| Part I, Line 3(c) | HSC had in 2011 a financial assistance policy that provides for free or discounted care. The level of Charity Care is determined by several factors such as family size, income level, and net personal assets. 100% charity care is provided to patients who are ineligible for governmental or other insurance coverage, and whose family income does not exceed 200% of the Federal Poverty Guidelines. Partial Charity Care is based on a sliding discounted scale of gross charges based on the factors mentioned above and is available for patients who are ineligible for governmental or other insurance coverage and whose family income was between 200% but does not exceeded 500% of the poverty guidelines. In addition, the financial assistance policy covers uninsured patients who had sufficient assets to pay for medical services but whose medical costs during the year were catastrophic. Finally, individuals that do not qualify for financial assistance under the organization's financial assistance policy may still be provided up to a 25% discount on billed services in certain circumstances. |
| Part I, Line 4 | HSC had in 2011 a financial assistance policy that provides for free or discounted care. The level of Charity Care is determined by several factors such as family size, income level, and net personal assets. 100% charity care is provided to patients who are ineligible for governmental or other insurance coverage, and whose family income does not exceed 200% of the Federal Poverty Guidelines. Finally, individuals that do not qualify for financial assistance under the organization's financial assistance policy may still be provided up to a 25% discount on billed services in certain circumstances. |
| Part I, Line 5 | The Hospital for Sick Children d/b/a The HSC Pediatric Center does not deny financial assistance to any patient eligible for free or discounted care under its financial assistance policy, regardless of whether or not the financial assistance budget was exceeded. |
| Part I, Line 6(a) | The Hospital for Sick Children d/b/a The HSC Pediatric Center has not prepared an annual community benefit report in 2011 but will conduct a community needs assessment in 2012. Once completed and based on the report's findings, HSC will formalize selective programs and/or services designed to meet the community needs identified in the report and to address the needs of the community that it serves, based on the organization's mission. The HSC Pediatric Center was not required to file a community benefit report with the District of Columbia government in 2011. |
| Part I, Line 7, column (f) | Due to the composition of its patient population (90% Medicaid reimbursed), the organization incurred very little bad debt expense during the 2011 tax year. However, the bad debt expense, $3,821, was excluded when determining the percent of total expense in column (f). |
| Part I, Line 7(a) | HSC experiences very low volumes of charity care because approximately 90% of its patient population is composed of Medicaid patients. Only 2% of its patient population is self-pay and many of those patients qualify for Medicaid shortly after being admitted. In determining the cost of charity care, Hospital for Sick Children d/b/a The HSC Pediatric Center (HSC) did not use worksheet 2 to calculate cost reported in Part I, line 7(a) but, rather, it used the cost-to-charge ratio calculation of total cost divided by total charges per the 2011 DC Medicaid cost report. The amount of charity care is reported in accordance with the District of Columbia's State Health Planning and Development Agency (SHPDA) requirements that specified health care facilities within the District provide a reasonable volume of uncompensated care through the year. This level was set at 3% of the organizations total operating costs per its audited financial statements, minus the amount of reimbursement under XVIII and XIX of the Social Security Act. Further, the SHPDA regulations permit the carry forward of any excess amounts in any preceding years, HSC had an excess of uncompensated care in 2010, which allowed it to meet the requirements in 2011, thus HSC was in full compliance with SHPDA's regulations in 2011. |
| Part I, Line 7(b) | The Hospital for Sick Children d/b/a The HSC Pediatric Center (HSC) provided medical care to a patient population that is approximately 90% Medicaid. In 2011 HSC incurred a loss of approximately $1.2 million. A significant portion of this loss was driven by reimbursement from Medicaid being less than the actual cost of providing patient care services to Medicaid patients. The following schedule details these costs not reimbursed by Medicaid: Schedule of Non-Allowable Costs from Medicaid Cost Reports 12/31/11 - DC Bed Tax assessment ($382,212.50) - DC Medicaid provider tax assessed against each licensed bed. - Tax-exempt bond interest expense ($151,132.82) - The Interest expense on HSC's Hospital Revenue Bonds, which are refunding bonds of an initial tax-exempt bond issuance to demolish and reconstruct a portion of the facilities owned by HSC. - Other ($202,492.80) - non-allowable contract services, catering, website design, etc. - Total non-allowable costs = $735,838.12 These unreimbursed costs are necessary to provide adequate medical care to the patient population that HSC serves. The total unreimbursed costs are reported on Part I, Line 7(b) as unreimbursed Medicaid. |
| Part III, Line 4 | N/A - Due to the composition of its patient population (90% Medicaid reimbursed), the organization incurred very little bad debt expense during the 2011 tax year. |
| Part III, Section B, Line 8 | N/A, HSC does not participate in the Medicare program. |
| Part III, Line 9(b) | HSC patients who are uninsured for the relevant services, who are ineligible for government or other insurance coverage, and who have family income not in excess of 200% of the Federal Poverty Guidelines or between 200% but not exceeding 500% of the Federal Poverty Guideline will be eligible to receive 100% or partial charity care, respectively. The determination of Charity Care should be made before providing services when possible but all efforts will be made to establish whether the patient is eligible for Charity Care before leaving the hospital and after the patient has been discharged. Also, when possible the organization makes a prospective determination that certain patients qualify for Charity Care during the collection process, although the patient has not submitted a completed financial assistance policy. Once charity care eligibility has been established, the patient's account in whole or in part, is written-off to Charity Care and all collection efforts cease. If the patient receives partial Charity Care, all efforts will be made to place that patient on an extended payment plan. For those patients that do not initially apply or qualify for Charity Care, the organization continues to monitor whether the patient may qualify for Charity Care. If the patient is found to be eligible for financial assistance, at any point (including once collection efforts have begun), the organization will approve the patient for Charity Care. |
| Part V | The Hospital for Sick Children d/b/a, The HSC Pediatric Center is a self -contained sub acute, specialty children's hospital. |
| Part V, Section B, Line 13g | HSC publicizes its financial assistance policy in many ways. It posted the policy within several areas of the Hospital generally frequented by patients, guarantors, etc. The policy is distributed to patients on admission, upon request and could be disseminated during any phase of the collection process. Also, HSC as mandated by its Charity Care policy publishes, on an annual basis in a local newspaper the availability of Charity Care services. |
| Part V, Section B, Line 15e | HSC does not conduct any collection action(s) as specified in this section, until all reasonable efforts to determine a patient(s) eligibility are exhausted. |
| Part V, Section V, Line 16e | HSC does not contract with any 3rd party to take collection action(s) until all reasonable efforts to determine a patient(s) eligibility are exhausted. |
| Part V, Section B, Line 18d | HSC is a long-term sub-acute care specialty hospital; it does not operate an emergency room or provide emergency medical care. |
| Part V, Line 19d | HSC experiences a very low number of financial assistance eligible patients because its payor mix is approximately 90% Medicaid, 8% managed care or third party insurer, and only 2% self-pay. Many of those patients that are classified as self-pay qualify for Medicaid shortly after being admitted. As a result, HSC's cost of charity care in 2011 was only $19,816. The Hospital's financial assistance policy provides for: (i) 100% Charity Care to HSC's patients who are uninsured for the relevant service(s), who are ineligible for government or other insurance coverage, and who have family incomes not in excess of 200% of the Federal Poverty Guidelines, (ii) Partial Charity Care to HSC's patients who are uninsured for the relevant service(s), who are ineligible for governmental or other insurance coverage, and who have family income in excess of 200% but not exceeding 500% of the poverty guidelines, and (iii) financial assistance to uninsured patients who had sufficient assets to pay for medical services but whose medical costs during the year were catastrophic. In no circumstances does any such individual that qualifies for financial assistance under the Hospital's financial assistance policy pay more than the amount generally billed to commercial insurers for such services. |
| Part VI, Line 2 | The Hospital for Sick Children d/b/a The HSC Pediatric Center has not prepared an annual community benefit report in 2011 but will conduct a community health needs assessment in 2012. Once completed and based on the report's findings, HSC will formalize selected programs and/or services designed to meet the community needs identified in the report and to address the needs of the community that it serves, based on HSC's mission. |
| Part VI, Line 3 | The HSC Pediatric Center has a strong commitment to providing services to families at no cost to them if at all possible. Many of the families who receive services from HSC have already qualified for Medicaid by the time of their arrival at HSC due to prior services at other facilities. If a family arrives at HSC with private insurance, often times they qualify for Medicaid by the time they are discharged to care at home. Families are informed of the availability of financial assistance at the time of their admission to HSC. They are given written copies of the statement: "Notice of Availability of Uncompensated Services" in the appropriate language. If they believe they may be in need of the services, an application is filled out at the time of admission. If a family does not qualify for Medicaid they would be eligible to pursue the available uncompensated services. If patients are not eligible for Medicaid and other governmental programs, subsequent to discharge, HSC both informs and attempts to qualify them prospectively for the Charity Care services provided by HSC. |
| Part VI, Line 4 | The HSC Pediatric Center serves children and young adults up to the age of 21 in the Washington DC metro area who have special health care needs. HSC serves the Washington DC metro area which is a diverse community that covers two states and the District of Columbia. Specific geographic and demographic characteristics of the community we service include the following: 1.There are approximately 59,000 SSI and special needs children under the age of 18 in the Washington, DC metro area. 2.Of those 59,000 children, 6,065 were District of Columbia special needs children. Of the DC special needs children approximately 5,000 were enrolled in Health Services for Children with Special Needs, Inc, HSC's sister organization that provides care management services to children who have SSI or SSI related diagnoses. 3.Maryland has a higher prevalence of children with special health care needs (CYSHCN) than does the nation as a whole. In 2010, more families of CYSHCN in Maryland are living in poverty than in the five previous years. 4.Prince Georges County in Maryland, a county bordering Washington, DC is an ethnically and socioeconomically diverse population with a higher rate of being uninsured than surrounding MD counties. There are 12,381 special needs children in Prince Georges County Maryland. 5.Of the 59,119 children with special health care needs 32,262 have behavioral health diagnoses, 10,719 have cerebral palsy, developmental delays, or failure to thrive. The other 16,137 children have various other medical conditions. The primary CSHCN conditions treated at the HSCPC are conditions requiring longer term transitional care and/or specialty rehabilitation care prior to their discharge to home or nursing/group home. These conditions include cerebral palsy, anoxic brain damage, spina bifida, failure to thrive related to prematurity, chronic lung disease, and intestinal or other feeding disorders. 6.In 2011, 89% of HSC's patients were covered by Medicaid or Medicaid Managed Care. 38% were MD Medicaid or MD Medicaid MC, 53% DC Medicaid/MC, and 9% Virginia Medicaid. 7.365 children were admitted to HSC in 2011. 20,385 days of care were provided to those children. |
| Part VI, Line 5 | While HSC has not directly participated in community building activities, its parent company, the HSC Foundation a section 501 (3) organization that is a supporting organization of HSC participated, funded, or led activities related to community building and community health improvement activities in 2011. Examples of those activities included: - Opened the National Youth Transitions Center, a new, state-of-the art building that serves as a unique place for national collaboration, active learning, and capacity building focused on youth transitions issues - Created the National Youth Transitions Collaborative with an initial membership of 37 local, regional and national organizations, and established a Leadership Council to provide Collaborative oversight - Established the National Veterans Center within the National Youth Transitions Center - Leveraged the National Youth Transitions Center to obtain a grant in support of a California-based convening on healthy development among minority youth - Partnered with Congressional offices and George Mason University's LIFE Program to expand the Congressional Internship Program - Created The HSC Foundation Youth Transitions Fellowship in partnership with American Association of People with Disabilities and selected the first Fellow from a national pool of applicants - Hosted a briefing on Capitol Hill to educate Congressional members and staffers on the critical issues facing returning wounded veterans - Funded seven internship programs for youth with disabilities - Provided over 15 sponsorships for disability events, garnering local, regional and national acknowledgment |

| Process of reviewing return | Part VI Section B Line 11 | The Form 990 is prepared by an accounting firm and then reviewed by the controller and vp of finance. Once comments from the controller and vp of finance have been incorporated into the Form 990, the Form 990 is made available to the Finance Committee of its parent organization, HSC Foundation, for their review, followed by a meeting of the HSC Foundation Finance Committee in which the Form 990 is presented and any questions are answered. After the Finance Committee meeting, the Form 990 is made available to all members of the organization's Board of Directors for their review and comments. |
| Conflict of interest | Part VI Section B, Question 12c | HSC falls underneath the HSC Foundation Conflict of Interest Policy. Under that policy, whenever a potential conflict arises for The HSC Pediatric Center board of directors and officers, it is brought to the attention of the President and Executive Committee of the HSC Foundation Board, which analyzes the situation and makes a determination of whether or not an actual conflict exists. If it is determined that an actual conflict exists, the President and Executive Committee reports to the The HSC Pediatric Center Board of Directors regarding the nature of the conflict and that person is thereafter recused from any discussion and vote regarding the matter. Whenever a potential conflict arises for employees of The HSC Pediatric Center management team and of the Company, it will be brought to the Board Chair by the President which analyzes the situation and makes a determination of whether or not an actual conflict exists. If it is determined that an actual conflict exists, steps will be taken to eliminate the conflict and communicated to the employee involved. The Board Chair and President will report all COI to the Executive Committee Foundation. |
| DETERMINING EXECUTIVE COMPENSATION | PART VI, Line 15, SECTION B | The organization relied on a related organization, HSC Foundation, to determine executive compensation. HSC Foundation has adopted an executive compensation policy. under that policy, the Organization uses a four step approach to executive compensation: 1. The executive committee is made up of independent Board members without a conflict of interest, to establish and determine the reasonableness of total compensation for the System's disqualified executives and forwards to the full board for approval. 2. Periodically, the Executive Committee will engage an independent consultant to prepare a detailed written report on appropriate comparability data for comparable positions. 3. The Executive Committee reviews and relies upon the report of the independent consultant, along with other individual and market data, then debates and fully documents its decision about what is reasonable compensation for disqualified executives 4. Once the Executive Committee develops executive compensation recommendations, they are presented to the System's full board for ratification. |
