ExplanationThe Children's Hospital of Philadelphia has established controls in place to monitor the use of grant funds both outside and within the United States. Expenditures are monitored based on the guidelines outlined by 45 CFR Part 74 Appendix E (OASC-3). It is our policy to follow the federal government-established principles for determining costs applicable to grants, contracts, and other agreements. The Hospital generally applies these same cost principles to non-federal funding. All costs posted to sponsored projects must comply with government and sponsor rules and regulations. Costs must meet several criteria: (1) costs being charged to a grant must be reasonable and necessary for meeting the objectives of the grant/project, (2) costs must be allowable in accordance with the sponsor rules and regulations, (3) costs must be allocable based on the benefit derived, cause and effect, or other equitable relationship, and (4) costs must be consistent with costs charged in similar circumstances to other sponsored projects.Explanation

Form990 Schedule H Part VI Form990ScheduleHPartVI (2 entries)

IdentifierIdentifierReturn ReferenceReturnReferenceExplanationExplanation
SCHEDULE H DISCLOSURESPART I, LINE 3C:The organization's financial assistance policy requires that available asset information be reviewed in all cases in addition to income; however, the policy also provides that a patient's/family's primary residence or vehicles needed for regular transportation are not considered in assessing available assets. The financial assistance policy also indicates that patients/families must be residents of CHOP's Primary Service Area. Exceptions to this residency requirement are granted on a case-by-case basis. Part I, Line 7: Total and net community benefit expenses have been assigned in Part I, Line 7 as follows. A ratio of patient care cost to charges based on Worksheet 2 (Schedule H Instructions) was applied to determine expense for financial assistance and Medicaid. Medicaid expense also includes a provider assessment paid to Pennsylvania and other states that require such payments. Costs for community health improvement and community benefit operations are also included. Part III, Line 4: CHOP's audited financial statements include the following statement regarding bad debt expense, accounts receivable, or allowance for doubtful accounts for the Controlled Affiliates: "Primary collection risks relate to uninsured patients and the portion of the bill which is the patient's responsibility, primarily co-payments and deductibles. We estimate our provisions for doubtful accounts based on a number of factors, including aging of the receivables and the historical collection experience. Receivables for patient services are recorded net of allowance for doubtful accounts of $142,385,000 and $109,028,000 as of June 30, 2012 and 2011, respectively." CHOP's patient accounts receivable is reduced by an allowance for uncollectible accounts for amounts that could become uncollectible in the future. In accounting for bad debt expense, uncollectible self-pay accounts (including patient account balances due after reimbursement from insurance) are written off as bad debt expense after exhausting collection efforts. In addition, certain amounts are recorded as bad debt expense after CHOP routinely analyzes the historical cash collections of its patient accounts receivable. Part III, Line 9b: CHOP's bad debt policy and partial financial assistance policies, as well as its financial assistance policy and all related internal policies and practices, provide that financial assistance (and partial financial assistance) cases are designated in the system in a payor category that is not sent to outside collections. Part V, Line 13g: Information regarding CHOP's financial assistance policy is available at http://www.CHOP.edu/visitors/financial-matters/financial-assistance.html. A written summary of the financial assistance policy is provided to patients upon admission. Part V, Line 19: For those families whose income is greater than 201% but less than 400% of the Federal Poverty Guidelines, CHOP provided partial charity care under its financial assistance policy. Prior to sending a bill for services to those eligible for partial charity care, the bill is reviewed by an individual in the Business Office to ensure the amount charged is not in excess of CHOP's lowest negotiated commercial insurance rate. Needs Assessment Description Part VI, Line 2: As part of its extensive research and community service activities, CHOP performs cause-specific assessments of community health needs. For example, CHOP is a key participant in the Philadelphia Collaborative Violence Prevention Center (PCVPC). PCVPC brings together academic institutions and community-based organizations to conduct community-based participatory research. PCVPC consists of a central research model that takes promising models of primary prevention programs for youth and adapts them for use in the West and Southwest Philadelphia communities in partnership with community members and community-based organizations. Examples include youth development programs in schools and afterschool settings, as well as restorative justice programs such as Youth Courts in Philadelphia schools. PCVPC has also evaluated community-level interventions such as the impact of greening of vacant properties on neighborhood crime. A key goal of the PCVPC is to learn how to best share research results with the community and incorporate community feedback into the research design. CHOP believes that this research-oriented focus on specific community needs is an effective method of meeting specific health needs of the communities it serves. Other CHOP programs that aim to decrease violence in the community are the Violence Intervention Program (VIP), which assigns a case manager to assault-injured youth seen in the emergency department or trauma unit, and the Children and Moms Program (CAMP) which addresses domestic violence issues in CHOP's clinic families. Many of CHOP's community programs are provided in collaboration with community agencies. Community health needs are identified and addressed through these collaborations. For example, CHOP's Connect to Protect Project is a community-level prevention research initiative. The Connect to Protect (C2P) Coalition strives to reduce HIV/AIDS rates among those most at risk, young men who have sex with men, and their networks through the collaborative efforts of local communities and health researchers. This initiative is a part of The Adolescent Initiative, an integrated system of clinical care, research, training and prevention with culturally and developmentally appropriate education to adolescents at risk for or infected with HIV. Another community program, CHOP's Center for Leadership Education in Neurodevelopmental Disabilities (LEND) develops partnerships that enhance the health infrastructure of communities CHOP serves by improving services and resources for children and families of children with, or at risk for, neurodevelopmental disorders including autism spectrum disorders. LEND fellows collaborate with community-based organizations and/or government agencies on projects to improve the health of children in homeless shelters, improve the health of children in the foster care system, improve employment opportunities for youth and young adults with disabilities, and improve access to public transportation systems for individuals with autism spectrum disorders among other projects. CHOP's on-going partnership with organizations through these and many other programs assist it in its on-going assessment of the needs of the communities it serves. CHOP's Homeless Health Initiative (HHI) is a voluntary health outreach program that represents a coordinated, multi-disciplinary approach towards the goal of helping at-risk children in shelters achieve optimal health and life potential, thereby breaking the cycles of homelessness and disenfranchisement. HHI has continued to meet regularly with shelter families and staff and community and city government stakeholders to better assess their needs, as well as collaborate and advocate for children experiencing homelessness. HHI hosted and organized two city-wide stakeholder meetings and organized speakers on a variety of topics including lead screening, infant development, and early intervention. HHI leadership actively participates on city committees such as the Trauma Informed Network, Homeless Services Children's Workgroup, and menu/nutrition planning committee. HHI has also continued to promote the Family Care Training Model-locally and regionally. Family Care is a train-the-trainer parenting model designed specifically to meet the needs of families in shelter. Finally, CHOP owns and operates PolicyLab, whose mission is to achieve optimal child health and well-being by informing program and policy changes through interdisciplinary research. PolicyLab is one of nine Centers of Emphasis within CHOP's Research Institute. PolicyLab researchers are methodological experts in the disciplines of epidemiology, health services research, program evaluation, and ethics, seeking evidence-based solutions to the most challenging health-related needs affecting children. The express mission of PolicyLab includes a commitment to transforming "evidence to action," bridging the gap between academic communities and the real world by engaging in research that is responsive to community needs and relevant to policy priorities. This focus on broadly but rigorously assessing child health and community needs as part of its research mission is a unique aspect of CHOP. In TY 2011, PolicyLab partnered with numerous stakeholders, including the US Department of Health and Human Services, the Commonwealth of Pennsylvania, City of Philadelphia, and other hospitals throughout Pennsylvania in traditional healthcare and other community settings to assess needs and identify the programs, practices, and policies that support the best outcomes for children and their families. Patient Education Of Eligibility Of Assistance Part VI, Line 3: CHOP's family health coverage program
Other InformationPart VI, Line 5:CHOP has a governing board - its Board of Trustees - in which independent persons representative of the community comprise a majority. CHOP operates a full-time emergency department open to all children, regardless of ability to pay. As described elsewhere in this Schedule H, CHOP treats a substantial number of Medical Assistance patients and, further, provides substantial amounts of financial assistance. In addition to a full range of patient care programs (many of which are specialized services that may not be available elsewhere), CHOP also operates extensive unparalleled research activities, and a broad range of community education and service programs. Some of these services and CHOP programs are described below. In addition to its financial assistance and other community benefit activities described elsewhere on this Schedule H, CHOP offers a number of services that uniquely reflect its integration of high quality patient care, innovative research and a commitment to education. Some of these CHOP specialized services include: a comprehensive center - one of only a few in the world - offering technologically advanced, multidisciplinary care for unborn babies with genetic or anatomical abnormalities; a cancer center integrated with CHOP's pediatric cancer research program (which is the largest pediatric cancer research program in the world), through which CHOP launched the world's first proton beam therapy service exclusively for pediatric patients; a program for ventilator-dependent children (including a home care program for ventilator dependent patients as an alternative to inpatient treatment); a Hematology Acute Care Unit (HACU) in the Sickle Cell Center which provides rapid evaluation and treatment for children with blood diseases; and, other clinical programs, ranging from primary care through tertiary and quaternary care that are routinely recognized as among the best in the world. CHOP operates or participates in a wide range of community programs. One category of programs focuses on prevention and outreach and includes such programs as the Community Asthma Prevention Program, a research-based asthma education program for Philadelphia communities, a Diabetes Center, and the Sickle Cell Center at CHOP that, in addition to providing patient care, focuses on patient and community education in underserved areas of Philadelphia. In 2011, CHOP's Adolescent Initiative served 163 HIV-infected youth and more than 3,000 high-risk HIV negative youth, providing an array of services including outreach case management to link infected youth to care in addition to comprehensive HIV care and primary care and other services. The Hospital also provides HIV counseling and testing services at several community settings including a secure youth detention facility and a homeless shelter for high-risk youth. Over the past decade, the Adolescent Initiative has served on average 393 high risk HIV negative youth per year with prevention case management. In addition, the Adolescent Initiative provides reproductive health education services at Health Resource Centers in two high schools in West Philadelphia. Free and confidential family planning services are also offered to adolescents at three of CHOP's primary care locations. CHOP's Youth Heart Watch program also holds free screenings throughout the community to find young people with potentially serious, yet undiagnosed, heart conditions. Early intervention programs are another aspect of CHOP's community programs and includes: Early Head Start for low-income pregnant women and their families, and Reach Out and Read, a national program supporting reading and literacy. An additional category of community programs focuses on improving access to health care and includes the Homeless Health Initiative, which provides medical education and prevention activities on-site in shelters and through the medical home at a CHOP primary care center. The Hospital also offers a Language Services Program which provides support to community members with limited English proficiency and our Center for Childhood Communication has a Family Wellness Program for children with hearing loss. CHOP community programs also focus on injury and violence prevention and include programs such as Safe Place: The Center for Child Protection and Health, which provides health care to children who have been maltreated and those in substitute care. The Hospital also provides a number of educational programs for foster parents and for child welfare and law enforcement professionals. CHOP operates a regional Poison Control Center providing toxicology services to the State of Delaware and 23 counties throughout Pennsylvania, other hospitals, healthcare providers and consumers. The Kohl's Injury Prevention Program offers a wide range of education and safety devices. In addition, CHOP's Special Babies Clinic provides programming for high-risk and preterm infants through its Special Trips for Special Babies program that facilitates visits to area attractions in an effort to foster greater childhood development for over 230 individuals, of which 99 percent are covered by public health insurance. In addition to community programs in Pennsylvania, CHOP also supports a Global Health Program, which sends CHOP staff to give care to children both inside and outside the United States' borders. Also, CHOP runs a Children's Intensive Emotional and Behavioral Program (CIEBP), which provides comprehensive partial hospital services in a behaviorally-based therapeutic setting for children between the ages of 5 and 12. The Program serves children with significant behavioral, emotional and social needs from throughout the South Jersey region. The overall mission of the program is to improve child and family functioning so that a child can remain and be productive in his or her home, school and community. CHOP's pediatric residents participate in the Community Pediatrics and Advocacy Program, an integral part of each trainee's rotations each year. Community-based projects include developing health curriculum for 5th graders at Penn Alexander School, helping young adults with special health care needs transition into adulthood, and partnering with the Philadelphia Fire Department to deliver pediatric Continuing Education Units for paramedics. The Community Nursing Advocacy Fellowship (CNAF) gives nurses the opportunity to serve in the community and is the only fellowship of its kind in the nation. Some of the projects these nurses have developed include creating a physical fitness program for kids who live in a homeless shelter, initiating a weekly meeting for teenage girls to address topics including sexual health and peer pressure, and developing pediatric guidelines for a low cost clinic and for Early Head Start home visitors. An additional category of CHOP community programs focuses on children with disabilities and includes the Leadership Education in Neurodevelopmental Disabilities (LEND) Program which advances the care of children with neurodevelopment and related disorders and develops community partnerships to improve services and resources for these children. CHOP's REACH (Rapport, Empowerment, Advocacy through Connections and Health) program prepares teens and young adults with special healthcare needs for a successful transition into adulthood by providing peer support, guidance and resources. The program also offers mentorship and training to help youth acquire the skills needed to develop and maintain partnerships among families, youth and providers as they move through transition. CHOP also was recognized by the Commonwealth of Pennsylvania's Department of Labor and Industry as an exemplary employer who employs individuals with disabilities. Since 2007, the CHOP Career Path team has worked closely with the Pennsylvania Office of Vocational Rehabilitation to train and place young adults with chronic illnesses and disabilities into real competitive positions. CHOP operates or participates in many research-based education programs such as the Center for Autism and Developmental Research and Epidemiology and "Connect to Protect" (C2P) a community-level prevention research project of the Adolescent Trials Network for HIV/AIDS Interventions. CHOP also operates the Healthy Weight Program, which aims to prevent and treat pediatric obesity. CHOP also provides financial and programmatic resources for various community needs and initiatives. Some of these initiatives include providing 500 U.S. Coast Guard approved life jackets and educational sessions on swim safety, hydration and skin protection at City of Philadelphia swimming pools. CHOP sponsors high school interns participating in summer jobs through the Philadelphia Youth Network's "WorkReady Philadelphia" Program, donates computers to area schools and community groups, such as the Philadelphia Youth Music Partnership, and participates in the Anti-Defamation League's "Walk Against Hate".

FORM 990, PART IV, LINE 12An audit is performed on an Obligated Group which comprises the Children's Hospital of Philadelphia, the Children's Hospital of Philadelphia Foundation, and First Medical Insurance Company. An audit is also performed on a consolidated basis which includes the obligated group, PGH Development Corporation, Children's Anesthesiology Associates, Ltd., Children's Anesthesiology Associates of NJ, Inc., Children's Health Care Associates, Inc., Children's Health Care Associates of NJ, Children's Surgical Associates Ltd., Children's Surgical Associates of NJ, Inc., Surgical Research and Education Foundation and Radiology Associates of Children's Hospital, Inc. Form 990, Part VI, SECTION B, Line 11B A copy of this return was reviewed by the Board Audit & Compliance Committee before the return was filed. Before the return was filed, the Form 990 was also made available to the entire governing body of the organization through an accessible shared computer network drive maintained by The Children's Hospital of Philadelphia. FORM 990, PART VI, SECTION B, LINE 12 The Children's Hospital of Philadelphia maintains a written Conflicts of Interest Policy that applies to, inter alia, all of its Trustees, officers, employees, members of the Medical Staff and researchers of The Children's Hospital of Philadelphia and its affiliates. The Policy requires annual conflicts of interest statements from: Trustees and officers of the Hospital and its affiliates; the President and others in Senior Management; administrative personnel serving at the level of manager and above and certain other categories of employees defined in the conflicts of interest policy (such as persons known to place orders with vendors); Department Chairs, division chiefs and other members of the Medical Staff (except that members of the Medical Staff who are not based at the Hospital are only required to submit statements bi-annually); members of the Hospital Research Staff; designated employees of practice plans affiliated with the Hospital; and other persons designated by management. The statement tracks the conflicts of interest policy, requiring each person to disclose information for the reporting period regarding the existence and nature of gifts, outside interests, outside activities and other matters constituting a potential, perceived or actual conflict of interest, and to certify that they have read the policy and answered fully, accurately and to the best of their knowledge. After confirmation that all questions have been answered, the statements are reviewed by the relevant Vice President, Department Chair or other executive (or their designee), tracked by the Office of Compliance and Privacy (OC&P), and all statements disclosing potential, perceived or actual conflicts are forwarded to the OC&P as well as the Office of General Counsel (OGC) for further review and follow-up as needed. The conflicts statements submitted by Trustees of the Hospital and Foundation, and members of Senior Management, are reviewed by the OGC and OC&P and the disclosures are summarized in memoranda distributed to and reviewed by the members of the Audit and Compliance Committee of the Hospital and Foundation Boards. The remaining conflicts statements containing affirmative disclosures are reviewed by staff in the OGC and OC&P. In reviewing annual statements where an actual, perceived or potential conflict is disclosed, where problems are identified that need to be addressed, the goal is to eliminate or manage the conflict going forward and ensure that, as to employees or others on the Medical or Research Staff, the relevant supervisor is aware of the issue. Summary information about compliance with the policy's requirement to submit annual statements and disclosures contained therein is provided in memoranda to the Audit and Compliance Committee of the Boards. In addition, if any matter involving a potential violation of the conflict of interest policy is brought to the attention of management during the course of the year, a review is conducted by either the relevant department's management, OGC or OC&P, as appropriate. FORM 990, PART VI, SECTION B, LINE 15 The compensation of executives and physicians/scientists is reviewed and approved by an independent Compensation Committee of the Board of Trustees. The Committee reviews and approves in advance the compensation to be provided to the CEO, all Vice Presidents (including Executive and Senior VP levels), Clinical Department Chairs, and all faculty physicians and scientists. For each such person, this process was last performed in 2011. In making its determinations, the Committee considers the performance of the organization and that of the covered individuals as well as related business judgment factors. It also considers market comparison reports prepared by an external independent compensation consultant with significant experience in performing executive and physician compensation assessments for not-for-profit healthcare organizations. The Committee's process is designed to qualify for the rebuttable presumption of reasonableness for those individuals who are disqualified persons. The peer group for executives generally includes large and complex academic medical centers and health systems. For physicians and scientists, the peer group generally includes academic medical centers. Information from other organizations may also be considered where appropriate for the position. FORM 990, PART VI, SECTION C, LINE 19 Form 990 and financial statements are made available upon request. The conflicts of interest policy is available on The Children's Hospital of Philadelphia's website.
