Part I, Line 6aST JAMES HEALTHCARE PRODUCED A COMMUNITY BENEFIT REPORT IN 2012 AND IT WAS MADE AVAILABLE TO THE PUBLIC. IT WAS PUBLISHED AS AN INSERT IN THE LOCAL NEWSPAPER WITH A DISTRIBUTION OF 15,000. IT ALSO WAS PLACED IN ALL OUR WAITING AREAS AND IS ON OUR PUBLIC WEBSITE.
PART I, LINE 7GSUBSIDIZED SERVICES AT ST. JAMES HEALTHCARE THAT WERE COUNTED AS COMMUNITY BENEFIT TOTALED $196,184. THIS AMOUNT INCLUDES PROVIDING ATHLETIC TRAINERS FOR THE HIGH SCHOOLS' ATHLETIC PROGRAMS. TRAINERS ARE PROVIDED TO 7 DIFFERENT SCHOOLS IN SOUTHWEST MONTANA, AND WITHOUT THIS SUPPORT, NONE OF THE SCHOOLS EXCEPT MONTANA TECH (THE COLLEGE IN BUTTE) WOULD BE ABLE TO PROVIDE THIS SERVICE TO THE STUDENT ATHLETES. THESE TRAINERS ATTEND ALL PRACTICES, GAMES AND TOURNAMENTS FOR EACH SCHOOL, AND ST. JAMES PAYS FROM 50 to 80% OF THEIR SALARIES TO PERFORM THIS SERVICE. IN ADDITION, ST. JAMES PROVIDES LOW COST ($5.00) SPORTS PHYSICALS FOR ALL THE ATHLETES. THE TOTAL AMOUNT FOR ATHLETIC SERVICES AND THE SPORTS PHYSICALS WAS $144,257. ST. JAMES HEALTHCARE ALSO PROVIDED A MOBILE PET VAN FOR PATIENTS THAT REQUIRE PET SCANS. SUCH EQUIPMENT IS NOT AVAILABLE IN THE AREA SO PATIENTS WOULD NEED TO TRAVEL TO ANOTHER FACILITY. THE VAN COMES TO ST. JAMES HEALTHCARE PERIODICALLY. SOMETIMES, THERE ARE NO PATIENTS BUT ST. JAMES STILL MUST PAY FOR THE VAN. WE CONSIDER ONLY THOSE PAYMENTS AS COMMUNITY BENEFIT DOLLARS. THE TOTAL IN 2012 WAS $29,150.
Part I, Line 7, COLUMN (f)THE AMOUNT IN COLUMN (F) IS BASED ON THE NET COMMUNITY BENEFIT IN COLUMN (E) DIVIDED BY THE TOTAL OPERATING EXPENSES MINUS BAD DEBT EXPENSE ($84,249,475 MINUS $2,004,919 = $82,244,556). The denominator is $82,244,556.
Part I, Line 7IN PART I, LINE 7, ST JAMES HEALTHCARE PROVIDES THE AMOUNTS OF FINANCIAL ASSISTANCE AND CERTAIN OTHER COMMUNITY BENEFITS AT COST. THE AMOUNTS REPORTED ARE FROM WORKSHEETS 1 THROUGH 8.
PART IIWe extend our care beyond our hospital's walls to improve the health of our community. Our community building activities demonstrate this commitment. Most of our community building activities are in the category of community support. These activities promote health in the community in a number of ways. We provide meals donations to the Mountain View Social Development Center so its clients have nutritious food each day. In addition, we provide soup to the local soup kitchen three times a week to help feed the homeless and those in need. Our employees are involved in a number of volunteer activities throughout the year that promote healthy living. These encompass everything from raising funds for the youth program at the local YMCA to helping distribute food at the local food bank. Also, for our casual dress Friday, employees can bring food donations for the Butte Food Bank. Service clubs throughout the community are focused on promoting healthy life styles for both children and adults, and St. James Healthcare's ("St James") staff actively participates in clubs such as the Kiwanis, the Chamber of Commerce, the Rotary, and the Exchange Club. In the area of community health improvement advocacy, we partner with public health agencies and serve on the boards and committees of these agencies that are focused on improving the health of the community. St. James is the lead agency in a city-wide diabetes network outreach program. Our full-time Diabetes Program Coordinator who is a registered nurse and Certified Diabetes Educator, has developed a community network of partners and members. The program serves both our adult and child population with individual as well as group sessions. Support groups, health fairs, and prevention programs for children and young adults are among the activities that members of our diabetes staff, dietary staff, nursing staff, and administrative staff work many hours to provide. No one has been denied service because of lack of funds. We also provide social workers for a bi-monthly Alzheimer's Caregiver Support Group at the Belmont Senior Citizen's Center. As a medically underserved area, Butte needs more physicians, and St. James actively recruits physicians to the area. When potential recruits make site visits to Butte, we host meetings with local government officials, local primary and secondary educational institutions, the Chamber of Commerce and other organizations to provide a positive and encompassing experience for the potential recruit. In a rural community physicians are an essential part of the community and we encourage them to make connections in this early phase by meeting many of its leaders. In 2012, we successfully recruited or assisted in the recruitment of ten physicians to our area. The recruits were a general surgeon, a vascular-thoracic surgeon, a pediatrician, a radiation oncologist, three emergency medicine physicians, an internal medicine physician, and two family medicine physicians. We also support our employees by encouraging them to volunteer with community organizations, including serving on community boards for organizations such as the local YMCA, Big Brothers and Sisters, Butte-Silver-Bow Public Health Department, City-County Home Health Service, McDonald Family Place, Butte Emergency Food Bank, The Butte Rescue Mission, Safe Space, and The Breast and Cervical Cancer Coalition. We are good citizens and partner with other organizations and agencies to support a thriving community. We support or are members of Butte-Silver Bow Chamber of Commerce, Butte Silver-Bow Local Development Corporation, Kiwanis, Exchange Club, 4-H, The Butte Education Foundation, Rotary, and athletic programs for children and young adults. In addition, St. James is looked to provide for health education in the community and to support and sponsor activities that promote health. The following are details on programs and other activities provided by St. James that promote and enhance the health of the overall communities it serves. * St. James works with public and private high schools throughout southwest Montana and the public college in Butte to provide athletic services that otherwise would not be available to area students, St. James pays a significant portion of the salaries and benefits (50% to 80% or more) for seven athletic trainers who are employed by St. James, but work in these schools with student athletes throughout the school year. St. James partners with physicians and clinics to provide very low-cost ($5) physicals for students wishing to participate in athletic activities. These dollars are then donated back to each participating school's athletic departments. * St. James co-sponsors with St James Healthcare Foundation a Ticker Tuner Fun Run for the community to promote health. *St. James and the Butte Community Diabetes Network hosted a spring through fall walking program, titled Walk N' Talk With A Doc. Each week a different member of our medical staff led a 1.5 mile walk on a community walking trail. The event gave people the opportunity to exercise while getting to know their physician outside the office. The format was an informal question and answer session during and after the walk. Over 300 residents and 15 physicians participated during the 10-week program.
PART III, LINE 4A COST TO CHARGE RATIO IS USED AS THE METHODOLOGY FOR DETERMINING THE AMOUNTS REPORTED AS COST OF BAD DEBT EXPENSE. FOR ALL SELF-PAY ACCOUNTS,A STANDARD 40% DISCOUNT IS FIRST APPLIED TO CHARGES AS A DEDUCTION OF REVENUE. BAD DEBT EXPENSE IS REFLECTED NET OF DISCOUNTS APPLIED AND PAYMENTS RECEIVED. THE ORGANIZATION DOES NOT INCLUDE IN BAD DEBT EXPENSE ANY AMOUNT THAT COULD REASONABLY BE ATTRIBUTABLE TO PATIENTS WHO LIKELY WOULD QUALIFY FOR FINANCIAL ASSISTANCE UNDER THE HOSPITALS' CHARITY CARE POLICY. THE FOLLOWING ARE THE RELEVANT PORTIONS OF THE COMPANY'S FOOTNOTE TO THE FINANCIAL STATEMENTS RELATED TO BAD DEBT: INCLUDED IN NET RECEIVABLES IS THE ALLOWANCE FOR UNCOLLECTIBLE RECEIVABLES. THE ALLOWANCE FOR UNCOLLECTIBLE RECEIVABLES IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS AND TAKES INTO CONSIDERATION HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTHCARE COVERAGE, AND OTHER COLLECTION INDICATORS. MANAGEMENT PERIODICALLY ASSESSES THE ADEQUACY OF THE ALLOWANCES FOR UNCOLLECTIBLE ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. THE RESULTS OF THESE ASSESSMENTS ARE USED TO MODIFY, AS NECESSARY, THE PROVISION FOR BAD DEBTS AND TO ESTABLISH APPROPRIATE ALLOWANCES FOR UNCOLLECTIBLE NET PATIENT ACCOUNTS RECEIVABLE.
PART III, LINE 8St. James Healthcare believes a significant portion of the shortfall from Medicare should be treated as community benefit. St. James provides the resources required to care for these patients and should receive some benefit for doing so. The costing methodology used is the cost to charge ratio because St. James believes this methodology most accurately reflects the actual cost of providing the services.
PART III, LINE 9BAn integral component of our Mission is to be good financial stewards. This requires us to determine which patients are in need of charity care and which are able to contribute some payment for care received. We maintain a balance that enables us to continue to provide charity care to those who need it most, and to ensure that we manage our resources so that we can continue to be here when people need us most. St. James Healthcare notifies patients of its financial assistance policy prior to admission, upon admission and upon discharge. In addition, we provide communication regarding patient bills. Patients are contacted multiple times about unpaid balances prior to initiating any collection action. If a patient is determined to be eligible for financial assistance at any time during the collection process, the account is reclassified as financial assistance and debt collection efforts are ceased. In 2012, St. James provided a total of $3,696,811 in traditional charity care and $2,343,522 for the unpaid cost of Medicaid.
Supplemental Information2. NEEDS ASSESSMENT - ST. JAMES HAS REGULARLY PARTICIPATED IN NEEDS ASSESSMENT SURVEYS TO IDENTIFY THE ONGOING AND CHANGING NEEDS OF THE COMMUNITY. THE MOST RECENT SURVEY WAS FINALIZED IN EARLY 2011 WITH THE NEXT SURVEY SCHEDULED IN 2014. IN 2011, THE ASSESSMENT EXAMINED THE POPULATION OF SILVER BOW COUNTY WHICH IS SERVED BY ST. JAMES AND WAS DONE BY PARTNERING WITH THE LOCAL PUBLIC HEALTH DEPARTMENT AND LOCAL SOCIAL AGENCIES. WITH THE ASSISTANCE OF OUTSIDE CONSULTANTS, THE ASSESSMENT DETERMINED WHERE GAPS IN SERVICES EXIST. FROM THE "GAPS" DOCUMENT, WE CONDUCTED FOCUS GROUP MEETINGS WITH SOCIAL AGENCY PERSONNEL TO DEVELOP OBJECTIVES AND STRATEGIES FOR THE FOLLOWING CATEGORIES OF NEED: AGING POPULATION; ENVIRONMENTAL FACTORS; SOCIO-ECONOMIC ISSUES; SOCIAL AND BEHAVIORAL PROBLEMS; AND HEALTH OUTCOMES AND FACTORS. WITH OBJECTIVES AND STRATEGIES IN HAND, ST. JAMES HAS BEGUN TO DEVELOP ACTION PLANS (SHORT TERM AND LONG TERM) FOR EACH OF THE FIVE AREAS. According to the 2011 Needs Assessment, the leading causes of death are heart disease and cancer. The rates of diabetes and COPD are also high. To combat this we have expanded our heart and cancer services and recruited new physicians in both areas. We have a monthly cancer support group facilitated by our Spiritual Care Department. We have also expanded our diabetes program and outreach to include a 19-week diabetes prevention program for those individuals at risk of developing diabetes. These classes are free to those without insurance or who cannot afford to pay using the guidelines of our FAP or a suggested contribution based on income level. Also identified were the community's access to medical providers, specifically those in primary care, urology, endocrinology, cardiology, psychiatry, pediatric dentistry, and licensed counselors that accept Medicaid. As a hospital we have successfully recruited all of these specialties with the exception of endocrinology and psychiatry. We continue to work with the Western Montana Mental Health Clinic to increase the availability of psychiatrists in the area. In regards to endocrinology, we continue to refer patients out of the area to specialists in this discipline. In response to this need and to other specialties not covered in our locale, we partnered with St James Healthcare Foundation's Patient Compassion Program to provide more assistance to those individuals who cannot financially afford to travel elsewhere for their services. In addition to addressing the health needs from the assessment, we examined ways we could impact one of the most serious socio-economic findings; the fact that more than 22 percent of our community's school age children live below the federal poverty level. In 2012, we donated $1,500 each ($3,000 total) to the two elementary schools in our highest poverty census tracts. The principals at each public school were asked to use the money to provide eyeglasses, warm winter clothing, etc. to those children in their school who had the greatest need. Our CHNA is posted on our hospital's public website and is also available upon request from St. James. Butte-Silver Bow County has also posted it on its website and it is available upon request from the Butte-Silver Bow Health Department. We have partnered with the Butte Community Council, a resource and referral agency committed to providing assistance to families for services for their children. This includes supporting early childhood professionals through education and services, promoting high quality affordable child care, and advocacy on behalf of children. It is also a USDA Child and Adult Care Food Program sponsor. We also support the AWARE Inc. Early Head Start program through volunteer efforts from our staff and funds from the St James Healthcare Foundation. 3. PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE - St. James Healthcare treats patients with respect and dignity regardless of their ability to pay. St. James Healthcare has a written financial assistance policy that explains eligibility criteria for financial assistance at various levels, including a 100% discount, and is based on federal poverty guidelines. We work with patients to help them understand their financial responsibility for care received, financial assistance available to them, and to establish payment programs in demonstration of our core value of respect. As part of our responsibility to educate, we inform our patients and their families of the availability of assistance, including government programs. We communicate this in a variety of ways to ensure that messages reach multiple audiences. Before, during and/or after admission, we encourage our self pay patients to visit with a financial counselor to discuss qualifications for financial assistance. The financial counselor works with the patient to complete a financial assistance form to determine the level of discount for which the patient may be eligible. We post financial assistance information in emergency and admissions areas, on billings invoices, in various areas around the hospital and clinic sites and on the hospital website. We provide written materials to patients that detail our financial assistance policy and how it is administered. We provide materials and education when patients are discharged, and include information in billing statements, including phone numbers and other methods to contact us with questions. We ensure that our financial counselors, admission employees, social workers and other employees understand our policies to be able to assist patients in the most appropriate way. We also have a partnership with Western Security Bank to provide payment plans and medical credit lines. This helps patients meet their financial obligations in a reasonable and dignified manner based on their ability to pay and allows them to continue to ensure the ongoing welfare of their families. This is done in accordance with our core value of respect. 4. COMMUNITY INFORMATION - The care site of St. James serves the communities of Butte, Montana, and the southwestern region of Montana. St. James is located in Butte and is the sole acute care provider in both the city of Butte as well as Silver Bow County which encompasses Butte. St. James provides over three-quarters of the inpatient care delivered to patients within Silver Bow County. St. James's primary service area is defined as Silver Bow County. Silver Bow County, located in southwestern Montana in a medically underserved area, is the eighth largest county in the state with a population of slightly more than 32,000. This geographic area also represents the patient origin of approximately 75% of St. James' inpatient discharges. The care site's secondary service area includes six other Montana counties - Beaverhead County, Deer Lodge County, Granite County, Jefferson County, Madison County and Powell County. St. James provides close to 75% of its inpatient care to patients living in the Butte community, with patients originating from the adjacent communities of Whitehall, Anaconda and Deer Lodge representing the next highest concentration of patients. St. James is the only acute care provider in its primary service area. Within the care site's secondary service area there are five Critical Access Hospitals providing care to patients in southeastern Montana. Those hospitals are Community Hospital of Anaconda (not-for-profit) in Anaconda, Barrett Memorial Hospital (government owned) in Dillon, Ruby Valley Hospital (government owned) in Sheridan, Granite County Medical Center (government owned) in Philipsburg and Deer Lodge Medical Center (not-for-profit) in Deer Lodge. Patients from the primary and secondary service areas requiring specialized tertiary/quaternary care will typically travel to Missoula, a 120-mile trip, Billings, a 225-mile trip, as well as other major metropolitan areas out of state. Mountainous passes between Butte and these larger cities cause isolation especially during the winter in our medically underserved region. We are a designated Health Professional Shortage Area (HPSA) and a Medically Underserved Areas/Populations (MUA/P) by the U.S. Department of Health and Human Services Health Resources and Services Administration (HRSA). Butte-Sliver Bow (Butte), the county seat, is the state's fifth largest city, but the population has decreased by 4.8% since 2000. State government, which includes higher education institutions, is the largest employer in Silver Bow County with 22.3% of the total employment. Health care is the top private sector employer at 13% with St. James Healthcare, the community's only hospital, as one of the largest single private employers. Additional Butte-Silver Bow statistics follow. *Median income for Silver Bow County households in 2012 was $40,039 compared to Montana median income of $45,344. * 37% of the local population is below 200% of the federal poverty level; 16.4% of the residents have an income below the poverty level *94.8% of Butt
