Nemaha Valley Community HospitalPart V, Section B, Line 5: Community members were invited to attend a meeting in which their input was requested to determine the health needs of the community. The community members identified the health care needs that should be prioritized. These members included medical professionals, government employees, leaders of non-profit organizations, and hospital employees.
Nemaha Valley Community HospitalPart V, Section B, Line 6a: Sabetha Community Hospital
Nemaha Valley Community HospitalPart V, Section B, Line 11: Priority 1: Childcare (Affordable & Accessible) Hospital administration has had discussions with daycare providers regarding options for employment by the Hospital to provide care for the children of Hospital staff. The reach of the Hospital on this priority is limited as continuous childcare is a problem much larger than the Hospital alone can solve.Priority 2: Mental Health (Diagnosis, Placement, Aftercare, Access to Providers)The Hospital has hired a full-time in-house licensed and certified mental health therapist. The Hospital also contracts with telehealth mental health providers including medication management. The Hospital continues to partner with Health Innovation Network of Kansas in their work with suicide prevention through speaking events, meetings with state-wide organizations, local television broadcasts, and other campaigns. In the Hospital's family practice clinic, drug, alcohol, and depression screenings are part of each patient visit. The Hospital's care coordinator, social worker, and community health worker assist patients with locating and continuing care through education and coaching.Priority 3: Housing (Affordable & Accessible)Access to affordable housing is not part of the mission and goals of the Hospital's critical operations. We recognize the issue but have limited capability to address the need.Priority 4: Substance Abuse (Drugs & Alcohol)Screening for drug and alcohol abuse is part of each patient visit within the family practice clinic. The Hospital employs a social worker and a community health worker. Both employees help educate, advocate, and connect individuals to available resources. They also assist patients in navigating the health system and continuing care as needed. The Hospital holds an annual community health fair at which several organizations provide education and resources regarding substance abuse including prevention and treatment. At local schools, the Hospital's respiratory therapy department provides educational sessions and demonstrations on the dangers of vaping.Priority 5: Chronic Disease Management (Cancer, Diabetes, In-Home Care)The Hospital's care coordinator, social worker, and community health worker assist patients with managing chronic disease by connecting them with resources, helping them set appointments, and continue care after hospitalization and in prevention of an inpatient stay. The Hospital has added an oncology department which will provide chemotherapy infusions to local cancer patients. The service expands access to care by allowing patients to stay close to home instead of driving 2-4 hours for treatment. Discounted lab tests are offered at the annual community health fair. The Hospital's dermatology provider presented skin cancer education, awareness, and prevention for a segment on our local television station. The Hospital's pain management services help relieve pain for cancer patients. The dietitian and diabetes educator hold monthly support group meetings for diabetics. They also offer individual management training.Priority 6: Obesity (Access to Healthy Foods/Exercise)The Hospital encourages utilization of its registered dietician in primary care and specialty care settings. The clinic is involved in the RETOOL weight loss study with KU Medical Center. Clinic providers refer patients to the registered dietician for weight loss management. Employees of the Hospital are allowed to see the dietician at no cost. The Hospital helps promote fitness events put on by the extension office. Through social media posts, the Hospital shares tips and education on healthy eating and exercise habits.Priority 7: Uninsured/UnderinsuranceHospital staff help patients enroll in Medicare and the Marketplace. Through the Hospital's financial assistance policy, discounted care is offered to patients who are uninsured or underinsured. Discounts range from 20% to 90%. The Hospital offers a prompt pay discount of 10% to self-pay or underinsured patients if they pay 90% of the total bill at time of billing. The Hospital also offers a 20% discount to uninsured patients who do not qualify for financial assistance.Priority 8: Youth Health Education (Addition)At local schools, the Hospital's respiratory therapy department provides educational sessions and demonstrations on the dangers of vaping. The nursing supervisor visited local schools educating students on hearing loss prevention. Stop the Bleed, AED, and CPR education is held at area schools and colleges. The diabetes educator provides presentations to area students regarding diabetes signs, symptoms, and how to support diabetic friends. Each spring, the therapy department and other volunteers provide a Learn to Bike Camp for anyone 4 years old and older.

Part I, Line 7:This amount was determined by multiplying a cost-to-charge ratio derived from Worksheet 2 for the tax year.
Part I, Line 7g:The net community benefit expense of subsidized health services include costs attributable to a rural health clinic in the amount of $1,127,108.
Part II, Community Building Activities:N/A
Part III, Line 4:Nemaha Valley Community Hospital provides for accounts receivable that could become uncollectible in the future by establishing an allowance to reduce the carrying value of such receivables to their estimated net realizable value. This estimate is based on the aging of its accounts receivable by payor and historical collection experience. See note A5 (page 7) of the audited financial statements regarding the allowance for uncollectible accounts.Part III Line 2:This amount was determined by multiplying a cost-to-charge ratio derived from Worksheet 2 for the tax year.Part III, Line 3:This amount was determined based on the overall income level of patients that submit all required documentation under the Hospital's financial assistance policy. As a tax-exempt hospital, we must provide necessary services regardless of the patient's ability to pay for that care. Making quality patient care available to all in our community, regardless of their economic means, qualifies this amount as a community benefit.
Part III, Line 8:Medicare costs were obtained from the Hospital's as-filed Medicare cost report for the tax year. Any shortfalls are a benefit to the Hospital's community since it represents health care provided at less than cost to elderly and needy residents of the Hospital's service area.
Part III, Line 9b:Nemaha Valley Community Hospital makes every effort to get correct financial and billing information and to educate patients on the eligibility requirements of charity care programs and payment options.
Part VI, Line 2:N/A
Part VI, Line 3:Notification associated with patient encounter.
Part VI, Line 4:The community served is primarily residents who reside in Nemaha County Kansas and consists of approximately 2,139 residents. The median family income is $70,221. 8.50% of the community residents live below federal poverty level. Neighboring hospitals are located in Sabetha, Pawnee County, NE, and Hiawatha. They are approximately 21, 20, and 28 miles away respectively.
Part VI, Line 5:The Hospital maintains a Facebook page which posts general health information topics and announces dates of general health-related group activities. The Hospital offers periodic diabetic educational programs and education programs for expectant mothers. The Hospital uses all excess funds to maintain and improve Hospital facilities and equipment and to insure the delivery of high quality patient care services.
