DescOn January 1, 1886, Altoona opened a 30-bed, two-story wooden structure and admitted 113 patients in its first year. The opening of the hospital was driven by the need of the Pennsylvania Railroad and the residents of the growing territory of Blair County. The railroad donated the land that was close to the rail lines which was a convenience for transporting injured railroad workers.UPMC Altoona is currently a 376-bed hospital serving primarily the more than 20 counties throughout Central Pennsylvania. It maintains a Level II Adult Trauma Center, providing comprehensive care for the most severely injured. UPMC Altoona cares for the behavioral health of the region through a 34-bed inpatient facility and offers outpatient therapy, including a 24-hour crisis center, school programs, and family-based drug and alcohol treatment. UPMC Altoona's medical mall brings outpatient services closer to patients by providing state-of-the-art diagnostic neurology testing, digital mammography, as well as comprehensive rehabilitation services.UPMC Altoona is committed to its mission of providing exceptional health care to all in need, regardless of ability to pay. It provided charity care and other uncompensated care, including Medicaid and Medicare shortfalls, at a cost of over $30,000,000 in the fiscal year ended June 30, 2015. UPMC Altoona also provided services to the community through free or low-cost outreach programs targeted at patients, patients' families, and the community. UPMC Altoona provides meeting space to various groups including the Alzheimer's Support Group, Parkinson's Support Group, Bariatric Support Group, Depression and Bipolar Support, and the Caregiver's Support Group. All of these programs and support, including direct donations, have assisted thousands of people in the community with the cost of almost $20,000,000 borne by the hospital in the fiscal year ended June 30, 2015. UPMC Altoona also funded schools of health professions and medical residency programs as well as medical research at a combined cost of $2,800,000 in fiscal year 2015.During fiscal year 2015, Altoona had 17,732 inpatient admissions which contributed to 82,862 inpatient days. There were 68,843 emergency department visits, and Altoona performed over 14,000 surgeries. UPMC Altoona had a total of 371,925 outpatient visits during fiscal year 2015. Of the patients that Altoona serves, 69% are participants in the federal Medicare and state Medicaid programs.UPMC Altoona works with other hospitals in Blair County and more than 40 community agencies and organizations to implement community health programs, supports Let's Move Blair County, which promotes obesity prevention in children and families, and helps promote employee health at area businesses by performing screenings and providing information at their health and wellness fairs.UPMC Altoona provides support for local food banks, homeless shelters, and domestic abuse shelters, and their Healthy Living Club provides community education programs to help people lead healthier lives, with topics such as stroke awareness, child safety, and good nutrition.Free medications are distributed by UPMC Altoona to patients upon discharge who lack the resources or insurance coverage to the cover the costs of medications or copays as well as to patients at the free medical clinic. UPMC Altoona provided free cab rides to outpatients from the Community Crisis Center and inpatient Behavioral Health Unit to the patients' homes or residential facilities.Desc

Schedule H, Part V, Section B, Line 6a-UPMC AltoonaUPMC Altoona hospital's CHNA was conducted with the two other acute care hospitals in Blair County: Nason Hospital and Tyrone Hospital.
Schedule H, Part V, Section B, Line 6b-UPMC AltoonaUPMC Altoona is part of the Healthy Blair County Coalition. The Coalition is a community partnership of organizations that cooperate to conduct the community health needs assessment of Blair County. The CHNA is conducted by the Healthy Blair County Coalition Steering Committee who is comprised of the following: UPMC Altoona; Nason Hospital; Tyrone Hospital; Partnering for Health Services; Pennsylvania Office of Rural Health; Altoona Family Physicians; Community Action Agency; Blair County Department of Social Services; Family Services, Inc.; Operation Our Town; Penn State Altoona; Blair Health Choices; Altoona Blair County Development Corporation; Blair Drug and Alcohol Partnerships; United Way of Blair county; Altoona Area School District; and Sheetz, Inc.
Schedule H, Part V, Section B, Line 11-UPMC AltoonaThe Hospital (UPMC Altoona) did not meet all of the needs identified in the Community Health Needs Assessment (CHNA) it conducted in collaboration with the Health Blair County Coalition (HBCC). The CHNA that was conducted: July - October 2012 identified the following 31 community needs, all of which impact and individuals/family's health and in turn the community's health, which are grouped by category: * Economics: Unemployment/Underemployment, Poverty, Lack of Jobs. * Education: children being adequately educated, unsafe school environment, bullying/harassment, use/availability of drugs. * Environmental: water and air pollution, noise pollution. * Health: alcohol/drug abuse, smoking and tobacco, adults with mental illness/emotional issues, children with mental health or emotional issues, lack of affordable medical care, HIV/AIDS, diabetes, and obesity. * Housing: shortage of affordable housing, substandard housing, overcrowded housing. * Leisure Activities: shortage of recreational facilities, lack of cultural activities. * Safety: crime, gangs. * Social: teen pregnancy, racial or ethnic intimidation, family violence, gambling, and lack of affordable daycare for children. * Transportation: inadequate public transportation, poor roads and traffic conditions. Following a needs prioritization process and evaluation, the HBCC chose to develop strategies to address the following top priority needs identified: Strategy 1: Promote a Healthy Lifestyle (to address: obesity, diabetes and encourage a healthy lifestyle) Strategy 2: Alcohol and Other Substance Abuse (to address alcohol/drug abuse) Strategy 3: Mental Health Needs of Children/Adolescents (to address (to address children with mental health or emotional issues) Strategy 4: Smoking/Tobacco (to address smoking and tobacco use) UPMC Altoona has chosen, in partnership with the HBCC, to address the following community needs: 1. Promote a healthy lifestyle: to address obesity, physical inactivity, and diabetes. 2. Mental health needs of children and adolescents. The other 2 community hospitals and the remaining 47 other community partners that are part of the HBCC are addressing some of the other 31 issues identified above. However neither UPMC Altoona - as a single community hospital, nor the HBCC with 75 community members, has sufficient financial and human resources to meet all of the 31 identified community needs. Many of these needs, which are common in many communities, are significant and are such that society has been working long term to address. However UPMC Altoona has committed resources and efforts, and is working with other community partners, to meet its above identified needs. UPMC Altoona, in partnership with the HBCC, has developed strategies to address its identified community health needs. Although the hospital is not able to address all of the needs identified in Blair County, UPMC Altoona is addressing the identified needs, which will result in healthier individual and families, which will in turn result in an overall healthier community. HBCC Work Group Accomplishments: As part of the implementation strategy to address the county needs identified, the Healthy Blair County Coalition for the following work groups: Healthy Lifestyle, Alcohol and Other Drug Abuse, Mental Health needs of Children and Adolescents, Poverty, and Tobacco-Free Community. The following is a summary of the accomplishments of each work group to date: 1. Healthy Lifestyle: * Adopted the national Let's Move Initiative and held a launch event in 5 communities. Poverty * In cooperation with other community partners, the Bridges Network sponsored and/or assisted with 5 poverty simulations in Blair, Bedford, and Cambria counties. * Members of the work group have worked with and/or expanded programs that provide resources to low-income students at the elementary, secondary, and higher education levels. * Assisted in developing a Fuel Bank Program to more efficiently provide financial assistance to low-income individuals and families. * Bridges Out of Poverty training is available to interested community organizations. * 5. Tobacco-Free Community * The work group developed and distributed a webinar in order to provide information and resources for businesses and organization on how to become 100% tobacco-free workplaces. * The work group conducted a meeting with career services personnel and guidance counselors then developed a second webinar. The purpose was to educate students on issues related to seeking employment in companies that are currently or will be tobacco-free workplaces. In collaboration with the Blair County Drug and Alcohol Partnership, local hospitals, businesses, and other organizations, smoking cessation educational programs have been conducted. Some of the significant community health needs identified in the most recent CHNA are beyond the scope and resources of UPMC Altoona and all of the community partners who are part of the Healthy Blair County Coalition. For example: poverty was identified as a significant community need which may directly impact the health of an individual or family and the HBCC formed a work group to address the issue of poverty - the Poverty Work Group which implemented the Bridges out of Poverty program to address poverty in Blair County. However the issue of poverty remain a significant issue which impact the health of individuals in Blair county. Also other issues, identified in the top 25 community challenge issues, identified in the most recent CHNA which are beyond the resource of UPMC Altoona and the HBCC to address include: lack of jobs, unemployment/underemployment, crime, lack of cultural activities, shortage of recreational facilities, shortage of affordable housing, poor roads and/or traffic conditions, substandard housing, inadequate public transportation, ;lack of affordable care for children, gangs, water or air pollution, and race or ethical discrimination.

Schedule H, Part I, Line 3cFinancial assistance or charity care is available for eligible individuals who have health care needs and are uninsured, underinsured, ineligible for government programs or otherwise unable to pay for their own care. A patient may be determined to have financial need based on the following: 1. Indigency: If income falls below 200% of the federal poverty guidelines, the patient will have no financial responsibility. The patient is approved for 100% free care. 2. Low Income Sliding Scale: Assistance may be a discounted or reduced patient liability depending on the patient's income: a. If the combined family income is greater than or equal to 200% and less than or equal to 400% of the federal poverty guidelines, the patient is eligible for a reduced charity care. There will be an 85% reduction in the patient bill. The discounted charity care scale is listed below: F/S= Family Size 2014 Financial Assistance Income Guidelines F/S FINANCIAL 100% PATIENT 0% FINANCIAL 85% PATIENT 15% ASSISTANCE ASSISTANCE 1 $23,340 $46,680 2 $31,460 $62,920 3 $39,580 $79,160 4 $47,700 $95,400 5 $55,820 $111,640 6 $63,940 $127,880 7 $72,060 $144,120 8 $80,180 $160,360 + $8,120 $16,240 3. The patient will be liable to pay 15% of their calculated income towards their medical bill or the reduced amount whichever is less. 4. Medical Hardship/Catastrophic Care: If a patient is denied for financial assistance and cannot afford to pay the account balance, the patient may be considered medically indigent and may be considered for medical hardship/catastrophic care. The patient will be liable to pay 15% of their calculated income towards their medical bill. This is a one-time financial assistance adjustment for existing account balances. The remaining balance will be adjusted to free care.
Schedule H, Part I, Line 6aUPMC Altoona prepares an annual community benefits report. It is posted to and available to the public via the website WWW.UPMC.COM.
Schedule H, Part I, Line 7The costing methodology used to calculate the amounts reported in Part I Line 7 is a ratio of costs to charges method. The method employed for determining the ratio of cost to charges is in line with those described in Schedule H, Worksheet 2, "Ratio of Patient Costs to Charges."
Schedule H, Part I, Line 7, Column fThe total expense reported on Form 990 Part IX Line 25 Column A includes bad debt expense of $24,857,967. The expense used to calculate the percent of total expense for Form 990 Schedule H Part I Line 7 Column f are the total expenses net of bad debt
Schedule H, Part III, Section A, Line 4The costing method used to calculate the amounts in Schedule H Part III lines 2 and 3 is a ratio of cost to charges method. Discounts and payments in patients' accounts are deducted before the cost of bad debt expense is determined. The method employed is in line with those described in Schedule H, Worksheet 2, "Ratio of Patient Costs to Charges". The organization's bad debt expense consists solely of self-pay patients accounts deemed uncollectable. It is UPMC Altoona's contention that the cost of bad debt should be stated in Part I Line 7 of Schedule H as they represent the costs for provision of services to patients for which the entity has exhausted all recourse for reimbursement. The services provided to patients who present themselves are provided regardless of a patient's ability to pay as in line with the organization's charitable mission and service to our community. These expenses are incurred regardless of the efficiency of the provision of the related medical care and are deemed to have been medically necessary for the patient. UPMC Altoona reclassifies bad debt accounts as charity care by utilizing a predictive model PARO (Payment Assistance Rank Order). PARO was built as a socio-economic score that identifies those patients that are most likely to be the highest need of financial assistance. This is accomplished by analyzing consumer data and activity, court records, government records, property files, Census data and IRS data. PARO provides a systematic approach to Financial Assistance: " Predictive model that encompasses healthcare economics and credit policies; " Uniform assessment measure for every patient regardless of financial standing; " Accounts for patients that are unresponsive, illiterate, or otherwise challenged to apply; " Eliminate barrier to application and meets increased pressure to provide additional financial resources to Consumers; " Identify and support those Community needs efforts to provide assistance to those Consumers in need. There are two data points returned to UPMC Altoona in order to make the decision whether the patient would qualify for charity care. The first is the estimated income level based on the Federal Poverty Level guidelines. The second is the PARO score which utilizes an algorithm based approach and returns a score based on personal attributes of the patient. Data elements were combined to create two primary indicators of need, the PARO score and the Federal Poverty Level. PARO analyzed our historical charity care approvals to determine our PARO threshold and the FPL threshold for the historical approvals. The approved charity care accounts were compared against accounts that were in active accounts receivable. This was done to determine if the distributions were similar or if any pattern existed based on the approved accounts and the unknown accounts. If the patient's estimated income and PARO score are within the scores identified by the calibration of UPMC's existing charity care patients, the account is reclassified from bad debt to charity care. Every patient is able to apply for charity care; however, the automated strategy focuses on patients that do not complete the applications process. The US Department of Education estimates that 1 in 5 consumers are functionally illiterate. This, coupled with the low levels of participation in traditional banking methods, makes the application process virtually impossible for some consumers. See UPMC's Consolidated Audited Financial Statements page 6.
Schedule H, Part III, Section B, Line 8The costing methodology used to determine the Medicare allowable costs of care is a ratio of costs to charges method. The method employed for determining the ratio of cost to charges is in line with those described in schedule H, Worksheet 2, "Ratio of Patient Costs to Charges." It is UPMC Altoona's contention that all amounts calculated to be shortfalls in reimbursement for services provided to Medicare patients are truly uncompensated care that should be stated in Part I Line 7 of Schedule H of Form 990 as they represent costs for provision of services to patients for which the entity is unable to collect, regardless of the efficiency of provision of the related care costs.
Schedule H, Part III, Section C, Line 9bUPMC Altoona has a debt collection policy outlining collection practices for patients. If at any time the patient expresses an inability to pay, they are sent a Financial Assistance application. Information regarding Financial Assistance is printed on all collection letters.
Schedule H, Part VI, Line 2UPMC Altoona helped form and actively supports and participates in the Healthy Blair County Coalition (Coalition). The Coalition is comprised of over 75 community agencies, organizations, and services. The Coalition was formed in 2007 to conduct comprehensive community needs assessments, review and analyze data, formulate strategies to meet identified service gaps and needs, implement strategies to meet identified needs, and conduct subsequent post-implementation surveys to determine effectiveness of programs and services implemented. An assessment of the communities' healthcare needs are part of the initial and follow-up survey processes and involve a household survey, focus groups, and survey of local businesses, service providers, healthcare providers with special expertise and knowledge in helping to assess needs within our community. The most recent community needs assessment was conducted in FY 2012-13 and identified the following categories of community needs: Economics (unemployment/underemployment, poverty, lack of jobs); Education (children being adequately educated, unsafe school environment, bullying/harassment, use/availability of drugs); Environmental (water or air pollution, noise or other pollution); Health (alcohol and/or drug abuse, smoking and tobacco, adults with mental illness or emotional issues, children with mental health or emotional issues, lack of affordable medical care, HIV/AIDS, diabetes, obesity); Housing (shortage of affordable housing, substandard housing, overcrowded housing); Leisure Activities (shortage of recreational facilities, lack of cultural activities); Safety (crime, gangs); Social (teen pregnancy, racial or ethnic intimidation, family violence, gambling, lack of affordable daycare for children) and Transportation (inadequate public transportation, poor road and/or traffic conditions). From this total list of identified community needs, the Health Blair County Coalition Steering Committee, of which UPMC Altoona is a member, identified four areas of need to be addressed by the Coalition, although other identified needs are also being addressed by others in the community, 1. Obesity/physical activity/diabetes, 2. Alcohol and other substance abuse, 3. Mental health needs of children /adolescents, and Smoking/tobacco. Of these priority needs, UPMC Altoona is specifically addressing: obesity, diabetes Promote a Healthy Lifestyle), and the mental health needs of children/adolescents.
Schedule H, Part VI, Line 3UPMC Altoona communicates the availability of financial assistance to its patients by placing signage and distributing brochures about the Financial Assistance program in its Provider registration areas, admissions, emergency department and business office locations. In addition, UPMC includes information regarding how a patient can request financial assistance on the bills, follow-up correspondence and collection letters. Patients also may find information about financial assistance through access to the UPMC web site (www.upmc.com). Patients are encouraged to contact UPMC's trained financial counselors should they require assistance with completing applications. Contact telephone numbers ARE posted strategically throughout the hospital facilities so that patients have a resource for obtaining support with any financial assistance questions they may have.
