Part V, Line 4: Intended uses of the endowment fund.In 1995, the NBME established the Edward J. Stemmler, MD Medical Education Research Fund. The initial sum of $5,000,000 was invested and professionally managed with the goal of increasing the size of the Fund over time while providing total annual grants up to 5% of the trailing three-year average of the fund balance. The Fund makes grants to support research in topics relevant to the mission of the NBME. Specific investment guidelines were established by the NBME Finance Committee. As with all NBME invested funds, the Finance Committee monitors performance and recommends to the Executive Board any changes to the principal balance of the Fund. Since inception to date, the Stemmler Fund has awarded 74 grants for a total of approximately $6.2 million.
Part X : FIN48 FootnoteNBME qualifies as a tax-exempt organization under existing provisions of the Internal Revenue Code (IRC). Additional Supplemental InformationForm 990, Schedule I, Part II, Line 1a - GrantsEach school is listed with the approved grant as follows:1. Harvard Medical School Proposal #1213-044, "Academic and Professional Lapses in Medical School: A Prospective Assessment of their Sequelae".2. Ohio State University Proposal #1112-064, "Virtual Patients Simulations to Assess Data-Gathering and Clinical Reasoning".3. University of Iowa Proposal # 1112-001, "Simulation Approaches for Training in Fluoroscopically Guided Orthopaedic Trauma Surgery". Note that the University of Michigan Proposal #0809-037 and University of Illinois Proposal #0708-012 reimbursements of unused funds of $5,913.07 and $3,947.36 respectively are not listed on the schedule in Part II.Contributions:NBME periodically makes contributions to organizations and/or community programs in order to assist in furthering the exempt purpose of those organizations. The total amount of contributions made in 2013 was $113,901.
Grantmaker's Description of How Grants are UsedTHE EDWARD J. STEMMLER, MD MEDICAL EDUCATION RESEARCH FUNDOF THE NATIONAL BOARD OF MEDICAL EXAMINERS (NBME)PurposeThe purpose of the Stemmler Fund is to provide support for research or development of innovative evaluation methodology or techniques, with the potential to advance assessment in medical education or practice at eligible medical schools.2013-2014 Goal: The goal of the Stemmler Fund's 2013-2014 Call for Letters of Intent is to provide support, through a program of annual grants, for research or development of innovative assessment approaches that will enhance the evaluation of those preparing to, or continuing to, practice medicine. Expected outcomes include advances in the theory, knowledge, or practice of assessment at any point along the continuum of medical education, from undergraduate and graduate education and training, through clinical practice. Pilot and more comprehensive projects are both of interest. Collaborative investigations within or among institutions are not only eligible but encouraged, particularly as they strengthen the likelihood of the project's overall contribution and success.Eligibility: The eligible applicants for Stemmler Fund grant awards are the medical schools accredited by the LCME or the AOA. Additional eligibility requirements include:- The number of Letters of Intent submitted by any eligible school is not limited. However, an individual may not be principal investigator on more than one Letter of Intent. The designated principal investigator must represent the primary applicant institution and must be responsible for project oversight and reporting. (The applicant institution's rules apply regarding eligibility of their faculty, staff, or associates to serve as investigators.)- A maximum of two applications representing a revised Letter of Intent previously submitted to the Fund program will be accepted by the NBME. That is, applications addressing the same substantive study topic by the same investigators may be submitted for Fund consideration no more than a total of three times.- The primary investigators of prior Fund grants are eligible to reapply and compete (with new applicants) for one additional award through the Stemmler Fund Call for Letters of Intent for the same line of study. Current award recipients are eligible to compete again only if any current NBME support is expected to terminate satisfactorily by the beginning of the 2013-2014 project period (usually July 1, 2014).Selection Process:1. All submitted Letters of Intent will be reviewed by NBME staff to ensure eligibility and completeness.2. The Stemmler Fund Steering Committee will then evaluate Letters of Intent on the extent to which the proposed research is innovative or will advance assessment in medical education or practice, and addresses an important problem or issue in medical education or practice.3. The Stemmler Fund Steering Committee will then meet to select a subset of Letters of Intent (e.g., 10-15) and invite the principal investigators to submit full proposals.4. All Invited Proposals will be reviewed by NBME staff to ensure completeness.5. Peer reviewers with expertise in measurement, research design and methodology will then evaluate Invited Proposals on the relevance and significance of the proposal to the purpose and goals of the Stemmler Fund, the adequacy of the research design or methodology, and the qualifications of the principal investigator and project staff.6. The Stemmler Fund Steering Committee will then review all proposals and peer reviews, giving careful consideration to the adequacy of the budget, timetable, and other key project resources in each proposal, and select awardees. The NBME will provide feedback detailing the comments provided by reviewers of applications not selected for funding.Grant Fund Use Monitoring:Grant recipients are required to submit an interim and final report which includes a Final Project Narrative and a Final Project Fiscal Report.

ExplanationFirst Class Travel and Travel for CompanionsNBME's Business Travel policy states that "NBME reimburses reasonable and actual air and train travel at the most economical coach fare." First-class travel and travel for companions are not specifically authorized in the policy. In limited circumstances, certain NBME Officers and Key Employees may be permitted first-class travel if authorized by the President. First-class travel and spousal travel for the President is included in the compensation package. The cost incurred for travel for companions is included in the employee's taxable earnings.Tax Indemnification and Gross-Up PaymentsNBME has a Supplemental Executive Retirement Plan (SERP) and Highly Compensated Employee program (HCE) for which the payments are grossed up for Federal, State, and local income taxes. Therefore, the amount reflected on the employees' W-2 includes taxes remitted by NBME for taxes on the employees' behalf. Client Note 1Client Note 1 - Note 1 - Form 990 Part I, line 6 - VolunteersNBME utilizes an extensive network of volunteers, many of whom are medical school faculty members and practicing physicians, to accomplish its mission. These volunteers contribute the highest levels of expertise in their profession to a range of activities including test development and governance. They receive a small honorarium in recognition of their service.Note 2 - Form 990 Part VI, line 1b - Donald E. Melnick, as President of NBME, is an ex officio member of the NBME Executive Board.Note 3 - Schedule F Part II, line 1d - The net amount of grants outside of U.S. is $37,167.20, includes a grant awarded to the University of British Columbia in the amount of $55,057.20 and a reimbursement of unused funds from the University of Toronto of $17,890.00.1. The University of Toronto Proposal #1011-017 reimbursement of unused funds.
Form 990, Part III, Line 4d: Other Program Services DescriptionOTHER PROGRAM SERVICES 4: The Post-Licensure Assessment System (PLAS) is a joint activity of the NBME and the Federation of State Medical Boards (FSMB). PLAS comprises two programs: the Special Purpose Examination (SPEX) Program and the Assessment Center Program (ACP). SPEX is a one-day, multiple-choice examination of current knowledge requisite for the general, undifferentiated practice of medicine and is intended to be used for reasons of endorsement or reciprocity of licensure. The ACP provides comprehensive objective and personalized assessments of physicians for whom there is a question regarding clinical competence. Together, the PLAS programs have assessed thousands of physicians over the past decade for reasons that range from endorsement of licensure to license reactivation after disciplinary action. In 2012, the Assessment of Professional Behaviors (APB) multisource feedback program was rebranded as the Observational Assessment (OA) team to better reflect NBMEs work in multiple domains. In 2013, the OA team collaborated with the Association of Pediatric Program Directors to study the feasibility of using a subset of pediatric competencies (and the associated milestones) to assess the readiness of fourth-year medical students and pediatric interns to serve in the inpatient setting. New assessment tools were employed using multisource feedback based on both immediate observation of learners and on cumulative observation over days and weeks, with participating pediatric residency training programs. By mid-2013, 97 learners had been assessed. Research and Development Programs include the Center for Innovation and the Stemmler Medical Education Research Fund. The Center for Innovation is charged with development of new products and services and exploration of new ventures that facilitate NBME's strategic vision and mission. The Stemmler Medical Education Research Fund assists research in topics relevant to the mission of NBME and has supported 74 grant awards for a total of over $6.2 million since 1995. In addition to these defined programs of research and development, each program area and service unit is charged to carry on a program of research and development relevant to the work of the program or unit, for example, development of new item formats for USMLE, exploring relationships between prior measures and certifying exam performance for our clients and developing new testing methods for clients. These research and development activities result in the publication of dozens of research reports in scientific journals and presentations at scientific meetings each year.Note: The expenses reflected above do not include institutional indirect expenses. OTHER PROGRAM SERVICES 5: NBME's International Programs unit provides students, graduate physicians, medical schools, and regulatory agencies with valid and reliable information regarding individual knowledge and overall curriculum effectiveness. The International Foundations of Medicine (IFOM) examinations represent a key component of NBME International Programs. The Clinical Science Examination (CSE) covers the core of clinical knowledge in medicine, surgery, pediatrics, obstetrics and gynecology, and psychiatry expected of students in the final year of undergraduate medical education. The Basic Science Examination (BSE) incorporates the common core of knowledge expected of students who have completed the preclinical curriculum and are about to begin study of clinical medicine. In 2013, approximately 3,000 candidates sat for the IFOM CSE. The NBME International Programs unit also offers a portfolio of consultative, faculty and institutional development, and assessment programs in the international undergraduate medical education, certification and licensure, and graduate medical education environments. Consultative services often include an on-site evaluation of the overall assessment capability for medical schools. The International Programs unit is also involved in various research projects related to the validity and reliability of various assessment tools.Note: The expenses reflected above do not include institutional indirect expenses. OTHER PROGRAM SERVICES 6:
Form 990, Part VI, Line 4: Description of Significant Changes to Organizational DocumentsAt the Annual Meeting in April 2013, the members approved changes to the NBME Bylaws, largely in response to the new District of Columbia Nonprofit Corporation Act of 2010. Among other changes, the amendments to the Bylaws add and consolidate rules governing the conduct of meetings, clarify that the members advise on policy and budget issues, and authorize the use of electronic forms of notice. To take advantage of the opportunity to provide broader indemnification as permitted by the DC Act, the Bylaws were updated to align with the Act and in December 2013, the NBME filed amended and restated Articles of Incorporation.
Form 990, Part VI, Line 6: Explanation of Classes of Members or ShareholderThe members of the NBME are responsible for electing its board of directors, the Executive Board, recommending policy for the organization proposed by the Executive Board, receiving reports on the annual budget, and approving amendments to the by-laws of the organization. The members of the NBME include its Executive Board, up to twenty-two representatives of the test committees, twenty-two representative members nominated by key constituent medical organizations, elected members-at-large, and honorary members. The members of the NBME meet annually. The officers and members of the Executive Board, elected by the NBME membership, meet four or five times a year.

Form 990, Part VI, Line 7b: Describe Decisions of Governing Body Approval by Members or ShareholdersSee Schedule O - Form 990, Part VI, line 6.
Form 990, Part VI, Line 11b: Form 990 Review ProcessThe Form 990 is reviewed by the NBME President, Chief Financial Officer, and General Counsel prior to filing the return with the Internal Revenue Service (IRS). The review is conducted via a formal meeting during which material disclosures and significant changes in the document from the previous year's return are reviewed and discussed to ensure appropriate and complete disclosure. A copy of the Form 990 was provided to and reviewed by the NBME Executive Board and Audit Committee members prior to filing the return with the IRS. Any revisions necessary as a result of their review of the Form 990 were made prior to filing with the IRS.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of ConflictsThe National Board of Medical Examiners has a written Conflict of Interest Policy for its Members as well as a Fiduciary Duty and Conflict of Interest Statement for its Officers, Key Employees and Executive Board Members. These individuals are required to review and sign the applicable policy annually. Submission of completed statements by Officers, Key Employees, and Executive Board Members is enforced and annual disclosures of interests that could give rise to conflicts are monitored by the Secretary of the Board. Potential or real conflicts of interest are required to be reported to the Secretary of the Board or its President. If deemed appropriate, the party will be recused from the relevant proceedings.
Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key EmployeesPeriodically, typically every three years, a Compensation Survey of Peer Organizations is conducted by an outside independent compensation consulting firm. The recommendations resulting from the survey are reviewed by NBME Human Resources and the Compensation Subcommittee of the NBME Executive Board. Officer employees and Key Employee compensation is reviewed and confirmed annually by the Compensation Subcommittee. Payments to non-employee officers are reviewed and approved by the Executive Board with interested officers recused. In setting CEO compensation, the Compensation Subcommittee uses the Compensation Survey of Peer Organizations and the results of the detailed annual CEO Evaluation of performance report from the CEO Evaluation Subcommittee of the Executive Board. The CEO compensation is set and approved by the Compensation Subcommittee of the Executive Board which then reports to the Executive Board that it has reviewed the relevant information and made a decision.
Form 990, Part VI, Line 19: Other Organization Documents Publicly AvailableNBME governing documents, Conflict of Interest Policy, and financial statements are available upon request to the NBME Secretary of the Board.
