Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - St. Bernard Hospital. The assessment incorporated data from both quantitative and qualitative sources. Quantitative data input includes primary research (the PRC Community Health Survey) and secondary research (vital statistics and other existing health-related data); these quantitative components allow for trending and comparison to benchmark data at the regional, state and national levels. Qualitative data input includes primary research gathered through two Informant Focus Groups. The "community" defined for this project, includes all residential Zip Codes within the service area St. Bernard Hospital and Health Care Center (St Bernard Hospital and Health Care Center Service Area or SBJJCC Service Area). The sample design used for this effort consisted of a random sample of 408 individuals age 18 and older in the St. Bernard and Health Care Center Service Area. All administration of the surveys, data collection and data analysis was conducted by Professional Research Consultants, Inc. (PRC). As part of the community health assessment, two focus groups were held at St. Bernard Hospital: one on June 19, 2012, with key informants from South Chicago (including health professionals, social service providers, and other community leaders), and another on June 21,2012, comprised of key informants from throughout Cook County (including representatives from public health; physicians, other health professionals, social service providers, and other community leaders.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - ST BERNARD HOSPITAL. Identified "Health Improvement Areas" and 2014 Response by St. Bernard Hospital: Access to Care. During 2014, St. Bernard has worked to expand patient access to care through several efforts, including the following: * Helped uninsured and under-insured patients apply for coverage under the Affordable Care Act * Became a primary owner of Better Health Network, a new community-based health plan established in accordance with Illinois Department of Healthcare and Family Services regulations as an Accountable Care Entity (ACE). Better Health Network is currently enrolling plan members and will eventually serve up to 40,000 Medicaid recipients living in communities on the South and West Sides of Chicago and Cook County. Provided free bus and train cards and cab fare to patients without transportation * Provided free medications to patients without prescription drug coverage * Modified facilities to improve accessibility for people with disabilities * In a venture with Community Care Alliance of Illinois, St. Bernard became a medical home on the South Side of Chicago for people with special needs * Opened a specialty clinic staffed by physicians with specialties that are in short supply on the South Side of Chicago. Cancer Services. During 2014, St. Bernard began investigating opportunities for establishing a cancer care program, including infusion therapy. Currently, St. Bernard is exploring possible partnerships with other area health care providers. Chronic Kidney Disease. During 2014, St. Bernard continued providing inpatient renal dialysis, as well as emergency room renal dialysis. St. Bernard coordinated care with local dialysis centers. Diabetes. St. Bernard continues to provide specialty care for patients with diabetes; in particular, the open wound care center helped treat the wounds and skin conditions that disproportionately affect those living with diabetes. Family Planning. During 2014, St. Bernard Hospital's Women's Wellness Center provided family planning and prenatal care services to women, regardless of ability to pay. Heart Disease and Stroke. St. Bernard's cardiac specialists provided care through the hospital's specialty clinic. Through a partnership with Rush University Medical Center, St. Bernard was able to transfer patients with stroke symptoms from the emergency room directly to Rush. HIV/AIDS. During 2014, St. Bernard screened all expectant mothers for HIV, and made referrals to infectious disease specialists when appropriate. Tobacco Use. St. Bernard continued working with the Respiratory Health Association in Englewood to reduce tobacco use on the South Side of Chicago. Injury and Violence Prevention. St. Bernard Hospital is a founding member of Teamwork Englewood, a community organization that focuses on safety and promotion of healthy lifestyles in the community. St. Bernard hosted several events for Teamwork Englewood's Violence Prevention initiatives during 2014. (See events below). In addition, St. Bernard Hospital coordinated and hosted several events focused on violence prevention with the 7th District of the Chicago Police Department. Maternal and Infant Health. St. Bernard Hospital has operated its Women's Wellness Center for five years, providing obstetric care to uninsured women from the hospital's service area, regardless of ability to pay. During 2014, the hospital began discussion of the feasibility of expanding the Women's Wellness Center to include general gynecological care, in addition to obstetrics. Mental Health and Mental Disorders. The Community Health Needs Assessment documented a considerable need for mental health services. In response, St. Bernard now operates an outpatient behavioral health clinic. St. Bernard has also created a new inpatient medical-behavioral clinic, one of the only such facilities in Chicago. Finally, we are planning a partial hospital program for mental health treatment to open in the near future. Nutrition, Physical Activity, Healthy Weight: St. Bernard currently provides nutrition counseling for patients, employees and community members, to provide information about nutrient-rich diets that improve health, and to discuss particular dietetic needs of patients, based on individual health concerns. Oral Health. See Dental Center information above. Respiratory Diseases. In 2014, St. Bernard Hospital expanded its pediatric asthma program by adding a Pediatric Asthma Outreach Coordinator to conduct patient information and education. In this position, the new Coordinator interviews patients and caregivers to assess any known asthma triggers in their living environment; to provide asthma education on disease process, self-management, medication, tools and techniques; to complete an Asthma Action Plan for home/school and medical providers; to assist parents/caregivers with scheduling follow-up visits with medical provider, and to link families to St. Bernard Pediatric Asthma Clinic. In addition, St. Bernard began providing pediatric patients with asthma "kits," including hypoallergenic pillow cases, mattress covers, and spacers, and other materials to provide to patients and families, as needed, to remediate home triggers. These key programs are continuously monitored for performance and quality. Ongoing improvements will be made, as need, to insure program success. Finally, two needs identified by the Community Health Assessment, Vision Care and Care for Patients with Chronic Disabling Conditions, were not addressed in 2014, due to a lack of specialists at the hospital. St. Bernard is currently exploring options to add services in these areas in 2015.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part I, Line 7g Subsidized Health ServicesThe organization did not include, as subsidized health services, any costs attributable to a physician clinic.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceA cost-to-charge ratio was used for all of the figures reported in the table. The cost-to-charge ratio was derived from Worksheet 2, Ratio of Patient Care Cost-to-Charges.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountThe Hospitals allowance for uncollectible accounts for self pay patients, which includes uninsured patients and residual copayments and deductibles for which managed care has already paid, decreased from 73% of self-pay accounts receivable at December 31, 2013 to 72% of self-pay accounts receivable at December 31, 2014. In addition, the Hospitals self-pay write-offs decreased $5,542,413 from $16,993,260 for fiscal year 20132 to $11,450,847 for fiscal year 2014. The Hospital has not changed its charity care or uninsured discount policies during fiscal year 2014 or 2013. The Hospital does not maintain a material allowance for uncollectible accounts from Third-Party payers, nor did it have significant write-offs from third-party payers. There has been a migration of Self Pay and General Assistance patients to Medicaid & Charity Care.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnotePatients' accounts receivable are reduced by an allowance for uncollectible accounts. In evaluating the collectability of patients' accounts receivable, the Hospital analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for uncollectible accounts and provision for bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for uncollectible accounts. For receivables associated with services provided to patients who have third party coverage, the Hospital analyzes contractually due amounts and provides an allowance for doubtful accounts and a provision for bad debts, if necessary. For receivables associated with patient responsibility (which includes both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill), the patients are screened against the Hospital charity care policy and uninsured discount policy. For any remaining patient responsibility balance, the Hospital records a provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the standard rates (or the discounted rates if negotiated) and the amounts actually collected after all reasonable efforts have been exhausted is charged off against the allowance for uncollectible accounts.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsRatio of Cost to charge to total charges by Payer
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceOf the more than 86,216 patients seen annually at St. Bernard Hospital, 68% of them receive Medicaid as coverage for their health care services. Another 11% are on Medicare; and 15% of our patients are self-pay patients. Our patient mix necessitates that a fundamental component of St. Bernard Hospital's community benefit strategy is the provision of charity care to those who cannot afford to pay for their care. The Hospital performs normal accepted Collection procedures on legitimate balances owed to the Hospital. Schedule H, Part VI, Line 2 Needs assessmentThe hospital periodically commissions the community needs assessment to provide information to the healthcare needs of the community. The latest community needs assessment was done in 2012. The hospital also participates in community building and community healthcare advocacy groups to help identify needs and coordinate efforts to provide quality healthcare services to the community. The hospital participates in local health fairs and operates a mobile medical van that goes to schools providing immunizations and school physicals. From its work the van identified that there was a lack of dental services for children. The hospital has opened a dental clinic geared towards pediatric patients to fill that need. In 2010 the Hospital opened a Women's Wellness Clinic to provide free healthcare services to uninsured women of child bearing age.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceSigns are posted in the waiting areas of the hospital as well as the hospital website announcing the availability of charity care. Hospital Patient Access staff assist uninsured or underinsured patients in applying for any governmental assistance programs they may qualify for as well as the hospital's charity care program. Hospital bills to patients also have information on applying for financial assistance.
Schedule H, Part VI, Line 4 Community informationThe urban communities surrounding the Hospital from which the Hospital draws most of its patients are some of the poorest communities on the South Side of Chicago. Within the last 30 years, they have continued to lose population and have seen housing abandonment and commercial disinvestment grow. High rates of unemployment have had a disastrous effect on the social fabric necessary to maintain a stable community. People who have choices often leave and move to neighborhoods with greater amenities, leaving behind those with fewer options. Drugs and violent crime are rampant while 35% of the population lives below the federal poverty guidelines. Sixty eight percent of the patients the hospital serves are on Medicaid, while 15% are self-pay, mostly with no payment source. The area is a federally designated underserved area. The hospital serves over 750,000 patients in its primary and secondary service areas. There are no other hospitals in its primary service area, but there are three other hospitals located in its secondary service
Schedule H, Part VI, Line 5 Promotion of community healthSt. Bernard Hospital is a 210-bed Catholic community hospital founded in 1904 by the Religious Hospitallers of St. Joseph under the sponsorship of Catholic Health International. The Sisters of the Religious Hospitallers' enduring commitment to the hospital and the Englewood community for 110 years epitomizes St. Bernard's mission: to care for the sick and promote the health of South Side Chicago residents, while sustaining values of respect, dignity, caring and compassion for all persons. The hospital operates 210 licensed beds including medical/surgical, obstetrics, pediatrics, intensive care, emergency medicine, inpatient mental health, and medical detox services. The Englewood community, although challenged by the impacts of economic hardship, looks to St. Bernard Hospital as an institutional anchor. It is the community's single largest employer, employing more than 900 people. Over 45% of its employees live in the Hospital's primary and secondary service area. The majority of it's Board of Trustee members live in its primary or secondary service area. It maintains an open medical staff of 168 members and extends medical staff privileges to all qualified physicians in its community for all specialties. St. Bernard takes a lead role in improving the neighborhood and the lives of its residents by participating in economic development activities, such as building affordable homes in Bernard Place through its Housing Development subsidiary. As the community's major health care facility, the hospital provides services ranging from emergency medicine and obstetrics to mental health and substance abuse recovery. In keeping with modern demands, St. Bernard is proud to provide state-of-the-art technological capabilities that greatly enhance the hospital's high quality of care and promote patient safety. Of the more than 85,718 patients seen at St. Bernard Hospital each year, close to seventy-four percent receive Medicaid or Medicare as coverage for their health care. The hospital provides more than $16.9 million in charity care each year, or 9.9% of total patient revenue. Community outreach addresses unmet health care needs of children through our baby immunization tracking service, Pediatric Mobile Health Unit, and on-site Dental Center. As a setting for education, St. Bernard is a vibrant clinical training classroom for medical and nursing students, including Emergency Medicine Residents, Surgery, OB, Psychiatric medical students, and nursing assistant and x-ray tech students. St. Bernard remains a constant, driving force helping to strengthen the foundations of the community by delivering high quality, comprehensive health care services, developing affordable housing, and fostering opportunities for economic growth.
