PART V, LINE 4TO THE ORGANIZATION'S KNOWLEDGE, NO FINAL PURPOSE WAS ESTABLISHED.
Part X, line 2THE FIN 48 FOOTNOTE IN THE AFS READS AS FOLLOWS: The Center was recognized as a public charity generally exempt from federal income taxation under section 501(c)(3) of the Internal Revenue Code pursuant to a determination letter issued by the IRS in July 1997. The Center is entitled to rely on this determination as long as there are no substantial changes in its character, purposes, or methods of operation. Management has concluded that there have been no such changes, and therefore the Center's status as a public charity exempt from federal income taxation remains in effect. The state in which the Center operates also provides general exemption from state income taxation for organizations that are exempt from federal income taxation. However, the Center is subject to both federal and state income taxation at corporate tax rates on its unrelated business income. Exemption from other state taxes, such as real and personal property taxes, is separately determined. The Center had no unrecognized tax benefits or such amounts were immaterial during the periods presented. For tax periods with respect to which no unrelated business income was recognized, no tax return was required. Tax periods for which no return is filed remain open for examination indefinitely. Information returns (Form 990) and tax returns (Form 990-T) for the year ended December 31, 2013 were due on May 15, 2014 and have been extended to August 15, 2014. The Center has filed information returns and tax returns for the tax year 2012. Current accounting standards address uncertain income tax positions in financial statements by prescribing the minimum recognition threshold a tax position is required to meet before being recognized in the financial statements. Management believes there is no impact on the Center's accompanying financial statements related to uncertain income tax positions. IN ADDITION, SUBSEQUENT TO THE COMPILATION OF THE AFS, THE ORGANIZATION FURTHER EXTENDED THE FORM 990 AND FORM 990-T TO NOVEMBER 15, 2014

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3The Hospital for Sick Children dba The HSC Pediatric Center ("HSC") has a financial assistance policy that provides for free or discounted care to individuals that qualify under the organization's financial assistance policy. The level of financial assistance is determined by several factors, including: the individual's annual income, family size, and liquid assets, as well as whether the individual is covered by governmental or other insurance. Free care is provided to patients who are ineligible for governmental or other insurance coverage, and whose family income does not exceed 200% of the Federal Poverty Guidelines. Discounted care is based on a sliding discounted scale of gross charges and is provided to patients who are ineligible for governmental or other insurance coverage and whose family income is between 200% and 500% of the poverty guidelines. When possible, the organization may make a prospective determination that certain patients qualify for free or discounted care, although the patient has not submitted a completed financial assistance policy. Presumptive charity care is determined based on defined criteria for destitute status (homeless, illegal alien, incarcerated, etc.). Once charity care eligibility has been established, the patient account in whole, or in part, is written-off and all collection efforts on the written off portion of the account cease. If the patient receives partial Charity Care, all efforts will be made to place that patient on an extended payment plan. In addition, individuals that do not qualify for financial assistance under the organization's financial assistance policy may still be provided up to a 25% discount on billed services in certain circumstances.
Part I, Line 4HSC has a financial assistance policy that provides for free or discounted care to the medically indigent, which is a separate, supplemental determination of financial assistance for patients that are not eligible for financial assistance under the primary section of the financial assistance policy, but for whom the resulting financial liability for medical treatment represents a catastrophic loss. "Catastrophic" is defined as a situation in which the self-pay portion of the HSC medical bill is greater than the patient's ability to repay with current income and liquid assets in 18 months or less. The factors taken into consideration when evaluating a catastrophic assistance application are the following: current medical debt, liquid assets, living expenses, projected medical expenses, annual income, family size, and spell of illness.
Part I, Line 5HSC does not deny financial assistance to any patient eligible for free or discounted care under its financial assistance policy, regardless of whether or not the financial assistance budget was exceeded.
Part I, Line 7, column (f)Bad debt expense was excluded when determining the percentage of total expense in column (f).
Part I, Line 7(a)HSC experiences very low volumes of charity care for several reasons. First, approximately 91% of the hospital's patient population is composed of Medicaid patients. HSC is a long-term pediatric specialty hospital that provides specialized care to chronically ill children and young adults, which care is not currently provided by other providers in HSC's service area. Second, less than 1% of the hospital's patient population is self-pay and many of theose patients qualify for Medicaid after being admitted to the hospital facility. Third, HSC does not operate an emergency room or provide emergency medical care. In determining the cost of charity care, HSC did not use worksheet 1 to calculate cost reported in Part I, line 7(a) but rather, it used the cost-to-charge ratio calculation of total cost divided by total charges per the 2013 DC Medicaid cost report. The amount of charity care was reported in accordance with the District of Columbia's State Health Planning and Development Agency (SHPDA) requirements that specified health care facilities within the District provide a reasonable volume of uncompensated care through the year. This level was set at 3% of the organizations total operating costs per its audited financial statements, minus the amount of reimbursement under XVIII and XIX of the Social Security Act (i.e., medical revenue). HSC was in full compliance with the SHPDA regulations in 2013.
Part III, Line 4N/A - Due to the composition of its patient population (91% Medicaid), HSC incurred an immaterial amount of bad debt expense during its 2013 tax year. As a result, the organization does not believe that any of its bad debt expense is attributable to patients eligible under the organization's financial assistance policy. The footnote from HSC's financial statements related to bad debt expense reads as follows, "THe center reports net patient service revenue at the estimated net realizable amounts from patients, third party payers, and others for services rendered. Allowances for the excess of charges over anticipated patient or third party payer payments are included in the determination of net patient service revenue as reported in the statements of operations, whereas net uncollectible self-pay amounts are reported as an operating expense. Patient accounts receivable include charges for accounts due from the Maryland, Virginia, and District of Columbia Medical Assistance (Medicaid) programs, Tricare, CareFirst, other commercial and managed care insurers, and self-paying patients. Deducted from patient accounts receivable are estimates of uncollectible accounts related to self-paying patients, and allowances for the excess of charges over the payments to be received from third party payers that pay at less than full charges."
Part III, Section B, Line 8HSC does not participate in the Medicare program.
Part III, Line 9(b)The determination of a patient's qualification under the organization's financial assistance policy is attempted prior to the patient being discharged from the hospital. This may include a prospective determination that certain patients qualify for financial assistance, although the patient has not submitted a completed financial assistance policy. As soon as the patient's financial assistance eligibility has been established, the patient's account in whole or in part, is written-off and all collection efforts cease on the portion of the account that has been written off. If the patient receives discounted care, all efforts will be made to place that patient on an extended payment plan. For those patients that do not initially apply or qualify for financial assistance, the organization continues to monitor whether the patient may qualify for financial assistance throughout the collection process. If the patient is found to be eligible for financial assistance at any point (including once collection efforts have begun), the organization will approve the patient for financial assistance immediately .
Part VHSC is a long-term pediatric specialty hospital.
Part V, Line 3The Community Health Needs Assessment (CHNA) took into account input from representatives of the community served by the hospital facility, including persons with special knowledge of or expertise in public health, as well as leaders and representatives of medically underserved, low-income and minority populations. In particular, representatives from the DC Health Department and Prince George's County Health Department were interviewed, as well as individuals from community groups, such as Family Voices, and an array of physicians and other leaders in the disability and pediatric fields.
Part V, Line 6During the 2013 tax year, HSC adopted an implementation strategy that addresses community health needs identified in the CHNA written report. HSC began to execute the implementation strategy during 2013, including by undertaking the following actions: * After completion of a thorough business needs analysis, an outpatient rehabilitation clinic was opened in Prince George's County, Maryland to provide an improved outpatient delivery setting for patients needing therapy services in Maryland. * The Behavioral Health Center, located in Congress Heights, was further developed by bringing in a number of relevant practitioners, including child psychiatrists and non-physician behavioral rehabilitation specialists. The Center provides needed behavioral health services in an area that is more geographically accessible to a substantial number of our community's special needs children. * Conducted meetings with DC Public Schools ("DCPS") and Adventist Healthcare to begin collaboration efforts to start an early intervention plan at HSC, as well as meetings with autism service providers and DCPS to discuss collaborative efforts and programs for autistic individuals. Thus far, these efforts have not come to fruition due to lack of public funding for autism or early intervention outside of the DCPS school program. * Expanded the June Fair to include more community resources, provide trainings, and provide other information benefitting the special needs populations. * Enhanced the hospital's website to assist parents in becoming more knowledgeable about services provided and community resources available to them, including caregiver education and health management at home. * Met with DC Primary Care Association to help identify approaches to medical home and wrap around services. Volunteered to participate in both the DC and Maryland health information exchange efforts to be able to receive and share data relevant to integrated or coordinated care management.
Part V, Line 7In conducting the CHNA, the hospital determined that there were a number of unmet community health needs affecting improved health outcomes and experiences for our community population that we could not address for reasons given below. * HSCPC cannot directly affect the shortage of specific specialty physicians and other behavioral health providers, and in particular, child psychiatrists, in the community. * Despite successful efforts to overcome unmet need because of insurance coverage, CSHCNs often go without needed services because there are simply not enough providers within close proximity Appropriate institutional care for children age 22 and beyond. * Therapeutic Day Care for children under age 5 is the primary domain of social service and or educational agencies. While HSCPC cannot operate within these constraints currently, we do lend collaboration and expertise at various levels. The establishment of the National Youth Transitions system by the HSC Foundation is intended to address these realities. * The Hospital has limited capacity to implement a Medical Home model given its delivery model. It would also be difficult to facilitate coordinated care beyond what hospital patients needed during admissions/registrations or discharge planning. These issues are better handled by sister organizations HSCSN and the HSC Foundation whose missions more easily encompass these initiatives and stated challenges.
Part V, Section B, Line 12HSC has a financial assistance policy that provides for free or discounted care to individuals that qualify under the organization's financial assistance policy. Patients with sufficient assets to pay for care without becoming medically indigent, may not be eligible for financial assistance. However, the residence where a patient and/or the patient's family resides, automobiles needed to transport all working parties to and from work, savings accounts with less than two months of income and retirement accounts with less than $50,000 are always exempted from consideration as assets in considering whether the patient qualifies for financial assistance. Once charity care eligibility has been established, the patient account in whole or in part, is written-off and all collection efforts on the written off portion of the account cease. If the patient receives partial Charity Care, all efforts will be made to place that patient on an extended payment plan. Furthermore, HSC has a financial assistance policy that provides for free or discounted care to the medically indigent, which is a separate, supplemental determination of financial assistance for patients that are not eligible for financial assistance under the primary section of the financial assistance policy, but for whom the resulting financial liability for medical treatment represents a catastrophic loss.
Part V, Section B, Line 14HSC publicizes its financial assistance policy in many different ways. First, it posts notices regarding the availability of charity care in admissions, patient registration, and the business office. The information on the signs is in both English and Spanish. In addition, the policy is distributed to patients on admission and upon request, and is available at all patient registration desks and in all waiting areas. Furthermore, HSC, as mandated by its financial assistance policy, publishes a summary of its financial assistance policy, on an annual basis, in a newspaper of general circulation in the hospital's primary and secondary service areas.
Part V, Section B, Line 16HSC does not conduct any collection action(s) as specified in this section, until all reasonable efforts to determine a patient(s) eligibility are exhausted.
Part V, Section B, Line 18THE HOSPITAL FACILITY OR AN AUTHORIZED THIRD PARTY DID NOT UNDERTAKE ANY OF THE COLLECTION ACTIONS NOTED IN PART V, SECTION B, LINE 17 BEFORE MAKING REASONABLE EFFORTS TO DETERMINE ANY PATIENTS ELIGIBILITY UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY. IN ORDER TO HELP DETERMINE PATIENTS' ELIGIBILITY UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY, THE HOSPITAL UNDERTAKES A NUMBER OF ACTIONS, INCLUDING NOTIFYING PATIENTS OF THE FINANCIAL ASSISTANCE POLICY UPON ADMISSION AND UPON REQUEST. IN ADDITION, THE HOSPITAL DOCUMENTS ITS DETERMINATION OF WHETHER A PATIENT IS EILIGIBLE FOR FINANCIAL ASSISTANCE UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY.
Part V, Section B, Line 19dHSC is a long-term pediatric specialty hospital. As a result HSC does not operate an emergency room or provide emergency medical care. During 2013, there were not any patients that presented at HSC with symptoms or conditions requiring emergency care.
Part V, Line 20dHSC is a long-term pediatric specialty hospital with a payor mix that is approximately 91% Medicaid and less than 1% self-pay. Furthermore, HSC does not operate an emergency room or provide emergency medical care, so there are almost never walk-in patients to the hospital facility. During HSC's 2013 tax year, the hospital facility did not have any self-pay patients that qualified for discounted care under the hospital facility's financial assistance policy. As a result, the hospital facility was able to confirm that it did not charge any patients that qualified for financial assistance under the organization's financial assistance policy more than the amounts generally billed to individuals who had insurance covering such care, since the only patients that received financial assistance during the year received free care. In the process of preparing HSC's 2013 Form 990, the organization closely reviewed the hospital facility's financial assistance policy and determined that the financial assistance policy should be revised to ensure that, in the future, if there are patients qualifying for discounted care at the hospital facility, they are not charged more than the amounts generally billed to individuals who had insurance covering such care.
Part V, Line 22THE HOSPITAL FACILITY DOES NOT CHARGE ANY INDIVIDUALS THAT IT KNOWS ARE ELIGIBLE FOR FINANCIAL ASSISTANCE AN AMOUNT EQUAL TO THE GROSS CHARGE FOR ANY SERVICE. THE HOSPITAL USES THE CHARGE MASTER RATES FOR A SERVICE AS A STARTING POINT AGAINST WHICH THE DISCOUNTS MANDATED IN THE HOSPITAL FACILITY'S FINANCIAL ASSISTANCE POLICY ARE APPLIED TO DETERMINE THE AMOUNT ACTUALLY BILLED TO PATIENTS ELIGIBLE UNDER THE FINANCIAL ASSISTANCE POLICY. THE HOSPITAL FACILITY WILL NOT COLLECT PAYMENT FROM ANY PATIENT ELIGIBLE UNDER THE FINANCIAL ASSISTANCE POLICY IN EXCESS OF THE REDUCED AMOUNT THAT IS ACTUALLY BILLED TO SUCH FINANCIAL ASSISTANCE PATIENT.
Part VI, Line 2, Needs AssessmentHSC CONDUCTED ITS FIRST COMMUNITY HEALTH NEEDS ASSESSMENT IN ACCORDANCE WITH SECTION 501(R) DURING THE 2013 TAX YEAR.
Part VI, Line 3, Patient education of eligibility for assistanceHSC has maintained a strong commitment to providing services to patients at no cost to them, if at all possible. Many of the patients who receive services from HSC have already qualified for Medicaid by the time of their arrival at HSC, due to prior services at other facilities. If a patient arrives at HSC without Medicaid coverage, HSC will help to refer the patient to the appropriate contractors to help qualify the patient for Medicaid. Often times the patient will qualify for Medicaid by the time they are discharged to care at home. HSC attempts to inform patients and persons who are billed for patient care about its financial assistance policy in many different ways. First, it posts the policy within several areas of the Hospital that are generally frequented by patients and patient guarantors, including at patient registration sites and in waiting rooms. In addition, the policy is distributed to patients at the time of their admission. Also, upon request at any point, the financial assistance policy will be distributed to patients. If a patient or the person who is billed for patient care believes they may be in need of financial assistance, a financial assistance application is filled out at the time of admission. When possible, HSC may attempt to qualify the patient prospectively for financial assistance.
Part VI, Line 4, Community informationThe HSC Pediatric Center serves children and young adults up to the age of 21 in the Washington DC metro area who have special health care needs. HSC serves the Washington DC metro area which is a diverse community that covers two states and the District of Columbia. Specific geographic and demographic characteristics of the community we service include the following: 1. There are 59,119 SSI and special needs children under the age of 18 in the Washington, DC metro area. 2. Of those 59,119 children, 6,065 were District of Columbia special needs children. Of the DC special needs children approximately over 5,000 were enrolled in Health Services for Children with Special Needs, Inc, HSC's sister organization that provides care management services to children who have SSI or SSI related diagnoses. 3. Maryland has a higher prevalence of children with special health care needs (CWSHCN) than does the nation as a whole. In 2010, more families of CYSHCN in Maryland are living in poverty than in the five previous years. 4. Prince Georges County in Maryland, a county bordering Washington, DC is an ethnically and socioeconomically diverse population with a higher rate of being uninsured than surrounding MD counties. There are about 12,000 special needs children in Prince Georges County, Maryland. 5. Of the approximate 59,000 children with special health care needs, approximately 32,000 have behavioral health diagnoses, and approximately 11,000 have cerebral palsy, developmental delays, or failure to thrive conditions. Approximately 16,00 children have various other medical conditions. The primary CWSHCN conditions treated at the HSC hospital facility are conditions requiring longer term transitional care and/or specialty rehabilitation care prior to their discharge to home or nursing/group home. These conditions include cerebral palsy, anoxic brain damage, spina bifida, failure to thrive related to prematurity, chronic lung disease and intestinal or other feeding disorders. 6. In 2013, 91% of HSC's patients were covered by Medicaid or Medicaid Managed Care. 35% were MD Medicaid or MD Medicaid MC, 42% DC Medicaid/MC, and 14% Virginia Medicaid. 7. 305 children were admitted to HSC in 2013. 16,729 days of care were provided to those children.
Part VI, Line 5, Promotion of community heatlhWhile HSC has generally not directly participated in community building activities its parent company, the HSC Foundation, a section 501(c) (3) organization that is a supporting organization of HSC, participated, funded, or led activities related to community building and community health improvement activities in 2013. Examples of those activities included: * Developed "Making My Mark" curriculum created by and for young people with disabilities, with support from the National youth transitions center and the Youth transitions collaborative. This curriculum is designed to provide young people with disabilities the tools to make their mark in their journey of independent living. * Worked with a polling firm to create and then promote the results of the first survey of its kind to study the political impact of people with disabilities. Called Power in Numbers, the survey created a new tool that youth with disabilities can use in their self-advocacy efforts. The survey was covered by CBS radio and was featured on morning and afternoon programs of approximately 20 affiliates nationwide, as well as numerous disability publications. * Hosted an informal after-work intern event held at National Youth Transition Center. The event was open to interns with disabilities in the Washington DC metro area as well as career professionals interested in mentoring and/or sharing their perspective with interns. The event provided opportunities to network, practice essential soft skills and receive information on workplace accomodations. It was well-received, and efforts are now underway to replicate the event on a continuous basis. * Partnered with the Institute for Community Inclusion to host the "High Expectations" meeting at the National Youth Transitions Center, which resulted in the publication, Community Health Worker Report. * Hosted the DC Disability Mayoral Forum, which brought in candidates so District residents could learn their views on disability-related issues. * Sponsored the Autism Campus Inclusion Leadership Academy, which brought students from across the country to the National Youth Transitions Center to work on leadership and advocacy skills. * Grew the Youth Transitions Collaborative to 47 local, regional and national organizations. * Provided over 10 sponsorships for disability events, garnering local, regional and national acknowledgement.

PART II, LINE 7All costs of issuance with respect to the Bonds were paid from funds other than proceeds of the Bonds.
Part IV, Line 7The tax-exempt bond issuance undertaken by the organization in 2010 currently refunded a prior tax-exempt bond issuance from 2005, which 2005 bond issuance had currently refunded the Hospital Revenue Bonds, Series 1991A, issued by the organization on June 12, 1991 to finance the construction of a hospital facility. All of the proceeds of the 2010 tax-exempt bond issuance were spent immediately upon receipt to currently refund the 2005 bond issuance. As of the issuance date of the 2010 tax-exempt bonds, there were no unexpended proceeds of the 2005 bonds, and there were no transferred proceeds to the 2010 tax-exempt bond issuance from prior bond issuances. As a result of the foregoing, none of the proceeds of the tax-exempt bond issuance were used to make any higher yielding investments or nonpurpose investments, which are regulated under Section 148 of the Internal Revenue Code. Since the organization has not used the proceeds from the bond issuance to make any higher yielding investments or nonpurpose investments, the organization has not yet established written procedures to monitor the requirements of Section 148. It is the organization's intention to adopt such a policy at such time as another tax-exempt bond issuance is undertaken by the organization, pursuant to which the proceeds of the tax-exempt bond issuance will not be spent immediately upon purpose investments.

Part VI Section B, Question 12cThe HSC Foundation has a written conflict of interest policy that covers itself and all of its direct and indirect subsidiaries (including The Hospital for Sick Children). Under that policy, whenever a potential conflict arises for The HSC Pediatric Center board of directors and officers, it is brought to the attention of the President and Executive Committee of the HSC Foundation Board, which analyzes the situation and makes a determination of whether or not an actual conflict exists. If it is determined that an actual conflict exists, the President and Executive Committee reports to the HSC Pediatric Center Board of Directors regarding the nature of the conflict and that person is thereafter recused from any discussion and vote regarding the matter. Whenever a potential conflict arises for employees of The HSC Pediatric Center management team, it will be brought to the Board Chair by the President which analyzes the situation and makes a determination of whether or not an actual conflict exists. If it is determined that an actual conflict exists, steps will be taken to eliminate the conflict and communicated to the employee involved. The Board Chair and President will report all COI to the Executive Committee Foundation.
PART VI, Line 15, SECTION BThe organization relied on a related organization, HSC Foundation, to determine executive compensation of the organization's CEO. In addition, the organization determines the compensation of the officers and key employees of the organization, as discussed below. The organization and the HSC Foundation both use a four step approach to determine the executive compensation: 1. The executive committee is made up of independent Board members without a conflict of interest, to establish and determine the reasonableness of total compensation for the System's disqualified executives and forwards to the full board for approval. 2. Periodically, the Executive Committee will engage an independent consultant to prepare a detailed written report on appropriate comparability data for comparable positions. 3. The Executive Committee reviews and relies upon the report of the independent consultant, along with other individual and market data, then debates and fully documents its decision about what is reasonable compensation for disqualified executives 4. Once the Executive Committee develops executive compensation recommendations, they are presented to the System's full board for approval.
