ExplanationThe Hospital belongs to certain organizations that engage in lobbying activities on behalf of their hospital members. Since this is a nonallowable expense, these organizations inform their members the portion of their dues that should be considered applicable to lobbying expenses so that the appropriate adjustment can be made. In an effort to determine which dues include lobbying expenses, the Hospital reviewed the invoices for all dues payments in excess of $10,000 and scheduled all other payments we were made aware of through various inquiry.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Children's Hospital of Alabama. In 2022, Children's of Alabama (COA) engaged the Public Affairs Research Council of Alabama (PARCA) to conduct the 2023-2025 community health needs assessment (CHNA) designed to identify local and statewide child health concerns and how to best support efforts to address the concerns. For the 2023-2025 CHNA, COA and PARCA gathered data from four focus groups, over 1200 online & telephone surveys, and interviews of nine key stakeholders. Targeted constituencies for the online survey and focus groups included parents, school nurses, administrators, pediatric/family medicine physician practices, community advocates, and youth. 510 stakeholders responded to the online survey, compared to 553 in 2019 and 481 in 2016. Fifteen metrics at the state and national level were also analyzed for secondary data information. The next formal query of the community for CHNA will occur in FY25. In the interim, COA receives community input by integrating with various community-facing groups (e.g., United Way Health Action Partnership/Bold Goals, Alabama Department of Mental Health, Alabama Department of Public Health, etc.) as well as having regular conversations with local and state elected officials to hear emerging health issues that may not have been identified in the most recently completed CHNA. Based on those meetings and conversations, COA develops strategies to address pressing needs in real-time and fold those initiatives into the next CHNA.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Children's Hospital of Alabama. During 2024, COA continued to emphasize previously identified areas of focus: nutrition & exercise, mental & behavioral health, parenting education and child safety. Nutrition & exercise were addressed by COA's involvement collaboration with The Hunger Solutions Institute and several other related agencies to host Feeding Change: Nourishing Alabama's Future in Montgomery in November 2024h. Part of the conference included a Food as Medicine track with four breakout sessions, one of those led by COA physician and provider teams that oversee our Food is Medicine program, and highlighted best practices for Food is Medicine activities that are happening in the state. Mental health was addressed via COA's operation of the Psychiatric Intake Response Center (PIRC), which received approximately 2,110 calls and consulted in 4,400 psychiatric cases in the Emergency Department for 2024. PIRC-identified mental health providers were present in all counties in Alabama. The Center also provided ongoing behavioral health consultation and presentations to area school systems. COA continues to operate its grief counseling program, the Amelia Center, serving over 2,200 children in 2024 and providing behavioral health case consultation and treatment service to primary care practices enrolled in the HRSA-funded PATHS (Pediatric Access to Telemental Health Services) initiative. Over 120 practices and 450 providers participated with PATHS in FY24. Children's CHIPS Center (Children's Hospital Intervention and Prevention Services) conducts educational sessions and trainings to promote health and healing for those affected by suspected child abuse and neglect. Over 250 children were treated by the CHIPS center in FY24, providing over 3100 counseling sessions. COA recently rebranded its patient health and safety programming to the Children's Health Education and Safety Center (HESC). The HESC encompasses Safe Kids, ThinkFirst, and Alabama LifeStart and several other community facing programs. HESC's programs focus on general safety, brain and head trauma and sudden cardiac arrest. In total, the HESC's programs reached over 14,500 children and teens, and over 7,500 adults from in 2024. The HESC is a participant in a grant administered by the Alabama Department of Transportation to provide education for child passenger safety and teen driving, including pedestrian safety and wheeled sports safety. Through COA's Car Seat Program, checked 642 car seats were checked, 323 children and 344 parents/caregivers were educated at a Car Seat Clinic on the Children's of Alabama campus. Off Campus, COA held 2 community events in 2024, with one being in Florence and the other in Birmingham. At these off-campus events, COA checked 41 car seats, educated 28 children and 22 parents/caregivers. COA also participated in community events in Trussville and Tuscaloosa as part of the Child Passenger Safety Technician Certification Courses. Additionally, COA provided educational opportunities for 1,341 children and 720 parents/caregivers regarding transportation related topics, including pedestrian, wheeled sports, school bus, golf carts and ATV's. As a part of Pediatric Vehicular Heatstroke Campaign, COA educated 1,607 parents, caregivers, healthcare, and other professionals about the dangers of leaving children in the car alone. Lastly, COA held 3 Child Passenger Safety Technician (CPST) Certification Courses that certified 33 new CPST's across the state. Through these events and programs, COA provided 498 child restraints to children and their caregivers. COA also provided 190 booster seats to our partner programs so they could distribute them to booster seat appropriate children. COA' teen driving programs have touched thousands of students in 2024 through several programs. URKeys2Drv with mini summits, U TLK U TXT U CRSH, and other individual teen driving programs have been hosted across the state. In 2024, COA added tailgating events at high school sports games - this included having the game announcer provide teen driving facts to spectators, and COA staff set up a "game show trivia wheel with all transportation related questions for the teens to answers for a prize. Additionally, educational materials were provided to parents/caregivers and teens. Through COA's brain and spinal cord injury prevention program, COA staff educated 9,753 children/teens and 968 adults. These were done through a variety of different age-appropriate programs. Grants also played a factor in this, as grant specifications allowed COA to provide bike helmets after appropriate educational programs. These programs are for children of all ages, starting as young as pre-kindergartners and ending with our teenaged drivers and passengers. During the year, COA hosted School Nurse Workshops and Spotlights, as well as clinical skills labs. Our School Nurse Workshops are held in the summer since the nurses are out of school and have more time to attend longer sessions. However, COA also hosts Spotlights during the school year that are an hour in length so that the nurses can attend while working. The clinical skills labs are held at the beginning of each school semester so nurses with new students needing care are up to date on their education. COA hosted 601 nurses for skills labs, 955 for Spotlights and 1,111 for School Nurse Workshops. The topics for spotlights were headaches, obesity, sudden cardiac arrest, and firearm safety. The topics for Summer School Nurse Workshops included cystic fibrosis & immunizations & allergies, anxiety & adrenal insufficiency & dermatology, and a diabetes workshop. COA also provides a monthly newsletter, The Pulse, that provides medical, social, and mental health information, as well as information specifically about Children's of Alabama. This newsletter is available to all school nurses, and serves as a way of continuing to foster open communication. COA maintains a website (myschoolnurse.net) that is continually updated with educational information and offerings for school nurses. The HESC continued to designate schools as Heart Safe Schools through Alabama LifeStart, a program modeled after National Project Adam. The HESC also is a participant in a grant administered by the Alabama Department of Community and Economic Affairs grant to provide a multi-pronged state-wide education program on pediatric vehicular heatstroke. Through our other programs, we have provided safety education surrounding home safety, safe sleep for infants, firearm safe storage, and more. The Alabama Poison Information Center (APIC) housed at COA received over 50,000 incoming calls in 2024, offering free and confidential poison information and treatment recommendations to the public and health care providers 24 hours a day. In 2024, the APIC participated in 168 hours of community health events, engaging with the public and distributing poison information to >5,600 Alabamians. Along with Healthy Child Care Alabama (HCCA) the APIC provided 2,193 poison prevention programs reaching a total of 11,966 parents throughout all 67 counties. HCCA nurses distributed 12,221 poison prevention items provided by the APIC.

Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation10726249
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceThe costing methodology used to complete Line 7 was an overall cost-to-charge ratio computed using the IRS Form 990 Worksheet 2, ratio of patient care cost to charges.
Schedule H, Part II Community Building ActivitiesIn 2024, COA participated in initiatives as defined under Part II, Community Building Activities. COA employees continued to train allied health professionals and other students from area colleges, universities, and two-year institutions virtually and in-person. COA also provided myriads of programs and initiatives aimed at non-patient care, education, and research. COA personnel continued to engage with community organizations and stakeholder groups ranging from community mental health roundtables to maternal and child health stakeholder groups. COA continued its support of organizations statewide including, but not limited to, KidOne Transport, Childcare Resources, Alaquest, the YWCA, the YMCA, United Way of Central Alabama, VOICES for Alabama's Children, Alabama Partnership for Children, the Exceptional Foundation, and the Bell Center. COA's Child Intervention & Prevention Services (CHIPS Center) continued to train school administrators and educators on child abuse prevention, identification, and intervention. COA's Health Education and Safety Center educated school nurses and administrators on topics about mental and behavioral health and clinical competencies. Children's of Alabama executives and staff continued in 2024 to serve on a variety of community boards, nonprofit and professional organizations at the local, state and national levels.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountReceivables from patients, insurance companies and third-party contractual agencies are stated at net realizable value. The Hospital evaluates the collectability of its patient accounts receivable based on the length of time the receivable is outstanding, historical collection experience, review of large balance accounts and trends in health care insurance programs to estimate the appropriate allowance for doubtful accounts and provision for bad debts. For receivables associated with services provided to patients with third-party insurers, the Hospital estimates contractual allowances based on the terms of third-party insured contracts and estimates an additional allowance for doubtful accounts on patient responsible balances after third-party payments. For receivables associated with self-pay patients, the hospital records a significant provision for bad debts in the period of service based on its experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. These allowances are recorded on an estimated basis and adjusted as needed in future periods. Doubtful accounts are written off against the allowance after adequate collection effort is exhausted and recorded as recoveries of bad debts if subsequently collected.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBad debts attributable to patients eligible for charity care were calculated based on the outstanding patient balance in excess of a reasonable amount the average person could pay without creating a medical hardship. For 2021 this amount is $24,600, which approximates the income limit for a family of 4 in the federal poverty guidelines. Charity care determinations are based on completing the required application and meeting defined criteria of the hospital. Many times, patients fail to complete paperwork required despite efforts to educate and inform patients of this option. However, based on socio-economic and other known factors, it is highly likely that these patients would meet established charity care criteria if the application process was completed.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsMedicare cost & charges primarily relate to renal (ESRD) and SSI patients covered under Medicare. Given our small Medicare population, the costs/charges could fluctuate significantly year over year based on level of care. These would not be considered a community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceA guarantor will receive four bills before the account is turned over to a collection agency. After 120 days, if there is no attempt to pay - the account will be sent to a collection agency.

Schedule H, Part VI, Line 2 Needs assessmentCOA engaged the Public Affairs Research Council of Alabama (PARCA) to conduct the 2023-2025 community health needs assessment (CHNA) designed to identify local and statewide child health concerns and how to best support efforts to address the concerns. PARCA distributed an online survey to 30 organizations with the request that the survey be forwarded to the organizations' membership lists. These organizations and distribution lists were selected because of their professional and geographic diversity, and because of their experience with children of many backgrounds and in a variety of settings. Stakeholders from all 67 counties were sent the survey; responses were received from 66 counties. A telephone survey of randomly selected residents in Alabama was conducted; there were 752 survey respondents. In addition, selected statewide stakeholders participated in interviews and focus groups. Stakeholders and responding parties included parents, youth leaders, pediatricians, community advocates, business leaders, and the community at large. Data from these groups both corroborated and elaborated on themes that emerged from survey data. Children's of Alabama's CHNA findings and related implementation strategies were released on December 20, 2022.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceEach patient that presents as a self-pay patient is directed to a financial counselor. The financial counselor educates patients and their families about government and financial assistance programs and assists with related applications. If a patient is not counseled by a financial counselor on the date of service, statements are sent with the information needed to apply for financial assistance by the business office. In addition, the Financial Assistance Policy is posted on the Hospital's website and there is signage throughout the Hospital informing patients of the availability of a financial assistance program.
Schedule H, Part VI, Line 4 Community informationThe Hospital's primary service area is Birmingham / Hoover metropolitan statistical area as defined by the U.S. Office of Management and Budget. The Hospital's secondary service area is defined as the rest of the state, and we care for patients from every county in the state. demographic data including race, gender, and age is captured through the registration process. For 2023, Caucasians, African Americans, Hispanics, and other races accounted for 53%, 30%, 8%, and 9%, respectively, of the patients served while 37%, 46%, and 17% were ages 0-4, 5-14, and 15+ years, respectively. Economic demographic data is not specifically captured.
Schedule H, Part VI, Line 5 Promotion of community healthChildren's of Alabama is Alabama's primary site for pediatric health care and health education and training for community colleges and universities in Alabama and beyond through its affiliation with the School of Medicine at The University of Alabama at Birmingham. COA is Alabama's sole level one pediatric trauma center, transplant programming, and houses the largest pediatric burn center in the Southeast. Our community health promotion to the larger, non-patient community of youth and adult stakeholders, parents, and guardians focused in 2024 on implementation strategies tied to the 2023-2025 CHNA priorities including, but not limited to, nutrition & exercise, mental & behavioral health, parenting education and child safety. COA's Health Education and Safety Center encompasses Safe Kids, ThinkFirst, and Alabama LifeStart. These three programs focus on general safety, brain and head trauma and sudden cardiac arrest. More than 6,600 school-aged children and 1,300 parents and adults were reached by the Center in 2019-2022. Ur Keys 2 Drive' aimed to inform adolescents and teens on the importance of safe driving, biking and ATV usage. School nurse workshops educated more than 1,000 school nurses in 2024 alone on a wide range of topics. COA's Hospital Intervention & Prevention Services Center (CHIPS) addressed thousands of youth and adult stakeholders in schools on the prevention of child abuse. The Hospital's Alabama Poison Information Center (APIC) fielded thousands of calls from the public representing counties statewide and other states on poisonous interactions and COVID-related questions. In accordance with the IRS' 501(c)3 requirements defining community health promotion, Children's has an open medical staff and is governed by a diverse board comprised of statewide community leaders. Children's retains a strong focus on collaboration of health promotion with key stakeholders statewide. During FY24, Children's of Alabama continued addressing the needs of the larger pediatric community through the promotion of its mission and commitment to those who rely on the hospital's care.
