Income Based CriteriaIf applicable, describe the income-based criteria for determining eligibility for free or discounted care under the organization's charity care policy. Also describe whether the organization uses the asset test or other threshold regardless of income to determine eligibility for free or discounted care. When Catholic Health Initiatives (the ultimate parent organization to St Anthony Hospital) established its charity care policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among the 69 urban and rural communities in 19 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines ("FPG"), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. St Anthony Hospital bases its charity care eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain charity care write-offs, based on a sliding scale, ranging from 25%-100% of charges.IncomeBasedCriteria
Annual Community Benefit ExplnIf the organization's community benefit report is contained in a report prepared by a related organization, rather than a separate report prepared by the organization, identify the related organization. St Anthony Hospital prepares its own annual written community benefit report. Its community benefit report is not contained in that of a related organization.AnnualCommunityBenefitExpln
Costing Method UsedProvide an explanation of the costing methodology used to calculate the amounts reported in the table. If a cost accounting system was used, indicate whether the cost accounting system addresses all patient segments (for example, inpatient, outpatient, emergency room, private insurance, Medicaid, Medicare, uninsured or self pay). Also indicate whether a cost-to-charge ratio was used for any of the figures reported in the table. Describe whether this cost-to-charge ratio was derived from Worksheet 2, and, if not, what kind of cost-to-charge ratio was used and how it was derived. If some other costing methodology was used besides a cost accounting system, cost-to-charge ratio, or a combination of the two, describe the method used. A cost accounting system was not used to compute amounts in the table; rather costs in the table were computed using the organization's cost-to-charge ratio. The cost-to-charge ratio covers all patient segments. The cost-to-charge ratio for the year ended 6/30/10 was computed using the following formula: Operating expense (before restructuring, impairment and other losses) divided by gross patient revenue. Based on that formula, $44,512,000/$86,901,000 results in a 51.2% cost-to-charge ratio. Worksheet 2 was not used to derive the cost-to-charge ratio.CostingMethodUsed
Percent Of Total Expense ExplnIf applicable, state the bad debt expense included on Form 990 Part IX, line 25, column (A) but subtracted for purposes of calculating the percentage in this column. Total bad debt expense reported on Form 990 Part IX, line 25, Column A was $3,228,997PercentOfTotalExpenseExpln
Bad Debt Footnote(A) Provide the rationale and the costing methodology used to determine the amount reported in Part III lines 2 and 3. Describe how the organization accounts for discounts and payments on patient accounts in determining bad debt expense. (B) Also describe the method the organization uses to determine the amount that reasonably could be attributable to patients who likely would qualify for financial assistance under the hospital's charity care policy, if sufficient information has been available to make a determination as to their eligibility. Also, provide if applicable, the text of the footnote to the organization's financial statements that describes bad debt expense. If the organization's financial statements include a footnote on these issues that also includes other information, report only the relevant portions of the footnote. If the organization's financial statements do not contain such a footnote, state that the organization's financial statements do not include such a footnote and explain how the financial statements account for bad debt, if at all. (A) Costing methodology for amounts reported on line 2 is determined using the organization's cost/charge ratio. When discounts are extended to self-pay patients, these patient account discounts are recorded as a reduction in revenue, not as bad debt expense. (B) St Anthony Hospital does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care. St Anthony Hospital does not issue separate company audited financial statements, however, the organization is included in the consolidated financial statement of catholic health initiatives. The consolidated footnote reads as follows: "The provision for bad debts is based upon management's assessment of historical and expected net collections considering historical business and economic conditions, trends in health care coverage, and other collection indicators. Management periodically assesses the adequacy of the allowances for uncollectible accounts based upon historical write-off experience by payer category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility".BadDebtFootnote
Shortfall As Community BenefitPart III, Section B, Line 8 (A) Describe the costing methodology used to determine the Medicare allowable costs reported in the organization's Medicare Cost Report, as reflected in the amount reported in Part III, line 6. (B) Describe, if applicable, the extent to which any shortfall reported in Part III, line 7 should be treated as a community benefit, and the rationale for the organization's position. (A)St. Anthony Hospital is designated as a Critical Access Hospital ("CAH"). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (CoP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, and St. Anthony Hospital, due to its designation as a CAH, received cost-based reimbursement for Medicare purposes, St. Anthony Hospital will not experience Medicare related shortfalls. (B)However St. Anthony Hospital and its affiliates, believe that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays.ShortfallAsCommunityBenefit
Debt Collection PolicyIf the organization has a written debt collection policy and answered "yes" to Part III, line 9b, describe the collection practices set forth in the policy that apply to patients who it knows qualify for charity care or financial assistance, whether or not such practices apply specifically to such patients or more broadly to also cover other types of patients. St Anthony Hospital's debt collection policy provides that St Anthony Hospital will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through St Anthony Hospital community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet the St Anthony Hospital community assistance policy is required to be returned immediately by the third-party collection agent to St Anthony Hospital for appropriate follow-up. St Anthony Hospital requires its third-party collection agents to include a message on all statements indicating that if a patient or patient guarantor meets certain stipulated income requirements, the patient or patient guarantor may be eligible for financial assistance. All of Catholic Health Initiatives "hospitals" contracts with third party collection agencies include the following standards: * Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; * Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and * No Catholic Health Initiatives' collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect.DebtCollectionPolicy
Needs AssessmentDescribe how the organization assesses the health care needs of the communities it serves. Describe the community the organization serves, taking into account the geographic area and demographic constituents it serves. Describe how the organization's community building activities, as reported in Part II, promote the health of the communities the organization serves. Provide any other information important to describing how the organization's hospitals or other health care facilities further its exempt purpose by promoting health of the community (e.g. open medical staff, community board, use of surplus funds, etc). Community Benefit Annual Report FY 2010 Summary Introduction St. Anthony Hospital in 1902 was founded by the Sisters of St. Francis to provide healthcare to the rural farming and ranching community of Pendleton in Eastern Oregon. St. Anthony Hospital has an open medical staff with privileges available to all qualified physicians in the area. The St. Anthony Hospital ("SAH") Board of Trustees is made up of a majority of independent persons representative of the community. SAH continues to engage in the training and education of health care professionals who provide service to this community In 1997, St. Anthony Hospital joined Catholic Health Initiatives as a market-based organization. This was a blending of the health care ministries of multiple congregations of women religious. While each congregation had carried out the health care ministry of Jesus for many decades, they did so in different locations and in accordance with different traditions. Still, they shared a commitment to building healthy communities in which everyone, especially the poor and vulnerable, would have access to services needed for healthy minds, bodies and spirits. For this reason, SAH is committed to serve all persons in the community on a non-discriminatory basis. SAH operates an emergency room that is open to all persons regardless of ability to pay. In forming Catholic Health Initiatives, all of the founding congregations embraced a commitment to building healthy communities. Catholic Health Initiatives' mission is: To nurture the healing ministry of the Church by bringing it new life, energy, and viability in the 21st century. Fidelity to the Gospel urges us to emphasize human dignity and social justice as we move toward the creation of healthier communities for persons of all ages. St. Anthony Hospital is a qualified Critical Access Hospital. In addition, SAH provides services through a federally qualified rural health clinic, called the Urgent Care Center, and Home Health & Hospice programs. In keeping with our mission, SAH provides care for all regardless of ability to pay. Last year SAH gave $2 million to underprivileged individuals and the broader community in charity care, community education, outreach and unpaid costs of Medicaid. Quality healthcare is delivered in a service area of over 116,000 residents in Umatilla, Union, Morrow and Grant counties. This consists of rural, primarily agricultural communities, in Northeastern Oregon. The vast majority of the population is Caucasian, with Hispanics making up most of the minority population at around 20%. Pendleton is the largest city in the area with approximately 17,000 people, followed by Hermiston with a population of approximately 14,000 and La Grande with approximately 11,000 residents. All towns in the four county area have 6,000 or less residents. Community Benefit Approach Multiple Avenues Leading to Healthy Communities Catholic Health Initiatives has created multiple avenues that all lead toward healthy communities. They include: * New and continuing community benefit activities * Support for international health missions * Community Health Services Organizations * Grants for projects and long-term partnerships that promote community health * Investments in organizations that serve disadvantaged populations Thirteen years or a century from now, the process of building healthy communities may be completely different than it is today; however, it will always be an emphasis of Catholic Health Initiatives. New and Continuing Community Benefit Activities Every Catholic Health Initiatives market-based organization is committed to community benefit, which can be defined as the provision of health services and compassionate care to the poor and vulnerable as well as the broader community. In addition to providing uncompensated care to those in need, these activities include wellness programs; community education programs; and special programs for vulnerable populations such as children, the elderly, the disabled and the medically underserved. Assessing Health Care Needs St. Anthony Hospital draws from several sources to identity the health care needs in the community. These include the Oregon Housing and Community Service Poverty Report, the Oregon Healthy Teens Report, Healthy Aging in Oregon Counties Report, Keeping Oregonians Healthy Report, County Health Rankings, Umatilla County Health Department and Head Start. In July, 2008 St. Anthony Hospital formed a Community Health Improvement Coalition in order to: * assess the health needs of the community, * identify strengths and areas of improvement regarding existing health services, * develop a plan to address gaps, * identify key community organizations and players to respond to the gaps, and * Establish on-going relationships with community partners. The assessment process utilized both a questionnaire and focus group format in order to include individuals within the community, as well as all hospital employees and Board members. The data was combined with reports generated from county, state and independent health assessments pertaining to the broader Pendleton community served by St. Anthony Hospital. The following were the top identified needs of the community: -Shortage of physicians/Access to Healthcare -Child/Infant care -Affordable housing/Homelessness -Employment opportunities -Affordable healthcare -Health education -Mental health issues/addiction issues -Domestic violence -Prevention education: Smoking & Obesity -Palliative & End of Life Care QUALITATIVE DESCRIPTION OF COMMUNITY BENEFIT Community Outreach for the Poor Pharmacy Indigent Care Program SAH provides prescription medications to persons who do not have the means to purchase them. Provisions of these medications helps the recipients to recover more quickly from their illnesses, better manage chronic conditions, and avoid costly hospitalizations. In fiscal year 2010, prescription drugs valued at $6,222.63 were provided to 224 individuals. Care Rides SAH collaborates with the City of Pendleton by paying $2,000 for one quarter of the cost of transportation for low-income patients in the community. This helps ensure the community has easy access to health care. Without this assistance, over 1,500 rides could not have been provided. This services helps patients timely address their needs, decreasing the cost of care for all. Salvation Army Meals SAH helps provide weekly meals at the Salvation Army, with over 5,541 meals served to the underprivileged last year, at a cost of $142,701. St. Anthony Hospital has faithfully provided this service to those in need in our community for many years. This year SAH went even further to make sure this service was not interrupted when the Salvation Army facilities. where these services were normally provided. had to shut down operations for a number of months because of building code safety issues. During this time the hospital opened its doors and provided the meal on site; keeping with its mission to serve the poor. Tonya's House Tonya's House provides support to homeless teenage girls. They provide food, shelter, clothing, mental health, tutoring, transportation to appointments, and health care. SAH supports this service through donations and volunteer hours valued at $4,593. These girls would have nowhere else in the community to go if this service was not provided. Many of the girls come from unhealthy home environments where their lives are at risk because of domestic violence and/or substance abuse. Over 600 girls' needs were served. Pioneer Relief Nursery SAH provides supplies, utilities and free space for Pioneer Relief Nursery ("PRN"). The nursery provides respite childcare and training to parents who have challenges with childcare. The goal is to prevent child abuse and neglect in Umatilla County through intensive family support. PRN addresses the cycle of child abuse and neglect by providing therapeutic early childhood classrooms, parenting education, planned respite and comprehensive family support. PRN utilizes research-based strategies to help families build skills to cope with the stress of raising children in families with risk factors such as poverty, substance abuse, domestic violence and isolation. This last year this program served 156 families and 350 children. Only 2% of the children served by PRN reportedNeedsAssessment
Patient Education AssistanceDescribe how the organization informs and educates patients and persons who may be billed for patient care about their eligibility for assistance under federal, state, or local government programs or under the organization's charity care policy. Patients who qualify for charity care discounts shall be identified as soon as possible, either before services are provided or after an individual has received services to stabilize a medical condition. If it is difficult to determine a patient's eligibility for a charity care discount prior to the provision of services, such determination shall be made as soon as possible but shall not exceed a period of 18 months after the provision of such services. St Anthony Hospital offers financial assistance on all services provided to all patients if the service is deemed a medical necessity. Medical necessity is defined as any procedure reasonably determined to prevent, diagnose, correct, cure, alleviate, or prevent the worsening of conditions that endanger life, cause suffering or pain, resulting in illness or infirmity, threatening to cause or aggravate a handicap, or cause physical deformity or malfunction, if there is no other equally effective, more conservative or less costly course of treatment available. St Anthony Hospital will follow the hospital's admission criteria to determine the patient's medical necessity for patients who apply for charity discounts. Patients receiving emergency services shall be treated in accordance with the hospital's emergency services policy, developed in accordance with EMTALA and other requirements. Reasonable registration processes shall include asking whether an individual is insured and, if so, the name of the insurance program, if such inquiry does not delay screening or treatment. The registration process shall not discourage patients, from remaining for further evaluation. Financial issues will only be discussed after the patient has been screened and stabilized. Once EMTALA requirements are met, patients identified through the registration process as being without Medicare/Medicaid, other local health care financial assistance or adequate health insurance shall receive a packet of information that addresses the financial assistance policy and procedures, including an application for such assistance, or immediate financial counseling from staff including the presentation of the application for financial assistance. In addition to the financial packet, emergency inpatient admissions will be assisted by the admissions clerk in contacting the Oregon Health Plan. After the patient's information is submitted to the Oregon Health Plan, the patient or guarantor will receive an application for medical assistance. Even though OHP will not usually become retroactive if the patient and or guarantor qualify they would be eligible for future medical assistance through the OHP. The non-emergency patient scheduling an admission or other procedure shall receive a packet of information that addresses the financial assistance policy and procedures, including an application for such assistance, or financial counseling assistance from staff, including the application for financial assistance, if they are without Medicare/Medicaid, other local health care financial assistance or adequate health insurance. In both instances, the packet of information clearly indicates that St Anthony Hospital provides care, without regard to ability to pay, to individuals with limited financial resources, and explains how patients can apply for financial assistance. Assistance is available for patients not proficient in reading, writing or speaking English. St Anthony Hospital clearly posts signage in English to advise patients of the availability of financial assistance. Signs will be posted in other languages in instances where 10% or more of the local population speaks a foreign language to be determined by Administration. Staff members shall communicate the contents of signs to people who do not appear able to read. St Anthony Hospital identifies the availability of financial assistance in information booklets provided to patients and in general information provided on the St Anthony Hospital website. Physician practices or clinics that are an integral part of St Anthony Hospital or its non-profit subsidiaries shall adopt this charity care policy. These organizations shall comply with the same charity care policy and procedures adopted by the St Anthony Hospital board of trustees for the tax-exempt healthcare provider.PatientEducationAssistance

Affiliated Health Care SystemIf the organization is part of an affiliated health care system, describe the respective roles of the organization and its affiliates in promoting the health of the communities served. St. Anthony Hospital, along with its affiliated outpatient facilities are part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Englewood, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 73 hospitals; 40 long-term care, assisted- and residential-living facilities; and two community health-services organizations. Together, these facilities provided $590 million in charity care and community benefit in the 2010 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain - provides economies of scale and purchasing power to the MBOs. Cost savings associated with centralized services for the fiscal year ended June 30, 2010 include more than $105 million in supply chain expense, approximately $30 million for insurance costs and more than $10 million as the result of a system-wide effort to coordinate and negotiate technology and equipment purchases. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society.AffiliatedHealthCareSystem
