PART V, SECTION B, LINE 11Four main topics surfaced from the CHNA that BayCare Alliant Hospital is addressing. All four topics have action plans that are either implemented or are being currently addressed. 1) Access to Necessary Medical Care The hospital has established a formal fundraising strategy and an annual fundraising goal with respect to the hospital's charity fund. The goal of the charity fund is to provide philanthropic support for the continued improvement of the health of its patients and community. In particular, the fund seeks to provide support to patients who are uninsured or financially compromised and who need continued medical treatment upon their return to the community. The amount of funds raised and the number of patients assisted is tracked on a quarterly basis. During FY 2014, the hospital was successful in increasing available funds by 19.0%. Numerous needy patients were assisted with post-discharge expenditures such as utilities to ensure a safe discharge, rent payments, pharmaceuticals, and dialysis costs. The hospital continues to remain as one of the leading long-term acute care hospitals in Florida with respect to providing care to the underinsured. In FY 2014, 7.9% of the hospital's patient days were for underinsured patients. The hospital is increasing access to necessary medical care through the construction of a 6-bed ICU unit. The 6-bed ICU has been formally approved by the hospital's Board of Director's with a construction start date of October 1, 2015. The construction plan includes installation of the necessary components required to establish an E-ICU. Patients residing in the ICU will often be receiving drips such as levophed, dopamine, and diprivan. The ICU will provide increased staffing levels and constant monitoring required by these patients leading to gains in patient safety and improved outcomes. 2) Communication and Education In order to increase awareness about programs/services available, the hospital has developed a formal plan with respect to communication and education with specific goals that have either been obtained or being currently addressed. Included in these goals are various educational sessions and symposiums aimed at the medical community. The hospital has additionally established goals intended to increase awareness of workers compensation providers, nursing leaders, hospitalists, and referral sources. Lastly, the hospital has embarked upon an aggressive campaign to educate the community about the new 6-bed ICU and the associated benefits. 3) Access to Bedside Dialysis The hospital has increased and improved access to bed side dialysis by paying for a portion of post-discharge dialysis services through letters of agreements for patients who are transferred from the hospital to nursing homes. Additionally, the hospital has hired a Medical Director of Nephrology to assist with the access and quality of bed side dialysis provided at the hospital. In particular, the physician is providing assurance with respect to access, consistency, quality control, and patient safety. 4) Transportation The hospital has continued to provide complimentary transportation services to patients who are financially needy or whose safety may be of concern. In FY 2014, the hospital expanded the hours in which these services are available.
PART V, SECTION B, LINE 13BAN UNINSURED DISCOUNT OF 40% IS AUTOMATICALLY GIVEN TO ANY PATIENT WHO DOES NOT HAVE INSURANCE COVERAGE OR BENEFITS. THERE IS NO INCOME OR ASSET TEST REQUIRED FOR THE UNINSURED DISCOUNT. PATIENTS RECEIVE AN ADDITIONAL 10% DISCOUNT IF THE ACCOUNT IS PAID WITHIN 30 DAYS.
PART V, SECTION B, LINE 16Line 16A https://baycare.org/about-us/financial-assistance/financial-assistance-pol icy Line 16B https://baycare.org/about-us/financial-assistance Line 16C https://baycare.org/about-us/financial-assistance
PART V, SECTION B LINE 18DLIEN ACTION RELATED TO COLLECTIONS IS LIMITED TO PATIENTS INVOLVING AUTO LIABILITY INSURANCE.
PART V, SECTION B, LINE 20EEVERY PATIENT BILL INCLUDES A PLAIN LANGUAGE SUMMARY OF THE FINANCIAL ASSISTANCE POLICY.
PART V, SECTION B, LINE 22DPATIENTS WHO ARE UNINSURED OR UNDERINSURED AND CANNOT PAY FOR HOSPITAL SERVICES ARE ELIGIBLE FOR CHARITY CONSIDERATION. THESE PATIENTS ARE SCREENED BY DESIGNATED TEAM MEMBERS IN OUR FINANCIAL ASSISTANCE DEPARTMENT. THE AGENCY FOR HEALTH CARE ADMINISTRATION (AHCA) DEFINES CHARITY ELIGIBILITY AT 200 PERCENT OF THE FEDERAL POVERTY GUIDELINES, UNLESS THE TOTAL HOSPITAL BILL IS MORE THAN 25 PERCENT OF THE PATIENT'S ANNUAL INCOME. MEDICAID RECIPIENTS WHO HAVE EXCEEDED THEIR COVERAGE LIMITS ARE ALSO CONSIDERED FOR CHARITY CARE. BAYCARE ALLIANT HOSPITAL, INC GOES ABOVE AND BEYOND THE AHCA REQUIREMENTS BY PROVIDING ADDITIONAL "HARDSHIP" CHARITY FOR PATIENTS WHO ARE AT 250 PERCENT OF THE FEDERAL POVERTY GUIDELINES. ONCE A PATIENT IS IDENTIFIED AS ELIGIBLE FOR FINANCIAL ASSISTANCE HIS/HER ACCOUNT BALANCE IS WRITTEN OFF 100% TO CHARITY WRITE-OFFS AND ARE NO LONGER BILLED FOR SERVICES PROVIDED.

Part I LINE 3CPatients who are uninsured or underinsured and cannot pay for hospital services are eligible for charity consideration. BayCare Alliant Hospital, Inc. goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days.
PART I LINE 7Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program. Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits.
PART I LINE 7 COLUMN FBAD DEBT EXPENSE OF ($124,749) WAS EXCLUDED ON FORM 990, PART IX, LINE 25,COLUMN (A), BUT SUBTRACTED FOR PURPOSES OF CALCULATING THE PERCENTAGE IN THIS COLUMN.
PART III LINES 2, 3, & 4Lines 2 & 3 Bad debt expense is reported as total bad debt for the facility. The amount of bad debt expense attributable to patients eligible for financial assistance is calculated as a charge ratio, derived from data sampling. The resulting charge ratio is then applied to total bad debt accounts of the organization, which calculates the bad debt attributable to finance assistance. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid. Line 4 The organization's financial statements include a footnote that describes bad debt expense on page 6 of the Baycare Alliant Hospital, Inc. Notes to Consolidated Financial Statements.
PART III LINE 8Cost reports were used to report Medicare allowable costs. Medicare defines allowable costs as those appropriate and helpful in developing and maintaining the operation of patient care facilities and activities. It specifically excludes certain costs that are not directly related to patient care. The hospital incurs additional expense related to the provision of care to Medicare patients that Medicare has deemed non-allowable. This additional expense includes costs of physician services (emergency on-call fees, hospitalist program, recruitment, etc.), advertising costs, cafeteria costs for meals sold to visitors, etc. The hospital attempts to collect coinsurance and deductibles from Medicare beneficiaries. To the extent collection efforts are unsuccessful, Medicare reimburses the hospital at 70% of unpaid amounts. The following table reconciles the surplus or shortfall from Line 7 to the actual surplus or shortfall. The additional costs were allocated to Medicare based upon Medicare's percentage of total allowable costs. The unpaid coinsurance/deductibles were estimated using historical collection results. Line 7 Surplus or (Shortfall)$625,156 Additional non-allowable costs and unpaid/non-reimbursed coinsurance/deductibles ($96,631) Total Surplus or (Shortfall) $528,525
PART III LINE 9BPatients who are unable to pay are encouraged by BayCare Health System representatives, via personal interviews, signage, on patient billing statements, brochures or Customer Service phone calls, to submit financial information to the Financial Assistance Department to determine eligibility for programs, such as County, Medicaid, Disability, Victims of Crime, Charity, etc. For those patients who provide all the necessary documentation and qualify for charity according to the Financial Assistance policy, (defined in Part I, line 3c), patients' account balance would be written off completely to charity and not billed to the patients.
PART VI LINE 2 - NEEDS ASSESSMENTBayCare Alliant Hospital, Inc is committed to meeting the needs of the community it serves. Our quality philosophy is modeled around understanding our customers' needs in the communities it serves. BayCare Alliant Hospital, Inc addresses community health status assessments by accessing existing third party databases profiling health status information for geographies it serves. The assessments provide a profile of health status indicators in comparison to state averages and, if available, national benchmarks. In addition BayCare Alliant Hospital, Inc conducts physician community need studies that outline physician deficits by specialty for the geographic area served. Studies are also conducted to identify gaps in geographic access to services such as primary care, outpatient services and inpatient services. All of the above processes occur on an ongoing basis to assist BayCare Alliant Hospital, Inc in developing initiatives and programs/services to address identified health care needs in the communities it serves.
Part VI Line 3 - Patient Education of Eligibility for AssistanceBayCare Alliant Hospital, Inc Financial Assistance team members are dedicated to assisting patients in obtaining assistance through federal, state and local government programs or through the BayCare Alliant Hospital, Inc financial assistance policy. Signage and brochures are available, as well as team members whose full responsibility is to assist patients on inpatient units. The Financial Assistance team interviews patients for all available programs, assists the patients in completing applications to government agencies and for hospital charity care, advises patients regarding available community resources for health care, reviews and approves patient requests for charity care, and provides education and support to the patient throughout the assistance process. In addition to the aforementioned comprehensive process BayCare Alliant Hospital, Inc also informs and educates patients who may be billed for patient care, but may be eligible for charity or other programs, via patient billing mailers and customer service representative calls. The goal in using these various means is to effectively communicate with the entire patient population so they are informed and educated about their eligibility for assistance.
Part VI Line 4 - Community InformationBayCare Alliant Hospital is in a suburban setting serving parts of Pasco and Pinellas counties. The average income is lower than both the state and national averages. The population served is predominantly Caucasian and high-school or higher educated. Hispanics are the second largest ethnic group representing 10.4% of the population. 14.5% of households have annual household income below $15,000 per year. BayCare Alliant Hospital is a long-term acute care hospital connected with the BayCare Health System that serves west central Florida. The area served by BayCare Alliant Hospital has 12 hospitals with 4 being Not-for Profit. There is 1 federally designated medically underserved area and four medically underserved populations in BayCare Alliant Hospital's service area. With the service area expanding and the over 65 population expected to grow 13.5% in the next five years, the health care needs of our service area are expanding and changing. The population served by BayCare Alliant Hospital is expected to grow 3.2% in the next 5 years. This expected growth is slightly lower than the expected growth rate for the United States (3.5%). However, the population Age 55+ is expected to grow 11% in the next 5 years. Based on Florida inpatient discharge data for the period of 10/01/13-09/30/14, the payor mix for the geographic area consists of 56.8% Medicare/Medicare HMO, 13.7% Medicaid/Medicaid HMO, 19.8% Commercial Insurance, 7.3% Self-pay, and 3.3% Other.
Part VI Line 5 - Promotion of Community HealthBayCare Alliant Hospital, Inc. is a 48-bed, long-term acute care hospital (LTCH) affiliated with BayCare. It is the only facility of its kind located within a 20 mile radius of Dunedin, Florida. The 27,253-square-foot facility provides diagnostic, medical treatment and rehabilitation for patients experiencing complex medical conditions due to chronic disease or injuries. The hospital takes a multi-disciplinary, family-centered approach with physicians, nursing and other health care specialists working together toward the goal of the best possible recovery for patients. BayCare Alliant is located inside Mease Dunedin Hospital. This proximity enables BayCare Alliant to provide the community with access to high quality patient care and minimizes patient transportation. Long-term hospitals located within general acute care hospitals are often referred to as "hospitals within a hospital" or "HIH's". A typical stay at a long-term acute care hospital is 27.25 days. The board is made up of community members who want to provide the community with access to compassionate patient care of superior quality. The Hospital has provided educational sessions to the physician and clinical community. In addition, the Hospital has lectured to local organizations and sponsored charitable events. There is a critical need for the Hospital given that it is the only LTCH within the vicinity, and given that it serves the most highly acute and chronically ill segment of the community.
Part VI Line 6 - Affiliated Health Care SystemBayCare Alliant Hospital, Inc. is part of BayCare, a leading, not-for-profit health care system that connects individuals and families to a wide range of services at 13 hospitals and hundreds of other convenient locations throughout the Tampa Bay and central Florida regions. Inpatient and outpatient services include acute care, primary care, imaging, laboratory, behavioral health, home care and wellness. BayCare's Mission is to improve the health of all we serve through community-owned, health care services that set the standard for high-quality, compassionate care. On the web: http://www.baycare.org In 2014, BayCare hospitals provided 144,475 discharges and 544,678 emergency room visits. BayCare's network also includes 3,419 licensed beds, an expanding physician network, a comprehensive home health agency, three wellness centers, four ambulatory surgery centers, seven urgent care centers, 13 outpatient imaging facilities, outreach laboratories, retail pharmacies, and two stand-alone, behavioral health centers. BayCare has been developing an overall strategy to address the community's growing needs around post acute and ambulatory services. With a continued focus on establishing the foundation for population health management, BayCare strives to improve the area's access to affordable, high-quality health care. Creating the infrastructure to manage risk and population health is a multi-year initiative that encompasses aligning physicians, acquiring insurance competencies, developing information systems, and expanding care management capabilities. In this regard, BayCare has continued to expand its physician practices to 154 access points with 411 employed physicians. In 2014, its clinically integrated network, BayCare Physician Partners, grew to 943 specialists, 388 primary care physicians and 157,000 covered lives. BayCare also created a new organization, BayCare Physician Partners ACO, LLC, and applied to participate in the Medicare Shared Savings Program. BayCare will further mature its population health initiatives by working with the Centers for Medicare and Medicaid, and managing Medicare beneficiaries in the market. BayCare was founded in 1997 when several of the area's not-for-profit hospitals came together, united by the common Mission to improve the health of all they served. As an integrated health system, BayCare's hospital members believed they could reduce costs by consolidating administrative functions and services, and improve quality by sharing best practices. BayCare is currently one of the largest employers in the region with 23,600 employees and one of the few health systems in the nation with a top-decile, AA2 credit rating.
