ExplanationExplanationReturn ReferenceReturnReferenceIdentifierIdentifier
Part I, Line 6a: Holy Cross Hospital issues it's own community benefit report in addition to a related organization, Catholic Health East, issuing a consolidated report that includes Holy Cross Hospital's activities.
Part I, Line 7: Costing methodology used is cost-to-charge ratio derived from worksheet 2, ratio of patient care cost-to-charge ratio.
Part I, L7 Col(f): The bad debt expense included on Form 990, Part IX, Line 25, Column A, but subtracted for purposes of calculating the percentage in this column is $0.00. Bad Debt is not included in expenses.
Holy Cross Hospital's Community Outreach Department is the sponsor of the National School Lunch Program and Summer Food Service Program. Responsibilities include: application, planning, coordination, implementation, monitoring, etc. of all aspects of the programs in targeted lower-income (and Title I) parochial and private schools. As a result of these programs, the overall health of students has improved due to increased access to nutritious, quality food provided in portion controlled servings. This program has also provided economic relief for the families whose children receive free or reduced rate lunches/snacks.Part I, Line 7e:
Part III, Line 4: The figures were calculated based on the cost-to-charge ratio. The cost-to-charge ratio was derived as defined in the Form 990 Schedule H instructions and using the worksheets provided for in the Form 990 Schedule H. The cost-to-charge ratio derived from above was applied to total bad debt write-offs to calculate the costs for line 2. The organization derives the cost-to-charge ratio by using Worksheet 2 - Ratio of Patient Care Costs to Charges. Worksheet 2 computes the adjusted patient care costs by taking Total Operating Expenses and subtracting non-patient care activities, Medicaid provider taxes, total community benefit expense and Bad Debt expense. We then divide the adjusted patient care costs by gross patient charges. While we make all reasonable efforts to identify patients eligible for charity care prior to billing, such pre-billing identification is not always possible. This arises largely with patients treated in our Emergency Department ("ED") on an outpatient basis. Frequently, these patients do not complete an application for financial assistance, therefore making a definitive determination difficult. The estimated amount of bad debt for patients who qualify for financial assistance is calculated for line 3 based on a review of charity write-offs and estimating the amount of that cost that could be bad debt using the cost-to-charge ratio as described above. Per the organization's audited financial statements in regards to determining bad debt: "The Company provides an allowance for uncollectible accounts for estimated losses resulting from the unwillingness of patients and other third-party payors to make payments for services. The allowance is determined by analyzing historical data and trends. Accounts receivable are charged off against the allowance for uncollectible accounts when management determines that recovery is unlikely and the Company ceases collection efforts."
Part III, Line 8: None of the shortfall is treated as community benefit.
Part III, Line 9b: Collection efforts are placed on hold for patients that are known to qualify for charity care or financial assistance.
Part V, Section B, Line 14g: The Funding Assistance program is referenced on the hospital billing statements, and on signage at each point of patient registration and the emergency discharge office.Holy Cross Hospital, Inc.
Part V, Section B, Line 20d: The hospital uses the appropriate Medicaid payment rate for inpatients and outpatients.Holy Cross Hospital, Inc.
Part VI, Line 2: Holy Cross Hospital, Inc. actively conducts a Community Needs Assessment every (3) years which is inclusive of primary and secondary research methodologies - focus groups, key informant interviews, studies, findings, and a group process (including community members). In addition, local data including information from the health data warehouse, and the Quality of Life Assessments provided by the Broward Regional Health Planning Council are assessed as part of an ongoing and continuous environmental scan. During the months of March - June, 2010 Holy Cross conducted its Needs Assessment. In addition to the resulting Needs Assessment product, the Community Health Improvement Plan 2011-2013 was also developed.
Part VI, Line 3: Holy Cross Hospital's Admissions Clerks and Patient Financial Services Department are knowledgeable of federal, state, and local programs and assist patients in applying for these benefits. In addition, Financial Assistance applications for the hospital's charity care program are readily available for hospital and medical group patients. Signs informing patients of the program's availability are posted at the Admissions desk in the hospital. In addition, patients may glean information and applications on-line, via the in-house Medicaid application agency, from their Case Manager, Medical Group Office manager, or admissions clerk.
Part VI, Line 4: Holy Cross Hospital (HCH) is located in Fort Lauderdale, Florida, Broward County's largest city. Broward County is the second most populated county in the State of Florida and ranks high in diversity. In Broward County, 14.3% of the population is 65 and over, which is less than the percentage for the entire state of Florida. From 2000 to 2010, the greatest population increase occurred among the 45-69 age groups; and the greatest population decrease occurred in the 30-44 age group and the 70 and over age group. The Holy Cross Hospital Service Area (SA) is defined geographically by where 80% of Admissions are derived, with 65% Admissions defining the HCH Primary Service Area (PSA) and 15% defining the HCH Secondary Service Area (SSA); admissions beyond the HCH SA are defined as Other.
Part VI, Line 5: Holy Cross Hospital has a very active and dynamic Community Outreach ("CO") Department. Through this department, a majority of the community based services are performed. The CO Department facilitates access to healthcare to populations in need by identifying, advocating, educating, procuring and providing health related resources and disease prevention to better serve and improve the health of lower income, minority, and underserved populations. 2012 Year in review....Our works embody the physical, mental, emotional, and spiritual health of all age individuals-from newborn babies to the end of life. Our target audiences through differing in age always include the poor, disenfranchised, and those who lack access to healthcare services because they are uninsured, underinsured or undocumented. The work we do here today in our own backyard is not so different than the work that began in 1846 in Pittsburgh when, under the auspices of the Sisters of Mercy, a new hospital was opened. Although the hospital had no written mission statement there was the acknowledged commitment that care be provided to persons of every class, condition and religious persuasion and that assistance would be offered to the poor and destitute to the utmost limit of the mean of the institution. This statement was printed in the local newspaper of the time and continues to be the essence of our mission today. In 2012, 166 years later, 13 staff members and 2 loyal volunteers that comprise the CO team continue this "unspoken and unwritten mission statement." Last year, we served more than 36,000 members of our "more needy socioeconomic community." Children and seniors represented almost three-quarters of this number and were provided with health education, necessary vaccines, navigation, resources, counseling and screening services. In addition, CO coordinates Thanksgiving Baskets, Angel Tree gifts and Back to School Backpack distributions for more than 1,400 associates and their children. The programs we deliver make a huge difference in the lives of the community we serve. They include teaching a preschool child good dental hygiene; decreasing the obesity epidemic; assisting seniors to maintain their health and independence; early diagnosis of life threatening diseases and linkages into health care...we connect people. All of us at Holy Cross Hospital have a commitment to service-something that makes us difference and unique.Goodstart ProgramOur Commitment to those who are most vulnerable begins with our youngest population, our preschool age children. Program Goals: Increase knowledge and instill good health behaviors.Outreach to Children and YouthThe Goodstart Program provides health education and wellness promotion to at-risk preschool students in the community at their daycare facility. In addition, monthly caregiver workshops are presented to educate providers on childrens' health issues. At these workshops, caregivers obtain their necessary contact hours in order to maintain licensure and good standing for employment. In partnership with Broward Impact, Goodstart presents monthly in-service for seniors working with young children. Partnership with the Jan Moran Collier City Library promotes health literacy through facilitation of a monthly book club. The combined efforts of this mulit-generational program yield thousands of hours of health and wellness education and impart essential information and skills. Summary of those served in 2012:Pre-students - 2,338School age - 153Caregivers - 982Seniors - 135School HealthProgram Goals: Improve health to promote student learning. The Healthy School programming provides access for thousands of students to school-based vision, hearing, immunization, and scoliosis screening; health education classes and immunization. In addition, classes on CPR, blood borne pathogens and student-related health concerns (diabetes, HIV/AIDS, etc.) are made available to school personnel. School Health also provides outreach health education programming in non-traditional school settings, such as the Broward County Juvenile Detention Center. So what does it take to accomplish this seemingly fundamental task? Holy Cross Hospital located in Broward County, FL in the northern part of urban South Florida attempts to provide some answers to the question. With more than 3million people who call "home" to this community, Holy Cross's Community Outreach program has provided outreach services for more than 15 years - including and especially underserved youth. In addition to providing vision, hearing and scoliosis screening, childhood vaccinations, and health education classes (all for free), the program recognized the need to increase and expand its services and plant some new seeds that would help our community "Grow" Healthy Kids.Seeded with grant funding from The Health Foundation of South Florida in 2005, the program began providing teachers, students, and parent volunteers with the tools they needed to improve poor nutrition and physical inactivity. Utilizing an innovative train-the-trainer model and providing teachers with all of the resources they needed to implement lesson plans and activities in their classrooms the program was an immediate success. In addition, it also broke barriers by taking classes outside of the classroom walls and out of doors. On-site produce gardens and fruit bearing trees were planted, cultivated, and gleaned by students, teachers, and parent volunteers! Students then consumed the "fruit of their labors" - for many their first taste at fresh vegetables. From an urban oasis of food deserts came interested, creative and motivated children who began to grow healthy attitudes toward the food they were growing and eating. New attitudes towards food, nutrition labels, the earth, green programs, and sustainability were growing with students and consequently their parents. The program flourished, becoming a National School Lunch Sponsor, an on-site train-the-trainer food service program for the schools who eventually sought and gained self sponsorship! All along, improving and whetting the nutritional appetites of its students and decreasing out-of-pocket costs for parents who were eligible for free lunch. The program leveraged an additional millions in federal funding to the local area.Today, the Growing Healthy Kids program continues to evolve and spread beyond schools and into whole communities - now providing community gardening expertise to over 60 community garden partners. This year the program budded a new Pilates for Girls class designed to improve the mental and physical health of students and their teachers. 7-months into the program, the girls are feeling more fit and measurements demonstrate great improvements. Where will the program continue to sprout and grow? Wherever it is needed in order to continue its mission - and all of our mission - to Grow Healthy Kids!School GardensNumber of school gardens:622 gardens at 62 schoolsCommunity Gardens:Number of community gardens - 53Number of participants - 5,750Number of students participating in nutrition activities in the classroom 2,350. On Saturday, October 6th, 11a-12p, SICC: In partnership with the South Florida Hunger Coalition, Holy Cross Hospital Community Outreach presented the "Coalition of Immokalee Workers." The CIW is a community-based group of workers fighting for better wages, working conditions and protection of their dignity. Community members joined us to learn how agricultural labor is one of the most abusive, least regulated industries in the country. Attendees also learned about the Campaign for Fair Food and Fair Food Program in partnership with conscientious consumers who are signing groundbreaking agreements with retailers.

They are our neighbors, our children's friends, sometimes our coworkers and their families. Because of an unprecedented economic crisis, local food pantries cannot collect and stock enough food to distribute to the increase numbers of the food insecure. Our Response to our community's cries of hunger:* Holy Cross Hospital has been committed to assisting in the collection, distribution, donation, facilitation, and/or provision of food to small food pantries in Broward County. Everyday, associates contribute dry food items, canned goods and change in the cafeteria and Heart Healthy Cafe. These coins, dollars and cans added up to: more than 3 tons of food distributed to Broward's food pantries in 2012.HomelessBroward County has struggled with homelessness over the past decade. Almost a quarter of all households in the County experience chronic homelessness, meaning they have serious health issues, mental health, or substance use disorders and either remain homeless constantly or cycle in and out of homelessness. Our Response to our community's homelessness issue:* Placement of Parish Nurse at homeless day shelter and church based homeless feeding program* Placement of Outreach screening services, including: breast health, blood pressure, blood sugar, cholesterol, and HIV testing and linkages into care system for those requiring additional follow up services* The re-modeling of a Wellness Center with homeless service partner, Hope South Florida intended to provide on-site health care services and education for homeless families entering into the homeless systemNumber of homeless individuals served in 2012 - 5,044Response to Our Associates' Needs: Taking Care of Our OwnDonations by our associates for our associatesBack-to-school Backpacks in 2012Number of donated backpacks provided to children of our associates, 490ThanksgivingNumber of donated Thanksgiving baskets provided to our associates and their families, 328Angel TreeNumber of donated gifts provided to children of our associates, 484.The Mercy Family Life Center, a medical group facility providing primary care to homeless, uninsured, underinsured and undocumented refugees has provided essential direct care services to hundreds of patients. Form 990, Part VI, Section B, line 11The organization took steps to educate management, members of the board, and members of appropriate subcommittees of the board on the compliance requirements mandated by the Form 990. The form was reviewed by management and then submitted to a subcommitte thereof which reported its findings to the board.
Form 990, Part VI, Section B, line 12cCatholic Health East's Policy 103 sets forth the organization's conflict-of-interest policy and processes. Annually, all those serving CHE in a fiduciary capacity, including directors, officers, Sponsors' Council representatives, and key employees receive a copy of the policy and an annual disclosure statement to be completed. Disclosures of financial interest or other reportable circumstances as defined in the policy are submitted and reviewed by the organization's CEO and Board Chair. Summary information is reported to the entire board and available to the board throughout the year as business comes before the board or management for action. The policy contains a continuing affirmative obligation on all affected individuals to disclose compensation or other circumstances throughout the year which may rise to the level of an actual or apparent conflict. The determination of whether a disclosed financial or other interest constitutes a conflict of interest is made by the board or an appropriate committee thereof comprised of dis-interested persons and without the participation of the affected individual except to respond to questions about the disclosure. The policy further addresses the procedure for the board's further consideration of the proposed transaction/matter without the participation of the affected person and the documentation of the proceedings. Lastly, the policy addresses potential disciplinary action for violations of the policy. The policy is available to the public on the CHE website.

The committee reviews the market analysis, approves any salary adjustments for the executive population, considers both reasonableness and effectiveness of all remunerative programs, and establishes the detailed performance expectations which are incorporated into the incentive plan.
