DescriptionSt. Peter's built 24 state-of-the-art operating rooms in 2011.At St. Peter's, surgeons operate in the area's newest and best surgical suites, featuring the latest diagnostic and surgical technology. It's why more than 15,850 surgeries are performed at St. Peter's each year.In St. Peter's operating rooms, surgeons view patient X-rays on large television monitors rather than traditional X-ray films. These "filmless" operating rooms give doctors a more accurate picture of a patient's condition. In hepatobiliary (liver, biliary, pancreas), St. Peter's was the first hospital in the Capital District to use the Spyglass Direct Visualization System - a fiber-optic camera, about the size of a pencil point, that is inserted into the upper digestive tract and threaded through the stomach and into the bile ducts. This life-saving tool allows doctors to quickly and precisely locate and examine tissue - without having to perform laparoscopic surgery.Supported by an expert support staff, surgeons at St. Peter's have the tools and the advanced technology to transform and save lives.Minimally invasive procedures, including robotic, computer-assisted and image-guided surgeries, are performed in the areas of cardiac, vascular, gynecologic, neurological, orthopedic, otolaryngologic, thoracic, urologic and general surgery. Types of procedures being performed at St. Peter's Hospital:- Bariatric Surgery - Breast Surgery - Cardiac Surgery - Colorectal Surgery - Dental/Maxillofacial Surgery - General Surgery - Gynecologic Oncology - Gynecological Surgery - Hepatobiliary (Liver, Biliary, Pancreas) - Minimally-invasive Surgery - Neurosurgery - Orthopaedic Surgery - Otolaryngology Surgery - Pediatric Surgery - Plastic and Reconstructive Surgery - Surgical Oncology/Cancer - Thoracic Surgery - Urological Surgery - Vascular Surgery Robotic Surgery: The da Vinci Surgical SystemThe surgeon's surgical capabilities are enhanced by the da Vinci surgical system, a totally "intuitive" laparoscopic surgical robot. The system is so precise, the instruments recheck their position by computer 1,500 times per second. St. Peter's gynecologic oncologists were the first in their specialty in New York state to use the state-of-the-art da Vinci surgical system. This new system integrates robotics and extremely sophisticated computer technology with the skills of the surgeon. It allows the surgeon to perform minimally invasive procedures in a manner never experienced before. Robotic-assisted laparoscopy combines dexterity and precision with advanced 3-D imaging to overcome the surgical limitations of a standard laparoscopy. The new technology gives surgeons precise control and visualization, bringing a whole new dimension and flexibility to surgery. The system's camera gives the surgeon a highly detailed, three-dimensional view of the patient's body. Advanced imaging allows much greater precision, thereby allowing for small incisions, more exact placement of sutures and more exact use of the operating instruments. In addition, it is possible to return to regular activities in one to two weeks, instead of the traditional six weeks that may be required for a complete recovery. In orthopaedic surgery, certain patients can have a total hip replacement thanks to the Birmingham hip.In urologic surgery, St. Peter's also uses the Holmium Laser Enucleation of the Prostate (HoLEP), a minimally invasive laser system that can shrink an enlarged prostate.Other laser procedures at St. Peter's include the Nd:Yag Laser, which shrinks esophageal, bronchial and bladder tumors and urethral polyps.Opthalmologic/Ophthamologic is the area in which surgeons treat retinopathy of infants; and the CO2 Laser, which can remove or vaporize certain types of skin lesions, as well as shrink or remove certain types of throat cancers and lesions. It is also used by gynecology, urology and ENT Surgeons.St. Peter's offers many more advanced surgical procedures, including:- Mammosite(R) breast radiation therapy; - robotic gynecologic and prostate surgery; - breast MRI; - needle localization; - intensity modulated radiation therapy (IMRT); - PET (positron emission tomography); - computed tomography (CT or CAT scanning); - Neurosign 100 Facial Nerve Monitor; - uterine balloon thermotherapy; - video assisted endoscopy; - image guided neurosurgery; - sentinel node gamma guidance system; and, - shock wave lithotripsy. Surgical Services: In 2012, St. Peter's Hospital's operating room serviced 7,639 inpatient surgical cases (Orthopedic - 2,658, General Surgery - 1,020, Neurological - 972, Thoracic - 552, Gyn & Gyn Oncology - 397, Gen Colorectal - 413 and various others). The operating room also serviced 8,246 outpatient cases (General Surgery - 2,916, Ear Nose and Throat - 1,031, Orthopedic - 642, Urology Closed - 744, Gyn & Gyn Oncology - 1,279, Neurological - 529 and other cases).Description

DescriptionSt. Peter's Cardiac and Vascular Center is a world class facility providing state of the art diagnostic, treatment and rehabilitative services. Led by a premier team of cardiologists, cardiac surgeons and vascular specialists, our integrated team approach helps patients achieve the best possible outcomes.Since opening in 1985, St. Peter's Cardiac & Vascular Center has been recognized for excellence by health and heart care organizations across the countryA nationally recognized, Top 100 Cardiovascular Hospital, St. Peter's Cardiac & Vascular Center is led by a premier team of cardiologists, cardiac surgeons, and vascular specialists. This world-class facility is a leader in the Capital Region and one of the most active centers in New York State for the diagnosis, treatment and rehabilitation of persons with cardiac and vascular disease. The Cardiac and Vascular Center's new cardiovascular surgery facilities are among the nation's most advanced surgical environments for surgery and post-surgical care of patients with complex heart and vascular disease. Among the innovative technology in the facility is a unique "true Hybrid Operating Room" - the ArtisZeego multi-access system for surgical angiography. Upon its completion, this new operating room system was reported by its manufacturer to be one of only 29 such installations worldwide.St. Peter's nationally recognized surgical team and state-of-the-art operating rooms are the perfect combination, and our integrated team approach helps patients achieve the best possible outcomes. In 2011 we welcomed a new cardiovascular surgeon, Chief of Cardiac and Vascular Services to our team. He will oversee a comprehensive process to further enhance and expand the scope of St. Peter's nationally renowned program to prevent and treat cardiac and vascular disease in northeastern New York State. In 2012 we recruited 2 new Cardiac Surgeons, who were very well trained from academic teaching hospitals. We also initiated a new ECMO (extracorporeal membrane oxygenation) to manage critically ill cardiac patients requiring surgical intervention and life-saving treatments. We have developed a system wide Cardiac and Vascular Service line, across St Peter's Health Partners. Development of a service line approach to CV care within a hospital or hospital system takes advantage of these facts and engages various members of the healthcare team in a manner that shifts responsibility for quality, cost and data management to those actually delivering the care.As part of this CV service line we are surgical back up program to our partners in Troy, NY, a free standing Coronary intervention catheterization laboratory. We have managed to support and save many lives in our collaboration with our sister program. In 2012 we expanded our surgical program to treat the most critically ill patients suffering form severe heart failure with the implementation of a left ventricular assist device program. This program supports these patients survival during intensive treatment or preparing for coordination of heart transplant. Our catheterization program expanded its services to percutaneous closure of atrial septal defects. Previously done through open heart surgery, some patients are candidates for closure of this heart defect thru less invasive measures. This allows for quicker recovery time and quicker return to activities of daily living. In 2012 we developed balloon valvuloplasty program to alleviate heart failure symptoms and prepare some of these patients for minimally invasive valve surgery. Our goal is to initiate a TAVR (Transaortic valve replacement) program in 3rd quarter 2013.St. Peters Cardiac Catheterization Lab underwent the voluntary and rigorous ACE(TM) process to demonstrate an exceptional commitment to providing the best possible care to patients. The in-depth, independent review examines quality-assurance processes, facility equipment, personnel, and clinical outcomes information. All data collected is measured against nationally accepted standards for the highest-quality cardiovascular care. We were awarded ACE (Achieving cardiovascular excellence) in July 2013 and we were the first program in NY State to achieve this accreditation.St Peter's Echocardiography laboratories underwent an evaluation by ICAEL (Intersocietal Accreditation program) through this achievement; our facility has demonstrated its commitment to quality patient care. Our emergency response program meets or exceeds the national average in responding to persons suffering a myocardial infarction.By treating the person with emergency angioplasty the damage to the heart muscle is limited. The arrhythmia program continues to be a regional referral center to treat patients suffering from cardiac rhythm disturbances that can be debilitating or life threatening. Our goal in 2013 is to establish an Atrial Fibrillation center of excellence, St Peter's hospital is the largest Electrophysiology center in the capital region. The cardiac rehabilitation and wellness program expanded to provide therapy and rehabilitative services to patients suffering from chronic pulmonary diseases. Thru exercise, education and lifestyle management persons with lung disease can increase their functional capacity. The St. Peter's program performed over 3800 invasive cardiology /arrhythmia procedures, 600 surgical procedures and a full complement of non invasive diagnostic testing, prevention and rehabilitation services.St. Peter's Cardiac Cath Lab performed 2,463 cases in 2012. We performed 1,731 Diagnostic Procedures and approximately 682 PCI's.Description

ExplanationExplanationReturn ReferenceReturnReferenceIdentifierIdentifier
Part I, Line 7: The costs reported on Schedule H, Part I, Lines 7a, b, and f are all derived from cost-to-charge ratios. These cost-to-charge ratios are calculated at the department level. Lines 7 e, h, and i are from the cost of the actual community benefit programs conducted.
Part II: Programs listed under Community Building Activities (F) in CBISA:Elizabeth Street Park ImprovementsSt. Peter's, as a member of the Strategic Alliance with the Albany County Department of Health, is committed to creating Healthy Communities in the City of Albany's Arbor Hill, West Hill, North Albany and South End neighborhoods. The new playground was installed at Elizabeth Street Park during a "build day" with the help of City Department of Recreation employees and volunteers from St. Peter's and the First Church in Albany. Unstructured, child-directed play has been proven to help kids develop physically, socially and cognitively, yet today's kids have less time and fewer opportunities to play than any previous generation. The new playground equipment at Elizabeth Street Park is centrally located, and will benefit countless children and families. Sister Maureen Joyce Center ImprovementSt. Peter's and the United Way of the Greater Capital Region joined together to show our community first hand that SPH is committed to improving the quality of life and extending our Mission to those in need. The Sister Maureen Joyce Center cares for our most vulnerable neighbors-the poor, the homeless, veterans in need, the mentally ill and those addicted to drugs and alcohol. Our team spent a couple of hours painting the fence, preparing it for the winter months. The facility is a wonderful place-both inside and out-for the community that truly benefits from the services they provide.Asthma CoalitionSt. Peter's Staff participates on a steering committee on a monthly basis to coordinate community activities related to Asthma for the Underinsured and Uninsured children of the capital region.HCDI - Healthy Capital District Initiative Board The Healthy Capital District Initiative (HCDI) is a not for profit that has helped over 35,000 needy children and adults in the capital region get health services that they might not have accessed otherwise. Healthy Capital District Initiative (HCDI) represents an ongoing partnership effort to develop a strategic action work plan designed to improve the health of the residents of the Albany, Schenectady and Rensselaer Counties. HCDI was undertaken in the spirit of Communities Working Together For A Healthier New York, an initiative to use community forums to identify high-priority health concerns. Facilitated Enrollment Services-through St. Peter's participation on HCDI-Healthy Capital District InitiativeImprove access to care through enrollment in Family Health Plus, Child Health Plus A (Medicaid), Child Health Plus B (HMOs). In 2012, HCDI staff successfully enrolled the following number of individuals in government sponsored insurance programs:- Medicaid 1369- CHP - B 615- Children 1642- Adult 1256Children's Oral Health through St. Peter's participation on HCDI - Healthy Capital District InitiativeHCDI's Seal a Smile program provides exams, cleaning and sealants (where appropriate) to children who are uninsured or who have Medicaid. The statistics for 2012 are:- Total Children Seen 3326- Total Children Visits 4987- Total Child sealants 1446- 43 Schools & Head Start programs servedHealthy Hearts on the Hill CoalitionHealthy Hearts on the Hill Coalition is a collaborative initiative of the Center for Excellence in Aging & Community Wellness, and Koinonia Primary Care aimed at increasing awareness and participation in heart-healthy behaviors. Supported by lay advocates and community partners, such as St. Peter's Hospital, this coalition aims to reach its goals of increasing access to health care, physical activity and better nutrition in the West Hill, Arbor Hill, and South End communities of Albany, New York.Leukemia & Lymphoma Society BoardExecutive Director on Board of Leukemia & Lymphoma Society NY/VT Chapter is a member of the Board of Directors as well as the Patient Services Committee. It is also important to note the following activities as part of Coalition Building:St. Peter's Addiction Recovery Center & Homelessness ProgramThe Addiction Recovery Center for Healing (ARCH). Bad Debt expense was reported by distinguishing between that portion of bad debt that resulted from contracted rates (ex. unpaid coinsurance, co-payments, deductibles) vs. that potion of bad debt estimated to be more representative of billed charges. Bad debt resulted from unpaid liabilities stemming from contractual arrangements was included as the full amount of bad debt recognized. That portion of bad debt estimated to be more representative of billed charges was reduced to an estimated cost based on an RCC by major service line (ex. inpatient, outpatient, home care).Part III, Line 3:A percentage of the Part III, Line 2 bad debt expense is reported on Line 3. This percentage is based on the self pay accounts with no payments that were transferred to bad debt as compared to the all other payors. The rational is that these self pay patients would have qualified for the charity care program had they applied.Part III, Line 4:The text of the footnote that describes bad debt expense in contained on page 13 of the attached consolidated financial statements.Part III, Line 8:The shortfall on Part III, Line 7 represents the difference between the revenue related to Medicare patients (including revenue received for DSH and other pass-through reimbursements) and the costs associated with providing these services. The revenue and costs were both obtained from the Medicare Cost Report.Part III, Line 9b:In order to preserve the dignity of patients and to facilitate the process of securing necessary information, SPH strongly prefers to perform financial screening upon scheduling, admission or registration (discharge processing in the ED) as part of the overall financial counseling process as described in the CHE Key Performance Indicator System policies 1. SPH refers patients who represent increased financial risk as a result of the amount they are expected to owe "out-of-pocket" to a Financial Counselor for assistance in applying for alternative payment programs, determining Charity Care eligibility, establishing payment plans or other payment arrangements. 2. SPH makes financial counseling services available to all patients.Part III, Line 2:
Part V, Section B, Line 12h: The payment that St. Peter's Hospital accepts from the patient for a service shall not exceed the payment the hospital would have received from Medicare. The discount from charges explicit in this policy is meant to approximate such payment with administrative simplicity. Should the discount calculated for any specific instance result in an amount greater than the amount that Medicare would pay for the same services in a hospital setting, the discount will be further adjusted to equal the amount that would have been expected from Medicare.St. Peter's Hospital
Part V, Section B, Line 14g: We provide information about availability of the financial assistance program on patient statements.St. Peter's Hospital
Part VI, Line 2: St. Peter's participates in collaborative planning through the Healthy Capital District Initiative (HCDI) to assess community health needs. As part of the Healthy Capital District Initiative, St. Peter's collaborated with local public health agencies, community agencies, the business community and other providers to create the 2009 Community Health Profile. HCDI member organizations include Albany County Department of Health, Albany Medical Center, Catholic Charities of the Catholic Diocese of Albany, Capital District Physicians' Health Plan, Ellis Hospital, Fidelis Care, Northeast Health/Samaritan Hospital/Albany Memorial Hospital, Rensselaer County Department of Health, Schenectady County Public Health Services, Senior Whole Health, Seton Health/St. Mary's Hospital, St. Peter's Health Care Services, Whitney M. Young, Jr. Health Services. St Peter's administered a paper and pencil community needs assessment that targeted the five zip code areas that represented communities with the highest Community Need Index scores (12202, 12204, 12206, 12210, 12144). The five zip codes are located in the primary service area of St. Peter's Hospital and are within a five mile radius of the hospital. St. Peter's participates in the Albany County Strategic Alliance for Health initiative intended to combat the concurrent epidemics of cardiovascular disease, diabetes, and obesity. In collaboration with the Healthy Capital District Initiative, St. Peter's participated in the production of a televised Community Health Forum to provide citizen input to the state-mandated Community Health Assessment. The one-hour program featured a studio audience of about 80 public health experts and stakeholders as well as a broad range of consumers from the three counties. Also call-in questions and comments from viewers were broadcast live.
Part VI, Line 3: As members of the Catholic healthcare ministry who are deeply committed to caring for those who are poor, St. Peter's Hospital has established respectful and effective procedure for addressing the needs of those persons who are unable to pay for all or most of their care. Patients who represent increased financial risk as a result of the amount they are expected to owe "out-of-pocket" are referred to a Financial Counselor for assistance in determining Charity Care eligibility. As a not-for-profit Catholic Healthcare facility St. Peter's has remained consistent in the administration of our charity care program. St. Peter's prides itself on providing free and reduced health care services to those who are in need that live in our community. Annually, St. Peter's budgets an amount for Community Benefit, which includes our financial aid program. This information is posted throughout the facility, on our user-friendly web site, on all patient-billing statements, in the Ambulatory Care Network clinics and by word-of-mouth.St. Peter's has a number of programs in place to provide a comprehensive financial counseling to our patients. Uninsured patients are screened by a financial counselor at the point of registration to identify which type of program they may be eligible for. St. Peter's invested in software to enable us to improve the up-front financial counseling process to validate patient information upon registration which enables us to assist patients who need it most in real-time without any delay. We have formed a Charity Care workgroup which meets bi-weekly to review Charity Care cases that assist us to continually improve our systems and processes to best serve the needs of the most vulnerable. The greatest challenges regarding St. Peter's Financial Aid Program has always been to encourage the patients to complete the Charity Care application along with providing any documentation that demonstrates the patients' income levels. Ongoing patient education is required in order for us to make a determination on providing free or reduced cost care. More information about our Financial Assistance program is available online.Public notice of the availability of financial assistance is made through each of the following means:- Notices are posted in a visible manner in locations where there is a high volume of inpatient or outpatient admitting/registration, such as emergency departments, billing offices, admitting offices, and hospital outpatient service settings.- Information regarding financial aid (Charity Care) is included on all patient liability statements (bills) and statements sent to patients indicating: a) that if the patient meets certain income requirements that the patient may be eligible for a government sponsored program or for financial assistance from the hospital, and b) a hospital contact name and phone number that patients may call in order to gain information on the hospital's Charity Care, reduced payment, and other financial assistance policies.- Availability of assistance, contact names and phone numbers posted on St. Peter's Hospital web site.- Uninsured patients are provided with a copy of the Charity Care Application and Brochure upon registration.- St. Peter's annually makes available to the public on a reasonable basis the costs of charity care provided and the unreimbursed costs of care provided to beneficiaries of government programs that serve the poor.- Posted notices are in the primary language(s) of the service area and in a manner consistent with all applicable federal and state laws and regulations.
Part VI, Line 4: St. Peter's identifies Albany and Rensselaer Counties as its primary services areas and portions of Schenectady, Saratoga and Columbia Greene Counties for its secondary service areas in planning for the Community Service Plan. St. Peter's Hospital defines its Primary Service Area as the zip codes that account for the top 65% of inpatient discharges and its secondary service area as the zip codes that make up the next 15% of inpatient discharges. This method of defining the service area is the standard used by the corporate governing body of St. Peter's Hospital. St. Peter's Hospital is located in Albany, the County Seat and State capital of NY near the state's eastern border. Over 40% of St. Peter's 2004 inpatients resided in Albany zip codes. In general, persons in the community served by St. Peter's tend to be better educated and have a higher income than those in the U.S. as a whole and the state of NY. There is a lower rate of unemployment and fewer persons without health insurance than the state or national comparisons. There are 191,936 families in the service area, 16.1% of which are single parent families. (U.S. 15.7%). There are 336,040 housing units in the service area. 57.9% are owner occupied, 34.6% are renter occupied and 7.5% are vacant. (U.S. 59.9%, 30.4%, and 9.7% respectively). 7.0 % of families live below the poverty level, compared to 7.7% of all Albany County families and 11.7% of all families residing in NY. (U.S. 9.0%). 14.6% of area residents aged 25 or older have not graduated from high school, compared to 13.8% of Albany County adults and 21.0% of adults statewide. (U.S. 19.4%). 29.3% of residents aged 25 or older have received Bachelor's Degrees, compared to 33.4% of Albany County adults and 27.3% of adults statewide. (U.S. 25.6%). The unemployment rate in the service area is 5.6%, which is lower than the county level of 7.1% and the state's level, also 7.1% (U.S. 5.7%). In 2004, 15.0% of NY residents had no health insurance; 8.6% of children had no health insurance. In 2003, 8.3% of Albany County residents had no health insurance. In aggregate, 18% of non-elderly adults and 12% of children are uninsured in the United States.
Part VI, Line 5: In February 2011, St. Peter's Cancer Care Center hosted the Clinical Breast Exam (CBE) Skills Update Training through the University at Albany School of Public Health. This free training was offered to Registered Nurses, Physicians Assistants, Nurse Practitioners and Physicians administering clinical breast exams to women. Patient needs identified as part of the community planning process include Access to Quality Health Care and Chronic Diseases. St. Peter's has incorporated this information in its planning process. In addition to these ongoing community planning processes, St. Peter's developed a ten-year strategic planning process in 2008, focused on the care, services and structure necessary to meet community needs in the next decade. The process included review of a comprehensive community needs assessment, current market and demographic data and numerous focus groups of key stakeholders; the sponsors, Board members, physicians and management staff. In addressing the Prevention Agenda, St. Peter's Health Care Services addressed the priorities of Access to Quality Health Care and Chronic Disease with a focus on health coverage, dental care, breast cancer, lung cancer, colorectal cancer, diabetes, coronary heart disease and congestive heart failure through our goal to reach out to those most in need in order to demonstrate an improvement in the health of the vulnerable populations in the communities we serve. In collaboration with our community partners, we committed to and completed NYS Prevention Agenda measures: - Increase the percentage of adult New Yorkers with health coverage- Increase the percentage of adults who see a dentist - Increase the percentage of breast cancer cases diagnosed at an early stage- Reduce the incidence of lung cancer.- Reduce the prevalence of adults with diabetes and hospital complications of Diabetes- Reduce mortality rate for colorectal cancer- Reduce coronary heart disease hospitalizations- Reduce CHF hospitalization rateSt. Peter's provided Open Medical Staff Grand Rounds in 2012 on the following topics:- NYS Mandated Infection Control Training- Preventing A-Fib related strokes with anticoagulants- Esophageal Rupture w/lung cancer & multiple system disease- Update from NYS DOH - Bureau of Narcotic Enforcement- Inflammation & Chronic Disease: bringing the fire under control?- Calcium Channel Blockers & Beta Blockers: Toxicity & Mgmt.- FM Case: Hypokalemia with Non-Anion Gap Acidosis- Ultrafiltration with Aquadex in Pts. with Fluid Overload & CHF- Evolution of Human Diet & Its Connection to 21st Century Complex Disease- Neurologic Paraneoplastic Syndromes - Case Report- Perspectives on End Stage Renal Disease in the Elderly- Alpha-1 Antitrypsin Deficiency- Estimation of Kidney Function for Drug Dosing in Obesity- Atrial Fibrillation Management 2012- Omega-3 Fatty Acids and Inflammation- Improving Acute Ischemic Stroke Care- Surgical Treatment of Mitral Valve Disease- Metastatic Melanoma Treatment
Part VI, Line 6: As a member of Catholic Health East, our Vision, Mission and Core Values compel us to identify unmet community needs to demonstrably improve the health of vulnerable populations. In 2012, Catholic Health East provided over $368 million in community benefit programs and services. But faces speak louder than numbers. Individuals and families whose lives have been touched and changed through interaction with our ministry's community benefit programs and services are the guiding force to continue to serve our communities. Remaining faithful to extending the mission of person-centered care, especially for those who poor in a changing economic and political environment, CHE strives to expand its participation in the community benefit process with a partnership in assessing the need, planning, implementing and evaluation of the community benefit provided at each Regional Health Corporation (i.e. St. Peter's) Community benefit is grounded in the belief that those living in poverty have a moral priority for services. Programs designed to increase access to health care and to improve the health and lives of low-income and homeless persons should have top priority.
