IdentifierIdentifierReturn ReferenceReturnReferenceExplanationExplanation
SchH_P01_S00_L03cSchedule H, Part I, Line 3cCharity care will be extended to patients who are determined to be financially indigent. The determination will be based on poverty guidelines published by the federal Register. Charity care may reduce the patients bill by 25%, 50%, 75% or 100% based on the financial information received.
SchH_P01_S00_L06aSchedule H, Part I, Line 6aThe Hospital prepares an annual community benefit report, publishes the report and makes it freely available in hard copy to the public. The Report is also available at the hospital's website: www.altoonaregional.org.
SchH_P01_S00_L07Schedule H, Part I, Line 7Two costing methodologies were used: 1. Actual expenses, and 2. Cost to charge ratio derived from Worksheet2 - ratio of patient care cost to charge also cost from Medicare 2011 cost report.
SchH_P01_S00_L072fSchedule H, Part I, Line 7, Column fBad debt 20,847,096
SchH_P01_S00_L07gSchedule H, Part I, Line 7gARHS included in the subsidized health services costs attributable to its 2 physician clinics (AFP and Williamsburg) and its women's health center. AFP cost: $4,795,543, Williamsburg Clinic cost: $932,512, Women's Health Center cost: $593,687 (these include all actual expenses).
SchH_P02_S00_L00Schedule H, Part IIARHS community building activities promote the health of the community we serve in the following ways: physical improvement and housing: participation in community revitalization efforts and the use of health system owned properties/houses by other non-profit community organizations
SchH_P03_S0A_L04Schedule H, Part III, Section A, Line 4The Health System reports patient accounts receivable for services rendered at net realizable amounts from third-party payers, patients and others. The Health System provides an allowance for doubtful accounts based upon a review of outstanding receivables, historical collection information and existing economic conditions. As a service to the patient, the Health System bills third-party payers directly and bills the patient when the patient's liability is determined. Patient accounts receivable are due in full when billed. Accounts are considered delinquent and subsequently written off as bad debts based on individual credit evaluation and specific circumstances of the account.
SchH_P03_S0B_L08Schedule H, Part III, Section B, Line 8ARHS considers 100% of the Medicare shortfall to be a community benefit. The costing methodology is the allocated costs from the 2011 Medicare Cost Report.
SchH_P03_S0C_L09bSchedule H, Part III, Section C, Line 9bIf a patient contacts the hospital and indicates an inability to pay in full, a payment arrangement is utilized to help the patient pay off the bill within 3 - 12 months. If the account cannot be paid off within 3 - 12 months, a minimum amount of $50 per month is required. If the patient is unable to meet this schedule, they need to provide budget information to justify a lower monthly payment.
SchH_P05_S0B_L04Schedule H, Part V, Section B, Line 4The Needs Assessment was conducted in collaboration between Altoona Regional Health System, Nason Hospital, and Tyrone Hospital, as well as with many other community organization and agencies.
SchH_P05_S0B_L07Schedule H, Part V, Section B, Line 7: In its final document: Blair County Community Plan: March 2012, the Blair County Needs Assessment Partnership (now called the Healthy Blair County Coalition) identified 15 "top priority" needs/goals that had been part of a larger group of needs identified through the needs assessment process. The Partnership formed 4 workgroups: Alcohol and Other Drugs, Economy, Crime, and Healthcare. The Alcohol and Other Drugs Workgroup developed 3 major goals and 18 strategies to address these 3 major goal areas. The Crime Workgroup identified 5 major goal areas and 17 strategies to meet these identified need areas. The Economy Work Group developed 4 major goals and 18 strategies to meet these identified goal areas. The Healthcare Work Group identified 3 major goal areas and 7 strategies to meet these identified goals areas. The hospital took a leadership role in working with the Healthcare Work Group to develop strategies designed to meet the identified community healthcare needs, as did the other work group relevant to their specific needs. The top three healthcare needs identified by the Healthcare Work Group were Goal 1: Increase access to care for physical, mental and substance abuse services in Blair County. Strategies included: a) Advocate for the recruitment of additional psychiatrists to serve consumers. The hospital implemented this strategy/met this need by successfully recruiting additional psychiatrists to its Behavioral Health Department. Additionally, the hospital partnered with Primary Health Care to increase access to physical, mental, and substance abuse services. Primary Health Network has 32 clinic sites/service locations in Pennsylvania, employs over 130 physicians, dentists, physician assistants, certified nurse practitioners and other healthcare professionals, and a support staff of over 400. In the past year PHN provided services to over 80,000 people, including those served in Blair County. The hospital worked PHN to support the development of: Altoona Community Health Center - which has provided increased access to physical health services; and Altoona Behavioral Health - which has provided increased access to mental health and substance abuse services. Also the hospital provides health care and dental care to the community through its free clinic: Partnership for a Healthy Community. Also for Goal 1: the hospital continues, on a local and state level, to advocate for insurance initiatives to reduce the number of uninsured persons. For Goal 2: increase community awareness of existing physical, mental, and substance abuse and social service agencies in Blair County, the hospital continues to support the development of the 211 information system in Blair County. The hospital educates community residents regard services availability through its Community Education office and provides referrals to community agencies, programs, and services. For Goal 3: enhance coordination of referrals and services among providers in Blair County, the hospital has not yet met this goal, but continues to work with the over 100 community service providers to enhance service coordination and referral. One proposed strategy to accomplish this is to form a workgroup to develop and implement a common application form acceptable to all participating providers to better facilitate client access to services. Although there are important HIPPA considerations regarding a common application form, these concerns may be adequately addressed via appropriate IT safeguards.
SchH_P05_S0B_L13Schedule H, Part V, Section B, Line 13Other: The following statement appears on all patient bills: " If you are experiencing financial hardship and are unable to pay this bill, charity/financial aid may be available if you qualify. Please contact us at (814.889.2333) or Toll Free in PA at 800.443.4050 to obtain information about charity/financial aid and how to apply for it."
SchH_P05_S0B_L17Schedule H, Part V, Section B, Line 17The following statement appears on all patient bills: " If you are experiencing financial hardship and are unable to pay this bill, charity/financial aid may be available if you qualify. Please contact us at (814.889.2333) or Toll Free in PA at 800.443.4050 to obtain information about charity/financial aid and how to apply for it." Also an authorized third party (Financial Health Strategies) when contacting the patient for payment will make the patient aware of the hospital's free care program if the patient explains that they will have a difficult time paying the bill. In cases where an inpatient bill exceeds $10,000 we turn it over to another third party (Healthcare Receivable Specialists) who will attempt to qualify the patient for Medical Assistance and let them know about the hospital's Free Care Program.
SchH_P05_S0B_L19Schedule H, Part V, Section B, Line 19For any patient that qualifies for free care, the patient's bill will have a 60% allowance applied before a further reduction of 25%, 50%, 75%, or 100% would apply based on their income level as it relates to the free care tables within our policy.
SchH_P06_S00_L02Schedule H, Part VI, Line 2Altoona Regional Health System (ARHS) helped form and actively supports and participates in the Blair County Community Needs Assessment Partnership (Partnership). The Partnership is comprised of over 50 community agencies, organizations, and services. The Partnership was formed in 2007 to conduct comprehensive community needs assessments, review and analyze data, formulate strategies to meet identified service gaps and needs, implement strategies to meet identified needs, and conduct subsequent post-implementation surveys to determine effectiveness of programs and services implemented. An assessment of the communities' healthcare needs are part of the initial and follow-up survey processes and involve a household survey, focus groups, and survey of local businesses.
SchH_P06_S00_L03Schedule H, Part VI, Line 3Patient education of eligibility for assistance: the Health System informs and educates patients, their families, and others regarding eligibility and the application process for federal, state, and local governmental financial assistance programs.
