DescriptionSt. Luke's Episcopal Hospital (SLEH), as an integral part of the Health System, contributes to enhancing community health by delivering superior value in high-quality, cost-effective acute care. As a community teaching hospital and a tertiary referral center, SLEH serves both the Greater Houston area and the global community. In collaboration with its medical staff, SLEH is dedicated to excellence and compassion in caring for the whole person-body, mind and spirit. St. Luke's Episcopal Hospital (SLEH) STATS Hospital Admissions 31,487 Emergency Room Visits 30,677 Surgeries 22,830 Births 2,788 Outpatient Visits 111,389 St. Luke's Community Emergency Centers Pearland 25,215 Holcombe 14,563 San Felipe 12,593 Patient Care In its annual survey of "America's Best Hospitals," U.S. News & World Report ranked SLEH among the top healthcare institutions in the nation in six major areas of care. Texas Heart Institute (THI) at St. Luke's Episcopal Hospital was recognized for the 21st consecutive year in the top 10 for Cardiology and Heart Surgery. Number four overall, it is the only heart center in Houston, and the Southwestern United States, to be listed among the top five in its category. SLEH also ranked among the nation's top 50 in Diabetes and Endocrinology (35); Ear, Nose and Throat (43); Gastroenterology (30); Geriatrics (37); and Urology (31). As SLEH faces the difficult challenges of an evolving healthcare environment, enhancing the patient and physician experience through improved operations will be critical. In March, SLEH partnered with GE Healthcare to implement a new generation of patient flow techniques. This 18-month project will include new technology to facilitate improved patient throughput across SLEH. Efficiencies associated with these efforts are expected to reduce the patient's length of stay by nearly a day. SLEH's Medicare Case Mix Index (CMI), which reflects the clinical complexity and resource needs in its population of patients, ranked tenth nationally. Accreditation SLEH became the first hospital in Texas accredited as a Heart Failure Institute by the Healthcare Accreditation Colloquium. The accreditation is based on proficiency in managing heart failure while placing emphasis on quantity and quality of life. SLEH's Women's Center is now the first imaging site in Houston and Southeast Texas to receive accreditation in Breast Magnetic Resonance Imaging by the American College of Radiology. New Program SLEH inaugurated a Pancreas Transplant Program. The innovative program treats patients needing a pancreas transplant, pancreas-after-kidney transplant or combined kidney-pancreas transplant. Outstanding Leadership SLEH received an Outstanding Leadership Award from the U.S. Department of Health and Human Services for its achievements in eliminating ventilator-associated pneumonia. SLEH was one of only two Texas hospitals among 37 national hospital and healthcare facilities recognized, and the only Houston hospital to be recognized. The award highlights hospital efforts in the areas of prevention of central-line associated bloodstream infections and ventilator-associated pneumonia. Giving Back SLEH won LifeGift's first Donate Life Texas Registry challenge. More than 480 employees registered or indicated they were already registered to be an organ donor-translating into 10 percent of the hospital's workforce. SLEH competed against three other Texas Medical Center institutions in the challenge. Auxiliary During National Volunteer Week in April, SLEH Auxiliary's 117 members were recognized for donating 69,949 service hours in 2010-valued at $1,494,118. The 15th Annual Easter project, "Food for Life," sponsored by the SLEH Auxiliary, collected food and monetary donations from employees to benefit the Southeast Area Ministries Food Pantry. The group serves eight zip codes in the southeast area of Houston, along the Gulf Freeway from Gulfgate to Friendswood and Clear Lake, and includes Western Pasadena and South Houston, an area with a high incidence of hardship. St. Luke's Medical Clinics Two Houston area St. Luke's Medical Clinics (San Felipe and The O'Quinn Medical Tower at St. Luke's) offer adult primary care provided by board-certified physicians in Internal Medicine. Services include: Treatment of acute and chronic medical conditions Annual physicals, immunizations and preventive care EKGs and labs on-site St. Luke's Community Emergency Centers (CEC) SLEH's brand of care has become a neighborhood fixture with the St. Luke's Community Emergency Centers ("CEC")-freestanding emergency centers offering comprehensive adult and pediatric care. With almost 52,400 visits annually, the CECs are operational 24 hours a day, seven days a week, and include onsite laboratory testing and diagnostic imaging services and consultations by specialists, if needed. One of the most visited free standing emergency centers in Houston; St. Luke's CEC-Holcombe is now in its eleventh year of providing high-quality emergency care.Description

Form990 Schedule H Part VI Form990ScheduleHPartVI (13 entries)

IdentifierIdentifierReturn ReferenceReturnReferenceExplanationExplanation
charity careSCHEDULE H, PART I, LINE 7The cost to charge ratio method was utilized to determine financial assistance at cost; however, st. Luke's internal cost accounting system was used to determine unreimbursed Medicaid costs. Subsidized health services amounts represent cash paid to other healthcare providers for indigient healthcare services. Total expenses per Form 990, Part IX, line 25(a) were reduced by bad debt expense in the amount of $35,075,773 to determine the percentage in schedule H, Part I, line 7(f). If the percentage reported in part i, line 7, financial assistance and certain other community benefits at cost, were to be calculated on a system-wide basis, the combined percentage for all of the hospital facilities of the st. luke's episcopal health system would be approximately 7.045%. St. Luke's Episcopal Hospital received supplemental medicaid payments of $8,588,519 during 2011. These payments were included in the direct offsetting revenue amounts found in Schedule H, Part I, line 7b column d. These supplemental medicaid payments resulted in a reduction in net community benefit expense and percentages found in columns(e)and (f). If the direct offsetting revenue found in column (d) was not subtracted from total community benefit expense, the percentage in column (f) would be 13.01%.
FOOTNOTE FOR BAD DEBT EXPENSESCHEDULE H, PART III, SECTION A, LINE 4TEXT IN FOOTNOTES THAT DESCRIBES BAD DEBT EXPENSE: "Management regularly reviews collections data by major payor sources in evaluating the sufficiency of the allowance for uncollectible accounts. On the basis of historical experience, a significant portion of the Systems' self-pay patients will be unable or unwilling to pay for the service provided. Thus the System records a provision for uncollectible accounts in the period services are provided related to self-pay patients. Unpaid accounts are written off as bad debts upon reaching delinquent status."
RATIONALE FOR INCLUDING BAD DEBT AS COMMUNITY BENEFITSCHEDULE H, PART III, SECTION A, LINE 4ALTHOUGH ST. LUKE'S EPISCOPAL HOSPITAL STRIVES TO IDENTIFY ELIGIBLE CANDIDATES FOR CHARITY CARE OR DISCOUNTED CARE, THERE ARE CIRCUMSTANCES IN WHICH THE CANDIDATE DOES NOT ASSIST IN THE DETERMINATION PROCESS, AND CHARITY CARE/DISCOUNTED CARE GUIDELINeS CAN NOT BE MET, therefore THE ACCOUNTS BECOME a BAD DEBT EXPENSE FOR THE HOSPITAL'S RECORDS. IT IS VERY DIFFICULT TO ACCURATELY DETERMINE HOW MANY OF THESE ACCOUNTS WOULD HAVE MET THE CHARITY CARE/DISCOUNTED CARE GUIDELINES HAD THE CANDIDATES COOPERATED WITH THE APPLICATION PROCESS.
RATIONALE FOR INCLUDING MEDICARE SHORTFALL AS COMMUNITY BENEFITSCHEDULE H, PART III, LINE 8ST. LUKE'S EPISCOPAL HOSPITAL HAS ADOPTED THE COMMUNITY BENEFIT AND CHARITY CARE POLICIES OF THE ST. LUKE'S EPISCOPAL HEALTH SYSTEM, WHICH REQUIRE IT TO PROVIDE CARE TO ALL PATIENTS WHO PRESENT THEMSELVES FOR CARE. MEDICARE PATIENTS RECEIVE THE SAME HIGH QUALITY CARE AS PATIENTS THAT HAVE OTHER FORMS OF PAYMENT. AS MEDICARE REIMBURSEMENTS DO NOT FULLY COVER THE COST OF THE HEALTHCARE PROVIDED, ST. LUKE'S EPISCOPAL HOSPITAL CONSIDERS THE SHORTFALL AMOUNT AS AN ADITIONAL COMMUNITY BENEFIT TO THE ELDERLY MEMBERS OF THE COMMUNITY IT SERVES, AND THE COMMUNITY ENJOYS HIGH QUALITY HEALTHCARE FROM ST. LUKE'S EPISCOPAL HOSPITAL FOR ALL ITS CITIZENS, REGARDLESS OF METHOD OF PAYMENT. the cost to charge ratio is a costing methodology utilized in healthcare to estimate costs. the overall ratio of total costs to total charges is computed and the ratio applied to medicare patient charges to compute medicare costs.
COLLECTION POLICYSCHEDULE H, PART III, LINE 9BST. LUKE'S EPISCOPAL HOSPITAL HAS ADOPTED THE DEBT COLLECTION POLICY OF THE ST. LUKE'S EPISCOPAL HEALTH SYSTEM ("SYSTEM"). THE SYSTEM'S POLICY STATES THAT ST. LUKE'S EPISCOPAL HOSPITAL WILL NOT PURSUE COLLECTION PRACTICES AGAINST PATIENTS WHO HAVE QUALIFIED FOR charity care or other financial assistance once the account has received the charity adjustment. For those patients who have qualified for partial assistance, an adjustment is posted to the charity allowance and the remaining balance may be pursued as directed by our collection policy. In certain cases, a patient's qualification for charity care or financial assistance may not be readily determined; however, if after collection practices have begun it later becomes known that a person qualifies, the collection efforts will immediately cease for the charity care patient or be pursued at the lower partially adjusted amount.
Measures to publicize financial assistance policyPart V, line 13gSt. Luke's Episcopal Hospital provides financial counseling and screening for Medicaid eligibility. If an uninsured patient is admitted to a facility, the patient/guarantor is visited by an counselor who will first screen for Medicaid eligibility. If patient does not qualify for governmental assistance, the counselor reviews St. Luke's Patient Financial Assistance Program and assists the patient with completing the appropriate document(s). The document(s) are then forwarded for evaluation and qualification for financial assistance.
Individuals Eligible for Financial AssistancePart V, line 19dIndicate how the hospital facility determined, during the tax year, the maximum amounts that can be charge to PFAP-eligible individuals for emergency or other medically necessary care. Patient responsibility is based on total charges less an automatic uninsured discount of 40% after which a charity adjustment is applied. The charity adjustment for individuals with household income at or below 200% of the Federal Poverty Level (FPL) is the full amount of medically necessary charges. Individuals with household income in excess of 200% of FPL and below 400% of FPL are considered for charity assistance on a sliding scale. Balances remaining after the application of both the uninsured discount of 40% and the charity adjustment are considered the responsibility of the patient. Patients discharged from the emergency department without hospital admission will be expected to pay a fee of $100 per emergency department visit. Payment arrangements are available.
