Part I, Q. 3cIf applicable, describe the income-based criteria for determining eligibility for free or discounted care under the organization's financial assistance policy. When Catholic Health Initiatives (the ultimate parent organization to St. Anthony's Hospital Association) established its financial assistance policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among the 69 urban and rural communities in 19 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines ("FPG"), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. St. Anthony's Hospital Association bases its financial assistance eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain financial assistance write-offs, based on a sliding scale, ranging from 25%-100% of charges. Also describe whether the organization uses an asset test or other threshold, regardless of income, to determine eligibility for free or discounted care. An individual's income under the HUD guidelines is a significant factor in determining eligibility for financial assistance. However, in determining whether to extend discounted or free care to a patient, the patient's assets may also be taken into consideration. For example, a patient suffering a catastrophic illness may have a reasonable level of income, but a low level of liquid assets such that the payment of medical bills would be seriously detrimental to the patient's basic financial (and ultimately physical) well-being and survival. Such a patient may be extended discounted or free care based upon the facts and circumstances.
Part I, Q. 6aIf the organization's community benefit report is contained in a report prepared by a related organization, rather than in a separate report prepared by the organization, identify the related organization. St. Anthony's Hospital Association prepares its own annual written community benefit report. Its community benefit report is not contained in that of a related organization.
Part I, Q. 7, Column (f)If applicable, state the bad debt expense included on Form 990 Part IX, line 25, column (A) but subtracted for purposes of calculating the percentage in this column. Total bad debt expense reported on Form 990 Part IX, line 25, Column A was $4,160,661.
Part I, Q. 7(A) Provide an explanation of the costing methodology used to calculate the amounts reported in the table. If a cost accounting system was used, indicate whether the cost accounting system addresses all patient segments (for example, inpatient, outpatient, emergency room, private insurance, Medicaid, Medicare, uninsured or self pay). (B) Also indicate whether a cost-to-charge ratio was used for any of the figures reported in the table. Describe whether this cost-to-charge ratio was derived from Worksheet 2, and, if not, what kind of cost-to-charge ratio was used and how it was derived. If some other costing methodology was used besides a cost accounting system, cost-to-charge ratio, or a combination of the two, describe the method used. A cost accounting system was not used to compute amounts in the table; rather costs in the table were computed using the organization's cost-to-charge ratio. The cost-to-charge ratio covers all patient segments. The cost-to-charge ratio for the year ended 6/30/11 was computed using the following formula: Operating expense (before restructuring, impairment and other losses) divided by gross patient revenue. Worksheet 2 was not used to derive the cost-to-charge ratio.
Part I, Q. 7gTo the extent that the organization included any costs associated with physician clinics as subsidized health services in Part I, line 7g it must describe that it has done so and report in Part VI such costs included in part I, line 7g, line 1. There are no physician clinics included in subsidized health services.
Part III, Section A, Q. 4(A) Provide the rationale and the costing methodology used to determine the amount reported in Part III, lines 2 and 3. Describe how the organization accounts for discounts and payments on patient accounts in determining bad debt expense. Costing methodology for amounts reported on line 2 is determined using the organization's cost/charge ratio of 46.16%. When discounts are extended to self-pay patients, these patient account discounts are recorded as a reduction in revenue, not as bad debt expense. (B) Also describe the method the organization uses to determine the amount that reasonably could be attributable to patients who likely would qualify for financial assistance under the hospital's financial assistance policy, if sufficient information has been available to make a determination of their eligibility. Describe the rationale, if any, for including any portion of bad debt as community benefit. Also provide, if applicable, the text of the footnote to the organization's financial statements that describes bad debt expense. If the organization's financial statements include a footnote on these issues that also includes other information, report only the relevant portions of the footnote. If the organization's financial statements do not contain such a footnote, state that the organization's financial statements do not include such a footnote and explain how the financial statements account for bad debt, if at all. St. Anthony's Hospital Association does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care. St. Anthony's Hospital Association does not issue separate company audited financial statements. However, the organization is included in the consolidated financial statements of Catholic Health Initiatives. The consolidated footnote reads as follows: "The provision for bad debts is based upon management's assessment of historical and expected net collections considering historical business and economic conditions, trends in health care coverage, and other collection indicators. Management routinely assesses the adequacy of the allowances for uncollectible accounts based upon historical write-off experience by payor category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility."
Part III, Section B, Q. 8(A) Describe the costing methodology used to determine the Medicare allowable costs reported in the organization's Medicare Cost Report, as reflected in the amount reported in Part III, line 6. St. Anthony's Hospital Association is designated as a Critical Access Hospital ("CAH"). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (CoP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, St. Anthony's Hospital Association, due to their designation as a CAH, receives cost-based reimbursement for Medicare purposes and will not experience Medicare related shortfalls. (C) Describe, if applicable, the extent to which any shortfall reported in Part III, line 7 should be treated as a community benefit, and the rationale for the organization's position. St. Anthony's Hospital Association believes that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays.
Part III, Section C, Q. 9bIf the organization has a written debt collection policy and answered "Yes," to Part III, line 9b, describe the collection practices set forth in the policy that apply to patients who it knows qualify for financial assistance, whether or not such practices apply specifically to such patients or more broadly to also cover other types of patients. St. Anthony's Hospital Association's ("SAHA") debt collection policy provides that SAHA will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through SAHA's community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet the SAHA community assistance policy is required to be returned immediately by the third-party collection agent to SAHA for appropriate follow-up. SAHA requires its third-party collection agents to include a message on all statements indicating that if a patient or patient guarantor meets certain stipulated income requirements, the patient or patient guarantor may be eligible for financial assistance. All of Catholic Health Initiatives' hospitals' contracts with third party collection agencies include the following standards: - Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; - Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and - No Catholic Health Initiatives' collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect.
Part VI, Q. 1 (part II), and Q. 2, 4, and 5Q.2: Describe whether, and if so, how, the organization assesses the health care needs of the communities it serves. Q.4: Describe the community, or communities, the organization serves, taking into account the geographic area and demographic constituents it serves (for example, urban, suburban, rural, etc.), the demographics of the community or communities (for example, population, average income, percentages of community residents with incomes below the federal poverty guidelines, percentage of the hospital's and community's patients who are uninsured or Medicaid recipients, etc.) the number of other hospitals serving the community or communities, and whether one or more federally-designated medically underserved areas or populations are present in the community. Q. 5: Provide any other information important to describing how the organization's hospitals or other health care facilities further its exempt purpose by promoting health of the community (e.g. open medical staff, community board, use of surplus funds, etc) St. Vincent Morrilton Mission, Vision, and Tax-Exempt Purpose St. Anthony's Hospital Association d/b/a St. Vincent Morrilton celebrates 86 years of ministry this year. The Benedictine Sisters of St. Scholastica, joined by a Sister of Charity of Nazareth from St. Vincent Infirmary, members of Sacred Heart Catholic Church and other concerned citizens, came together to form the original ministry in 1925. Sixty nine years later in 1994, St. Vincent Morrilton became part of the St. Vincent Health System and in 1996 became a member of Catholic Health Initiatives. St. Vincent Morrilton, a 25-bed critical access facility is the primary source of healthcare for the residents of Conway and Perry Counties. St. Vincent Morrilton is led by a 12-member Advisory Board comprised of professionals who are either active in the local community or members of the St. Vincent President's Council. The Board serves in an advisory capacity and its Chairperson represents the local community as a member of the St. Vincent Health System Board of Directors. Services available at St. Vincent Morrilton include outpatient/same-day surgery, an intensive care unit, medical/surgical unit, 24-hour emergency room, orthopedic services, cardiology services, senior day care, rehabilitation services, imaging services, sleep medicine, wound care and laboratory services. An outpatient specialty clinic offers cardiology, orthopedics, oncology, neurology, gastroenterology, ear, nose, and throat, podiatry and nephrology specialties. St. Vincent Morrilton Petit Jean Home Health services provide home health care for residents of the service area. St. Vincent Medical Clinic - Morrilton provides patient access to healthcare, serving as a rural health clinic reaching more than 5,750 patients this year. Community Benefit Approach The population in the St. Vincent Morrilton primary and secondary service areas is 34,000. These people live in the greater Morrilton area, comprising Conway, Perry Van Buren and a portion of Pope Counties. St. Vincent Morrilton serves a significant indigent population: 15.0% of the population in this service area (Conway, Perry, Pope and Van Buren Counties) live below the poverty level-higher than the state average of 13.6% (2004 Small Area Income and Poverty Estimates, U.S. Census Bureau). St. Vincent Morrilton is the only full-service hospital within a 20-mile radius, and is therefore the primary source of healthcare in the area. Qualitative Description of Community Benefit Community Outreach for the Poor Donation of Supplies: During FY 11, St. Vincent Morrilton donated $10,008.80 in supplies to the Conway County Christian Clinic, Baptist Medical Missions International and a local Veterinarian. The Conway County Christian Clinic offers free healthcare to many residents of Conway County and partners with St. Vincent Morrilton to provide services through grant funding obtained by the Clinic. Community Outreach for the Broader Community St. Vincent Morrilton Service Excellence Employee Council raised 130 school supply items for the local Morrilton Elementary School. Community Outreach Health Fairs: St. Vincent Morrilton participated in two community outreach health fairs for local businesses in the area. Bosch Security Systems, specializing in audio speaker production and equipment, held its first health fair for its employees. St. Vincent Morrilton participated in the planning of the fair. It also provided information about breast and colon cancer prevention, as well as the importance of getting a flu shot. St. Vincent Morrilton was able to service the 171 employees who work for the company. The University of Arkansas Community College of Morrilton (UACCM) also held a health fair for its employees. St. Vincent Morrilton provided cholesterol screenings, blood pressure checks, and body mass index analysis. On this occasion, St. Vincent Morrilton was able to serve 160 employees who work for the College. Collaborative Efforts to Improve Community Health Body, Mind & Soul: St. Vincent Morrilton, in conjunction with the Winthrop Rockefeller Institute, an affiliated outreach campus of the University of Arkansas System, hosts an annual "Ladies Night Out" which features health and wellness topics that impact a woman's body, mind, and soul. Developed by a group of women from the local community, this event consists of various representatives from the community providing information on diabetic education, cancer, exercise techniques, weight management, osteoporosis, heart health/stroke, stress management, mental health/depression, menopause, sleep disorders, and spirituality/rejuvenation. Dr. Brent Scroggins, family practice physician, spoke and answered questions on topics of interest to all women such as managing your weight, healthy food choices, and balancing your diet with proper exercise. Over 150 women attended the event. This annual event was created to offer educational programs that will help encourage healthier lifestyle choices for individuals and families across Central Arkansas. Heart Fair: In honor of National Heart Month, heart education was provided in collaboration with the St. Vincent Morrilton campus and the University of Arkansas Community College (UACCM) in Morrilton. The ninth annual "Love Your Life, Love your Heart" Community Health Fair, hosted by St. Vincent Morrilton, was held on February 18th at the College and was supported by nursing students, community volunteers, support staff, and participating vendors. Chesapeake Energy sponsored the first 100 full panel cholesterol screenings. The Heart Fair featured blood pressure checks, body mass index analysis, nutrition seminars, various other screenings and information provided by a range of vendors, all supporting healthy hearts. Later in the month, a nutrition and healthy heart luncheon was hosted by dieticians from both campuses and provided education on healthy lifestyle eating, exercise and heart awareness. Dr. Erick McCarthy, Jessica Rohlman, and Laura Meek presented information about exercise, personal health and nutrition. A healthy cooking demonstration was provided by St. Vincent Morrilton Clinical Dietitian, Mary Jane Cahill, RD, LD. Pig Out Community Festival: St. Vincent Morrilton participated in the Great Arkansas Pig-Out Community Festival this past year. This is an annual two-day community event. St. Vincent Morrilton provided a first aid tent and information to the community regarding services that are offered. Hand sanitizer stations were placed throughout the park by St. Vincent Morrilton to reduce bacteria and create prevention awareness. Community Building Activities Economic Impact: Johnny Campbell, Radiology Department Coordinator became a member of the Morrilton Area Chamber of Commerce Board as of January 2011. The Board guides economic and leadership development related to the City of Morrilton, its business community and surrounding areas. Johnny also serves on the Planning and Zoning commission which oversees planning and zoning ordinances for the City, insuring that all pertinent regulations are followed by various businesses and private entities. Angie Longing, RN, BSN, MHA, Chief Nursing Officer serves on the Board of Directors at Brookridge Cove Rehabilitation and Care Center. Brookridge Cove is one of two short- term and long-term care facilities in Morrilton. Ms. Longing was one of the first Board members, so she played a role in the adoption of bylaws, policies and procedures. They also adopted a new 401K plan for employees. Christy Hockaday, FACHE and Administrator/CEO of St. Vincent Morrilton at that time completed second term as President of Conway County Economic Development Corporation in December, 2010. Physicians Rural Health Grant: St. Vincent Morrilton worked with Morrilton Medical Clinic and physicians Kyle Trauth, MD and Lydia Trauth, MD to apply for state funds to be able to sponsor these two physicians in return for their commitmCommunity Building Activities, Needs Assessment, Community Information
Part VI, Q. 3Describe how the organization informs and educates patients and persons who may be billed for patient care about their eligibility for assistance under federal, state, or local government programs or under the organization's financial assistance policy. St. Anthony's Hospital Association ("SAHA") includes information concerning its financial assistance policy on its website. In addition, SAHA prominently displays its financial assistance policy in both English and Spanish in obvious locations throughout the hospitals, including the emergency rooms and other patient intake areas, as well as in SAHA outpatient facilities. A financial assistance brochure is also presented to each patient at registration. In addition, SAHA registration clerks are trained to provide consultation to those who have no insurance or potentially inadequate insurance concerning their financial options including application for Medicaid and for financial assistance under SAHA's financial assistance policy. Upon registration (and once all EMTALA requirements are met), patients who are identified as uninsured (and not covered by Medicare or Medicaid) are provided with a packet of information that addresses the financial assistance policy and procedures including an application for assistance. SAHA registration clerks read the organization's medical assistance policy to those who appear to be incapable of reading, and provide translators for non English-speaking individuals. SAHA's staff will also assist the patient/guarantor with applying for other available coverage (such as Medicaid), if necessary. Counselors assist Medicare eligible patients in enrollment by providing referrals to the appropriate government agencies.Patient education of eligibility for assistance
Part VI, Q. 6If the organization is part of an affiliated health care system, describe the respective roles of the organization and its affiliates in promoting the health of the communities served. St. Anthony's Hospital Association, along with its affiliated outpatient facilities are part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Englewood, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 73 hospitals; 40 long-term care, assisted- and residential-living facilities; two community health-services organizations; two accredited nursing colleges; and home health agencies. Together, these facilities provided $612 million in charity care and community benefit in the 2011 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain -- provides economies of scale and purchasing power to the MBOs. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society. St. Anthony's Hospital Association operates with its wholly owned affiliates and community partners, along with its fundraising arm, the St. Vincent Morrilton Foundation, to serve the health care needs of the Morrilton, Arkansas communities.Affiliated Entities

METHODS USED TO ESTABLISH CEO COMPENSATIONSch J, Part I, Q. 3Compensation for the top management official was established and paid by Catholic Health Initiatives (CHI), a related organization. CHI used the following to establish the top management official's compensation: (1) Compensation Committee; (2) Independent Compensation Consultant; (3) Written Employment Contracts; (4) Compensation Survey or Study; (5) Approval by the Board or Compensation Committee.
POST-TERMINATION PAYMENTSSch J, Part I, Q. 4aPost-termination payments are addressed in executive employment agreements for Catholic Health Initiatives ("CHI") and related organizations' employees at the level of Vice President and above, including the MBO CEOs. These employment agreements require that in order for the executive to receive post-termination payments, these individuals must execute a general release and settlement agreement. Post-termination payment arrangements are periodically reviewed for overall reasonableness in light of the executive's overall compensation package. St. Vincent Infirmary Medical Center (SVIMC) uses the same methodology regarding severance payments for vice presidents and above. For the calendar year 2010, David Hall, MD received $46,834 in severance pay from SVIMC. The severance payment was included in the individual's reportable compensation for Schedule J.
SUPPLEMENTAL NON-QUALIFIED DEFERRED RETIREMENT PLANSch J, Part I, Q. 4bDuring the 2010 calendar year Catholic Health Initiatives ("CHI"), a related organization, maintained a supplemental non-qualified deferred compensation plan for MBO CEOs and other CHI employees at the level of Senior Vice President and above. The following reportable individuals were eligible to participate in that plan: Peter Banko and Mary Elizabeth O'Brien. During 2010 the following contributions were made by CHI to the deferred compensation plan: Peter Banko $37,318 Mary Elizabeth O'Brien $52,785 During 2010 the following distributions were made by CHI from the deferred compensation plan: David Hall, M.D. $399,689

Process for Determining CEO's CompensationForm 990, Part VI, Q. 15aThe organization's CEO's compensation is paid by CHI. CHI has a defined compensation philosophy. Both the executive and non-executive compensation structures and ranges are reviewed annually in comparison to market data. CHI uses The Hay Group as the independent third party to assess executive compensation programs and to ensure the reasonableness of actual salaries and total compensation packages. Compensation of the senior most executives is reviewed annually. The Hay Group reviews both cash and total compensation for overall reasonableness, for adherence to CHI's compensation philosophy, and for comparability to the not-for-profit healthcare market. This independent review is delivered by Hay Group to the HR committee of the CHI Board of Stewardship Trustees annually at their September meeting and minutes are shared with the full board at the December meeting. The last review was September, 2011. In addition, in December 2009, hay group completed a comprehensive review of all positions at the level of vice president and above to determine and validate appropriate compensation levels.
