Schedule D, Part V, Line 4Schedule D Part V Line 4 - The organization has interpreted the NYS Prudent Management of Institutional Funds Act Law, NYPMIFA as requiring the preservation of the historical dollar value of the corpus of the permanent restricted endowment funds absent of donor stipulations to the contrary. The net appreciation of the permanent restricted endowment funds absent of the donor stipulations to the contrary. The net appreciation of the permanently restricted net assets is considered temporarily restricted net assets until those amounts are appropriated for expenditure by the Organization in a manner consistent with the standard of prudence prescribed by NYPMIFA. If the fair value of the investments assets falls below the level of the donor, NYSMIFA requires the Center to retain the fund in perpetual duration. There were no such deficiencies as of December 31, 2013. To ensure the preservation of the corpus of the permanently restricted endowment funds, the total fair value of all commingled investment assets are classified first to the level of the permanently restricted endowment funds corpus, an endowment fund deficiency receivable from the unrestricted net assets would be recognized until the deficiency is recovered.

Schedule H, Part I, Line 7Arnot Ogden Medical Center uses a Cost to Charges ratio to calculate the Financial Assistance and Community Benefit Section. The medical Center offers a variety of services to the community it serves as part of an integrated delivery system. It is necessary to financially subsidize some of these services as the revenue generated does not cover the cost of providing the services.
Schedule H, Part IIArnot Health, in collaboration with partners from HP2and SHPT, developed broad based plans to address chronic disease as a public health priority, focusing on reducing obesity and tobacco use. Community Health Improvement Plans CHIP from both counties place emphasis on three key areas 1 health promotion activities to encourage healthy living and limit the onset of chronic diseases 2 early detection opportunities that include screening populations at risk and 3 successful management strategies for existing diseases and related complications. These strategies, recommended by the Health Impact Pyramid, are based on the interventions evidenced base potential to address health inequities, the ability to measure success and potential reach, potential for broad partner support and collaboration, and political feasibility. This method is based on findings from such organizations as the Institute of Medicine of the National Academies and their report, Accelerating Progress in Obesity Prevention, Solving the Weight of the Nation or the CDCs, Recommended Community Strategies and Measurements to Prevent Obesity in the United States. For each focus area under the selected Prevention Agenda, Prevent Chronic Disease priority, Chemung and Steuben County Partnerships identified objectives and goals that include improvement strategies and performance measures with measurable and time framed targets over the next five years. The proposed chronic disease prevention strategies are evidence based or promising practices. They include activities currently underway by partners and new strategies to be implemented. All proposed strategies in Chemung and Steuben Counties are supported by the HP2 and SHPT partnerships. Chronic disease prevention strategies will be implemented in multiple sectors, including at local schools, worksites, businesses, community organizations, and with providers, to make it easier for residents to make healthy choices. By planning and implementing strategies from many levels of the Health Impact Pyramid, and through our network of diverse partnerships, we can create an environment that is conducive to physical activity, good nutrition and tobacco cessation. The Chemung and Steuben County partnerships have included many suggested activities from the States Prevent Chronic Disease Plan that will encourage increased physical activity, better nutrition and tobacco cessation strategies that aim toward reducing and preventing the incidence of chronic disease in Chemung and Steuben Counties. Some of these interventions include environmental, policy and systems changes to create communities that support physical activity, improved nutrition and breastfeeding, and tobacco cessation. Chemung County will continue its work through the Creating Healthy Places to Live, Work and Play grant and improve parks and playgrounds, establish community gardens to improve access to fresh fruits and vegetables, and work with local restaurant partners to make eating out healthier. Steuben County will continue to work with the cross county Southern Tier Tobacco Awareness Community Partnership, STTAC to establish tobacco free policies and urge members of the Regional Economic Development Council to consider health in their projects. Both counties plan to pursue joint use agreements with local school districts. Promote breast feeding policies at worksites, and create resource guides that promote the counties many opportunities for physical activity and healthy foods, including local farmers markets, parks and hiking trails. Other strategies include the clinical and community solutions such as provider 51 referrals to Diabetes Prevention Programs, Chronic Disease Self Management Programs and Tobacco Cessation Education. One exciting aspect of the CHIP is the unlimited possibilities offered by technological advances. Arnot Health and other local providers are beginning to implement Electronic Health Records, EHR. When fully functional, the benefits of EHRs will include improved quality and convenience of patient care, accuracy of diagnoses, enhanced care coordination, increased patient participation in their care, improved health outcomes and increased practice efficiencies and cost savings. EHRs will give providers decision support tools and available resources to better prevent and manage chronic disease. Arnot Health is currently working towards consolidating existing technologies within the healthcare system, with the objective of getting everyone on the same system organization wide. Consolidating technology within the healthcare system will improve communication among care providers, and between patients with their providers, enhance patient care coordination, improve chronic disease management and health outcomes. Arnot Health is increasing its focus on population health management and working towards increasing the systems abilities and competencies related to population health management. Areas of focus include realigning our health care delivery system to be more patient and ambulatory centric; increasing patient engagement with primary care providers using PCMH as a model; improving care coordination; and improving the management of chronic disease. Successful consolidation of IT systems will, contribute towards the integration and interoperability of data, and data exchange among care providers. Create robust and functional analytics which can identify higher risk patients to better manage chronic disease and improve health outcomes. Equip providers with the competencies needed to manage health care in a more predictive and less reactive way. Reduce preventable utilization of higher cost health care services such as emergency room or inpatient settings. Chemung and Steuben Counties Community Health Improvement Plans propose training primary care providers to talk to their patients about their weight, physical activity, diet and tobacco use and offering professional training opportunities which include programs on prevention, screening, diagnosis and treatment of overweight, obesity and diabetes. Residents and medical students will benefit from this training through the healthcare systems graduate medical education program. Arnot Healths graduate medical education program is offered in partnership with Lake Erie College of Osteopathic Medicine, LECOM. Another CHIP strategy is to investigate the possibility of incorporating referrals to community resources to prevent or manage chronic disease through a link in EHR. Through the use of new technology, follow up calls can be made to check on patient compliance, and provide the opportunity to document and measure the effectiveness of healthcare interventions. 52 Arnot Healths service area includes two Medically Underserved Areas, MUA. Elmiras Census Tract 0007.00 in Chemung County and Troupsburg and Tuscarora Town Service Area in Steuben County. Poor access to care is associated with a higher incidence of chronic disease, poorly managed chronic disease and an increased incidence of preventable hospitalizations. In both counties, the ratio of residents to primary care providers exceeds the NYS average. Arnot Health, through its Arnot Medical Services provider practice group, continues to aggressively recruit providers for the counties most underserved areas, and expand its graduate medical program to help meet identified needs within the MUAs. As Arnot Health works in partnership with Chemung Countys HP2 and Steuben Counties SHPT to implement the CHIPs, we will continue to identify emerging best practices to reduce obesity and tobacco use. Data measures will be developed to assess the effectiveness of our plans in impacting the communitys health, and set mid-course corrections where needed. Promising cases for return on investment will be shared with policymakers. The specifics of Chemung and Steuben Counties Community Health Improvement Plans are outlined in the following CHIP charts. Strategies, goals, objectives, improvement strategies, performance measures with measurable time-framed targets over a three year period are included in the CHIP charts.
Schedule H, Part III, Section A, Line 4The Organizations allowance for doubtful accounts increase from 11.1 percent of gross accounts receivable at December 31, 2012 to 11.4 percent of gross accounts receivable at December 31, 2013. In addition, the Organizations bad debt write offs increased approximately 1,600,000 from approximately 25,300,000 for fiscal year ended 2012 to approximately 26,900,000 for fiscal year end 2013. The increase in write offs was to account for negative trends experienced in the collection of amount from self pay patients in fiscal year 2013. The Organization has not changed its charity care or uninsured discount policies during fiscal year 2013. The Organization does not maintain a material allowance for doubtful accounts form third party payers, now did it have significant write offs from third part payers.
Schedule H, Part III, Section B, Line 8The Medical Center provides Emergency, Stroke, Cardiac, Dialysis, Wound Care, Hyperbaric and other services that help prevent loss of life and extensive severity due to prompt treatment which benefits the community.
Schedule H, Part III, Section C, Line 9bSelf pay collection staff reviews any prior accounts to check for commercial insurance or Medicaid that may need to be applied to current self pay accounts. These accounts are reviewed using the self pay worklist. If insurance is located, self pay collector must verify eligibility for the date of service. The Self pay staff will change the health plan to the correct one for all covered dates of service and ARNOT Billing will submit claim or claims. If no insurance is located, confirm if a patient may have qualified for Medicaid. If Medicaid coverage is approved self pay staff will change the health plan to Medicaid for all covered dates of service and follow correct billing process. Self Pay Collections Staff attempts to collect full balance due by check or credit card. If they are unable to make the full payment, Self pay staff will attempt to make payment arrangements and to notify patient about the Arnot Ogden Medical Center Community Care Program. If the patient wants to apply for the community care program, the patient will be sent an application for the community care program and will be informed of documentation they will need to submit with the application. The Self Pay Collectors documents all activity in Soarian, and enters any necessary hold to support resolution. Self pay collections staff enters the payment plan and documents comments in Soarian. Payment plan statement is demanded and sent to the patient. If patient does not enter into a payment plan agreement, or fails to pay as agreed, account will go through the standard dunning and collection agency activity. See Guarantor Billing and Collections.

Schedule H, Part VI, Line 3The Arnot Ogden Medical Center communicates our Community Care Program in the following ways: Signage in waiting rooms, Brochures in waiting rooms and Admission packages, Notations on billing notices, Staff education so they can share availability of program and contact information with patients. Communication and education of shared County Dept of Social Services staff so they are informed of the program details and contact information. Communication of programs with Collection Agencies and collection attorneys so they can share the availability of the program and contact information with prospective enrollees. The information is also posted on our website, www.aomc.org.
Schedule H, Part VI, Line 4Arnot Health participated in a comprehensive year-long process to assess community needs across county lines with partners such as: The Chemung County Health Department; Steuben County Public Health; other hospitals and healthcare agencies in Chemung, Steuben, and Schuyler Counties; a diverse collaborative of community partners and community residents. The year-long process of Assessing Community Health Needs was facilitated by a consultant contracted by Chemung County Health Department and technical assistance from the S2AY Rural Health Network in Steuben County. Arnot Health, in partnership with the dual-county collaborative network, used the MAPP process (Mobilizing for Action through Planning and Partnership), to collect and analyze data from a variety of sources. Valuable input was gained from surveying Chemung and Steuben County residents about their current health, and opinions on county-wide health challenges. Areas of disparity were also assessed within each county. Findings from the Community Health Assessment (CHA) identified some overlapping areas of public health concerns and disparities in Chemung and Steuben Counties: Chemung County Priority: Prevent Chronic Disease. 1. Focus area-Reduce obesity in children and adults. 2. Focus area-Reduce illness, disability, and death related to tobacco use and secondhand smoke exposure. Disparity-Reduce percentage of lower income individuals who smoke, including those with mental health and substance abuse issues. Steuben County Priority: Prevent Chronic Disease. 1. Focus area-Reduce obesity in children and adults. 2. Focus area-Reduce illness, disability, and death related to heart disease and hypertension. Disparity-Promote tobacco cessation, especially among low socioeconomic populations and those with mental health illness.
Schedule H, Part VI, Line 5Arnot Health works collaboratively with the Chemung County Health Department, Chemung Valley Rural Health Network, Cornell Cooperative Extension, other area hospitals, faith based organizations, and community partners to assess public health needs within the community. Programs to prevent obesity and diabetes, such as a free 6 week community walking program called Step It Up, are planned, implemented and evaluated in partnership. Pre and post program health screening data is shared between agencies, and outreach efforts are coordinated to maximize reach and community health impact. Arnot Health has a leadership role in the NYSDOH grant funded Creating Healthy Places to Live, Work, and Play partnership. This diverse collaborative of over 30 community partners across public, private, and non profit sectors is working together to plan, implement and evaluate environmental, policy and systems changes to promote physical activity and nutrition. The Creating Healthy Places partnerships goal is to prevent obesity and diabetes through rehabilitating Elmira parks and playgrounds, increasing safe access to open public places for physical activity, building community gardens to increase access to fresh fruits and vegetables and working with local restaurants to offer healthier options on their menus.
