DescriptionThe New York Hospital Medical Center of Queens (NYHQ), located in the Borough of Queens in New York City, is a 519 bed licensed, not-for-profit medical center which is a corporate member of the New York-Presbyterian Healthcare System. NYHQ has received a determination letter from the Internal Revenue Service that it is exempt from federal income taxes as an organization described in Section 501(c)(3) of the Internal Revenue Code. The Medical Center is a voluntary, not-for-profit acute care community teaching hospital located in the Flushing section of Queens, New York. Presently, the Medical Center provides medical and surgical services and has a certified bed capacity of 519. It was incorporated in 1974 as Booth Memorial Medical Center and changed its name to The New York Hospital Medical Center of Queens in 1992 in conjunction with its affiliation with The Society of the New York Hospital (now, The New York and Presbyterian Hospital). Services and Programs The Medical Center offers diagnostic and therapeutic services on an inpatient and outpatient basis in approximately 64 medical specialties and sub-specialties. NYHQ has advanced capabilities in areas such as cardiac, general, and minimally invasive surgery. It receives tertiary inpatient referrals in infectious disease, nephrology/dialysis, radiation oncology, interventional radiology, and gastroenterology. It operates a Level III Neonatal Intensive Care Unit and a Level I Trauma Center (both designed to handle the most severe cases). The Medical Center provides the following programs as Centers of Excellence: The Neuroscience Institute, Cancer Center, Breast Center, Eye Center, Pediatric Asthma Center, and Obesity Surgery. NYHQ has been designated by the New York State Department of Health ("DOH") as an AIDS Designated Center. The Ambulatory Care division provides adult and pediatric primary/preventive care at 20 sites throughout Queens County. West Building The West Building officially opened at New York Hospital Queens in the fall of 2010. This large state-of-the-art building, which we refer to as the Heart Hospital and the Bone and Joint Hospital deliver both outpatient and inpatient services. This new facility adds 80 certified patient beds, bringing the hospital's total to 519. Ten new state-of-the -art operating rooms were added to handle some of the most complicated Orthopaedic, Heart & Vascular surgeries. A new hybrid operating room - one of few in the tri-state area - is another new West Building resource. This special OR enables doctors to perform minimally invasive surgeries, reducing patient length of stay from 7 to 10 days to one overnight stay in the hospital. New Urgent Care Center The Health Care Efficiency and Affordability Law for New Yorkers (HEAL-NY) state grant program funded The Urgent Care Center and an expansion of the main Emergency Department. The Urgent Care Center includes a patient treatment area, nursing station as well as dental and ophthalmic emergency areas. Cardiac Services The Medical Center offers a variety of cardiac services including diagnostic and interventional cardiology, cardiac surgery, congestive heart failure and rehabilitation centers and cardiac research, education and prevention programs. The Cardiac Catheterization Laboratory at NYHQ is one of only two interventional cardiology programs in Queens County, New York. The three catheterization laboratories at NYHQ performed a combined 3,056 procedures in 2011. Now, physicians in the Electrophysiology Laboratory can implant new MRI-safe pacemakers for our patients. NYHQ, in collaboration with The New York and Presbyterian Hospital ("NYPH"), operates a cardiac surgery program at the NYHQ campus. The Cardiac Health Center directs the Medical Center's education and prevention programs. Lectures and special events are regularly presented to the general public at the site and at community centers and health fairs throughout Queens County. The Cardiac Health Center provides primary and secondary cardiac rehabilitation programs, including nutrition services, monitored fitness exercise, and psycho-social counseling. Surgical Services In 2011, NYHQ performed 29,624 inpatient and ambulatory surgical procedures. These operations ranged from minimally invasive surgical procedures such as laparoscopic cholecystectomy and arthroscopic orthopedics to major open procedures such as aortic aneurysm repair and ablative and reconstructive surgery. NYHQ's Department of Surgery has encouraged use of inpatient and outpatient minimally invasive surgery, which reduces hospital stays and recovery times. Primary Care and Community Outreach NYHQ places particular emphasis on community health programs and outreach initiatives. The hospital team reaches out to Queens residents to support health education and awareness - offering free health fairs, health lectures, screenings, support groups and blood drives. A current calendar of events can be found at nyhq.org on the website home page. This commitment to the community is expressed in its continued strategic expansion of community-based programs and sites to enhance access throughout the Medical Center's service area, and includes: Save-A Life Campaign The Women's Auxiliary partnered with the NYHQ Community Advisory Council to raise approximately $30,000 from the community to purchase a top-of-the line cardiac monitor/defibrillator for an NYHQ ambulance. The new machine is designed to bring an Emergency Room physician - virtually - to a patient. With this device, vital signs, heart rhythms and other important information is sent wirelessly to the receiving hospital. The Trude Weishaupt Memorial Satellite Dialysis Center (the "Dialysis Center") The Dialysis Center provides hemodialysis, chronic ambulatory peritoneal dialysis, and patient and family education. NYHQ's renal hypertension division also provides outpatient ambulatory consultative services including the diagnosis and prevention of kidney stones, hypertension treatment and evaluation, and the management of kidney diseases. The Dialysis Center provided 39,715 treatments in 2011. Medical and Dental Staff As of December 31, 2011, there were approximately 1,600 physician and dentist members of the medical and dental staff of the Medical Center holding appointments in three categories: attending staff, emeritus staff , and consulting staff. The medical and dental staff is organized into thirteen clinical departments: ambulatory care, anesthesiology, cardiothoracic surgery, dental and oral medicine, emergency medicine, medicine, obstetrics and gynecology, orthopedics and rehabilitation, pathology, pediatrics, radiation therapy, radiology and surgery. The clinical departments include approximately 15 major services, subdivided further into 64 medical specialties and sub-specialties. Graduate Medical Education The Medical Center serves as an academic medical center through the continuing education of medical residents and fellows, medical students, physician assistant students, professional and technical students. In addition to the programs listed above, the Medical Center participates in training residents in obstetrics/gynecology, dental medicine, orthopedics, pediatrics, radiation oncology, emergency medicine and general surgery through rotating residencies affiliated with various facilities including NYPH (Cornell Medical Center), Memorial Sloan-Kettering Cancer Center, SUNY Health Science Center at Brooklyn and The Hospital for Special Surgery.. For The Year Ending December 31, 2011, NYHQ reported the following statistics: Laboratory Procedures (Excl. Blood Bank & CDL) 1,919,572 Radiology Procedures 274,180 Blood Bank 162,085 Surgical Procedures 14,330 Deliveries 4,139 Cardiac Catheterizations 3,056 Cardiac Catheterizations & PCI 3,789 EEG/EKG 77,508 Ambulatory Surgery/Endoscopy OPD 15,294 Emergency Encounters (Admits and T&R) 121,466 Ambulatory Care Visits 141,264 Patient Discharges 36,086 Average Length of Stay 5.00 Full-Time Employees 3,515 The Medical Center provides quality medical care regardless of race, creed, sex, sexual orientation, national origin, handicap, age, or ability to pay. Although reimbursement for services rendered is critical to the operations and stability of the Medical Center, it is recognized that not all individuals possess the ability to pay for essential medical services and, furthermore, the Medical Center's mission is to serve the community with respect to health care and health care education. Therefore, in keeping with the Medical Center's commitment to serve members of the community, the Medical Center provides the following: charity care to the indigent; care to persons covered by governmental programs at below cost; and health care activities and programs to support the community. These activities include wellness programs, community education programs, health screenings, and a broad variety of community support services.Description

ExplanationINDIVIDUALS PRESENTING FOR MEDICALLY NECESSARY SERVICES THAT ARE UNINSURED AND RESIDE IN THE STATE OF NEW YORK WITH INCOME LEVELS BELOW 400% OF THE FEDERAL POVERTY GUIDELINES ARE ELIGIBLE FOR CHARITY CARE. THE MEDICAL CENTER UTILIZES A PRESUMPTIVE ELIGIBILITY METHODOLOGY TO EVALUATE WHICH PATIENTS ARE ELIGIBLE FOR CHARITY CARE. PART I, LINE 7, COLUMN F: BAD DEBT EXPENSE IS $9,080,936. PART I, LINE 7: THE HOSPITAL UTILIZED A COST TO CHARGE RATIO AS CALCULATED IN USING FORM 990, SCHEDULE H INSTRUCTIONS WORKSHEET 2. PART III, LINE 4: The Hospital utilized worksheet A and A/R liquidation reports to calculate the amounts reported on lines 2 and 3. The Medical Center makes available free care programs for qualifying patients under its charity care and financial aid policy. During the registration, billing and collection process, a patient's eligibility for charity care is determined under the Medical Center's charity care and financial aid policy. The Medical Center obtains additional financial information for uninsured or under-insured patients who do not or have not supplied requisite information to qualify for charity care. The additional information is used by the Medical Center in determining whether to qualify patients for charity care and/or financial aid in accordance with the Medical Center's policies. For patients who were determined by the Medical Center to have the ability to pay but did not, the uncollected amounts are reported as bad debt expense, and is included in supplies and other expenses). Distinguishing between bad debt and charity care is difficult in part because services are often rendered prior to full evaluation of a patient's ability to pay. PART III, MEDICARE LINES 5,6 & 7 Lines 5,6,7 do not include certain medicare program revenue and costs and thus do not reflect all of the organizations revenues and costs with the participation in Medicare programs. The revenue and costs excludes professional health education, subsidized health services, and medicare managed care activity. If all these revenue and costs were included the medicare surplus of $8,072,533 would be a medicare shortfall of ($15,549,904). Medicare net surplus per Schedule H 8,072,533 Medicare GME net cost (10,590,753) Medicare net cost of subsidized health services (5,615,782) Medicare managed care net costs (7,415,901) Total net costs associated with the Medicare Program (15,549,904) PART III, LINE 8: THE HOSPITAL UTILIZED THE AMOUNTS REPORTED ON THE MEDICARE COST REPORT TO DETERMINE THE MEDICARE ALLOWABLE COSTS. TOTAL INPATIENT AND OUTPATIENT COSTS LESS GME AND IME AMOUNTS WERE THE BASIS FOR THE ALLOWABLE COSTS. PART III, LINE 9B: UNDER NEW YORK STATE LAWS THE HOSPITAL IS REQUIRED TO MEET THE FOLLOWING CRITERIA WITH RESPECT TO THE FOLLOW UP COLLECTION PROCESS. WHILE LIENS ON PRIMARY RESIDENCES ARE PERMITTED, FORCED SALE OR FORECLOSURE INITIATED BY THE HOSPITAL IS NOT. NO ACCOUNT MAY BE SENT TO COLLECTION WHILE THE FINANCIAL AID APPLICATION PROCESS IS STILL PENDING. WE MUST PROVIDE WRITTEN CONSENT TO A COLLECTOR BEGINNING A LEGAL ACTION FOR COLLECTION. THE FINANCIAL COUNSELING DEPT WILL PROVIDE MINIMUM 30 DAYS NOTICE TO A PATIENT FOR SUBMITTING AN ACCOUNT FOR COLLECTION. COLLECTION ACTIVITY DURING MEDICAID PROCESS IS NOT ALLOWED. COLLECTION VENDORS MUST CONFORM TO OUR FINANCIAL AID POLICY. PART V, LINE 19D: Highest volume payor when calculating the maximum amount that can be charged to outpatients and the Medicaid rate when calculating the maximum amount that can be charged to inpatients. NEEDS ASSESSMENT: THE HOSPITAL UTILIZES THE COMMUNITY ADVISORY BOARD TO ASSESS THE HEALTH CARE NEEDS OF THE COMMUNITY. THE BOARD IS COMPRISED OF MEMBERS OF THE COMMUNITY, BUSINESS LEADERS, LOCAL POLITICIANS AND SENIOR HOSPITAL OFFICIALS AND MEETS REGULARLY TO DISCUSS THE NEEDS OF THE COMMUNITY. Patient Education of eligibilty for assistance: The New York Hospital Medical Center of Queens provides care to all without regard to financial condition or mode of payment. IdentifierPART IIdentifier
ExplanationCertain officers and key employees of the New York and Presbyterian Hospital that are identified in Part VII as officers or Trustees of New York Hospital Medical Center of Queens are responsible for executing the mission and management of The New York and Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2011 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. In 2010, the plan was redesigned in anticipation of changes in deferred compensation rules in the not-for-profit environment and to make the value of the benefit easier to understand for participants. The redesigned plan maintains the target level of SERP benefits and modifies the vesting schedules to commence after five years of participation in the SERP, in prorated amounts through age 65. Consequently, for certain individuals, there is an increase in the amount reflected in the SERP compensation due to the change in the vesting and amortization periods. As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individuals listed in Part VII that are compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York Presbyterian Hospital and other related organizations in the aggregate. PART I, LINE 4B MR. STEPHEN S. MILLS PARTICIPATES IN A 457(F) PLAN WITH $45,000 CONTRIBUTED ANNUALLY. THE SUPPLEMENTAL RETIREMENT BENEFIT WILL BE PAID IN THE FORM OF A LUMP SUM WITHIN 30 DAYS OF RETIREMENT. IF EMPLOYMENT ENDS BEFORE THE 65TH BIRTHDAY (FOR A REASON OTHER THAN FOR CAUSE), THE LUMP SUM WILL BE RECEIVED WITHIN 30 DAYS AFTER SUCH TERMINATION. IF AT ANY TIME BEFORE AN AMOUNT IS PAYABLE HERE UNDER IT, IS SUBJECT TO TAXATION PERSUANT TO SECTION 457(F) OF THE INTERNAL REVENUE CODE OF 1986, AS AMENDED (THE "CODE") OR ANY SUCCESSOR THERE TO, OR ANY STATE OR LOCAL LAW, THE NEW YORK HOSPITAL MEDICAL CENTER OF QUEENS SHALL PAY FROM, AND CHARGE AGAINST, THE FUND THE AMOUNT OF WITHHOLDING OR DEDUCTIONS DUE AS A RESULT OF SUCH TAX TREATMENT. THE SUPPLEMENTAL RETIREMENT BENEFIT IS ADMINISTERED BY A COMPENSATION COMMITTEE APPROVED BY THE BOARD.Explanation
