DescriptionInpatient Service - The Uniontown Hospital is a 196-bed community hospital that delivered state-of-the-art primary inpatient services to approx. 9,900 inpatients during the fiscal year ended June 30, 2011. Since 1903, the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of the persons ability to pay. The Uniontown Hospital has earned several recognitions from HealthGrades, the nations leading independent healthcare ratings organization. These include a Five-Star Rating for joint replacement surgery, the Award of Excellence for gastrointestinal care and surgery, and the Distinguished Hospital Award for patient safety. The Hospital draws the majority of its patients from Fayette County. Its primary and secondary service areas only slightly extend into the neighboring counties of Westmoreland, Somerset and Washington Counties. Although the population of Fayette County has been decreasing since the 1990s, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby healthcare facilities, the growth in the regions uninsured population, and the increase in the regions elderly population. The Uniontown Hospital offers lines of service that are unavailable elsewhere in Fayette County. In addition to being the primary source of orthopedic care, it is the only hospital to offer obstetrics, interventional cardiology and radiation oncology services.Description

Other traditional outpatient services include Laboratory, Radiology, Physical Therapy and Same Day Surgery which accounted for 190,500 patient visits. Services also include the Diabetes Center which provides a comprehensive approach to diabetes management, including prevention, education, treatment and research.Description

DescriptionPCC/IRU/BHU - The Uniontown Hospital offers the Progressive Care Center PCC, the Inpatient Rehabilitation Unit IRU and the Behavioral Health Unit BHU. The PCC is the Hospitals sub-acute unit and had a 38-bed capacity during the fiscal year ended June 30, 2011. The PCC principally serves the needs of Medicare patients that are too sick to be appropriately cared for in a traditional skilled nursing facility, but not sick enough to justify remaining in an acute care setting. There were 723 patients in PCC during the year ended June 30, 2011 that stayed a total of 10,463 days. The IRU had an 11 bed capacity during the fiscal year. The IRU provides rehabilitation services principally for Medicare patients who have been hospitalized due to reasons such as a stroke or an orthopedic procedure. The number of patients in the IRU was 164 with total patient days of 1,922 during the fiscal year. The BHU began operations July 1, 2010 and is an 18-bed secured unit principally for older adult and geriatric patients in and around Fayette County. The goal of treatment is to achieve diversion and re-integration of the patient back into the community or to a lower level of care. During the fiscal year ended June 30, 2011, the BHU had 235 patients that stayed a total of 2,385 days.Description

IdentifierIdentifierExplanationExplanation
Part I line 7 column fThe amount of expenditures used to calculate column f were reduced by bad debt expense of 4,362,358.
Part I line 7Uniontown Hospital used the best available data to calculate the cost of charity care reported in line 7 for certain categories, the best available data was derived from a cost accounting system in other categories, specific cost-to-charge ratios from worksheet 2 were calculated and applied to that category.
Part III line 4The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable. That footnote reads as follows The Health System, which includes the Uniontown Hospital, reports patient accounts receivable for services rendered at net realizable amounts from third-party payers, patients and others. The Health System provides an allowance for doubtful accounts based upon a review of outstanding receivables, historical collection information and existing economic conditions. As a service to the patient, the Health System bills third-party payers directly and bills the patient when the patients liability is determined. Patient accounts receivable are due in full when billed. Accounts are considered delinquent and subsequently written off as bad debts based on individual credit evaluation and specific circumstances of the account.
Part III line 4Line 2 was determined by multiplying the provision for uncollectible accounts reported on the hospitals financial statements by the cost to charge ratio computed on Worksheet 2. Line 3 was estimated by reviewing data from the US Census Bureau to determine the percentage of the population in the hospitals service area that falls below federal poverty guidelines. The resulting percentage was applied to line 2 to estimate line 3.
Part III line 8Serving patients with government health benefits, such as Medicare, is a component of the community benefit standard that tax-exempt hospitals are held to. This implies that serving Medicare patients is a community benefit and that the hospital operates to promote the health of the community. The Medicare cost report was utilized to determine the amount report on line 6.
Part III line 9bThe Uniontown Hospitals Revenue Cycle Principles and Policies include seven policies approved by their Board of Directors. One of the policies is titled Financial Need Discounting and establishes discounts ranging from 0 to 100 based upon the patients income as compared to the federal poverty guidelines after the payment of a Minimum Patient Liability. The Minimum Patient Liability ranges from 10-100 depending on the service. According to the policy titled Patient Payment Expectations, patients are given four monthly billing statements to resolve their financial obligation. Each statement will indicate that the patient needs to resolve their financial obligation to avoid the potential for future service delays. The patient has 120 days from the date of the initial billing statement to resolve their financial obligation. Patients are given multiple options to make payment including major credit/debit cards. The organization actively pursues collection of accounts in default unresolved for greater than 120 days in a way consistent with a relationship built on
Part III line 9btrust and compassion. Insensitive collection practices are avoided and collection agencies are only used in situations where contact cannot be made with the patient. Patients that have accounts in default may be asked to take financial accountability before scheduling non-emergency services. Collection activities do not include judgments.
Part VI Line 2The Fayette Regional Health System leadership team develops a strategic plan every three years. This strategic plan establishes several priorities for Uniontown Hospital, all of which are focused on meeting the ever changing healthcare needs of the community. The leadership team develops this strategic plan with knowledge gathered from direct contact from patients patient satisfaction surveys outside consultants professional health care groups such as the Hospital Association of Pennsylvania and the Healthcare Financial Management Association and research obtained from various sources including the federal and state government and various other outreach programs and sources. In addition, the leadership team develops a Long Range Financial Plan that projects out five fiscal years and is updated annually along with the annual Financial Plan. The Hospital is currently working on its Community Health Needs Assessment to meet the new IRS requirements.
Part VI Line 3As part of the Revenue Cycle policy entitled Service Access, it states under Patient Advocacy that the health care system will actively assist all individuals in seeking resolution to their financial obligations. This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Convenient office hours, in a convenient location, will be established for patients seeking assistance. Currently this program is called the Safety Net Access Program SNAP. SNAP is available to all residents of Fayette County and patients will be assisted by Financial Care Consultants that are part of the Patient Financial Services department. The Hospital has brochures located in its waiting rooms referencing all financial services and patient obligations. In addition, the Patient Access staff makes the brochures readily available to the population at the time of registration.
Part VI Line 4The Uniontown Hospital is a 196 bed community hospital which is located 50 miles southeast of Pittsburgh in the City of Uniontown, Fayette County. Since 1903 the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of a persons ability to pay. The approximate population of Fayette County is 144,000. The current unemployment rate is 10.1. Approximately 17.3 of the population is over 65 with 17.3 of the population below the poverty level and 25.6 of the population eligible for Medical Assistance. Source Pennsylvania Department of Health 2011 Health Profile.
Part VI Line 5The general health and well being of the community residents is central to the mission of Uniontown Hospital. Part VI Line 5Continued such as a student immunization program, a program to reduce teen pregnancy and smoking, and several health issue related task forces.
