The Hospital is dedicated to providing comprehensive healthcare services to all segments of society, including the aged and otherwise economically disadvantaged. In addition, the Hospital provides a variety of community health services at or below cost. The Hospital maintains a financial assistance program to patients who are financially unable to pay for the healthcare services they receive. It is the policy of the Hospital not to pursue collection of amounts determined to qualify as charity care. Accordingly, the Hospital does not report these amounts in patient service revenue. The Hospital determines the costs associated with providing charity care by aggregating the direct and indirect costs, including salaries, benefits, supplies, and other operating expenses, based on the overall cost to charge ratio for the Hospital. The costs of caring for charity care patients for the year ended December 31, 2024 were approximately $110,000.Desc

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 6a:A copy of the community benefit report is available at the hospital.
Part I, Line 7:Charity care expense was converted to cost on line 7a based on an overall cost to charge ratio addressing all patient segments. Unreimbursed Medicaid on line 7b was calculated using the costing methods to prepare the cost reports but did not result in a community benefit in 2024. The cost for subsidized health services reported on line 7g was determined using the Medicare Cost Report. Community health improvement services, line 7e, health professions education, line 7f, and cash and in-kind contributions, line 7i, are reported based on amounts reported in the general ledger.
Part II, Community Building Activities:Stewart Memorial has a program called This is Home that community members can apply to have yard work and minor repairs completed in the spring by staff members. This past year we had 12 community members that utilized this service. Services provided by hospital personnel included raking leaves, washing windows, cleaning gutters and minor repairs. In addition, Stewart Memorial Community Hospital staff serve on community economic development and chamber committees. They volunteer their time with community activities and possible development. Other members of our team volunteer to serve on the disaster preparedness committee serving our county. They serve to prepare the community in case a disaster occurs and to use funds allocated to prepare our facility and community with needed safety equipment or upgrades.
Part III, Line 2:The amount reported on line 2 represents implicit price concessions. The Hospital determines its estimate of implicit price concessions based on historical collection experience with various classes of patients.
Part III, Line 3:The estimated amount of the Organization's implicit price concessions attributable to patients eligible under the Organization's charity care policy is calculated based on the percentage of individuals living below the poverty level in Calhoun County, Iowa. 8.61% can reasonably be considered a community benefit as it would have been written off to charity care.
Part III, Line 4:The footnote to the Organization's financial statements can be found on page 12 and 13 of the attached audited financial statements.
Part III, Line 8:Stewart Memorial Community Hospital is a critical access hospital with a Medicare shortfall. This is considered a community benefit because providing services to Medicare patients is essential to the patients and the community and increases their access to healthcare services. Both the total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by Centers for Medicare and Medicaid Services.Hospital services reimbursed on a fee schedule are not included in the Medicare calculation per the 990 instructions. Had this been reported the total loss from Medicare services would have increased by $889,880.Medicare fee schedule revenue $ 402,131Medicare estimated costs of care relating to payments (1,292,011)Net (Shortage) $ (889,880)
Part III, Line 9b:The Hospital must determine whether an individual is FAP-eligible or provide required notices during a notification period ending 120 days after the date of the first billing statement. Although the Hospital may use extraordinary collection efforts after the 120-day notification period, we must still accept and process a FAP application for an additional 120 days. The total period during which we must accept and process FAP applications is 240 days from the date of the first billing statement. If the hospital receives a FAP application during the application period, we must suspend any extraordinary collection efforts started until the application is processed, and if the individual is FAP-eligible, must seek to reverse the extraordinary collection efforts and promptly refund any overpaid amounts.
