DescCommunity Care - In keeping with PAMC's commitment to serve all members of its community, the medical center also provided a wide range of activities which included wellness programs, community education programs, and special programs for the elderly, handicapped or medically under-served, along with a variety of broad based community support programs. Most of these programs are offered for the cost of materials and professional services only, or at no fee at all as a community service. PAMC's wellness department reflects the shift in healthcare focus from treatment to prevention. The concept of wellness is to make sick people well and to keep well people from getting sick. Research is proving that exercise and proper diet can help reverse many chronic diseases and reduce the use of medication in many illnesses. Health talks have been given to schools, industries, social and civic groups. Health information, counseling, and audiovisuals have been provided to the community.Desc

Parkview Adventist Medical CenterPart V, Section B, Line 5: Community input in 2010, in a collaborated effort between the University of Southern Maine and Market Decisions, Inc., as assessment was conducted and designed to identify the most important health issues in the state of Maine, both overall and by county, using scientifically valid health indicators and comparative information. The assessment identified priority health issues for better integration of public quality and cost effectiveness of services to residents of Maine. The project represents Maine's efforts to share information that can lead to improved health status and quality of care available to Maine residents, while building upon and strengthening Maine's existing infrastructure of services and providers. The data used for determining the community health needs of Cumberland County was primarily obtained from the Onemaine community health needs assessment (CHNA). Additionally, Eastern Maine Health System, MaineGeneral Health, MaineHealth Systems, The University of New England Center for Health Planning, Policy and Research (CHPPR) and the University of Southern Maine's Muskie School for Public Health Expertise used a methodology developed by CHPPR over decades of work. The assessment integrates primary data from a telephone survey to heads of households with secondary data retrieved from state databases (ED usage, Mortality, Cancer Registry, Etc). That data is reviewed in the context of multiple health related domains to develop a composite view of health status, behavioral risks and barriers to access and care. Results were compared to national and state benchmarks to produce priorities.
Parkview Adventist Medical CenterPart V, Section B, Line 11: The identification of the issues of the community health needs assessment reveals a deficit in services to counter both adult and youth health risk behaviors. Additionally, the assessment outlines that nutrition and physical activity being the primary determinants of obesity, need to be encouraged both in schools and at home to prevent diabetes. Smoking is indiciated in many preventable deaths, and most smokers start in their teens. Similarly, alcohol and drug use behaviors are established early in life, usually before the legal age. These risk behaviors have been identified in clusters. Trends in high risk behaviors, including Cumberland and Sagadahoc, show individuals with high rates of one risk behavior often have high rates of others. Please note that the Organization filed for bankruptcy in 2015; therefore it no longer maintains an active website.

Part II, Community Building Activities:Parkview's Community Outreach was strong in 2014, reaching out to all ages in many.PARKVIEW COUCH to 5K ($4,500. In freebies given away)In 2014, PAMC was one of the major sponsors of Music on the Mall in Brunswick. We created a first of its kind Parkview Couch to 5k as a way to reach more people and to encourage the whole community to get out and exercise. Beginning in February, a trainer we hired began holding once weekly warm up sessions for as many people in the community who wanted to be part of this. Over the next 12 weeks, she trained them to cover more distance as they worked hard to reach their goal of literally getting off the couch after a long winter and walking or running a distance of 5k (3.2 miles). More than 65 people took part in the training session. The first event was at the end of May on the first night of Music on the Mall; the second one was on the final evening of the Music on the Mall at the end of August. Each participant was given a Couch to 5K t-shirt for participating, plus fruit, granola and water. Additionally, we handed out PAMC branded items at each of the summer's Music on the Mall events which promoted exercise and healthy living. Multiple Health Fairs ($1,200. in freebies given away) -Brunswick High School Bath-American Vets at Knights of Columbus, Brunswick-Senior Expo in Freeport-Portland Health Expo Healthy Maine Streets ($20 in freebies given away)-In 2014, Parkview became an active member of the Healthy Maine Streets initiative. Part III, Line 2:The cost to charge ratio calculated on IRS worksheet 2 was used in the calculation of cost on IRS worksheet 1 and 3.
Part III, Line 4:The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable for services rendered at net realizable amounts from third-party payers, patients and others. The medical center provides an allowance for uncollectible accounts based upon a review of outstanding receivables, historical collection information and existing economic conditions. As a service to the patient, the Medical Center bills third-party payers directly and bills the patient when the patient's liability is determined. Patient accounts receivable are due in full when billed. Accounts are considered delinquent and subsequently written off as bad debts based on individual credit evaluation and specific circumstances of the account. The Medical Center grants credit without collateral to patients, most of whom are local residents and are insured under third-party agreements. Additions to the allowance for uncollectible accounts are made by means of the provision for uncollectible accounts. Accounts written off as uncollectible are deducted from the allowance and subsequent recoveries are added. The amount of the provisions for uncollectible accounts is based upon management's assessment of historical and expected net collections, business and economic conditions, trends in federal and state governmental healthcare coverage, and other collection indicators. The Medical Center determines the actual write-off of patient accounts after a reasonable amount of time has passed since the first billing (at least 180 days) and reasonable collection efforts made. The estimate of bad debt expense attributable to patients eligible under the Medical Center's charity care policy is based on a 1 for 1 basis. In other words, for every patient that applies and is approved for charity care, there is another patient that doesn't apply, but would be approved.
Part III, Line 8:Serving patients with governmental health benefits, such as medicare, is a component of the community benefit standard that tax-exempt hospitals are held to. This implies that serving medicare patients is a community benefit and that the hospital operates to promote that health of the community. The hospital is reimbursed under PPS for medicare beneficiaries.
Part III, Line 9b:Because our charity care policy allows for a percentage-based charity approval on their income compared to the federal poverty level, some of those accounts that qualify for less than 100% write-off, might still, have outstanding balances. Those outstanding balances are subject to the same collection policy procedures as if they did not qualify for charity care.
Part VI, Line 2:See Part V, Section B
Part VI, Line 3:Patient education of eligibility for assistance: When a patient is registered, this information is discussed and the policies and paperwork are made available to the patient so they know we have these programs in place. We also reiterate these programs if questioned by a patient during the billing process.
Part VI, Line 4:Brunswick is located in the northernmost pocket of Cumberland County, Maine, and is considered part of the Portland-South Portland-Biddeford metropolitan area. According to the 2011 census, Brunswick has 14,500 residents of which 91% are white, 2.8% asian, and 2.3% two or more races. Median income is $42,654 per household and is on par with the average for the county. Many of Parkview's patients live just over the county border in Sagadahoc County, with a population of 35,200, a poverty rate of 10.5%, and median income of $52,071.
Part VI, Line 5:Parkview Adventist Medical Center is engaged in a number of activities and programs, which are of benefit to the community.Parkview is well known as the place/hospital to go to for evidence-based health and wellness information. In 2010, the Community Health Information Program (CHIP) was created. It began a partnership between Parkview and the local library in Brunswick (Curtis Memorial). Providing books, DVDs, computer programs and more to the local community, the CHIP program also helps to provide expanded collections at 11 other, smaller libraries in the state through a grant program. (Parkview contribution: $2,000)Another innovative program that Parkview offers FREE to the community and the whole country is the Healthy Living television series. Hosted by Sheryl S. McWilliams and featuring local physicians and health care professionals (repeat co-host Dr. Timothy Howe, MD), Healthy Living is a 30-minute health program that specifically focuses on lifestyle measures and daily choices that affect a person's well-being, their risk of disease and/or ways to reduce risk, reverse symptoms. The program is available on PEG media at no charge and is downloaded by television stations around the country and the globe. Twelve shows are recorded each year. (Parkview contribution: $500)A program started at Parkview and now attended by local residents and folks from around the country is Life Style Choices. This is a comprehensive 2-week reconditioning program for adults who have or who are at risk of developing some sort of chronic disease. The average participant will experience reductions in cholesterol, blood sugar, weight, triglycerides and blood pressure. In some instances, participants with Type 2 Diabetes have experienced a reversal of their disease. Dr. Timothy Howe, MD plays an active role in LSC; along with Sheryl S. McWilliams, R.N. (PAMC Wellness Director), Daniel Lopez, (Physical Therapy), Stephanie Homon (PAMC Nutritional Services) and Alice Willard-Michaels (PAMC Diabetes Educator), as well as guest speakers including Mary Penner, NP, Lyn Howe.In 2014, Life Style Choices was offered 4 times throughout the year and the groups were near-capacity. Brunswick Times Record Publisher/Advertising Director Larry Hubner attended the class in 2014 and spotlighted the Life Style Choices program on-line and in spacious ads. In addition, these FREE programs are offered throughout the year:Vegetarian Cooking Classes: given by Dr. Timothy Howe and his wife, Lyn; and Mary Penner, NP.Living Well Class: This program is aimed at the older adult who has a chronic disease in need of better management. It uses a group discussion with facilitation.Breathe Free: This is a 9-session tobacco cessation program that is taught by Dennis Farley, Cardio-Pulmonary Technician. Farley was awarded a special, Gold-Star award in 2014 from the Maine Tobacco Free Hospital Network for his tireless efforts in helping community members and PAMC employees to kick the habit! Living with Diabetes: This FREE series of talks throughout the year is given by Diabetes Educator Alice Willard-Michaels. She gives the community the opportunity to learn different methods to deal with their different levels of Diabetes.
