DescriptionSee Schedule O.Patient Care: Mercy Catholic Medical Center provides quality healthcare regardless of race, creed, sex, national origin, handicap, age or ability to pay. Although reimbursement for services rendered is critical to the operation and stability of Mercy Catholic Medical Center, it is recognized that not all individuals possess the ability to purchase essential medical services, and further that our mission is to serve the community with respect to providing healthcare services and healthcare education. Mercy Catholic Medical Center remains focused on our mission to further the healing ministry of Jesus Christ, especially for the poor and underserved community. Mercy Catholic Medical Center provides free care, subsidizes the care of Medicare and Medicaid patients where the government reimbursement is below costs, and sponsors health programs and activities for the community. These programs include wellness programs, community education programs, special programs for the elderly, handicapped, medically underserved and a variety of broad community support activities. Mercy Catholic Medical Center serviced 19,807 discharges providing 88,911 patient days of which: Patients covered under the Medicare program were 9,045 discharges and 44,898 patient days. Patients covered under the Medicaid program were 7,439 discharges and 24,115 patient days. Mercy Catholic Medical Center provides care to persons covered by governmental programs at below cost. Recognizing its mission to the community, services are provided to both Medicare and Medicaid patients. To the extent that reimbursement from Medicaid is below cost, Mercy Catholic Medical Center recognizes these amounts as charity care in meeting its mission to the community. The unreimbursed value of providing care to these patients was $2,754,791 for the twelve month period ended December 31, 2012. Charity care is also provided through many reduced price services and free programs offered throughout the year based upon the activities and services which Mercy Catholic Medical Center believes will serve a bona fide community health need. These include (1) Benefits for the Broader Community: (a) Community Health Improvement Services and Community Benefit Operations: $940,381, (b) Subsidized Health Services: $18,312,607, (c) Cash in Kind Contributions to Community Groups: $49,227, The Total Benefits for the Broader Community was $19,302,215. ExplanationExplanationIdentifierIdentifierReturn ReferenceReturnReference
Part I, Line 6a: The annual community benefit report for Catholic Health East (CHE) includes Mercy Medical Center.
Part I, Line 7: The figures were calculated based on the cost-to-charge ratio. The cost-to-charge ratio was derived by dividing total operating expenses by total gross patient charges. The cost-to-charge ratio was applied to total bad debt write-offs to calculate the costs for line 2.
Part III, Line 2:The figures were calculated based on the cost-to-charge ratio. The cost-to-charge ratio was derived by dividing total operating expenses by total gross patient charges. The cost-to-charge ratio was applied to total bad debt write-offs to calculate the costs for line 2.Part III, Line 3:While we make all reasonable efforts to identify patients eligible for charity care prior to billing, such pre-billing identification is not always possible. This problem arises largely with patients treated in our Emergency Department ("ED") on an outpatient basis. Frequently, these patients do not complete an application for financial assistance therefore making a definitive determination difficult. The estimated amount for patients who likely would qualify for financial assistance is calculated for line 3 based on a review of bad debt write-offs to include only bad debt for uninsured patients, and estimating the amount of that cost that could be charity based on a review of applications for financial assistance. A reasonable estimate based on the percentage of patients whose applications do not meet the criteria for financial assistance is used to estimate the amount that could qualify for financial assistance. Part III, Line 4:For financial reporting, an allowance for doubtful accounts is recorded for estimated losses resulting from the unwillingness of patients to make payments for services. The allowance is determined by analyzing historical data and trends. Accounts receivable are written off against the allowance for doubtful accounts when management determines that recovery is unlikely and collection efforts cease.
Part III, Line 8: Mercy Catholic Medical Center has reported a surplus on providing Medicare services of $3,319,805 on Part III, Line 7 on Schedule H. This surplus agrees to the amount disclosed in the 2012 Cost Report. Mercy Catholic Medical Center also incurred unpaid costs of Medicare programs of $18,644,626 during 2012. These unpaid costs relate to the following: 1) The cost and reimbursement for fee-schedule reimbursed services are not considered in the Medicare cost report. 2) The cost-to-charge ratio methodology used in the cost report differs from the methodology that Mercy Catholic Medical Center uses to calculate the unpaid cost of Medicare services. 3) The cost report does not include unpaid costs of Medicare Advantage plans. 4) The cost report numbers do not include revenues and costs of graduate direct medical education. This $18,644,626 amount represents the costs of services provided to Medicare patients by the Hospital for which no payment is expected or received. Medicare services are provided to eligible patients regardless of their ability to pay, and therefore any shortfall is considered to be a community benefit provided by the Hospital. When considering the total unpaid cost of Medicare for the Hospital for all services, including those which are not reported on the 2012 cost report, the total unpaid cost of Medicare services for Mercy Catholic Medical Center is $15,324,821.
Part III, Line 9b: Self-Pay balances are transferred to professional collection agencies when the accounts complete a patient statement billing cycle (e.g., 120 days) with no payment from the patient or proof of eligibility for Charity Care or other programs. Accounts with applications pending for Charity Care or other assistance programs are held until a decision is made. A pending application is defined as an application that has been fully completed by the patient, submitted and is in the process of being determined for eligibility.
Part V, Section B, Line 18e: The organization does not take any of the actions listed on Line 17 with respect to patients eligible for assistance under the organization's FAP.Mercy Fitzgerald Hospital
Part V, Section B, Line 18e: The organization does not take any of the actions listed on Line 17 with respect to patients eligible for assistance under the organization's FAP.Mercy Philadelphia Hospital
Part V, Section B, Line 20d: No amounts are invoiced to patients who qualify for free care under our financial assistance policy (FAP). The maximum amount invoiced to patients who qualify for discounted care under our FAP is 50% of the Self Pay Fee Schedule. In the case of discounted care, the actual amounts settled in most, if not all, cases are at levels well below amounts generally billed individuals who have insurance covering emergency or other medically necessary care.Mercy Fitzgerald Hospital
Part V, Section B, Line 20d: No amounts are invoiced to patients who qualify for free care under our financial assistance policy (FAP). The maximum amount invoiced to patients who qualify for discounted care under our FAP is 50% of the Self Pay Fee Schedule. In the case of discounted care, the actual amounts settled in most, if not all, cases are at levels well below amounts generally billed individuals who have insurance covering emergency or other medically necessary care.Mercy Philadelphia Hospital
Part V, Section B, Line 21: The organization initially invoices patients who are eligible for discounted care under its FAP in amounts in excess of what it would receive from third-party payors. Final amounts due the organization from such patients in most, if not all, cases are settled at levels significantly below what would be received from any such payors for the services rendered. The organization in 2013 executed a policy where patients eligible for discounted care are billed are rates no higher than the prevailing Medicaid rates.Mercy Fitzgerald Hospital

The organization in 2013 executed a policy where patients eligible for discounted care are billed are rates no higher than the prevailing Medicaid rates.Mercy Philadelphia Hospital
Part VI, Line 2: The organization conducts a Community Health Needs Assessment at the acute care operating unit level. This is to be conducted every three years with an annual update. The assessment includes an Annual Improvement Plan for the identified community health needs priorities. The needs are assessed by conducting and analyzing primary and secondary research. Primary research includes surveying key community organizations in each service area with the purpose of identifying key health care needs and issues faced by the community. Secondary research is publicly reported information including demographic data, socio-economic data, and vital health statistics.
Part VI, Line 3: The organization is deeply committed to caring for the health care needs of persons who are poor or are who experiencing extreme financial burdens. As a general matter, our charity care policy is predominately displayed in our ED and Business office. More specifically, the organization has established respectful and effective procedures for addressing the needs of those persons who are unable to pay for all or most of their care. In order to preserve the dignity of these persons and to facilitate the process of securing necessary information, the organization will perform financial screening upon scheduling, admission or registration (discharge processing in the ED) as part of the overall Financial Counseling process as described in the Key Performance Indicator System Policies. Patients who represent increased financial risk as a result of the amount they are expected to owe out-of-pocket will be referred to a Financial Counselor for assistance in applying for alternative payment programs (e.g., Medical Assistance) determining Charity Care eligibility, establishing payment plans or other financing arrangements. Financial Counseling services will be made available to all elective, urgent and emergent patients. Patients with insurance other than that afforded low income individuals will not have their patient liability unpaid balance, coinsurance, deductible or non-covered services written-off to Charity Care unless financial hardship can be proven.
Part VI, Line 4: Mercy Fitzgerald Hospital (MFH) is located in Darby (19023), Delaware County. Its primary and secondary service area comprises 11 zip codes across Southeastern Delaware County and West/Southwest Philadelphia. This is a diverse service area where 71% of total population is a minority. The overall population is expected to decrease 0.7% between 2012 and 2017 while the minority population is projected to increase 4.3%. A significant decrease of 5.8% is projected in the female population child bearing age group. With an average household income of $49,740 and unemployment at 8.8%, there are pockets of neighborhoods that are economically distressed and areas that have been designated as medically underserved and primary care health professional shortage areas. Mercy Philadelphia Hospital (MPH) is located in Kingsessing (19143), West Philadelphia. Its primary and secondary service area comprises five zip codes across west and southwest Philadelphia. Sections of the service area have been designated by the state and federal government as a medically underserved (MUA) and/or primary care health professional shortage (HPSA) areas and are also economically distressed. The service area has a high minority population, 85% of the total population. The overall population is expected to decrease by 1.1% between 2012 and 2017. With an average household income of $38,166 and unemployment at 8.4%.
Part VI, Line 5: Our community benefit activities include going directly into underserved communities/populations and providing health screenings, immunizations and health care education. Our community benefit programs also ensure establishing primary care physician offices in medically underserved areas. In addition, we maintain a 24-hour emergency room, an open medical staff and a board comprised largely of independent members of the community. The majority of the board is composed of independent members and the organization extends privileges to all qualified physicians in the community.
Part VI, Line 6: The organization is a member of the Mercy Health System of Southeastern Pennsylvania. The System operates four hospital facilities -- Mercy Fitzgerald, Mercy Philadelphia, Mercy Suburban, and Nazareth that are dedicated to addressing the specific medical needs of the community served. In addition, the System sponsors two programs that are designed to improve the quality of life for senior citizens. These programs are operated by Mercy Home Health and Mercy LIFE. Finally, the System offers comprehensive managed care programs for Medicaid enrollees through a partnership. All of the services provided by the System attempt to serve the needs of the community at-large with a special emphasis on the poor and disadvantaged.

Form 990, Part VI, Section A, line 6SEPA is the sole corporate member of the organization.
Form 990, Part VI, Section A, line 7aThe Board of Directors of SEPA has the authority to appoint members of the governing body.
Form 990, Part VI, Section A, line 7bThe governing body can make recommendations for certain types of actions that require approval by the Board of Directors of SEPA. Some examples include appointing or removing members of the governing board, amending Articles of Incorporation, and amending bylaws.
Form 990, Part VI, Section B, line 11The organization took steps to educate management, members of the board and members of appropriate subcommittees of the board on the compliance requirements mandated by the Form 990. It continues to maintain a process to address questions or concerns surrounding tax reporting requirements. The Form 990 is reviewed by management and the SEPA Audit & Compliance Committee, which is a subcommittee of the SEPA Board of Trustees. The full board is appraised of the committee's findings from their review.
Form 990, Part VI, Section B, line 12cThe organization has adopted SEPA's Policy 96-04-09 which sets forth the organization's conflict-of-interest policy and processes. Annually, all those serving the organization and affiliates in a position to affect business decisions, including directors, officers, members of a board designated committee, and key employees and physicians, receive a copy of the policy and annual disclosure statement to be completed. Disclosures of financial interest or other reportable circumstances as defined in the policy are submitted and reviewed by SEPA's Board or senior management as defined in the policy. The determination of whether a disclosure constitutes a conflict of interest is made by SEPA's Board or an appropriate committee thereof comprised of disinterested persons and without the participation of the affected individual except to respond to questions about the disclosure. Lastly, the policy addresses potential disciplinary action for violations of the policy. The policy is available to the public upon request.
Form 990, Part VI, Section B, line 15The organization relies on CHE, a related Section 501(c)(3) organization, and on CHE's procedures to determine the compensation of the CEO. For other employees, the organization has adopted SEPA's process for determining compensation. This process includes: The board has an independent committee review and approve all elements of remuneration for all disqualified parties, as well as other key management. The board/committee has an established compensation philosophy which details the objectives of market positioning and pay elements. The committee engages with external consultants to provide market data comparing SEPA's roles to similarly sized health systems utilizing both title and job content comparisons. The committee reviews the market analysis, approves any salary adjustments for the executive population, considers both reasonableness and effectiveness of all remunerative programs and establishes the detailed performance expectations which are incorporated into the incentive plan. All of these discussions and decisions are documented through the provision of meeting minutes.
Form 990, Part VI, Section C, line 19Organizational Articles of Incorporation, Corporate Bylaws, governance policies believed to be of interest to the public, conflict-of-interest policy, and IRS Form 990 are available to the public upon request.
