK IS ONLY ACTIVATED WHEN ISSUES OF CONCERN ARE BEING CONSIDERED AT THE FEDERAL AND STATE LEVELS. IT IS ESTIMATED THAT DURING THE FISCAL YEAR ENDING MAY 31, 2015, ALL LOBBYING ACTIVITY BY THE EXECUTIVE DIRECTOR DID NOT EXCEED 30% OF TOTAL WORK-RELATED DUTIES AND RESPONSIBILITIES. |

| FORM 990, SCHEDULE H, PART V, SECTION B, Line 5 | CHNA community Input OWENSBORO HEALTH COLLABORATED WITH PUBLIC HEALTH DURING THE ENTIRE CHNA intake PROCESS. IN 2011 THE GREEN RIVER DISTRICT HEALTH DEPARTMENT (GRDHD), WHICH INCLUDES THE COUNTIES OF DAVIESS, HANCOCK, HENDERSON, MCLEAN, OHIO, UNION, AND WEBSTER, BEGAN A COMMUNITY-WIDE PROCESS TO ANALYZE COMMUNITY HEALTH NEEDS AND IDENTIFY THE HEALTH PRIORITIES FOR THE REGION. THE COMMUNITY HEALTH ASSESSMENT, IS A FEDERAL REQUISITE FOR OBTAINING PUBLIC HEALTH ACCREDITATION. GRDHD UTILIZED THE STRATEGIC PLANNING FRAMEWORK KNOWN AS MOBILIZING FOR ACTION THROUGH PLANNING AND PARTNERSHIPS, OR MAPP, DEVELOPED BY THE NATIONAL ASSOCIATION OF COUNTY & CITY HEALTH OFFICIALS (NACCHO). FOR ADDITIONAL INFORMATION GO TO HTTP://WWW.HEALTHDEPARTMENT.ORG/ THE ASSESSMENTS AND INFORMATION COLLECTED BY THE GRDHD INCLUDED A SECONDARY DATA ASSESSMENT, AN ON-LINE AND IN-PERSON SURVEY, AND COMMUNITY FORUMS TO OBTAIN INFORMATION AND COLLABORATE WITH STAKEHOLDERS, ALL OF WHICH INFORMED THE IDENTIFICATION OF KEY COMMUNITY HEALTH ISSUES. A COMMUNITY PERCEPTION SURVEY ASSESSMENT WAS MADE AVAILABLE ONLINE AND IN-PERSON AT EACH OF THE SEVEN HEALTH CLINICS THROUGHOUT THE REGION. APPROXIMATELY 500 PEOPLE COMPLETED THE SURVEY, AND THOSE INVOLVED WITH THE DATA COLLECTION AND ANALYSIS SAID THAT THE SURVEY TARGETED THE UNDERSERVED POPULATION. ADDITIONALLY, THE GRDHD FACILITATED TWO COMMUNITY FORUMS HELD IN THE FIRST HALF OF 2012 TO ENGAGE WITH COMMUNITY STAKEHOLDERS AND COLLECT THEIR INPUT. THE COMMUNITY FORUMS WERE ATTENDED BY BETWEEN 75 AND 100 COMMUNITY MEMBERS REPRESENTING A DIVERSE GROUP OF NON-GOVERNMENTAL ORGANIZATIONS, BUSINESSES, HEALTHCARE INSURERS AND PROVIDERS, AND GOVERNMENT AGENCIES. THE FIRST FORUM, the community health forum, WAS DESIGNED TO IDENTIFY HEALTH NEEDS AS WELL AS IDENTIFY ALL RESOURCES CURRENTLY AVAILABLE TO MEET THOSE NEEDS. PARTICIPANTS WERE ASKED, "WHAT DO YOU THINK ARE THE PROBLEMS IN DAVIESS COUNTY?" AT THE SECOND COMMUNITY FORUM, STAKEHOLDERS WERE PRESENTED COMPLETE RESULTS OF THE SECONDARY DATA ANALYSIS, THE COMMUNITY SURVEY, AND THE STAKEHOLDER INPUT FROM THE COMMUNITY HEALTH FORUM. THEY WERE ASKED TO DISCUSS AND PRIORITIZE BY DOT METHOD THE GREATEST COMMUNITY HEALTH ISSUES BASED ON THE ANALYSIS, COMMUNITY PERCEPTION SURVEY, AND STAKEHOLDER INPUT. BARRON ENT., INC. FOLLOWING THIS PROCESS, OWENSBORO HEALTH ENGAGED THE HEALTHY COMMUNITIES INSTITUTE TO CONDUCT A MORE DETAILED DATA ANALYSIS. BOTH PRIMARY AND SECONDARY DATA WAS USED IN THIS ANALYSIS. THE SECONDARY DATA WAS DERIVED FROM OWENSBORO HEALTH'S HCI-CHNA SYSTEM, HTTP://WWW.OWENSBOROHEALTH.ORG/COMMUNITY-WELLNESS/HEALTH-NEEDS-ASSESSMENT/ THE SYSTEM INCLUDES THE MOST UP-TO-DATE PUBLICLY AVAILABLE DATA FOR APPROXIMATELY 140 COMMUNITY INDICATORS FROM OVER 20 SOURCES, COVERING OVER 20 TOPICS IN THE AREAS OF HEALTH, DETERMINANTS OF HEALTH, AND QUALITY OF LIFE. ADDITIONAL PRIMARY DATA CONSISTED OF IN-DEPTH PHONE INTERVIEWS WITH NINE COMMUNITY INFORMANTS INCLUDING A PUBLIC HEALTH EXPERT, PHYSICIANs and PUBLIC SERVANTs. interviews with HEALTHCARE AND SOCIAL SERVICE PROVIDERS WITH EXPERTISE AND SPECIAL KNOWLEDGE REGARDING THE NEEDS IN THE COMMUNITY Were SOUGHT ONCE THE COMMUNITY PRIORITY AREAS WERE NAMED IN AN EFFORT TO PROVIDE AN EVEN DEEPER UNDERSTANDING OF THE HEALTH NEEDS IN THE COMMUNITY. THE ADDITIONAL DATA COLLECTION REVEALED ADDITIONAL COMMUNITY NEEDS FROM THE ANALYSIS THAT WHILE NOT SELECTED AS PRIORITY HEALTH NEEDS WERE SO CLOSELY INTERREALTED AND INFLUENTIAL TO THE NAMED PRIORITY AREAS THOSE NEEDS WERE OFFERED AS ADDITIONAL AREAS OF CONCERN. |

| FORM 990, SCHEDULE H, PART V, SECTION B, Line 11 | How the hospital is Addressing Significant needs IN ADDITION TO THE CHNA, AND IN ACCORDANCE WITH REQUIREMENTS CONTAINED IN THE AFFORDABLE CARE ACT, OWENSBORO HEALTH ALSO ADOPTED AN IMPLEMENTATION PLAN WHICH DESCRIBES HOW OWENSBORO HEALTH PLANS TO ADDRESS THE IDENTIFIED HEALTH NEEDs CONTAINED WITHIN THE CHNA. SUBSEQUENTLY, OH FORMED AN INTERNAL IMPLEMENTATION STRATEGY TEAM COMPRISED OF STAFF MEMBERS FROM ACROSS THE ORGANIZATION INCLUDING REPRESENTATIVES OF: ADMINISTRATION; CANCER SERVICES; CARDIAC CARE; RESPIRATORY CARE; PHYSICIAN RECRUITING; COMMUNITY HEALTH AND WELLNESS; BEHAVIORAL HEALTH; NURSING; QUALITY; INFORMATION SYSTEMS; MEDICAL STAFF; FINANCE; STRATEGIC PLANNING; THE OH HEALTHPARK; GOVERNMENTAL, COMMUNITY AND LEGISLATIVE AFFAIRS; AND, COMMUNITY BENEFIT AND OUTREACH. Implementation Strategy Purposes: STRATEGY ONE - IDENTIFICATION AND INVENTORY OF EXISTING ACTIVITIES AND PROGRAMS, ADDRESSING TARGETED INDICATORS STRATEGY TWO - IDENTIFICATION OF GAPS IN ACTIVITIES AND PROGRAMS ADDRESSING TARGETED INDICATORS STRATEGY THREE - DEVELOPMENT OF A WRITTEN ACTION PLAN FOR ADDRESSING TARGETED INDICATORS STRATEGY FOUR - ONGOING EVALUATION, DEVELOPMENT AND REFINEMENT THE OWENSBORO HEALTH TAX YEAR 2014 IMPLEMENTATION STRATEGY UPDATE, AS MANDATED IN THE AFFORDABLE CARE ACT, REPRESENTS HOW THE OWENSBORO HEALTH REGIONAL HOSPITAL ALONG WITH ITS SYSTEM ENTITIES AND FACILITIES ARE WORKING COLLECTIVELY TO ADDRESS THE NAMED PRIORITY HEALTH INDICATORS. THE INTENT WITH OUR FIRST IMPLEMENTATION PLAN UNDER STRATEGIES ONE, TWO AND THREE, WAS TO OUTLINE A PLAN FROM WHICH ADDITIONAL PLANNING AND DECISION MAKING COULD TAKE PLACE ACROSS THE ORGANIZATION. A SIGNIFICANT COMPONENT OF THE PLAN WAS TO PINPOINT EXISTING ACTIVITIES THAT ADDRESSED TARGETED INDICATORS IDENTIFIED IN THE OWENSBORO HEALTH CHNA that was APPROVED BY THE OWENSBORO HEALTH BOARD OF DIRECTORS ON JANUARY 28, 2013. A PROJECT TO IDENTIFY ACTIONS AND ACTIVITIES ACROSS THE ORGANIZATION SPECIFICALLY TARGETED TO CHNA INDICATORS HAD NEVER BEEN UNDERTAKEN. OWENSBORO HEALTH COMMUNITY RELATIONS STAFF UTILIZED NUMEROUS SOURCES OF INFORMATION INCLUDING BUT NOT LIMITED TO INTERVIEWS, SURVEYS AND OWENSBORO HEALTH'S INTERNAL MECHANISM FOR TRACKING COMMUNITY BENEFIT. WHILE WE UTILIZE CB TRACKER, A SOFTWARE SYSTEM TO TRACK COMMUNITY BENEFIT ACTIVITIES FOR OWENSBORO HEALTH, THE UNDERSTANDING OF WHAT CONSTITUTES COMMUNITY BENEFIT REMAINS AN ONGOING EDUCATIONAL CHALLENGE FOR THE 3500 MEMBERS OF OUR ORGANIZATION. NUMEROUS STRATEGIES THAT ADDRESS COMMUNITY HEALTH NEEDS HAVE BEEN DEVELOPED AND IMPLEMENTED OVER THE YEARS. HOWEVER, MANY LEADERS IN THE ORGANIZATION ENGAGED IN THESE ACTIVITIES HAVING NEVER CONSIDERED THESE COMMUNITY HEALTH OUTREACH EFFORTS AS BEING CLASSIFIED UNDER THE TERM "COMMUNITY BENEFIT" AS IT RELATES IRS 990 REPORTING. THEREFORE NOT ONLY DOES THIS RESULT IN CONSERVATIVE REPORTING, IT DOES NOT ALLOW FOR A FULL PICTURE OF WORK BEING DONE. THEREFORE, ASCERTAINING THE VOLUME OF ACTIVITIES ADDRESSING CERTAIN INDICATORS UNDER THE FRAMEWORK OF AN ADOPTED CHNA AND DEFINED IMPLEMENTATION PLAN SPREAD ACROSS THE ORGANIZATION HAS PROVEN TO BE AN ENLIGHTENING, CHALLENGING AND BENEFICIAL EXERCISE. THE EXAMINATION AND DEVELOPMENT OF STRATEGIC PLANNING EFFORTS TO ADDRESS COMMUNITY HEALTH NEEDS BOTH WITHIN OWENSBORO HEALTH AND IN COLLABORATION WITH THE ENTIRE SYSTEM, HAS PROVEN TO BE ONE OF THE MOST VALUABLE ASSETS OF THE CHNA PROCESS. THE compilation of information INCLUDED IN THIS UPDATE REFLECTS MANY OF THE HIGHLIGHTED PROGRAMS AND STRATEGIES OWENSBORO HEALTH IS USING TO ADDRESS THOSE NEEDS. THIS Information HOWEVER, IS A WORK IN PROGRESS WITH MUCH WORK YET TO BE DONE. MORE SPECIFICALLY, DECISION MAKING AND IMPLEMENTATION PROCESSES ARE TO BE IDENTIFIED, AND GAPS IDENTIFIED, WHEREBY FUNDS CAN BE ALLOCATED AND OTHER RESOURCES, POLICIES AND EFFORTS CAN BE EXAMINED. WITH ONGOING EFFORTS TOWARDS FINALIZATION OF AN INVENTORY, OWENSBORO WILL MOVE NEXT TOWARDS MAPPING OUT AGREED UPON STRATEGIES UNDER EACH PRIORITY AREA, TAKING INTO CONSIDERATION CHOSEN METRICS AND DESIRED MEASURABLE OUTCOMES. THIS REVIEW AND EXAMINATION WILL BE APPROVED BY SENIOR LEADERSHIP AND THE OWENSBORO HEALTH BOARD OF DIRECTORS. STRATEGY FOUR, ONGOING EVALUATION, DEVELOPMENT AND REFINEMENT WILL TAKE PLACE FOLLOWING PLAN FINALIZATION AND APPROVAL. IMPLEMENTATION PLAN 3 YEAR HIGHLIGHTS AND UPDATES (REVISED FOR TAX YEAR ENDING MAY 31, 2015): - THE OWENSBORO HEALTH GIVES COMMUNITY BENEFIT GRANT PROGRAM WAS CHANGED TO ONLY CONSIDER PROPOSALS WHICH SOUGHT TO IMPACT THE NAMED PRIORITY HEALTH NEEDS. - IN FY15, Owensboro health GRANT SUPPORT AND IN PARTNERSHIP WITH COMMUNITY ORGANIZATIONS, OVER $700,000 IN GRANT FUNDS WERE PROVIDED TO IMPACT PRIORITY HEALTH NEEDS. - AS A RESULT OF THE CHNA, NEW PARTNERSHIPS RESULTING IN NEW PROGRAMS AND INITIATIVES TO ADDRESS PRIORITY HEALTH NEEDS HAVE BEEN FORMED. THESE ORGANIZATIONS HAVE ALSO BEEN ENGAGED IN COLLATION BUILDING AND LEADERSHIP DEVELOPMENT FOR OUR COMMUNITY MEMBERS. ADDITIONALLY, OUR INVESTMENT IN THE "PROJECT LEAD THE WAY" LIFE SCIENCES ACADEMY IS AN INVESTMENT IN TOMMORROW'S HEALTHCARE WORKFORCE. FORM 990, SCHEDULE H, PART III, LINE 2 BAD DEBT ESTIMATE PATIENT ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR BAD DEBTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE SYSTEM ANALYZES HISTORICAL COLLECTIONS AND WRITE-OFFS AND IDENTIFIES TRENDS FOR EACH OF iTS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR BAD DEBTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYOR SOURCES OF REVENUE IN EVALUATION OF THE SUFFICIENCY OF THE ALLOWANCE FOR BAD DEBTS. |
| Form 990, Schedule H, Part III, Line 3 | BAD DEBT ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER FAP FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS OR WITH BALANCES REMAINING AFTER THE THIRD-PARTY COVERAGE HAS ALREADY PAID, THE SYSTEM RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS HISTORICAL COLLECTIONS, WHICH INDICATES THAT MANY PATIENTS DECLINE TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE DISCOUNTED RATE AND THE AMOUNTS COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS WRITTEN OFF AGAINST THE ALLOWANCE FOR BAD DEBTS. FORM 990, SCHEDULE H, PART III, LINE 4 TEXT OF BAD DEBT EXPENSE FOOTNOTE SEE AUDITED FINANCIAL STATEMENTS - FOOTNOTE 8 (PAGE 20) |
| Form 990, Schedule H, Part III, Line 8 | Treatment of Medicare Shortfall as Community Benefit The costing methodology used is the cost to charge ratio calculated in worksheet 2 of Part I of Schedule H of this 990. The charges and payments are from the Medicare paid claims reports. As a Medicare designated Sole Community Hospital, we are the only provider in the region to provide certain services (psych and OB services, for example) to Medicaid patients and those costs are typically not covered by the payments received. We are ensuring access to services and care for the counties we serve. Therefore, all of the shortfall is considered community benefit. |

| Form 990, Schedule J, Part I, Line 1a | Tax Indemnification and Gross Up Payments The organization provides a nonqualified supplemental retirement plan for certain executive employees. Because benefits under the supplemental plan must be included in taxable income when they become vested, and as required by the supplemental plan's terms, the organization provides an additional benefit that covers the tax liability when it is incurred. The tax liability payments are themselves included in W-2 income in the year made to the executives, and are included in the figures disclosed on Schedule J, Part II. Club Dues During the reporting period, the organization paid the CEO and COO's membership dues in a social club. The club membership was used for business purposes. Any personal related expenses are treated as taxable wage income and fully included on the recipient's Form W2. During the reporting period, the organization also paid the CNO's membership dues in a health club. This benefit was phased out six years ago and only one remains.
