| Description | RCCO - REGIONAL CARE COLLABORATIVE ORGANIZATION In 2011, CHP was awarded the Regional Care Collaborative Organization (RCCO) contract by the Colorado Health Care Policy and Financing Department as part of Medicaid reform. When the RCCO project was first announced, local health care leaders (all members of CHP), wanting to maintain local control over health care decisions, responded with a unique community-based approach to building an Accountable Care Organization. Community Care of Central Colorado (aka RCCO Region 7) coordinates care within a patient-centered, medical home model for 60,001 Medicaid clients in El Paso, Elbert, Park and Teller Counties. | Description |
