Form990 Schedule H Part VI Form990ScheduleHPartVI (15 entries)

IdentifierIdentifierExplanationExplanation
PART VI LINE 1 - PART I, LINE 3CPatients who are uninsured or underinsured and cannot pay for hospital services are eligible for charity consideration. These patients are screened by designated team members in our Medical Assistance Department. The Agency for Health Care Administration (AHCA) defines charity eligibility at 200 percent of the federal poverty guidelines, unless the total hospital bill is more than 25 percent of the patient's annual income. Medicaid recipients who have exceeded their coverage limits are also considered for charity care. St. Anthony's Hospital goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days.
PART VI LINE I - PART I, LINE 6AN/A - The Community Benefit Report is available to the public.
PART VI LINE 1 - PART I, LINE 7Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients, including emergency room patients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program. Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits.
PART VI LINE 1 - PART I, LINE 7FBad Debt expense of $1,818,053 was included on form 990, Part IX, Line 25, Column (a), but subtracted for purposes of calculating the percentage in this column.
PART VI LINE I - PART IIN/A
PART VI LINE I - PART III, SECTION A, LINE 4The organization's financial statements do not include a footnote directly on bad debt expense. Bad debt expense at cost was calculated by the ratio of cost to charge method, using data from Schedule H. Total expense of the organization is reduced by the community benefit expenses reported on Part I, Line 7 (column C), and gross patient revenue is reduced by the direct offsetting revenue reported on Part I, Line 7 (column D). The resulting ratio of cost to charges is then applied to total bad debt expense of the organization, which calculates the bad debt expense at cost. We believe the method described above understates the true cost of bad debt. In many instances, bad debt results from non-payment of co-pays and deductibles, which would not be at charges but at a lesser amount. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid.
PART VI LINE 1 - PART III, SECTION B, LINE 8Cost reports were used to report Medicare allowable costs. Medicare defines allowable costs as those appropriate and helpful in developing and maintaining the operation of patient care facilities and activities. It specifically excludes certain costs that are not directly related to patient care. The hospital incurs additional expense related to the provision of care to Medicare patients that Medicare has deemed non-allowable. This additional expense includes costs of physician services (emergency on-call fees, hospitalist program, recruitment, etc.), advertising costs, cafeteria costs for meals sold to visitors, etc. The hospital attempts to collect coinsurance and deductibles from Medicare beneficiaries. To the extent collection efforts are unsuccessful, Medicare reimburses the hospital at 70% of unpaid amounts. The following table reconciles the surplus or shortfall from Line 7 to the actual surplus or shortfall. The additional costs were allocated to Medicare based upon Medicare's percentage of total allowable costs. The unpaid coinsurance/deductibles were estimated using historical collection results. Line 7 Surplus or (Shortfall) ($6,523,476) Additional non-allowable costs and unpaid/non-reimbursed coninsurance/deductibles $2,446,597 Total Surplus or (Shortfall) ($8,970,073)
PART VI LINE 1 - PART III, SECTION C, LINE 9BPatients who are unable to pay are encouraged by BayCare Health System representatives, via personal interviews, signage, patient billing mailers, brochures or Customer Service phone calls, to submit financial information to the Medical Assistance Department to determine eligibility for programs, such as County, Medicaid, Disability, Victims of Crime, Charity, etc. For those patients who provide all the necessary documentation and qualify for charity according to the charity care policy, (defined in Part I, line 3c), patients' account would be written off completely to charity and not billed to the patients.
PART VI LINE 1 - PART V, SECTION B, LINE 19DSt. Anthony's Hospital applies a discount of 40% and offers up to a 50% discount of charges for individuals who do not have insurance covering emergency or other medically necessary care. This discount or allowance is consistent with the average blended inpatient/outpatient rate of BayCare's commercial payer's contractual allowance rate.
PART VI LINE 2St. Anthony's Hospital is committed to meeting the needs of the community it serves. Our quality philosophy is modeled around understanding our customers' needs in the communities it serves. St. Anthony's Hospital develops community health status assessments by accessing existing third party databases profiling health status information for geographies it serves. The assessments provide a profile of health status indicators in comparison to state averages and, if available, national benchmarks. In addition, St. Anthony's Hospital conducts physician community need studies that outline physician deficits by specialty for the geographic area served. Studies are also conducted to identify gaps in geographic access to services such as primary care, outpatient services and inpatient services. All of the above processes occur on an ongoing basis to assist St. Anthony's Hospital in developing initiatives and programs/services to address identified health care needs in the communities it serves.
PART VI LINE 3St. Anthony's Hospital Medical Assistance team members are dedicated to assisting patients in obtaining assistance through federal, state and local government programs or through the St. Anthony's Hospital charity care policy. Signage and brochures are available, as well as team members whose full responsibility is to assist patients in the emergency room and on inpatient units. The Medical Assistance team interviews patients for all available programs, assists the patients in completing applications to government agencies and for hospital charity care, advises patients regarding available community resources for health care, reviews and approves patient requests for charity care, and provides education and support to the patient throughout the assistance process. In addition to the aforementioned comprehensive process, St. Anthony's Hospital also informs and educates patients who may be billed for patient care, but may be eligible for charity or other programs, via patient billing mailers and customer service representative calls. The goal in using these various means is to effectively communicate with the entire patient population so they are informed and educated about their eligibility for assistance.
PART VI LINE 4St. Anthony's Hospital is in a small city setting serving southern Pinellas county. The average income is lower than both the state and national averages. The population served is predominantly Caucasian and high-school or higher educated. Blacks are the second largest ethnic group representing 15% of the population and 14% of households are below poverty level. St. Anthony's Hospital is part of BayCare Health System that serves west central Florida. The area served by St. Anthony's has 7 hospitals with more For Profits than Not-for Profits. There are 2 federally designated medically underserved areas in St. Anthony's service area. Due to the aging of the population with little to no growth for the under 55 segment in the next five years, the health care needs of our service area are changing. The population served by St. Anthony's Hospital is expected to grow 0.1% in the next 5 years. The expected growth rate for the United States is 4.1%. While overall population growth is miniscule, the population Age 55+ is expected to grow 10%. The community experiences higher incidence rates compared to the state of Florida in the following areas: adults with asthma, HIV cases, AIDs cases, fully immunized kindergarteners and sexually transmitted diseases. Based on Florida inpatient discharge data for the period of 07/01/09-06/30/10, the payor mix for the geographic area consists of 51.3% Medicare/Medicare HMO, 15.5% Medicaid/Medicaid HMO, 12.1% Commercial Insurance, 10.4% HMO/PPO, 6.3% Self-pay, and 4.4% Other.
PART VI LINE 5St. Anthony's Hospital was founded in 1931 by the Franciscan Sisters of Allegany with a mission to serve and to build a hospital evoking the Franciscan sisters' faith and mission of treating everyone with the utmost kindness and consideration, regardless of circumstances. Through faith dedication and compassionate care, St. Anthony's has grown to become an established community resource. The mission is evident as 1,457 team members and nearly 500 highly qualified physicians work daily to achieve St. Anthony's mission of improving the health of the community. 2010 Overview In 2010, St. Anthony's Hospital prepared to continue its legacy of serving all in the community with care and compassion and celebrated many achievements. Construction Begins on New Emergency Center and Patient Care Tower St. Anthony's Hospital began construction on a new approximately 100,000 sq.-ft. Emergency Center and Patient Care Tower. The project is planned to include 32 private patient exam rooms; two critical cardiac care exam rooms; expedited triage and admissions; advanced CT Scan, digital imaging and point of care lab technologies; and a spacious waiting room with comfortable seating and wireless connectivity. Franciscan Sisters of Allegany Move Into New Convent In preparation for the new emergency center, St. Anthony's relocated and built the new St. Anthony of Padua Convent. With new convent, the Franciscan Sisters of Allegany now have a modern and updated home that will shelter both body and spirit. Named for the hospital's patron saint, the convent is a single story home with a chapel and meditation garden and common areas for prayer, dining and recreation. Nationally Recognized Breast Cancer Prevention and Care The Susan Sheppard McGillicuddy Breast Center at St. Anthony's Hospital received two important designations in 2010. The breast center was honored with accreditation from the National Accreditation Program for Breast Centers (NAPBC), a highly regarded national program. This accreditation comes just after receiving the Breast Center of Excellence designation from the American College of Radiology (ACR). The Susan Sheppard McGillicuddy Breast Center is now the only facility in St. Petersburg, Fla. with both the ACR designation and the NAPBC accreditation. St. Anthony's Triathlon Affiliates with Global Race Series The 27th year of the St. Anthony's Triathlon was a huge success with thousands of athletes swimming, biking and running along scenic areas of St. Petersburg. It also was announced that the 2011 Triathlon would become part of the Ironman tradition by joining a new global event series titled 5150 launched by the World Triathlon Corporation (WTC), producer of the Ironman triathlon events. ENERGY STAR Ranking Earned for Second Year For the second year in a row, St. Anthony's Hospital earned the U.S. Environmental Protection Agency's prestigious ENERGY STAR, the national symbol for protecting the environment through superior energy efficiency. This signifies that the hospital's buildings perform in the top 25 percent of similar facilities nationwide for energy efficiency. Commercial buildings that earn the ENERGY STAR use an average of 35 percent less energy than typical buildings and also release 35 percent less carbon dioxide into the atmosphere. Electronic Medical Record Phase II As part of BayCare Health System's multi-year, system-wide electronic medical record initiative, St. Anthony's Hospital went live with phase II, forever changing how clinicians document patient information. Today, entering patient information on the computer is the only way to perform many of the daily functions currently documented on paper charts. Community Outreach & Partnerships St. Anthony's Hospital has had a tradition of caring for people from all walks of life and has been at the forefront of community outreach for generations. This includes not only the care given within the walls of the hospital, but also in the community itself. The Mission Integration / Community Affairs Committee, is a committee of the St. Anthony's Hospital Board of Trustees, and made up of community members including representatives of diverse community organizations including a neighborhood association, the police department, Salvation Army, Catholic Charities, senior citizens, Family Services, St. Petersburg Free Clinic, physicians, school principal, and other businesses and social community leaders. The committee's aim is to keep St. Anthony's aware of community needs and facilitate collaboration to meet those needs. St. Anthony's leaders were ahead of their time when they established an entire department to promote good health in the community more than two decades ago. Through prevention and early detection of disease, we improve the quality of life and reduce cost and suffering due to preventable illness. STEPS to a Healthier Pinellas is an example of an organization that benefits from St. Anthony's outreach programs. St. Anthony's is proud to work with the Nina Harris Exceptional Student Education Center, which is dedicated to providing a safe, nurturing environment where special needs students can build self-confidence, independence and self-esteem to maximize their success at home, school and in the community. Students from the school work at the hospital in the areas of food and nutrition and environmental services to help with learning job skills. St. Petersburg Free Clinic St. Anthony's Hospital is committed to supporting community organizations such as the St. Petersburg Free Clinic. The St. Petersburg Free Clinic serves as "safety net" providing health care for the uninsured and underinsured in St. Petersburg. In 2010, St. Anthony's provided laboratory and basic imaging services for clinic patients. St. Anthony's Hospital actively recruits volunteer doctors, nurses, physical therapists and occupational therapists, to provide free medical care for the St. Petersburg Free Clinic. The hospital also held a food drive to collect donations for the clinic's food bank which serves the greater community. Health Fairs In 2010, St. Anthony's Hospital participated in seven health fairs which were held at a variety of locations, including Carillon Wellness Center and St. Anthony's Hospital, Catalina Marketing Health Fair, City of St. Petersburg Health Fair, Family Health Lecture, General Dynamics Health Fair, Homeless Health Fair and Screening, Tropicana Field and Tampa Bay Start! Heart Walk. Health Screenings St. Anthony's Hospital screened community members on many diseases and conditions, including diabetes, heart disease, hypertension, and stroke. The health screenings were conducted at many venues, such as schools, churches and at hospital-sponsored events. St. Anthony's Hospital pays for these services so patients will not be charged. In 2010, the total amount paid for was $12,377. Community Lectures With our mission to improve the health of the communities that we serve, St. Anthony's Hospital held several free community lectures, educating the community on a variety of health-related topics including breast cancer awareness, diabetes, heart health, nutrition and weight loss, orthopedics, senior health, sports safety and training, stroke and cardiac risk assessment. Support Groups St. Anthony's Hospital provides support groups to the community on a number of topics ranging from cancer to stroke care. In 2010, we had more than eight active support groups throughout Pinellas County, including Amyotrophic Lateral Sclerosis, Primary Lateral Sclerosis, Bariatric Surgery Support Group; and Hereditary Spastic Paraplegia Support Group; Brain Tumor Support Group; Continent Ostomy Support Group; Diabetes Support Group; Headache Support Group; Multiple Sclerosis Support Group and Stroke Support Group. In-Kind Donations of Meeting Space St. Anthony's Hospital provides space for community groups to hold meetings and events. The organizations supported include: Alcoholics Anonymous, Boys and Girls Clubs of the Suncoast, Foster Grandparents, Fighting Blindness Foundation, Lupus Foundation, Relay for Life, Seniors and Services, Gulfcoast Chapter of Medical Assistance, Susan G. Komen for the Cure, Multiple Sclerosis Support Group, Overeaters Anonymous, Myasthenia Gravis Support Group, Brain Tumor Support Group, the Junior League and Tampa Bay Healthcare Collaborative. One important connection with the community is that St. Anthony's is an active member of the Historic Uptown Neighborhood Association, enabling them to carry out many of their activities and meetings. The availability of free space enables these organizations to conduct meetings without the cost of renting a venue. Faith Community Nursing Faith Community Nursing (FCN) offers a unique partnership between two healing groups - St. Anthony's Hospital and churches in Pinellas County. This nursing subspecialty takes a whole person approach to health care, promoting wellness of body, mind and spirit within the context of the va
PART VI LINE 6St. Anthony's Hospital is part of BayCare Health System, a leading community-based health care system in the Tampa Bay area with 10 not-for-profit hospitals, 70 outpatient facilities, more than 200 access sites and 18,500 employees. Together, BayCare organizations provide a lifetime of expert medical care by connecting patients to a complete range of services, such as primary care, imaging, lab, behavioral health and home health care. BayCare's hospitals are Mease Countryside, Mease Dunedin, Morton Plant, Morton Plant North Bay, St. Anthony's, South Florida Baptist and St. Joseph's Hospitals. St. Joseph's Hospitals operate as one entity and files one Form 990, however it includes several distinct hospitals: St. Joseph's, St. Joseph's Children's, St. Joseph's Women's and St. Joseph's Hospital-North. BayCare was founded in 1997 when several of the area's not-for-profit hospitals came together united by a common Mission to improve the health of their communities through high-quality, community-based health care. With BayCare, these hospitals created a unique model for a successful organization. Since those ingrained in the community can best determine their population's needs, BayCare's hospitals are in three geographic regions with their own local boards. BayCare provides organizational and financial strength for the hospitals and has centralized certain services regionally such as finance, human resources, home care and behavioral health. This enables each hospital to remain focused on their shared Mission of serving the health care needs of the entire community, regardless of their ability to pay. Each BayCare hospital provides charity care and unbilled community benefit services too. This is an essential expression of BayCare's shared Mission. In 2010, BayCare provided $184.5 million in community benefits, which includes $80 million in traditional charity care, $11 million in unbilled community services and $93 million in unreimbursed costs of Medicaid and county welfare programs. As one of the largest employers in the Tampa Bay area, BayCare has also played a significant role in the creation of jobs in the Tampa Bay community. Even though unemployment has increased, BayCare has brought on more than 1,650 employees since the end of 2006. BayCare's comprehensive network provides patients with increased access to services, such as emergency care, inpatient, outpatient and home health. For example, BayCare HomeCare helps patients leave the hospital sooner and recover at home where they are more comfortable. In 2010, HomeCare provided approximately 599,000 home health visits to more than 50,000 patients through 11 offices located throughout Florida. Recognizing the need for mental health services, BayCare is the largest, not-for-profit provider of behavioral health services in west central Florida. BayCare Behavioral Health offers inpatient, outpatient, in-home, residential treatment, support group and case management services. BayCare's designated Baker Act-receiving facilities in Hillsborough, Pinellas and Pasco counties provide inpatient crisis stabilization and partial hospitalization programs as part of the continuum of mental health care. BayCare also is the only organization in the Tampa Bay area that operates dedicated psychiatric emergency rooms at each of its hospital-based, Baker Act facilities. In 2010, BayCare Behavioral Health opened the Morton Plant North Bay Recovery Center, a 72-bed acute care, behavioral health facility to treat adults, children and adolescents. It is the only hospital-based children's mental health facility in Pasco County. BayCare hospitals also benefit from faster technological advancements like the system-wide introduction of an electronic medical record (EMR) and a biometric patient identity program that uses cutting-edge, palm scanning technology. Both of these projects provide more comprehensive access to patient records with the goal of improving patient safety. BayCare has registered more than 1.5 million patients through its biometric, palm-scanning registration and added more than 2 million patient charts to its EMR repository. Because of these two systems, a patient's medical record is now available at any BayCare hospital or facility. With the scan of a palm, BayCare's facilities will be able access the patient's data and avoid duplicate information requests at different times in the care process. An EMR also provides real-time communication and care-team coordination that can lead to a more complete picture of the patient's needs and better outcomes. In an effort to offer greater communications, increased patient safety and continuous care, BayCare is also working to electronically connect our hospitals with physicians' offices through an electronic medical record. In addition, each facility can gain knowledge from best practices shared by other BayCare hospitals through a system-wide Quality program that helps drive higher performance to improve patient care, processes and technology. In 2010, every BayCare hospital participated in the Fifth Annual Quality Sharing Day, an event designed to help employees share and learn best practices that were implemented elsewhere in the health system. As part of the organization's commitment to the community, BayCare partners with Hillsborough Community College, University of Tampa, University of South Florida, Erwin Technical Center and St. Petersburg College to help educate future nurses and health care professionals. In addition, BayCare is an organizational sponsor for the United Way of Tampa Bay's Hands on Tampa Bay. The program provides volunteer support in a variety of service areas such as homelessness and hunger; education and literacy; children, youth, and family; seniors; animals and environment; and persons with disabilities. BayCare also supports organizations that work to foster economic development in the Tampa Bay area. These organizations include the Tampa Bay Partnership and the Pasco Economic Development Council.

For Board members, the review of conflicts or potential conflicts occurs at the Board or committee level. After disclosure of the Board Member's or Committee Member's actual or potential conflict, the following procedures for addressing the conflict of interest will be adhered to by each Board and all Committees with Board delegated powers, without exception: 1. Part VI and Part VIIForm 990, Part VI, Question 15a & 15b - Process used for Compensation Review and Approval The organization uses an independent compensation committee, appointed by the Board of Directors. The Compensation Committee's purpose is to provide oversight for the organization's executive compensation program, review and approve compensation and benefits for all "disqualified persons" subject to the Intermediate Sanctions regulations issued under Section 4958 of the Internal Revenue Code (including the Chief Executive Officer, Chief Administrative Officer & CFO, other system and entity executives, and other disqualified persons as defined in the Intermediate Sanctions regulations (i.e., voting members of the governing body, family members, former officers)), and establish the compensation philosophy for all other executives. This committee engages nationally recognized compensation consultants to assist them in review of executive compensation. The compensation consultants provide a review of each vice president and above in the system to determine if that employee's compensation is reasonable when compared against market standards. The data reviewed comes from compensation studies that include comparable compensation for similarly qualified persons in functionally comparable positions at similarly situated organizations. The organization keeps contemporaneous minutes of the compensation committees meetings and decisions. External consultants review compensation every other year, the last review occurring in 2009, but the compensation committee regularly monitors compensation and all other procedures are followed annually. Form 990, Part VI, Question 16b - Procedure to evaluate Joint Venture Arrangements The organization has a Joint Venture committee of subject matter experts who review potential arrangements with taxable joint ventures.
