The interpretation requires that the entity account for and disclose in the financial statements the impact of a tax position if that position will more likely than not be substantiated upon examination. MHSC has evaluated the financial statement impact of tax positions taken or expected to be taken and determined it has no uncertain tax positions that would require tax assets or liabilities to be recorded in accordance with accounting guidance for either December 31, 2013 or 2012.

SCHEDULE H, PART III, LINE 4The organization's financial statements includes the following footnote: "Patient accounts receivable are reduced by an allowance for doubtful accounts. In evaluating the collectability of accounts receivable, MHSC analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for doubtful accounts and provision of bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. For receivables associated with services provided to patients who have third-party coverage, MHSC analyzes contractually due amounts and provides an allowance for doubtful accounts and a provision for bad debts, if necessary. For receivables associated with self-pay patients (which includes both patients without insurance and patients with deductibles and copayment balances due for which third-party coverage exists for a portion of the bill), MHSC records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the discounted self-pay rates provided by policy and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts."
SCHEDULE H, PART III, LINE 8The extent to which any Medicare shortfall should be treated as Community Benefit: Medicare shortfalls should be reported as community benefit because the cost of providing healthcare to the patients exceeds the reimbursement. In summary, the healthcare services are provided or rendered at a loss. The methodology for calculating the loss in based on the as filed Medicare Cost Report. MEDICARE COSTING METHODOLOGY Medicare cost is based on the Medicare Cost Report, which combines routine and ancillary costs. The calculation of cost for ancillary services are based on direct routine costs, allocated based on the Medicare patient days as a percent of the total patient days.
SCHEDULE H, PART III, LINE 9BCharity Care provision: If a patient is deemed to qualify for 100% financial assistance the patient liability is written off entirely and there are no further collection activities. However, if a patient is qualified for a partial financial assistance write off (income falls between 201% and 350% of the federal poverty guidelines) the amount approved for financial assistance is written off based on a tier scale and written off as a partial assistance approval. This leaves the patient a small percentage as patient liability. Uninsured patients are encouraged to apply for Methodist's Charity Care Program (uncompensated care). Accounts will not be approved for legal action (beyond reporting to credit agencies) until the agency screens the patient for undercompensated care. Only those patients whose income exceeds 200% of federal poverty guidelines will be approved for legal action. Methodist Hospital will not garnish wages. It will, however, permit liens on homes or other real estate for the purpose of securing repayment at sale or refinancing.
SCHEDULE H, PART VI, LINE 2NEEDS ASSESSMENT The community health needs assessment brings together a variety of demographic and health data and information, gathered from both primary and secondary. The first phase of the community health needs assessment involved collections and analysis of numerous demographic and health status and data and information for Methodist Hospital Service Area and Service Planning Area (SPA) 3 - San Gabriel Valley. The following types of data were analyzed: population, household and family demographics; births statistics, death statistics, designated Health Professional Shortage Areas, large-scale household telephone surveys designated to measure health access, utilization of health services, and health behaviors. Wherever possible, the service area health experience was benchmarked to Health People 2020 targets. In addition, to assist with providing context to the demographics, Methodist Hospital Service Area is compared to Los Angeles County. The second phase of the needs assessment focused on qualitative primary research. Representatives from Methodist Hospital agreed that telephone interviews with health department contacts and representatives from public and private organizations would be a cost effective approach to gain an understanding of important health issues and potential opportunities to collaborate with these organization. DNA and Company, Inc., on behalf of Methodist Hospital, completed the 2013 community needs assessment.
SCHEDULE H, PART VI, LINE 3PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE Methodist Hospital informs Medical Staff and employees of available low and no-cost resources. This information is distributed through community outreach resource tables at employee fairs, internal bulletins, and management meetings. MH also provides Medicare counseling to MH staff and employees. Methodist Hospital distributes its financial assistance policy and relevant contact information to patients upon their admittance to the hospital. Externally, Methodist Hospital compiles and maintains information on available low-cost and no-cost local services, including direct contact persons for each program. It creates a comprehensive community outreach calendar for the hospital's website and distributes information through a community outreach brochure, the 50+ Bulletin, and the Health Ministries newsletter. It disseminates information that partners can publicize through their own internal mechanisms. In addition, it allocates resources to increase community access to health related services and delivers Medicare counseling to community groups.
SCHEDULE H, PART VI, LINE 4COMMUNITY INFORMATION As of 2013 Methodist Hospital's service area was comprise of approximately 581,067 persons. This represents a growth of one percent since the year 2010, with a forecasted increase of another two percent by the year 2018. The 2013 demographic distribution is as follows: Age group: 21 to 64 years - 59% Under 21 years - 29% 65 years and over - 12% Race/Ethnicity Hispanic - 50% Asian or Pacific Islander - 32% White - 15% Language Spoken at Home: English only - 30% Spanish - 40% Asian or Pacific Island Language - 27% Service area average household income - $73,800 Service area percentage below federal poverty level - 12.0% vs Los Angeles County average of 13.5% Number of hospitals serving the community - Four: Methodist, Monrovia Memorial, City of Hope, San Gabriel Valley (of these, 2 are specialty hospitals - City of Hope and Monrovia). It is worth noting that the population of residents 65 years and older, and that of Asian or Pacific Islanders, is greater than seen in Los Angeles County - wherein the Hospital is located - as a whole.
SCHEDULE H, PART VI, LINE 5PROMOTION OF COMMUNITY HEALTH Methodist Hospital's Board of Directors consists almost entirely of members who reside in its service area. The Board is dedicated to meeting the unique health needs and demands of its community. Additionally, the Hospital maintains an open medical staff which encourages physician coverage for needed services. During Fiscal Year 2013, Methodist Hospital pursued the following objectives to promote Community Health: - To continue to increase access to health care services for the community. - To continue to provide health education support and screening services for the public based on important health concerns, such as maternal child health, heart disease, cancer, and diabetes. - To continue to provide health education, support, and screening services for seniors through a dedicated 50+ Health Connection Program. - To continue to provide health education and training, information and resources for member congregations through a Health Ministries Program.
SCHEDULE H, PART VI, LINE 7STATE FILING OF COMMUNITY BENEFIT REPORT The Hospital files a Community Benefit Report in California.
