PART I, LINE 7 - EXPLANATION OF COSTING METHODOLOGYThe Kingston Hospital's costing methodology was based upon the 2012 New York State Institutional Cost Report and the 2012 Medicare (Form 2552) Cost Report. These cost reports are filed with the New York State Department of Health and the applicable CMS intermediary, respectively. The cost-to-charge ratio derived from The New York State Institutional Cost Report was used for the various sub-line items of line #7.
PART III, LINE 2 - METHODOLOGY USED TO ESTIMATE AMOUNT OF BAD DEBT EXPENSETHE AMOUNT REPORTED ON LINE 2 IS EQUAL TO THE PROVISION FOR BAD DEBTS PER THE AUDITED FINANCIAL STATEMENTS ("AFS"). THE EXPLANATION OF THE METHODOLOGY USED TO ESTIMATE THIS AMOUNT CAN BE FOUND IN FOOTNOTE 1 ON PAGES 9 AND 11 OF THE ATTACHED AFS.
PART III, LINE 4 - BAD DEBT EXPENSE AUDITED FINANCIAL STATEMENT FOOTNOTETHE TEXT OF THE FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE CAN BE FOUND ON PAGES 9 AND 11 OF THE ATTACHED AUDITED FINANCIAL STATEMENTS.
PART III, LINE 8 - methodolgoy used to determine the amount on line 6The costs on line 6 were taken directly from the cost report costs (D & E-Series where applicable).
PART III, LINE 9B - PROVISIONS ON COLLECTION PRACTICES FOR QUAL. PATIENTSThe Credit and Collection Policy for Self Pay Balances provides that, in some cases, non insured patients that do not qualify for Medicaid qualify for charity care. In those instances the Credit and Collection Department will follow the guidelines as set forth in the Charity Care policy. The Charity Care Policy provides that the Financial Counselor evaluates the information provided in patients Charity Care application in conjunction with Federal Income Poverty Guidelines to determine the write-off percentage. If the patient/guarantor qualifies for Charity Care based on the sliding scale, the patient will be required to pay the outstanding amount. A payment plan is established not to exceed 10% of the guarantors gross monthly income.
PART VI, Line 2 - NEEDS ASSESSMENTThe Kingston Hospital ("TKH") identifies unmet community health needs by participating in community coalitions, partnerships, boards, committees, commissions, advisory groups, and panels. On an annual basis, the hospital analyzes internal patient surveys and publicly available data on the market including demographics and health services utilization. Local needs assessments and reports, such as the latest Ulster County Community Health Assessment 2010-2013 and the community needs index developed by healthcare business of Thomson Reuters, are used as they became available to determine the types and locations of community benefit programs implemented. Using the community needs index, The Kingston Hospital gathers vital socio-economic and demographic factors to support internal decision making for resource allocation and to determine the geographic location of new programs to meet emerging needs. For each zip code, the community needs index methodology aggregates five socioeconomic indicators/barriers to healthcare access that are known to contribute to health disparity. The indicators are related to income (percentage of households over age 65 below poverty line; percentage of families with children under 18 below poverty line; percentage of single female families with children under 18 below poverty line), education (percentage of population over 25 without a high school diploma), culture (percentage of population that is minority including Hispanic/Latino ethnicity; percentage of population over age 5 that speaks English poorly or not at all), insurance ( percentage of population in the labor force, aged 16 or more, without employment; percentage of population without health insurance), and housing (percentage of households renting their home). During FY12, in addition to the continued use of the "Ulster County Community Health Assessment" report, we also use a range of other available needs assessments to identify and respond to local needs. In fiscal year 2009, we undertook a physician Community Needs Assessment Study, another key component at examining the community needs and access to quality healthcare providers. This study reviewed by specialty in Health Alliance of the Hudson Valley's ("HAHV") designated geographic areas comprising or existing within the confines of HAHV's Kingston primary service area. This community need was determined based upon demonstrable and documented objective evidence. The board of Trustees, via the Audit and Compliance committee of the Board, reviewed and approved the community need study/report. In order to define community need, we followed the 2007 Federal Register guidelines establishing the necessary link of community need in the Primary Service Area ("PSA") and physician recruitment activities: "The geographic area served by the hospital, is the area composed of the lowest number of contiguous zip codes from which the hospital draws at least 75% of its inpatients." HAHV demonstrated and documented Community Need whereby the physician to population ratio in our PSA for the respective physician's medical specialty is below the recommended ratio resulting in an FTE deficit on the "community need model." By fulfilling these two requirements, HAHV established the necessary link of community need in the PSA and physician recruitment activities. The analysis demonstrated a documented community need for additional certain medical specialties such as pulmonary and cardiology, as well as a need for most surgical specialties. HAHV is working with existing physician practices to recruit new physicians to these specialties to insure the community need and access are improved. In 2012, TKH continued to work collaboratively with private and public stake holders, the Ulster County Commissioner of Health and the County Executive to identify underinsured healthcare. This study was published in 2010 entitled "Healthy Ulster - A Public Health Approach". A recent report ranked Ulster County 33rd in health outcomes and 30th in health factors compared to other counties (62) in New York State. The leading causes of death in Ulster County are heart disease, cancer and stroke. Based on the above needs assessments, the major community needs identified for fiscal 2010-2013 were: * Identify community and government partners who could work in a collaborative way to improve access and delivery of Behavioral Health services. * The need to increase access to quality health care, especially for children, pregnant women, uninsured adults and seniors. * The need to obtain medical care for the underserved by enrolling eligible residents to Medicaid, MHIP and other insurance programs and by building a better system of care for the uninsured. * The need to eliminate racial and ethnic health disparities by providing culturally and linguistically competent care and target diseases that are more prevalent in minority populations. * To provide health education, disease prevention and chronic disease management (including obesity) programs to improve the health status of the community. * In summary, our activities focus primarily on positively impacting the health of our community with programs that are culturally and linguistically tailored to meet the unmet needs of women, infants, seniors, and racial and ethnic minorities. * To work with physician practices to recruit and retain obstetricians which would allow the hospital to continue to provide quality OB/GYN services. The Kingston Hospital's interdepartmental leadership, executive management, and its Board of Trustees plan, monitor and evaluate the Hospital's community benefit efforts. Initiatives are thoughtfully planned to ensure that links exist between the Hospital's clinical expertise and unmet community needs. To determine the needs of the community, The Kingston Hospital has a Chief Executive Officer review committee on community benefit (an internal, interdepartmental committee) that utilizes available data (E.G., needs assessments, hospital patient data, community needs index) and community input to develop the hospital's community benefit work plans. HAHV and Ulster County have formed leadership steering committees to focus on future planning and solutions to two key community health concerns: ED usage by the uninsured or under insured and innovative employee health initiatives. These steering committees meet bi-monthly.
PART VI, Line 3 - PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCEThe Kingston Hospital is committed to: * Providing access to quality healthcare services with compassion, dignity and respect for those we serve, particularly the poor and the underserved in our communities. * Caring for all persons, regardless of their ability to pay for services. * Assisting patients who cannot pay for part or all of the care they receive. * Balancing needed financial assistance for some patients with broader fiscal responsibilities in order to sustain viability and provide the quality and quantity of services for all who may need care in a community. In accordance with AHA recommendations, TKH has adopted the following guiding principles when handling the billing, collection and financial support functions for our patients: * Provide effective communications with patients regarding hospital bills * Make affirmative efforts to help patients apply for public and private financial support programs * Offer financial support to patients with limited means * Implement policies for assisting low-income patients in a consistent manner * Implement fair and consistent billing and collection practices for all patients with patient payment obligations TKH effectively communicates with patients regarding patient payment obligations. Financial counseling is provided to patients about their payment obligation and hospital bills. Information on hospital-based financial support policies and external programs that provide coverage for services are made available to patients during the pre-registration and registration processes and in response to patients seeking financial assistance. Information regarding the financial assistance is also provided by the onsite financial counseling staff in both inpatient and outpatient areas. Patient accounting also supports the financial counseling program by providing patients with information and applications while handling customer service calls. Our financial counselors and county Medicaid workers also provide guidance regarding the financial assistance program when necessary. Financial assistance is available on the back of every hospital bill. Financial counselors, both internal staff and a paid external vendor, make affirmative efforts to help patients apply for public and private programs for which they may qualify and that may help them obtain and pay for healthcare services. The hospital has onsite representatives from Fidelis Insurance Company to help patients who are in need of insurance coverage navigate through the application process. The patient does not need to have an account with TKH; they just have to be in a situation where they are in need of insurance coverage. Fidelis screens the patient to see which plan will fit them and they are certified to enroll the patients in many different insurance policies based on their insurance needs. Every effort is made to determine a patient's eligibility prior to or at the time of admission or service. However, determination for financial support can be made during any stage of the patient's stay after stabilization or collection cycle. TKH offers financial support to patients with limited means. This support is available to uninsured and underinsured patients who do not qualify for public programs or other assistance. Notification about financial assistance, including contact information, is available through the hospital website, hospital posters and flyers, financials assistance applications and hospital statements. TKH has established a written policy (Attachment A) for the billing, collection and support for patients with payment obligations. TKH makes every effort to adhere to the policy and is committed to implementing and applying the policy for assisting patients with limited means in a professional, consistent manner. TKH educated staff members who work closely with patients (including those working in Patient Registration and Admitting, Financial Assistance, Customer Service, Billing and Collections) about these policies with an emphasis on treating all patients with dignity and respect regardless of their insurance status or their ability to pay for services. All patient registration staff receives in-service training regarding the Financial Assistance Program. Patient Accounting also receives information about the program and how to handle patients seeking financial assistance. Financial counselors receive in depth training to handle financial assistance requests, process applications, and manage outcomes.
PART VI, Line 4 - COMMUNITY INFORMATIONThe Kingston Hospital, one of three hospitals within a two-market area, primarily serves the residents of Ulster County, an area with a total population of approximately 182,500. Our Primary service area consists of 22 zip codes in Ulster and Delaware County that account for the top 75% of discharges. Our secondary service area is comprised of zip codes in Ulster, Eastern Delaware and Northern Dutchess Counties that make up the next 15% of discharges. Approximately 18% of the 148,000 residents in our primary service area are minorities, compared to 39.5% for the State. Demographic analysis reveals that almost 22% earn less than $25,000 and 47% less than $50,000. The area also has a significant number of women of childbearing age (17%) and Adults 55+ (30%). Within our service area, zip codes to the South (New Paltz and Ulster Park), East (Highland), and North (Saugerties) of Kingston Hospital are the fastest growing zip codes, however Kingston still accounts for largest population density. Based on the NYS Interim Expanded Behavioral Risk Factor Surveillance System report for the period July 2008 - June 2009, The Ulster County Community Health Assessment reports that Ulster County's rate (13.4%) of uninsured adults is slightly below the estimated NYS average rate (13.3%).
PART VI, Line 5 - Promotion of community healthThe Kingston Hospital has a 10 member community board comprised of a majority of community members. Only one of the 10 Board members is employed by The Kingston Hospital or HealthAlliance, the Hospital's parent corporation (one HealthAlliance Executive). The medical staff of The Kingston Hospital is organized in the public interest. Medical staff privileges in the Hospital are open and available to all qualified physicians and we have a large, diverse medical and dental staff (356 members) in which most share joint privileges with Benedictine Hospital. No part of the income of The Kingston Hospital insures to the benefit of any private individual nor is any private interest being served. All surplus funds are reinvested into the facility, equipment or programs of the hospital to improve the quality of patient care, expand our facilities, and advance our medical training, education and research programs. The Kingston Hospital actively advocates on behalf of poor and vulnerable individuals, especially for healthcare coverage for all persons and improved community health. During 2012, we encouraged our employees, as healthcare workers, to write to their local senators and congressional representatives about the need for healthcare reform. The Kingston Hospital has a vibrant volunteer program, offering varied opportunities to members of the community to volunteer. Currently, Kingston Hospital had 89 volunteers contributing their time in 2012.
PART VI, Line 6 - AFFILATED HEALTH CARE SYSTEMThe Kingston Hospital is a member organization of HealthAlliance. Fellow member organizations include Benedictine Hospital, Margaretville Memorial Hospital (including Mountainside Residential Care Center) and Woodland Pond at New Paltz. As a not-for-profit health system, HealthAlliance reinvests its profits back into the community through programs to serve the poor and uninsured, manage chronic conditions like diabetes, health education and promotion initiatives, and outreach for the elderly. In Fiscal Year 2012, for TKH, this included nearly $2.7 million in such community benefits. Therefore, HealthAlliance takes a systems approach in its community benefit planning and implementation, and is consequently able to ensure that its member hospitals and other entities/affiliates are helping promote and address the health needs of their respective communities. Consolidation of Services: The HealthAlliance of the Hudson Valley is the active parent company to affiliate members Benedictine Hospital, a Catholic hospital, The Kingston Hospital, a secular hospital, Margaretville Hospital, a rural critical access hospital, Mountainside Residential Care Center, a skilled nursing facility and Woodland Pond, a continuing care retirement community. This integrated healthcare system is committed to providing quality and compassionate medical care for our patients, their families and our community. The consolidation of two acute care hospital campuses was a focus of HealthAlliance when incorporating in 2008, responding directly to New York State's Berger Commission mandate to form a corporate alliance between Benedictine Hospital and The Kingston Hospital to bring down the costs of hospital-based care. In addition to service consolidation between the two Kingston based hospitals, HealthAlliance also committed to streamlining operations and improving care at the other affiliate locations. Effective March 11, 2009, HealthAlliance became the active parent company of the aforementioned entities and a master alignment agreement was signed between the two Kingston hospitals that stipulated: Benedictine will remain a Catholic Hospital; The Kingston Hospital will remain a secular hospital; non-emergency, elective abortions and sterilizations will be performed by a separate corporation located in proximity to The Kingston Hospital. The two hospitals conducted a bed-need analysis and clinical consolidation study and made a recommendation for use in the HEAL 4 grant application. The Boards of both hospitals approved the following recommendations: each campus would eventually be licensed for 150 beds; clinical and non-clinical duplication will be eliminated to the extent possible; The Kingston Hospital will become the General Acute Care campus; and Benedictine will become the Specialty Acute Care campus. The results by 2010 solved many long-standing problems including retiring debt, improving operational performance of Benedictine and The Kingston Hospital by $15 million, achieving pay parity between employees at each hospital and implementing electronic health records technology. The hospital consolidation, long in the making, was a success. With the business of healthcare undergoing drastic changes in recent years, HealthAlliance has entered a new planning phase to respond to these changes. Many factors contributed to this new phase such as: operating at 70% occupancy, patient trends of delaying elective care, physicians doing more procedures in their offices or ambulatory facilities in lieu of a hospital and providing care in a mixed income community with high-need and lower reimbursed rates than competitors in neighboring counties. These factors will continue to negatively impact the efficiency of healthcare operations in Ulster County unless we are able to reduce the physical footprint. Despite significant operational improvements since 2008, HealthAlliance made the difficult decision to close and repurpose one of two acute care hospital campuses in the city of Kingston. The NYS Department of Health, other state regulatory agencies and HealthAlliance are engaged in frequent discussions regarding the community's current and future healthcare needs; in addition to assessing the potential impact to the greater Hudson Valley and its healthcare institutions. This open communication between all entities will continue throughout the transition process. Despite all of the transition of services in and among the two hospital campuses, many services the community still needs and depend on are still growing and thriving on the Kingston campus, such as the following: Outpatient Addiction Therapy: The hospital's out-patient clinic, BridgeBack, offers eight different programs, at various levels of care, including day rehab, family, gender adolescent programs and a "special needs" group and intensive outpatient. The "Special Needs" group is fast becoming very popular in the treatment community, especially with the local courts and social services departments. The program has been successful at reducing recidivism, as its primary target population is the client who traditionally has been difficult to engage and keep in treatment. BridgeBack continues to offer an adolescent program and works closely with local schools like Kingston High School who will send a student for an intake assessment they suspect needs a substance abuse program. A second location, Bridgeback North in Saugerties, was added to accommodate the growing need for Outpatient Addiction Therapy in the area. TKH's Methadone Treatment Program ("MTP") is located on the opposite side of the same building, which makes it very convenient for those clients who participate in both programs. Both BridgeBack and Methadone are licensed by the NYS Office of Alcoholism and Substance Abuse Services (OASAS). In order to meet its clients' special needs, the treatment team has been paying close attention to providing more individualized treatment. This has been effective in the adolescent program where the retention rate is varied. All of the substance abuse programs accept most insurance carriers. Charity applications and a sliding fee scale are provided to all clients who do not have an ability to pay for substance abuse services. The Kingston Hospital continues to sponsor an annual "Recovery Conference" for all professionals in the community and engages well-known presenters in the field of substance abuse. Stroke Designation Program: The Kingston Hospital administrative and clinical staff continues to place priority on keeping the hospital current with the best practices in disease management of the stroke patient. Our stroke patient management program offers a continuum of care from onset of stroke symptoms to long-term recovery. Four beds have been designated as a Stroke Step Down Unit. Staffs are telemetry and ACLS trained. All requirements for Stroke Designation have been fulfilled. Our Stroke Center is accredited by the New York State Department of Health. The Stroke Center is accredited by the New York State Department of Health. The Stroke Center participated in the American Stroke Association "Get with the Guidelines" Stroke Program and regularly inputs data resulting from efforts to improve acute stroke treatment and prevent secondary events. TKH received the 2012 American Heart Association/American Stroke Association's Get With The Guidelines Stroke Gold Plus Performance Achievement Award. The award recognizes TKH's commitment and success in implementing excellent care for stroke patients, according to evidence-based guidelines. To receive the award, Kingston Hospital achieved significant adherence to all Get With The Guidelines-Stroke Performance Achievement indicators for two or more consecutive 12-month intervals as well as compliance with six of 10 Get With The Guidelines-Stroke Quality Measures, which are reporting initiatives to measure quality of care. These measures include aggressive use of medications, such as tPA, antithrombotics, anticoagulation therapy, DVT prophylaxis, cholesterol reducing drugs and smoking cessation, all aimed at reducing death and disability, and improving the lives of stroke patients. Dialysis & Diabetes Education: The Kingston Hospital Dialysis Center offers Hemodialysis, Home Peritoneal Dialysis and Acute Dialysis Services. Our Kingston outpatient site currently operates 30 stations with three patient shifts on Monday, Wednesday, Friday and two patient shifts on Tuesday, and Thursday. We offer state of the art Dialysis machines and water treatment, beautiful and comfortable surroundings, TV capability, comfortable conference and waiting areas and beautiful art work. In addition, the in-hospital unit provides dialysis services for the in-house and acutely ill patients.
