Part XPart X : FIN48 FootnoteEMHS, its hospitals, and certain other affiliates have been determined by the Internal Revenue Service to be tax-exempt charitable organizations as described in Section 501(c)(3) or 501(c)(2) of the Internal Revenue code ("Code") and, accordingly, are exempt from federal income taxes on related income pursuant to Section 501(a) of the code. Accordingly, no provision for federal income taxes has been recorded in the accompanying consolidated financial statements for these organizations.Tax-exempt charitable organizations could be required to record an obligation for income taxes as the result of a tax position they have historically taken various tax exposure items including unrelated business income or tax status. Under guidance issued by the Financial Accounting Standards Board (FASB), assets and labilities are established for uncertain tax positions taken or positions expected to be taken in income tax returns when such positions are judged to not meet the "more-likely-than-not" threshold, based upon the technical merits of the position. Estimated interest and penalties, if applicable, related to uncertain tax positions are included as a component of income tax expense. The System has evaluated its tax position taken or expected to be taken on income tax returns and concluded the impact to be not material.Certain of the System's affiliates are taxable entities. Deferred taxes related to these entities are based on the difference between the financial statement and tax basis of assets and liabilities using enacted tax rates in effect in the years the differences are expected to reverse. The deferred tax assets and liabilities for these entities are not material.

Part V - Explanation of Number of Facility TypeN/A
Part VI - States Where Community Benefit Report FiledME
Part VI - Affilated Health Care System Roles and PromotionAs a member of EMHS, Acadia Hospital shares in the organization's goal of being the best rural healthcare provider by the year 2012. As a specialty hospital within our health care system, we provide assistance to our affiliates in an integrated fashion, using technology like telemedicine to bridge the distances faced by Maine citizens. EMHS offers its own initiatives, such as addressing childhood obesity, and the affiliates often support these efforts in their own communities. As autonomous hospitals, we also each provide our own unique programs to meet the needs of our local communities.
Part VI - Explanation Of How Organization Furthers Its Exempt PurposeThe Acadia Hospital is a nonprofit psychiatric and chemical dependency hospital that, along with Acadia Healthcare, a provider of substance abuse and community integration services, serves the entire state of Maine. It currently has the largest psychiatric staff in Maine with a total of 17 psychiatrists, neurologists, addictionologist/ internist, and family practitioner. The facility opened nineteen years ago in response to an urgent need for more hospital and community based mental health and chemical dependency treatment programs. In 2011, Acadia's Access Center received 181,373 calls requesting service. Total admissions amounted to 1,184 resulting in Acadia experiencing a 82.1% occupancy rate for the year.Acadia Hospital provides space for support groups, and provide crisis response services if requested from local communities. As in question five above, we provide many educational presentations in the community that focus on wellness and access to appropriate care, as well as anti-stigma work. We also offer an online mental health screening tool.See Schedule O, Part III, Line 4d for further details from the publication of community benefit projects of Eastern Maine Healthcare Systems entities.
Part VI - Community Building ActivitiesThe major foci of Acadia Hospital's community building activities include collaboration with other agencies and sharing our expertise with other organizations and individuals in an effort to strengthen communities through proactive activities. We also work to provide education on mental health and substance abuse issues with campaigns like our Close to Home opiate education campaign, and our wellness educational features which includes Mental Health Matters, Tour de Brain, and Life Strengthening Tools. We leverage social networking via our website, two Facebook profiles and a recently added Twitter profile, as well as a new YouTube channel. We support the efforts of other non-profits like the state's chapter of the National Alliance on mental Illness, and we participate educational programs like Mental Illness Awareness Week, Recovery Month and Suicide Prevention Day.
Part VI - Community InformationAs one of only two private, non-profit freestanding psychiatric and chemical dependency hospitals in the State of Maine, Acadia Hospital serves people from across Maine, with a majority of the patients originating in the Greater Bangor area. Geographically, Maine is a primarily rural state, and the demographics reveal an aging population with lower than national average earning power.
Part VI - Patient Education of Eligibility for AssistancePatient education begins at admission and continues throughout treatment and encompasses assistance with State health insurance coverage, COBRA, and Charity Care.It is the policy of The Acadia Hospital to treat all patients requiring care without regard for ability to pay and to assist patients in arranging for settlement of their financial obligation. For those qualifying under Acadia's Charity Care program, medically necessary services will be provided to Maine residents at no cost. Posters are displayed in patient care areas, information and forms are available online on the hospital website, and education is provided during the admission process and continues throughout treatment as the patient is assisted with the availability of programs.
Part VI - Needs AssessmentAcadia Hospital participated in the EMHS Community Health Needs Assessment. The EMHS Community Health Needs Assessment examines the health status of the residents of Bangor and Penobscot, counties and provides comprehensive research data and recommendations on a variety of critical target areas including community health education, access to primary care, care for chronic conditions, geriatric care, and mental health and substance abuse. As a follow up to the studies conducted in 2001 and 2007, EMHS contracted with the Center for Health Policy, Planning and Research (CHPPR) at the University of New England (led by Ron Deprez, PhD. MPH) to determine changes in health status and barriers to care since 2001. The 2007 Community Health Needs Assessment was funded again as a community service of Healthcare Charities, the philanthropic center of EMHS. EMHS participated in another Community Health Needs assessment late last year. Those results were made available earlier this year, and in late June, Acadia, as a member of EMHS, hosted a community health needs assessment forum at the Spectacular Event Center in Bangor so community leaders and public health officials could discuss a collaborative effort to best address the needs of residents in Penobscot County. We also look for trends within our patient population and stay involved with private and state organizations that track key data.

Part V, Line 6i - Describe Other Needs Identified6a Acadia Hospital plans to meet the prioritized needs through the following initiatives and resources: collaboration with a number of primary care offices and emergency departments lacking psychiatric resources by connecting them with psychiatry through tele-video technology. 6g Mental Health and Substance Abuse Acadia Hospital will participate in and be supportive of Downeast and Penobscot County District Coordinating Councils; being a provider of substance abuse education and intervention. For example, following the CHNA meetings, Acadia provided education in at least eight different community or professional forums on the abuse of Bath Salts - a new drug of abuse in the stimulant class that had begun to plague the area. Acadia also opened up quicker access to youth substance abuse services following feedback from the community on the negative effects of access delays.Part V, Section B, Financial Assistance PolicyLines 8 through 21Part V, Section B is optional for tax year 2011. Acadia Hospital Corporation has elected to not complete lines 8 through 21. The software will not allow AHC to leave these responses blank.Number of Hospital Faciltiy - 0
Part V, Line 5c - Description of Making Needs Assessment Widely AvailableIn conjunction with EMHS, Acadia Hospital hosted a community forum to present an overview of local assessment results and recommendations. Invitees included: Other area hospitals, Physician leaders, Federally Qualified Health Centers, Healthy Maine Partnerships, District Liaisons linked to Maine CDC, Home Health and Long Term Care leaders, Social Service Agencies, Leaders of the tribal communities, Business leaders, Legislative leaders, Representatives of the State administrationAttendees were provided an executive summary of the assessment as well as a summary table of data reflective of the service area. A presentation was made by EMHS staff who were members of the assessment development steering committee providing selected data results, trends over time and the priorities and recommendations as suggested by the research consultants. In addition to the community forums, the entire statewide assessment (both narrative and data sets) were posted to the EMHS website. After the forums, Power Point presentations used at the Forum as well as input collected in the breakout sessions were also posted on the website. Forum participants were encouraged to go to the website to review the assessment in detail, and access the data for local planning. Media releases were also sent to local news outlets in combination with the forums, encouraging new articles on the forum and inviting all members of the public to view the report on the website.Instructions on the website assist viewers to download and /or print sections of the report. Individuals without computer or printer access were provided a phone number where they can request a printed assessment.Number of Hospital Faciltiy - 0
Part V, Line 4 - List Other Hospital Facilities that Jointly Conducted Needs AssessmentThe needs assessment was developed as a statewide collaborative between the state's three largest health systems: EMHS (in central, eastern and northern Maine), MaineGeneral (in central Maine) and MaineHealth (in southern Maine). Multiple collaborators were involved in the dissemination of the assessment findings and establishment of priorities (see Section 5).Number of Hospital Faciltiy - 0
Part V, Line 3 - Account Input from Person Who Represent the CommunityOn a statewide basis, the research consultants developed an advisory committee that met two times during the assessment research and drafting of the publication. These organizations represented a broad spectrum of backgrounds: Healthy Maine Partnership Wabanaki Center (serving tribal populations); Maine Development Foundation; Maine Health Access Foundation (philanthropic foundation focused on access to care in Maine); Maine Center for Disease Control; Bingham Foundation (philanthropic foundation); Maine Health Management Coalition (representing the state's major employers, insurers and providers); Governor's Office of Health Policy and Finance (GOHPF); Advisory Committee for Health System Development (overseen by GOHPF); Maine Chapter, NAACP; City of Bangor, Department of Health and Welfare.Number of Hospital Faciltiy - 0
Part V, Line 1j - Description of Other Needs Assessment1a The Acadia Hospital serves northern, eastern and central Maine primarily, with some clients coming from southern Maine as well. Maine is a rural state and Acadia is located in Bangor, Maine, the regional hub.1b Acadia Hospital and its related physician practices utilize extensive electronic medical records, including disease registries, which provide a vast amount of data relative to patients in care. The needs assessment provides a much broader picture of the community as a whole, including the health status of individuals not in care, as well as functional health status and social-demographics (such as employment, income and education levels). The local units of the state's public health infrastructure (known as Healthy Maine Partnerships) are also integrated into the process so that their data relative to health, environmental and social measures are part of the community dissemination process.1c Acadia Hospital engaged multiple health resource leaders in the community forum invitation list (as described in Section 1g). Additional resources and leaders needed were identified in the breakout sessions (see question for breakout participants, Section 1g). In the nine-county service area, key collaborators going forward will include: include the Downeast Public Health Coordinating Council, Penobscot County Public Health Coordinating Council, and collaborations with a number of rural primary care offices and hospitals.1d-1e-1f EMHS, the parent company of Acadia Hospital, routinely conducts a community health needs assessment (Hereafter needs assessment) across the service area of all of its member hospitals. The most recent assessment, published in 2011, was conducted under a contract with the University of New England Center for Health Planning, Policy and Research (CHPPR) and the University of Southern Maine's Muskie School for Public Health. Using a methodology developed by CHPPR over decades of work, the assessment integrates primary data from a telephone survey to heads of households with secondary data retrieved from state databases (ED usage, Mortality, Cancer Registry, etc.). That data is reviewed in the context of multiple health related domains to develop a composite view of health status, behavioral risks, and barriers to access and care. Results are compared to national and state benchmarks to produce priorities and recommendations as prepared by the consultants.1g See question #31h On a statewide basis, the research consultants developed an advisory committee that met two times during the assessment research and drafting of the publication. A broad spectrum of backgrounds were represented: Healthy Maine Partnership, Wabanaki Center (serving tribal populations); Maine Development Foundation; Maine Health Access Foundation (philanthropic foundation focused on access to care in Maine); Maine Center for Disease Control; Bingham Foundation (philanthropic foundation); Maine Health Management Coalition (representing the state's major employers, insurers and providers); Governor's Office of Health Policy and Finance (GOHPF); Advisory Committee for Health System Development (overseen by GOHPF); Maine Chapter, NAACP; City of Bangor, Department of Health and Welfare. In the local area served by the assessment, multiple parties were engaged in dissemination of the assessment findings and establishment of priorities (see Section 5).Number of Hospital Faciltiy - 0
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsSchedule H, Part III, Section C, Line 9aEastern Maine Healthcare Systems (EMHS) has had a written billing and collections policy since September 2010. The policy applies to Acadia Hospital Corporation, a related organization of EMHS.All account guarantors who express an inability to pay inpatient and outpatient services will be screened for eligibility for charity care using an application and guidelines established by Acadia Hospital. An account may be reconsidered for charity care at any time when new information is available about a patients inability to pay.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitMedicare losses should be treated as a community benefit because the losses are incurred in performing an important public service, and Maine hospitals experience one of the lowest Medicare reimbursement rates in the country.
Part III, Line 4 - Bad Debt ExpensePatient and trade accounts receivable are stated at the amount management expects to collect from outstanding balances. Management provides for probable uncollectible amounts through a charge to earnings and a valuation allowance based on its assessment of the current status of individual accounts. Balances that are still outstanding after management has used reasonable collection efforts are written off through a charge to the valuation allowance and the applicable patient accounts receivable. The costing methodology used to determine the amount reported on line 2 is cost to charge ratio.
Part I, Line 7, Column F - Explanation of Bad Debt Expense$2,303,202 of bad debt expense, $17,893,240 of charity care, $17,939,578 of contractual allowances is included on Form 990, Part IX, line 25, column (A).
Part I, Line 7 - Explanation of Costing MethodologyRatio of Patient Care Cost-to-Charges is used in calculations.
Part I, Line 6a - Related Organization Community Benefit ReportThe Acadia Hospital Corporation community benefit report is contained in an annual community benefit report prepared by Eastern Maine Healthcare Systems which is the parent organization of all related organizations.

Form 990, Part III, Line 4d : Other Program Services DescriptionOTHER PROGRAM SERVICES 4: The Acadia Hospital is a nonprofit psychiatric and chemical dependency hospital that, along with Acadia Healthcare, a provider of substance abuse and community integration services, serves the entire state of Maine. Total admissions amounted to 1,184 resulting in Acadia experiencing a 82.1% occupancy rate for the year. OTHER PROGRAM SERVICES 5: Please see the following publication for details of community benefit projects at Eastern Maine Healthcare Systems entities.A former British prime minister, David Lloyd George, once noted that "You can't cross a chasm in two small steps." His point is that we shouldn't be afraid to take a big step if one is indicated. For some, the EMHS decision to become one of the federally recognized Pioneer Accountable Care Organizations (ACO) in the nation might seem to be a very large step. The nation certainly has a large chasm to cross, as identified in the Institute of Medicine's 2011 report, "Crossing the Quality Chasm." As our nation's healthcare delivery system has become unsustainably expensive, it has exposed the dysfunctional incentives and inefficiencies that grew within the system over the last 50 years. It is important to note that there is no single part of the system at fault. It has been a natural evolution of a myriad of parties-private payers, governmental payers, hospitals, physicians, and consumers alike. For those reasons, EMHS is proud to be among a few bold health systems in the nation that are leading health reform. While the Pioneer ACO is driven by the federal Medicare program, EMHS is mobilizing the same strategies for multiple patient populations in the state of Maine and elsewhere in northern New England. Our decision to pursue these redesigned models of care delivery and reimbursement should not seem to be such a big step for our constituents who have read our two previous annual reports. In 2009, we described the need to redefine hospitals not as brick-and-mortar places but as a continuous loop of community-based services. The nation's epidemic of chronic disease is most effectively addressed in the community, saving our hospitals for the crises and acute issues that demand the most advanced technology.Likewise, in 2010, we described the building blocks EMHS has been putting in place to manage a reformed healthcare system. These building blocks include advanced information technology that enable care managers to identify and monitor chronically ill patients to assure their optimal health status. This approach to care management has been pivotal to the success of the Bangor Beacon Community project-and regional "Little Beacons" emulating it. Beacon projects are proving real value by linking together multiple providers-both within EMHS and among externally aligned collaborators-using the same care management philosophy regardless of the site of care.We recently heard a colleague in a Pennsylvania health system describe their approach to accountable care as one of "never discharging a patient." Their focus is to manage seamless transitions of care from site to site and provider to provider, assuring appropriate levels of support throughout a patient's life. That is a fair description of how EMHS is approaching a reformed, accountable, value-based healthcare delivery model, and we are proud to safely lead the way across the chasm.M. Michelle Hood, FACHEEMHS President and CEOP. James Nicholson, CPAEMHS Board Chair OTHER PROGRAM SERVICES 6: EMHS (Systemwide)LEADERSHIP: M. Michelle Hood, FACHE, President and CEO and P. James Nicholson, CPA, Chair LOCATION: Brewer: EMPLOYEES: (Home Office) 438; DESCRIPTION: EMHS offers access to high quality healthcare, while striving for efficiencies to control costs. Beacon is supported by a three year, $12.7 million grant from the Office of the National Coordinator and ends in March 2013.EMHS (Home Office)- Beacon's successes include: updating state law to allow sharing of mental health information through electronic medical records; expanding connections to HealthInfoNet, Maine's statewide health information exchange; and improving primary care patient outcomes through Performance Improvement Committee activities.- EMHS and Husson University co-hosted a breakfast session with New York Times bestselling author, T.R. Reid. Reid's talk focused on his book, "Dispelling the Myth-How America Can Achieve High Quality, Lower Cost Healthcare."- All EMHS Home Office managers are completing leadership training called "EMHS University." The program is designed to ensure that all managers in the Home Office have received the appropriate information on policies, procedures, and skills necessary to be a great leader.- More than 40 Home Office employees were trained in basic Lean tools and skills. Lean is a process improvement program designed to reduce costs and duplications, and help improve quality. OTHER PROGRAM SERVICES 7: The Acadia HospitalLEADERSHIP: Daniel B.
