| Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistance | WYCKOFF HEIGHTS MEDICAL CENTER'S COSTING METHODOLOGY WAS BASED UPON THE 2024 AUDITED FINANCIAL STATEMENTS, 2024 NEW YORK STATE INSTITUTIONAL COST REPORT AND THE 2024 MEDICARE (FORM 2552) COST REPORT. THESE COST REPORTS ARE FILED WITH THE NEW YORK STATE DEPARTMENT OF HEALTH AND THE APPLICABLE CMS INTERMEDIARY, RESPECTIVELY. THE COST-TO-CHARGE RATIO DERIVED FROM THE NEW YORK STATE INSTITUTIONAL COST REPORT WAS USED FOR THE VARIOUS SUB-LINE ITEMS OF LINE #7. |
| Schedule H, Part II Community Building Activities | BEHAVIORAL HEALTH PROGRAM ACTIVITIES: WYCKOFF'S BEHAVIORAL HEALTH PROGRAMS HELP ADDRESS THE CRITICAL LACK OF MENTAL HEALTH AND SUBSTANCE USE SERVICES IN THE COMMUNITY. DESPITE NEARLY ONE IN FIVE INDIVIDUALS EXPERIENCING A MENTAL HEALTH CONDITION, BOTH BROOKLYN AND QUEENS HAVE A DEFICIT OF BEHAVIORAL HEALTH PROVIDERS - PARTICULARLY THOSE WHO TAKE PUBLIC INSURANCE - THUS CREATING A SIGNIFICANT UNMET NEED FOR CARE. TO IMPROVE ACCESS TO INTEGRATED, EVIDENCE BASED BEHAVIORAL HEALTH SERVICES, IN 2016 WYCKOFF LAUNCHED BEHAVIORAL HEALTH SERVICES ACROSS FOUR PRIMARY CARE CLINICS: POSITIVE HEALTH, ADULT MEDICINE, WOMEN'S HEALTH, AND PEDIATRICS. MOST PATIENTS ENGAGE IN THE COLLABORATIVE CARE PROGRAM; THIS INITIATIVE PROVIDES ROUTINE DEPRESSION, ANXIETY, DRUG AND ALCOHOL USE SCREENINGS OF ALL PATIENTS, AS WELL AS ACCESS TO ONSITE, INTEGRATED BEHAVIORAL HEALTH SERVICES. PATIENTS WITH POSITIVE SCREENING SCORES ARE REFERRED TO A CARE MANAGER, A BEHAVIORAL HEALTH CLINICIAN WHO COLLABORATES CLOSELY WITH THE PATIENT'S PRIMARY CARE PROVIDER TO IMPROVE SYMPTOMS OF DEPRESSION AND ANXIETY THROUGH A PATIENT DRIVEN MIX OF EVIDENCE-BASED INTERVENTIONS AND PSYCHOPHARMACOLOGY. HAVING BEHAVIORAL HEALTH PROVIDERS IN PRIMARY CARE REDUCES THE STIGMA OF SEEKING MENTAL HEALTH SERVICES AND IS SHOWN TO IMPROVE HEALTH OUTCOMES IN CONNECTION WITH A WIDE RANGE OF CHRONIC DISEASES. THIS INITIATVE WAS INITIALLY GRANT FUNDED BY HRSA FROM 2016 THROUGH 2021. SINCE THE GRANT FUNDING ENDED, WYCKOFF HAS INVESTED ITS OWN RESOURCES TO CONTINUE PROVIDING DEPRESSION AND ANXIETY SCREENINGS AND COUNSELING SERVICES IN PRIMARY CARE. WYCKOFF CONTINUES TO RECEIVE GRANT FUNDING FROM SAMHSA TO IMPLEMENT SUBSTANCE USE SCREENING, TREATMENT, AND REFERRAL TO CARE SERVICES INTEGRATED WITH HIV AND HEPATITIS MEDICAL SERVICES. IN OCTOBER 2022, WYCKOFF RECEIVED A NEW 5-YEAR GRANT AWARD FROM SAMHSA TO CONTINUE OFFERING COUNSELING, PATIENT NAVIGATION AND CASE MANAGEMENT SERVICES RELATED TO SUBSTANCE USE/ABUSE PREVENTION AND TREATMENT. WYCKOFF ALSO OFFERS A BEHAVIORAL HEALTH PROGRAM FOCUSED ON CHILDREN 0 to 5 YEARS OF AGE AND THEIR FAMILIES: THE HEALTHY STEPS PROGRAM IS GRANT-FUNDED BY THE OFFICE OF MENTAL HEALTH, AND SINCE 2016 HAS BEEN IMPLEMENTED IN THE WYCKOFF PEDIATRIC CLINIC. HEALTHY STEPS PROVIDES EARLY CHILDHOOD INTERVENTION SCREENINGS, EDUCATION, AND SUPPORT THAT ARE ORGANIZED INTO THREE TIERS OF SERVICE BASED ON INTENSITY AND THAT ARE RESPONSIVE TO EACH FAMILY'S NEEDS. HIV, HEPATITIS, AND SEXUAL HEALTH PREVENTION AND CARE ACTIVITIES: WYCKOFF OFFERS THE MOST ROBUST AND COMPREHENSIVE HIV, HEPATITIS AND SEXUAL HEALTH SERVICES IN THE LOCAL GEOGRAPHIC AREA. MANY OF THESE SERVICES ARE FUNDED BY CITY, STATE, AND FEDERAL GRANTS AND ALLOW WYCKOFF TO SERVE THE COMMUNITY IN A COMPREHENSIVE WAY WITH BOTH PREVENTION AND MEDICAL TREATMENT SERVICES INTEGRATED ALONGSIDE SUPPORTIVE SERVICES LIKE BEHAVIORAL HEALTH COUNSELING AND CASE MANAGEMENT. PREVENTION SERVICES INCLUDE SOCIAL MEDIA AND SOCIAL VENUE OUTREACH, INTEGRATED HIV/HCV/STI TESTING, SEXUALLY TRANSMITTED INFECTIONS (STIS) TREATMENT/VACCINATION, BIOMEDICAL PREVENTION (PRE-AND POST EXPOSURE PROPHYLAXIS), MENTAL HEALTH AND SUBSTANCE USE COUNSELING, PATIENT NAVIGATION INCLUDING BENEFITS NAVIGATION, AND INDIVIDUAL AND GROUP EVIDENCED BASED INTERVENTIONS. IN OCTOBER 2023, WYCKOFF RECEIVED A NEW 5-YEAR SEXUAL HEALTH PREVENTION GRANT FROM THE CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) WHICH WILL ALLOW WYCKOFF TO CREATE A SEXUAL HEALTH CLINIC THAT IS INTEGRATED WITH PRIMARY CARE SERVICES AT THE 1419 MYRTLE AVENUE WYCKOFF DOCTORS LOCATION. THE WYCKOFF INITIATIVE FOR SEXUAL HEALTH (WISH) IS A ONE-OF-A-KIND PROGRAM IN BUSHWICK, FOCUSED ON ESTABLISHING COMPREHENSIVE SEXUAL HEALTH SERVICES IN A PRIMARY CARE SETTING. THE WISH POGRAM GOALS ARE TO INCREASE THE STI, HIV, AND HEPATITIS TESTING UPTAKE IN OUR CLINIC THAT SERVES OVER 5,000 UNIQUE CLIENTS ANNUALLY; TO REDUCE THE TRANSMISSION OF COMMON STIS (GONORRHEA, CHLAMYDIA, AND SYPHILIS) IN OUR COMMUNITIES; TO INCREASE KNOWLEDGE OF AND ACCESS TO HIV PREVENTIVE TOOLS SUCH AS PREP AND PEP; AND TO ENGAGE OUR COMMUNITIES IN PLANNING AND DESIGNING PROGRAM ACTIVITIES AND SEXUAL HEALTH SERVICES THAT BEST MEET THEIR NEEDS AND PREFERENCES AND ARE STIGMA-FREE. | Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | Effective January 1, 2018, the Medical Center adopted ASU 2014-09 following the modified retrospective method of application. As a result, at the adoption of ASU 2014-09, the majority of what was previously classified as the provision for bad debts is now reflected as an implicit price concession (as defined in ASU 2014-09) and therefore is included as a reduction to net patient service revenue in the accompanying consolidated statement of operations. The amount reported on line 2 is the "Implicit Price Concessions is represented at cost within the "Uncompensated Care" amount of $9.8M on Page 17 of the 2024 audited financial statements. |
