LINE 3JSpecific conditions and groups were not addressed individually. However, our assessment did identify several conditions that required a higher rate of hospitalization among select communities experiencing health disparities. These communities have a higher rate of uninsured persons, low-income persons and minority groups. Charts illustrating the differences in health are in the assessment.
LINES 5, 6BInformation was collected through the use of a uniform survey that was distributed widely through communities in Nassau County to help ascertain the health perceptions and key concerns of residents, as well as identify service needs and barriers to care. It reached a broad spectrum of community residents from many different zip codes. Responses were analyzed according to two separate populations: Nassau County in its entirety and communities with health disparities. Key informant interviews were also conducted with Nassau County's health and human services providers. Emphasis was placed on groups that addressed one or more of the following populations: minorities/medically underserved, seniors, women's/children's services, special populations and individuals with disease-specific conditions. These organizations and their representatives included: 1. American Cancer Society, Syosset, NY - Director of Patient and Family Services 2. American Diabetes Association, Melville, NY - Director, Long Island Operations 3. American Heart Association, Plainview, NY - Regional Vice President, Field Operations 4. Catholic Charities, Hicksville, NY - Director of Development and Communications; Developer, Information and Referral 5. Circulo de la Hispanidad, Long Beach, NY - Executive Director; Chief Director of Services and Operations 6. FEGS Health & Human Services (Federation of Employment & Guidance Service System), Hempstead, NY - Senior Director; Director of HIV Services; Vice President, Long Island Operations 7. Health and Welfare Council of Long Island, Hempstead, NY - President and CEO 8. Island Harvest, Mineola, NY - SNAP Outreach Coordinator 9. Jewish Association Serving the Aging (JASA), Long Beach, NY - Director, Long Beach Services 10. LI GLBT (Gay, Lesbian, Bisexual, Transgender), Garden City, NY - Chief Operating Officer and Director of Development for Long Island 11. Long Island Council of Churches, Hempstead, NY - Director; Nassau County Coordinator 12. Mental Health Association of Nassau County, Glen Cove, NY - Director of Special Projects 13. Perinatal Services Network, Uniondale, NY - Coordinator 14. Planned Parenthood, Hempstead, Massapequa and Glen Cove, NY - Senior Vice President for Health Services, Planned Parenthood of Nassau County 15. Project Independence, Roslyn Heights, NY - Deputy Commissioner; Senior Citizen Program Development Specialist 16. Sustainable Long Island, Farmingdale, NY - Executive Director 17. United Way, Deer Park, NY - Senior Vice President
LINE 6AWinthrop conducted the CHNA in collaboration with Catholic Health Services, Long Beach Memorial Hospital, North Shore - LIJ Health System, and South Nassau Communities Hospital.
LINE 7DIn addition to listing the CHNA on the hospital's website and making a hard copy available upon request, Winthrop also included an article about the CHNA report in its Cornerstone publication that was mailed out to more than 100,000 community residents.
LINE 11As stated in the CHNA, the most pressing needs of the community identified are reducing obesity and the prevention and management of chronic disease (diabetes, asthma, cardiovascular disease and stroke, cancer). Winthrop offers educational programs, screening, and community outreach programs to address these needs. Mental Health & Substance Abuse were also identified as a significant health problem by our community. We have a referral system in place and will address the issue through public education efforts.
LINE 13BWinthrop extends discounts for financial assistance to patients based upon FPG levels for family size and household income up to 500% of the FPG.
LINE 22DWinthrop follows New York State guidelines to determine amounts that can be charged to FAP-eligible individuals by reviewing its Medicare, Medicaid, and its commercial insurance payor rates then applying the lowest rates to its service categories. The New York State guidelines are similar to Internal Revenue Code Section 501(r).

PART I, LINE 7The Hospital utilized a cost-to-charge ratio derived from Worksheet 2, Ratio of Patient Care Cost-to-Charges, of Form 990.
LINE I, LINE 7GRenal Dialysis, High School Clinic, Wound care program, E.R., Home Health Program, Long-Term Home Care Program, NICU, and Diabetes Education

PART III, LINE 2The total bad debt expense shown on this line represents actual amounts written off during the year related to balances due on patients' accounts for which payment was expected and pursued by the Hospital and/or its collection agencies but never received.
PART III, LINE 3The Hospital utilizes a process to identify amounts that are in a "bad debt" status but could qualify for financial assistance under the Hospital's financial assistance policy. These accounts are removed from bad debt and reviewed. If the criteria for financial assistance are met then they are reclassified from bad debt. These amounts are included in Part I of Schedule H.
PART III, LINE 4Per page 11 of the Audited Financial Statements, the Hospital presents the provision for bad debt expense, net totaling $26,311,737 and $21,918,874 for the years ended December 31, 2014 and 2013, respectively, as a reduction of net patient service revenue in the consolidated statement of activities. For the Form 990 bad debt expense has been reclassified from revenues to expenses.
PART III, LINE 8Medicare cost and charge information was extracted from the Medicare cost report and an RCC methodology was applied. The short fall represents unreimbursed costs incurred in providing services to Medicare recipients and therefore should be treated as a community benefit.

SCHEDULE J, PART ILINE 4A Garry Schwall separated from active employment on September 5, 2014. His compensation and certain benefits were continued through the end of the reporting period in exchange for certain service availability, non-compete, non-solicitation and other obligations and promises. LINE 4B THE ORGANIZATION ALSO MAINTAINS A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN (SERP) FOR THE SENIOR AND LONG-TERM EXECUTIVES OF THE ORGANIZATION, INCLUDING MR. COLLINS. THIS PLAN SEEKS TO PROVIDE PARTIAL REPLACEMENT INCOME AT RETIREMENT AFTER 20 YEARS SERVICE. THE BENEFITS UNDER THIS PLAN ARE FORFEITABLE IN THE EVENT OF VOLUNTARY TERMINATION BEFORE RETIREMENT AGE. THE ORGANIZATION'S OUTSIDE COMPENSATION CONSULTANT HAS OPINED THAT THIS PLAN IS WITHIN MARKET NORMS. PLEASE NOTE THAT WINTHROP WAS REQUIRED TO RECOGNIZE ADDITIONAL EXPENSE IN RESPECT OF PARTICIPANTS' BENEFITS UNDER THE SERP FOR 2014. THE PRIMARY FACTOR IN THIS ADDITIONAL EXPENSE IS HISTORICALLY LOW INTEREST RATES USED BY WINTHROP TO RECOGNIZE LIABILITIES ACCRUED UNDER THE SERP ON ITS FINANCIAL STATEMENTS. WINTHROP ADOPTED A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN ("SERP II") FOR 2 EXECUTIVES WHO WERE NOT ELIGIBLE UNDER THE SERP DESCRIBED IN THE PREVIOUS PARAGRAPH BUT WHO WERE NEGATIVELY IMPACTED BY LIMITS UNDER WINTHROP'S STANDARD RETIREMENT PLANS (LIMITS NOT APPLICABLE TO EMPLOYEES GENERALLY). THE PRIMARY PURPOSES OF SERP II ARE TO PROVIDE A SUPPLEMENT TO THE STANDARD RETIREMENT PLAN AND TO ENCOURAGE RETENTION. SERP II COVERS MS. CATALIOTTI AND MS. GAFFNEY AND THE 2014 CONTRIBUTION AMOUNTS FOR THEM WERE $60,022 AND $76,173, RESPECTIVELY. SERP II IS FORFEITABLE BY THESE EXECUTIVES IF THEY TERMINATE EMPLOYMENT BEFORE DECEMBER 31, 2021 (AND ADDITIONAL SERVICE REQUIREMENTS MAY APPLY).
schedule j, Part I, Line 7THE ORGANIZATION MAINTAINS AN INCENTIVE COMPENSATION PROGRAM THAT PAYS VARIABLE COMPENSATION BASED ON THE SATISFACTION BY THE ORGANIZATION AND THE EXECUTIVE, IF AND TO THE EXTENT SATISFIED, OF PRE-ESTABLISED QUALITY OF CARE, PROGRAM, FINANCIAL AND SIMILAR GOALS ESTABLISHED AT THE BEGINNING OF THE YEAR PRIOR TO PAYMENT. THE INCENTIVE COMPENSATION PROGRAM AWARDS ARE MADE BY THE COMPENSATION COMMITTEE IN ITS DISCRETION.
schedule j, PART IIPAUL HARNICK, M.D. AND JOHN F. ALOIA, M.D. ARE EMPLOYED PHYSICIANS OF THE ORGANIZATION AND ALSO SERVE AS UNCOMPENSATED MEMBERS OF THE BOARD OF DIRECTORS. ANTHONY GAMBINO, M.D., PHILIP RAGNO, M.D., AND RICHARD SCHWARTZ, M.D. ARE INCLUDED ON THE HOSPITAL'S FORM 990 DUE TO THE CONTROL RELATIONSHIP FORMED WITH ISLAND WIDE MEDICAL ASSOCIATES, PC. MISSION STATEMENTIt is the mission of Winthrop-University Hospital (referred to as "Winthrop" or "the Hospital") to provide high-quality, safe, culturally competent, and comprehensive healthcare services in a teaching and research environment, which improve the health and well-being of the residents of Nassau County and contiguous county areas based on a profound commitment to an enduring guiding principle - "Your Health Means Everything." The Hospital's Community Service Plan is guided by and reflects its mission statement - to improve the health and well-being of the residents it serves.
COMMUNITY PARTNERSHIPSWinthrop remains engaged with the community by maintaining relationships with community- and faith-based organizations, private organizations, local school districts, and legislators from the village, town, county, state and federal government. Our partnerships have been strengthened during 2014 due to our current CSP initiatives. HEALTH INITATIVES SUPPORTING THE NYS PREVENTION AGENDA During 2014, Winthrop added two per diem community health nurses to the outreach team, one of whom is bilingual, to help expand the availability of our programs. Programs are being conducted, evaluated and modified if needed. OBESITY - NUTRITION AND HEALTHY WEIGHT GOAL: Expand the role of health care and health service providers and insurers in obesity prevention. OBESITY SCREENINGS IN PRIMARY CARE PROVIDER OFFICES Strategy: 5-2-1-0 Program 5-2-1-0 is a healthy lifestyle plan - five servings of fruits and vegetables, no more than two hours of screen time (including TV and IPADS), one hour of exercise and zero sugary drinks. The objective is to teach children the basics of a healthy lifestyle; the intended goal is to achieve a long-term positive impact on their health. Through this program, primary care providers are encouraged to consistently document Body Mass Index (BMI), provide lifestyle counseling, and develop individual care plans and follow-up measures. The identification and counseling of children who are obese (BMI =/>95%) was implemented in 2013 in the WUH Hempstead Pediatric Clinic, an area suffering health disparities. Results for 2014 are as follows: 2014 - Counseling Documentation rate - 96%; up 17% from 2013 2014 - Follow-up rate with parents - 30%; 14% increase from 2013. PREVENT CHILDHOOD OBESITY THROUGH EARLY CHILD-CARE AND SCHOOLS. CHILDHOOD OBESITY SCREENINGS IN THE COMMUNITY Strategy: Bring the 5-2-1-0 Program to the Community Winthrop is collaborating with the Head Start communities located in Hempstead and Westbury to document BMI's, and provide nutritional counseling for families of children identified as having an unhealthy weight. The healthy lifestyle program was introduced at Head Start parent meetings. Moving forward, plans include providing nutritional counseling and support to parents of children who are identified as having BMIs greater than 95%ile. PROMOTE EXCLUSIVE BREASTFEEDING BABY FRIENDLY DESIGNATION Strategy: - Achieve Baby Friendly Designation; improve percentage of newborns who only receive breast milk when discharged from the Hospital. In 2014, Winthrop was awarded Baby-Friendly Designation by Baby-Friendly, USA. Winthrop is the first and only hospital on Long Island to achieve this distinct honor (as of December, 2014). Year 2014 - 45% of babies were only receiving breast milk when discharged from Winthrop, an increase of 5% from 2013 PREVENTING CHRONIC DISEASE - DIABETES PROMOTE EVIDENCED-BASED CARE A. NATIONAL DIABETES PREVENTION PROGRAM Strategy: The National Diabetes Prevention Program is a 16-week core program, followed by a once-a-month post-core program for eight months. Winthrop offered three core program groups and one post-core program during 2014. Sixty-three people enrolled and 50 (83%) completed the core program; all who completed enrolled for the post core program for eight months. During the fall of 2014, 46 people started the program - a total of 106 participants. B. PROMOTE CULTURALLY RELEVANT SELF-MANAGEMENT DIABETES PREVENTION AND DIABETES EDUCATION TO SOUTH ASIAN INDIAN COMMUNITY Strategy: The South Asian Indian community has a high prevalence of diabetes. Winthrop is targeting this population through community outreach to provide education about diabetes and diabetes prevention. In 2015, Winthrop reached out and provided a lecture to 250 individuals in the Asian Indian community in Hicksville. At-risk individuals were provided with educational materials. NUTRITION AND HEALTHY WEIGHT (OBESITY) -CHRONIC CONDITIONS PROMOTE CHRONIC DISEASE SELF-MANAGEMENT AND PREVENT OBESITY Strategy: Provide "Active Living" classes - A four-part free-of-charge series that addresses chronic condition management and the benefits of a healthy lifestyle. During 2014, Winthrop implemented three pilot sessions at the Hospital's Welcome Center; 63 people attended three sessions.
SERVING COMMUNITIES WITH HEALTH DISPARITIES IMPROVING ACCESS TO CAREWinthrop's Women's Wellness OB/GYN Center in Hempstead offers a wide variety of obstetrical and gynecological services, as well as on-site risk assessment, psychosocial counseling, postpartum depression screening and referrals, domestic and sexual abuse screening, nutritional and gestational diabetes education, assessment of living conditions and cultural diversity. The site features a Medicaid Enrollment Specialist for uninsured patients; Charity Care Application and self-pay rates are available as well. They participate with NYS Medicaid and many Medicaid Managed Care HMO plans. During 2014, the Center averaged 1327 visits per month with a total of 15,900 encounters. This is an increase of 6% from 2013. Financial services were provided to over 634 families during this time period. The Hempstead Pediatric Practice draws patients primarily from the Hempstead community but also serves children, from newborn to age 21, from the surrounding neighborhoods experiencing health disparities (Elmont, Freeport, Uniondale and Roosevelt). In addition, Winthrop's Pediatric Specialty Center in Mineola sends a Pediatric Pulmonologist and a Genetic Counselor to the Hempstead location on a regular basis to allow for easier access to these services. In 2014 alone, a total of 16,473 patients were treated at the facility, an increase of 14.4% from 2013. The Hempstead High School Health Center (HHHC) is a school-based Health Center, in a community with health disparities. During 2014, there were over 4000 visits to the Center by approximately 1,230 students. Comprehensive preventive, episodic and confidential healthcare was provided. Winthrop includes specific programs needed by the school's population, including: - Youth support groups in English and Spanish that address common adolescent issues such as acculturation, adolescent adjustment, alternative lifestyles, teen pregnancy and prevention, parenting teens, and violence prevention, including gang violence. - An Asthma Management Program that monitors and tracks students with asthma. This effort is particularly significant in Hempstead, where asthma rates are nearly double those of adjacent neighborhoods. Both staff nurses in the HHHC are certified asthma educators. Collaboration with primary care physicians in the community facilitates treatment that meets current medical standards, trains high school nurses and staff about asthma, offers access to medications and assists with obtaining health insurance for uninsured students. - An "Oral Health" Program that assesses dental needs and applies fluoride varnish.

ExplanationIn 2014, there was restructuring of the arrangement between the hospital and the physician practice entities that provide the services of their physicians to the hospital. The hospital formed the Physician Administrative Alliance (PAA) in order to centralize certain management functions of the practices. The PAA is headed by the Vice President of Network Development and is assisted by two physicians, one to represent the faculty and one to represent the community acquired physicians. The PAA responsibilities are to provide management development services such as business plan development and financial decision making. As a result of this restructuring, the hospital technically acquired control of the physician practices for Form 990 reporting purposes in 2014 and the physician practices thus became related organizations of the hospital.ExplanationTxt
