O01	Officer directors etc family relationship Part VI line 2	Mary Ellis, Director and bookkeeper, is the mother of Monnie Kelley, Director. Dr. Billy Sellers, a pediatrician, provides medical care to children of other Directors and is paid at ordinary and customary rates.
O02	Form 990 governing body review Part VI line 11	The board of directors reviews the Form 990 before filing with the IRS, either at the quarterly meeting of the board or at a special called meeting.
O03	Conflict of interest policy compliance Part VI line 12c	An interested person must disclose the existence of an actual or possible conflict of interest and be given the opportunity to disclose all material facts to the directors and members of committees with governing board delegated powers. The board will then decide if a conflict of interest exists. If appropriate, the board shall appoint a disinterested person to investigate alternatives to the proposed transaction or arrangement. If the governing board or committee has reasonable cause to believe a member has failed to disclose conflicts of interest, it shall afford the member an opportunity to explain the alleged failure to disclose.
