Debt Collection PolicyPart III, Line 9b: Each week, the statements for accounts are sent out through our EDI process for statements. These are sent from a clearinghouse who are now doing our statements.Patients are billed after their Medicare, BCBS and insurance has been filed and a payment received within a six-week period. The patient will be billed each month with the current balance. Statements are also sent with ins pending, WC pending and BCBS pending. This gives patient an idea on where there account is with insurance.A 1.5%/18% yearly/$.50 minimum interest charges is placed on all accounts after 90 days.Insurance billers verify statements before notice letters are printed. If no payment is received within 2 months of billings, the statement is sent with a notice letter requesting payment through the EDI process also. The billers will verify who should get a notice. Notices are also sent after 3 months and 4 months of no payments. If no response from the last letter within 5 days, a telephone call is issued to follow up on the account. When talking to the patient regarding their account, we can ask them if they would like to make payments to us, or if they can pay their account in full in 30 days, there would be a 5% discount given, or if they would qualify for our Healthcare Financial Aid Program, we can send them an application to fill out and return to us with the 2 denials we request. If no response to the telephone call, then the account will be submitted to the insurance billing clerks to verify the account status; then to the Business Manager and Administrator for collection approval. These accounts are then turned into a Professional Collection Agency. When the need arises for Charity Care considerations, consulting is done with the Business Office Manger and Administrator to determine if this is the best ethical decision. There are situations where the patient's lack of finances may warrant this.DebtCollectionPolicy

Other InformationPart VI, Line 6: Madelia Community Hospital staff, healthcare providers and emergency response organizations have been working collaboratively since 2002 to prepare for disasters in our region. Disaster preparedness plans have been created in the event of pandemics, floods, tornadoes, etc. The drills became a reality this year with the H1N1 flu pandemic. As a team we swung into action and implemented strategies developed by the Minnesota Department of Health to slow the spread of the disease as well as to protect the health of all Minnesotans. Madelia Community Hospital was quick to respond with educational opportunities in many different settings of the community, including the school system, daycare center, public forums and senior expos. Every patient who presented in the Emergency Room was given a small complimentary Flu Kit including bottled water, chicken noodle soup, Kleenex, hand sanitizer and printed educational material. A number of H1N1 clinics were also held throughout the flu season. Since March, The Madelia Community Hospital Home Care Department census grew by approximately 35% with the closing of several area home care agencies. This resulted in the addition of several new RNs and HHAs to the home care staff. Sharon Dexheimer, a Certified Caregiver Educator has also been added to the staff with the implementation of the Family Memory Care Program. Sharon Dexheimer provides area caregivers with education on Alzheimer's disease, assistance in locating community resources, development and implementation of a family care plan and short term or ongoing caregiver support. The program is provided at no cost by the Madelia Community Hospital through a grant from the Administration on Aging, Department of Health and Human Service and the Minnesota Board on Aging. In April, a weekly Ultrasound service became available by using a state-of-the-art ultrasound machine that uses sound waves to visualize organs and structures inside the body to determine if there are any abnormalities present. General ultrasound services now include: Renal, OB, Abdomen, Pelvic, Thyroid, Scrotum, Breast, Venous/Arterial, Aortas, and ABI Limited. Carotid Doppler ultrasounds are also provided. The staff at Madelia Community Hospital is committed to keeping patients safe as they seek care in our facility. Even though our infection control rates are very low, we strive for zero. In April, we launched a new hand washing campaign "Hand Hygiene Saves Lives." Hand washing is the number one prevention for hospital acquired infections. Through our program we are educating staff, patients, and visitors on the importance of hand hygiene. The data collected through our efforts is displayed in the reception area of the hospital and also at the back entry. In May, a team was developed to create processes that would make sure that Madelia Community Hospital patients who were discharged before test or procedure results were available would have proper follow up with a physician. It was also important that the patient be notified by the nurse if further follow up was needed. Staff at Madelia Community Hospital became aware of occurrences with family, friends, and in some cases it happened to them, where follow up on test results was not completed; it was happening in healthcare facilities all over. As this discussion continued, staff felt we needed to make sure this was not happening to patients in our facility. We will continue to monitor its success and continue to make improvements in this process. This program has been very successful.Living Well With Chronic Conditions, a free workshop helping to "Put Life Back in Your Life" was held twice this year by the Physical Therapy Department due to support from the Minnesota Department of Health. The workshops consisted of six weekly sessions designed to help participants with ongoing health conditions such as arthritis, diabetes, high blood pressure, anxiety, heart disease, anxiety and others. Taught by specially trained volunteer leaders, some who have health conditions themselves, the program covers a new topic each week and provides opportunities for interaction and group problem solving. This program will continue twice a year. Madelia Community Hospital kicked off Hospital Week with an Open House featuring blood pressure screenings, healthy snacks and beverages and health education materials. This was the perfect time to roll out an Eight-Week Physical Activity Pledge. During the Open House, community members were encouraged to join our Madelia Community Hospital team and sign up for an online activity tracker to promote better health. Hospital members then hosted events during the following 8-week period to support community members reach their goals. Events included: Pilates, total body conditioning, 30-minute cardio, kick boxing, and line dancing. Free live patient education webinars were held to recognize National Osteoporosis Awareness and Prevention Month. The first webinar, "How Strong are your Bones" offered a great opportunity for those interested in osteoporosis to be provided with an overview of osteoporosis and many of the risk factors for the disease, including medicines and medical conditions that can cause bone loss. The second webinar, "Nutrition for Your Bone Health" offered a great opportunity for those interested in learning more about food choices you can make to meet the nutritional needs for optimal bone health. The webinar also provided information about foods and nutrients you should consume in moderation and when to consider taking supplements to benefit your bones. In this new era of preventing, detecting and treating this disease, The Madelia Community Hospital continues to offer bone density tests, painless diagnostic tests, Monday through Friday. Madelia Community Hospital Foundation presented Mickayla Jones and Jasmine Eggestein with the 2010 Madelia Community Hospital Healthcare Scholarships for $500. Mickayla plans to attend South Central College in Mankato and intends to go into Psychology or Social Services. Jasmine will be attending the University of Tampa in the Pre-Med program. Jasmine is pursuing a career in medicine with a goal of becoming a pediatric oncologist physician. These scholarships are intended to inspire and encourage students to dedicate their lives to helping others.Hospital Staff members wanted to further promote a healthy lifestyle and are proud to sponsor the Madelia Farmer's Market! The Farmer's Market has been open every Wednesday from 4:30 to 6:30 p.m. across from the hospital on Drew Avenue. Over the summer the Farmer's Market has grown, both with customers and vendors offering a wide variety of products. Our Dietary Department is in it's 5th year of letting the patients choose their own meal; a choice of two or three entrees along with a variety of side itmes allow them to have a meal that fits their own taste and dietary needs. On average, 60 nutriouts meals are served per day, seven days a week including the holidays. Meals are served to patients, Amberfield Place residents, Meals on Wheels clients and staff members.Madelia Community Hospital Physical Therapy Athlete Care and Evaluation (A.C.E.) Clinic continues strong. The free program provides prevention, evaluation and treatment of area athletes, with the focus on getting the athletes back in the game. The Athlete's Injury Assessment evaluates the athlete's condition to determine their source of pain or limitation. The 30 minute session includes: general first aid care, rehabilitation suggestions, stretching & strengthening, guidelines for returning to the sport, taping and/or brace selection and if necessary, referral recommendations to an appropriate doctor. The program is under the direction of Certified Physical Therapists /Athletic Trainers Jamie Hitchcock and Jim Hitchcock. The A.C.E. Clinic has assessed 38 athletes through this year and hopes to be of assistance to the community again especially as a new school year begins! The Minnesota Department of Health recently designated Madelia Community Hospital as a Level 4 Trauma hospital. The staff at Madelia Community Hospital voluntarily participated in the intense designation process which shows their commitment to the community and region to help provide life-saving emergency measures to a severely injured person. Madelia Community Hospital met standards of commitment, clinical equipment resources and staff training. Madelia Community Hospital's Emergency Department is trained and equipped to treat many minor to moderate injuries, and to stabilize and triage severe injuries. Jen McLaughlin, RN, Emergency Department Manager has been working for more than a year to achieve this designation showing the Hospital's commitment to the community and region to help provide life-saving emergency measures to a severely injured person.OtherInformation
