Form 990, Part VI, Section A, line 1During the year the board of directors delegated broad authority to act on its behalf to an Executive committee composed of the Chair, Vice Chair, Secretary, and Treasurer. The Executive Committee is authorized to handle any matters referred to it by the Board of Directors. In addition, is authorized all powers of the Board during the interval between meetings of the Board, except and excluding purchase, sale, lease or encumbering of real property.
Form 990, Part VI, Section B, line 11A copy of the organization's final form 990 (including required schedules), as ultimately filed with the IRS, was provided to each voting member of the organization's Finance Committee and Board meeting on January 25, 2011 prior to its filing with the IRS. In addition, the organization's return preparer and management presented a Form 990 summary report at this meeting.
Form 990, Part VI, Section B, line 12cAs organization personnel, members of the governing body and advisory boards are covered under the organization's Conflict of Interest policy. In the event that a situation arises whereby an employee has a conflict of interest, he/she is obligated to report that potential to the President/CEO. Disclosure of a potential conflict of interest and the President/CEO's decision regarding the actions will be noted in a file kept by the President/CEO. In the event a situation exists whereby a member of the governing body or advisory committee has a conflict of interest, he/she is obligated to report that potential to the governing body. The governing body will then render a decision of that member's eligibility to vote on any particular issue. Disclosure of a potential conflict and the governing body's decision regarding the conflict will be noted in the minutes.
Form 990, Part VI, Section B, line 15The President/CEO's compensation is established and approved annually by the Executive Committee of the Board of Directors. The Committee utilizes data provided by independent compensation consultant studies to provide a basis for comparison. Additionally an appraisal of performance is also discussed with all Board members. The approval of the President/CEOs compensation is documented in the Executive Committee meeting minutes. The organization has a compensation program which applies to all personnel except the President/CEO. The policies and procedures define the general rules of operation of the compensation program and the President/CEO and Board retains the authority to revise the policy as needed for the effective management of the program. The compensation program was initially developed by an independent human resource consultant and is reviewed as needed. The salary structure is reviewed annually.
Form 990, Part VI, Section C, line 19The organization does not make its governing documents, conflict of interest policy or financial statements available to the public. However, the Federal Form 990 is available upon request.
Form 990, Part XI, Line 2c:The process of assuming responsibility for oversight of the audit of the organization's financial statements and selection of an independent accountant has not changed from the prior year.
Form 990, Part I, Line 1:Hope Hospice (later renamed Hope Hospice and Community Services; hereafter referred to as "Hope") was incorporated on September 11, 1981, after its formation by a group of local volunteers, and licensed by the State of Florida since 1984. Hope is a not-for-profit, 501 (c)(3)-status Community-Based agency. At its inception, Hope Hospice served only a few patients each day in the immediate Fort Myers, Florida area. Today Hope serves more than 2,200 people daily, in a nine-county, 10,000 square mile area in southwest and mid-Florida. Through the years, Hope's Mission and Vision have been constant. The Vision of Hope is to create an environment in which life's journey is cherished. The Mission of Hope is to provide exceptional care and support to every individual and their loved ones as they fulfill life's journey. Hope's Vision and Mission are supported by its Pillars of Excellence: Service - Provide extraordinary, timely care and service Quality - Provide excellence each day and demonstrate continuous improvement Stewardship - Create and manage the resources needed for our current and future success People - Recruit and retain staff who are engaged, empowered and committed The organization's goals are to remove all barriers to access; to be good stewards of all resources; and to provide the highest possible quality care. Hope's goals and initiatives are framed in response to strategic questions, including: - How do we identify and change habits that do not support good care? - How do we use lessons from other industries to improve our care and services? - How do we improve our ability to reduce pain and suffering? - How do we ensure we are the employer of choice? - How do we create and sustain a culture that leads to continuous improvement? Hope makes make certain that its organizational capabilities ensure: - No needless pain or suffering exists in the communities we serve - No helplessness felt by those we serve - No unwanted waiting for those we serve - No waste of resources or inefficiencies in providing our services "Hope Hospice" becomes "Hope Hospice and Community Services" In support of Hope's Vision, Mission and goals, an overarching question was, "How can we increase our continuum of care and services, giving more people access to the comfort and safety they need?" Traditionally, the starting point in hospice care is the physician's referral, indicating the patient has only six months or less to live. Beginning at the end, little thought is given to the patient's overall situation and circumstance. This can result in a less than optimal care experience and outcome for the patient and family. Hope looks at the entire patient situation and circumstances prior to developing a comprehensive plan of care. In recent years, Hope has recognized that the palliative care skills of hospice professionals need not be limited to the care of the dying, in hospice. These same skills can be applied to alleviate pain and suffering for those who are seriously ill yet not at the end of life. Hope has introduced a number of new care programs to meet those needs. Hope's innovation has created a new dimension in health care. Hope Hospice and Community Services provides a continuum of palliative care services unlike any other health care program in the United States. Hope's innovation is best described with two words: proactive coordination. Each care program and service has its own specific staff, delivering palliative care in a specific setting or program. Hope is uniquely applying the principles of palliative care (i.e. to prevent and relieve suffering while focusing on the quality of life for the individual) across a larger continuum of care beyond end of life care. Hope's palliative care philosophy encompasses every aspect of the individual's well being - body, mind and spirit. At the heart of the philosophy is the commitment to make sure the system is working to meet specific personal needs, not in trying to make a patient fit into a system. Services are available to people of all ages, in all programs, who are touched by serious illness, regardless of their ability to pay. The team includes physicians, nurses, pharmacists, social workers, personal care specialists, volunteers, chaplains, homemakers, bereavement counselors, massage therapists, art therapists and music therapists as needed to care for the individual needs of each person and family. Hope's programs of care benefit all demographics. Hope Hospice and Community Services serves the residents of Lee, Glades, Hendry, Highlands, Hardee, Polk, Charlotte, Desoto and Collier counties in southwest and mid-Florida, with a total population of 1.8 million, of which approximately 76 percent are white, 15 percent Latino, 8 percent African-American and 1 percent Asian. The area includes urban, suburban and rural communities. Hospice and other services are provided to everyone in need in Lee, Glades, Hendry, Highlands, Hardee and Polk counties. Hospice care is usually covered by Medicare, Medicaid or insurance, although Hope provides hospice care to everyone in need regardless of age, ethnicity, type of illness, the ability to pay or any other life circumstances. This comprehensive, coordinated program of care is offered to them by no one else in the entire area. It is of great benefit to many in the community who are particularly vulnerable in these times of a down turned economy and an uncertain future for US health care, including those with no medical coverage; no financial resources whatsoever; the homeless; veterans and others. Ensuring Quality Care In November 2010, the Dartmouth Institute for Health Policy and Clinical Practice published a study titled "Quality of End-of-Life Cancer Care for Medicare Beneficiaries." According to the report, 75.6 percent of cancer patients in the Fort Myers area who died during the period from 2003 to 2007 used hospice services during their last month of life - dramatically eclipsing the national figure of 55 percent, and among the highest rankings of all hospices. Hope is the only hospice serving the Fort Myers area. Hope was the first to receive the excelleRx Institute Excellence in Pharmacotherapy Outcomes award, in 2005. The purpose of the award is to honor outstanding achievements by hospice organizations dedicated to superior care and promotion of best practices. All of Hope's services and programs coordinate care along the continuum using the principles of palliative care. Each service is measured for its effectiveness and benefit to patients and families. Outcome measurements are in place throughout the continuum as part of the ongoing Quality Improvement Plan. In addition to regular, intensive review of all activities, Hope conducts frequent and regular patient surveys in patients' homes, in nursing and assisted living facilities and in Hope Hospice Houses. These patient surveys focus on: pain and symptom control; staff sensitivity; whether expressive and integrative therapies were offered; extended hours services and support; and whether medical equipment and supplies met needs. Outcomes are also measured by the Community Health Accreditation Program (CHAP), the national leader in improving the quality of care in the home care industry. One of CHAP's primary objectives is to enable hospices and other health care organizations to develop and maintain state-of-the art national standards of excellence. During their exhaustive review of Hope's operations, CHAP discovered what makes the organization great. Hope's high score was "extremely rare," they said, for several reasons, including: high employee morale and job satisfaction which contributes to a positive work environment; experienced management team and Board of Directors who are committed to quality care and standards of excellence; and collaboration with the educational community to increase students' awareness of palliative care opportunities.
Innovation in HealthCare: Meeting the Challenges In 2008, Hope Hospice and Community Services was among the country's first recipients of the Quality in Palliative Care Leadership Award, in recognition of its innovative patient care. The Leadership Award is presented by the National Consensus Project for Quality Care and the National Quality Forum to organizations working to improve the quality of American health care. Hope was one of only nine health care organizations in the country to be honored in the award's first year. Hope received the Circle of Life Award in 2004 for its innovative program that improves the care of individuals at the end of life and its strategy to make services available to anyone who needs hospice care, regardless of age, diagnosis, or ability to pay. Hope was recognized in 2003 by the American Pharmacists Association Foundation for its innovative and visionary approach in delivering a consistently high level of care to its patients. The recipients are recognized for pioneering innovative ways to improve the medication use process and increase communication between all members of the health care team. Because of the significant public health problem associated with the inappropriate use of medications, Hope pioneered innovative ways to improve the medication use process and increase communication among all members of the health care team. Hope Hospice created, implemented and maintains a Collaborative Practice Medication and management System to manage medication therapy. It involves pharmacists, Hope physicians and nurses, many independent referring physicians, and independent clinical pharmacy and medication distribution systems linked together by Web-based documentation and communication tools. Using integrated, state-of-the-art technology that enables professionals from different disciplines to participate in a patient's care and make informed decisions based on current information, Hope is able to deliver and unparalleled level of quality care to its patients. Unlike most other hospice and palliative care organizations, Hope has overcome a critical obstacle to patient comfort and care. While chemotherapy can be curative, it can also be a palliative treatment for pain management, even though the illness may be incurable. However, Medicare, the source of coverage for most hospice patients does not cover the costs of chemotherapy for hospice patients. As a result, most hospices will not accept a patient in those circumstances. The individual is forced to choose whether to remain on chemotherapy with minimal medical support, or enter hospice without chemotherapy. Hope Hospice and Community Services has a commitment to accept all patients in need of care, including those who need services for which there is no medical coverage. Hope does not force decisions and covers the costs of any necessary treatments. To the benefit of many, Hope looks at the prognosis as well as the needs when determining eligibility, helping to ease the transition from curative care to palliative care. If other hospices were to make this paradigm shift, the level of quality hospice care as well as the overall quality of health care would improve. The Future of Hope and the Future of Healthcare The United States is being challenged as never before to make the best use of our resources in healthcare in order to best meet the changing and increasing needs of the people in our communities. Healthcare providers must make very careful and strategic decisions to ensure that future needs will be met. Hope has taken a leadership role in raising awareness and shaping policy on the state and national levels. Hope's President and CEO, Samira K. Beckwith was appointed by Florida Governor Jeb Bush to help the state concentrate on providing more services to enhance the quality of life for senior citizens. Governor-elect Rick Scott recently appointed Beckwith to his Health and Human Services Transition Team, to address Medicaid reform, health, health-care administration, children and families, elder affairs, persons with disabilities and veterans' affairs. She has testified before Congress on the need for better legislation to provide for end-of-life comfort and care. Hope will continue to apply the principles of palliative care across a larger continuum beyond end-of-life care, in search of new opportunities to provide care to more people.
