DescriptionCarle Foundation Hospital is an Illinois not-for-profit Corporation established for the purpose of operating the Carle Foundation's 305-licensed bed hospital and related facilities and, conducting patient care related activities of the Foundation. The hospital, a level one trauma center, offers a full range of inpatient and outpatient diagnostic and therapeutic services for medical, surgical, obstetrical and gynecological patients. The hospital provides various ancillary support services to both inpatients and outpatients such as autopsy, blood bank, emergency services, respiratory therapy, operating room, occupational therapy, pastoral counseling, social services, pharmacy and physical therapy. Carle Foundation Hospital's mission is to serve people through high quality care, medical research and education. We make sure those who need care, get care, whether or not they can pay. Through our Community Care Discount Program for hospital services and our partnerships with area health agencies, access to dental, vision and general healthcare has been improved for those who are struggling financially. As the second tenet in our mission, research gives us tomorrow's answers, today. In collaboration with the University of Illinois scientists and the Carle Physician Group physicians, we are turning the latest medical knowledge into practical applications for the bedside. We are now conducting more than 99 active studies relating to cancer, gastro-intestinal disease, cardiovascular disease and more. Educational programs for both our professionals and the community ensure everyone is better prepared. For the community, we offer a myriad of opportunities like babysitting certification and new dad courses, along with free CPR and first aid classes. For our professionals, we have continuing education courses, three levels of management training classes and courses for physician leaders. Carle Foundation Hospital's continued success is rooted in our ability to adapt to a changing healthcare environment, plan diligently for our future and maintain a commitment to our patients and community. Not-for-Profit hospitals have a long standing practice of providing support, donations and services to their communities. This stems not from economic opportunity, but rather from a responsibility to address and improve the health of all the people they serve. At Carle Foundation Hospital, we also believe that we have an obligation to generously give back to our community in exchange for our tax exemption. All of these factors lead to Carle Foundation Hospital being a champion of access to healthcare and a pillar of community support. In FY10, the Carle Foundation's total community benefit was $63,377,550. Community benefit programs are based on assessed needs and designed to improve the health of those who live and work in our area, with a focus on serving those less fortunate. Areas of Community Benefit include: hospital services that operate at a loss, cash and in-kind donations, translational research, community education, community education, community-building activities and workforce development. Another role of the Community Benefit is to step in where government falls short. As State and Federal budgets get tighter, funding for important health-related services rarely cover costs and, at a minimum, is usually slow in coming. That is most clearly evidenced by the hospital's unpaid costs of Medicare and Medicaid in the amount of $27,807,505 included in the Community Benefit total above. In addressing what many call an epidemic, Carle Foundation Hospital strives to eliminate barriers that keep area residents from receiving the care they need. Carle Foundation Hospital's Community Care Discount Program is one of the most generous charity care discount programs in the country. We want to make sure that the people who qualify know about their options. Our advertising throughout the region is targeted to reach people who qualify so they are aware financial assistance is available. As a not-for-profit hospital, it is our obligation to reach out to those who cannot afford healthcare.Description

DescriptionThe Carle Community Care Discount Program (Charity Care) - As a tax exempt organization, Carle Foundation Hospital provides care to patients regardless of their ability to pay for that care or source of payment. We also recognize that some patients need help to pay their hospital bills. As a locally owned community hospital, we always provide the care first and then help with the financial challenges. Carle Foundation Hospital's Community Care Discount Program (Charity Care) provides discounts or free care to those who need it. The most current policy was re-written with the integration with Carle Clinic and became effective April 2010. An additional policy reflects our compliance with the Illinois Hospital Uninsured Patient Discount Act. Our Community Care Discount Program is continually evaluated and expanded as needed to meet the needs of our community. Evaluation involves input from administrative leaders, billing office staff, local consumer advocacy groups and patients. In FY10, charity care totaled $9,025,099 and served 3,569 people.Description

DescriptionResearch and Education Initiatives - Carle Foundation Hospital is actively involved in Research and Education Initiatives. The overall purpose of the Research Program is helping to discover new diagnostic and treatment technologies and methods that will improve the delivery and/or quality of healthcare. The Research Program is done in conjunction with area physicians and scientists, The University of Illinois and entrepreneurial companies. Carle Foundation Hospital's efforts in research and education have steadily grown in recent years with strategic emphasis now placed on Translational Research with The University of Illinois. As of June 2010, Carle Research Institute had 99 active, with another 20 pending, research studies underway. Of those, an additional 58 active and four pending studies were being conducted jointly by Carle physicians and University of Illinois scientists. Carle Research Institute projects are related to cancer, cardiology, gastroenterology, neuroscience and more. Also a significant part of research, the Carle Tissue Repository (CTR) collected 250 specimens in 2010 since receiving its first tissue donation last year. Joining forces with Carle pathology facilitated more stringent collection and processing procedures which allows a more robust end-product for research. The tissue repository provides human tissue obtained with consent as part of usual care. Specimens are stored securely adhering to strict rules that protect donor privacy and ensures the appropriateness of the research for which it will be used. These specimens are valuable resources for medical research that may benefit future patient treatments and outcomes. While not included in Carle Foundation Hospital Community Benefit totals, it is important to note that research has also been conducted for many years at the Carle Cancer Center by Carle Clinic physicians, now Carle Physician Group, following integration with the Hospital in April 2010. The legacy Carle Clinic began participating in clinical trials in 1978 and trials will continue to be provided to the community. Carle Cancer Center became a member of the Community Clinical Oncology Program (CCOP) in 1983. Founded by the National Cancer Institute the same year, CCOP is a network for conducting cancer prevention and treatment clinical trials by community medical practitioners. This network connects academic centers with community physicians who accrue patients to those trials. In fiscal year 2010, Carle Cancer Center participated in 205 active government-sponsored clinical trials. 1,210 patients, including 24 new accruals, were involved in trials receiving treatment, active follow-up or long-term follow-up. Also during the past year, Carle physicians have participated in 12 non-oncologic clinical studies in the areas of Gastroenterology, Urology, Cardiology and Plastic Surgery. Eighty-three patients, including 19 new accruals, were involved in trials receiving treatment, active follow-up or long-term follow-up. The studies are a mix of privately funded and standard care billed; however, additional costs not covered are at a loss to Carle. Upon completion, and FDA approval, medical drug and device companies will take beneficial findings to market to benefit the public. Besides patient and community education, significant resources are expended on the education and training of medical students, physician residents, nurses, allied health professionals and the general healthcare workforce. Through a variety of activities including significant donations, scholarship programs, and physician, nurse, and allied health education, more than $3.2 million was invested in programs that address community-wide workforce issues, strengthening the training and availability of professionals to care for our healthcare needs now and in the near and distant future. * Carle Scholars at Parkland College. Scholarships and staffing support for nurse education and nursing student recruitment, as well as scholarships for allied health professions. * Graduate Medical Education programs. Maintaining a geriatric fellowship and three medical residency programs, and serving as a clinical site for a fourth. There are 40-45 residents practicing on Carle's campus at any point in time. * Continuing Medical Education programs for regional providers who are not members of the Carle Foundation Hospital medical staff. This includes Carle Foundation Day, which is an event for providers held at the Forum. There are multiple presentations, speakers and displays which count toward required continuing medical education credit for physicians in the region who attend. * Support of the University of Illinois College of Medicine at Urbana-Champaign, with monies earmarked for the MD/PhD program.Description

FIN 48 Disclosure The Foundation adopted the guidance for accounting for uncertainty in income taxes on July 1, 2007. This guidance clarifies the accounting for uncertainty in income taxes recognized in an organization's financial statement. The guidance prescribes a more-likely-than-not recognition threshold and measurement attribute for financial statement recognition of a tax position taken or expected to be taken. Amounts requiring recognition under tax accounting guidance is reflected as a liability for uncertain tax benefits along with any associated interest and penalites that would be payable to the taxing authorities upon examination. There are no uncertain tax benefits identified or recorded as a liability as of June 30, 2010 and 2009.Explanation

Costing Method UsedTo compute and convert charity care, unreimbursed Medicaid, means-tested programs, and bad debts charges to cost for Part III, a consistent GAAP (Generally Accepted Accounting Principles) based cost-to-charge ratio was used across all payers. Although the methodology was similar to Worksheet 2, for simplicity purposes certain immaterial values were omitted. Other Benefits cost were reported at the actual expense incurred. Patients' receivable payments were recorded at time of receipt and related discounts were also recorded at actual amounts at the time of payment receipt. A separate GAAP based provision for estimated bad debts and discounts was recognized for accounts in process and pending adjudication and payment. The estimated portion was based on historical trends and adjusts to actual in future periods when the accounts' adjudication and payment occur. Accounts determined to be eligible for charity care were processed immediately for charity care discount with no collection effort. For accounts with insufficient information and documentation to determine charity care eligibility, the hospital consulted with a variety of alternative sources to help determine an individuals' financial means (or lack of means) to pay. Based on related trends, the hospital further developed a general estimate of charity care which continued to reside within bad debts.CostingMethodUsed

Shortfall As Community BenefitThe numerator(total expense) and denominator (total gross charges) of the simple ratio of patient care cost to charges is adjusted by eliminating non-patient care that generates other revenue, bad debt expense, Medicaid and other provider taxes and the total cost of community benefit activities and programs. Also, any gross patient charges for programs not relying on the ratio are eliminated from both the numerator and denominator of the ratio. These adjustments are designed to eliminate "double counting" of community benefit expenses. The resultant ratio aligns with Schedule H requirements. While the Medicare shortfall is not included in the form 990 community benefit reportable number, the cost of unreimbursed Medicare charges for FY10 was $27,345,930 (calculated using the stated method). As an Illinois-based hospital, we are required to report community benefits provided to the Office of the Attorney General by the Community Benefit Act of 2003. For that report, Illinois law defines government-sponsored indigent health care as the unreimbursed cost of Medicare, Medicaid, and other Federal, State or local indigent care programs. To reduce public confusion caused by variations in our reports, for consistency, we have included the Medicare shortfall in all of our community benefit-related reports. However, we do believe this is a community benefit because, as a hospital, we are stepping up to carry the burden of the government, assuring care to seniors , where 10.1% are 65 years of age and older in Champaign County alone. According to Family Service of Champaign County, in 2000, 8% of those 60 and older were poor-at or below the poverty level. Those 85 and older--the fastest growing demographic in the county-have experienced increasing costs over the past decade while living on fixed incomes determined 20 years ago. 38.2% of Carle Foundation Hospital's patients in FY 2010 were Medicare recipients.ShortfallAsCommunityBenefit
Debt Collection PolicyIt is the responsibility of our patients to request and complete a CCDP application. They are informed about Community Care on multiple occasions throughout the collection process. If a patient does not request or complete and return the application, then the balance is deemed their responsibility to pay. We currently do not presume eligibility except for traditional Medicaid and Homeless patients (based on current area shelters or Social Worker verification). Once all applicable discounts have been applied we make every attempt to work with the patient to set up payment arrangements on the remaining balance due. The current minimum is 5% or $25.00 a month. Out of pocket medical expenses are limited to 40% of the patient's annual gross income if the patient is uninsured or underinsured and earns at or below 400% of the FPL. If they cannot meet these guidelines our in-house collectors work with them to set up a temporary/short term payment arrangement, until they can make the minimum arrangement. If they are unable to make payment on the remaining balance after the CCDP discounts, then the balance can be listed with the collection agency. When the account is still in-house the minimum notifications are 2 collection letters, 2 phone calls and monthly itemized statements. If a patient does not respond to us then the account is final noticed (additional letter) and sent to the agency. Carle will not file collection suit liens on a primary residence nor do we allow the collection attorneys to use "body attachments". The agencies are aware of our Community Care Discount program (CCDP) and have been advised to refer the patients back to Carle if they deem the patient is unable to pay. We will make one more attempt to work with the patients to see if help is available. If Carle determines that the patient might qualify, we send the patient a CCDP application and put a hold on the account listed with the agency for as long as we have determined necessary, the average is 60 days. If the patient is then approved for CCDP the balance is adjusted and then the account is closed. If they are approved for less than 100% the adjustments are reported to the agency and the balance is left with the agency for collection.DebtCollectionPolicy

Needs AssessmentCarle Foundation Hospital uses existing data, informal discussions, and community needs assessments to determine if existing programs are on track: what needs to be added, deleted or enhanced; and where our focus needs to be placed in the future. Data was drawn from these sources and used to affirm and re-shape our plan, as needed: . Community Needs Assessment, published in 2004 as a community effort coordinated by the Champaign County Regional Planning Commission. This continues to be the primary guiding resource for the United Way. . United Way Summer Summit, a full-day planning session to assess current need and to set priorities. . Current and future workforce shortage statistics . Current state-wide health reports . Homeland Security initiatives . Input from CU Public Health and human service agencies (on-going) . A Senior Needs Assessment Focus Group, commissioned by Carle Foundation Hospital in FY08. . Oral Health in Champaign-Urbana, IL, a review prepared by the Champaign-Urbana Public Health District. . A study by the Health Care Advisory Board requested by Carle Foundation Hospital to look at ways in which other communities were tackling the problem of providing dental services to older seniors. . Unanticipated opportunities or challenges in the local markets with respect to community health and wellness offerings. . Champaign County regularly conducts disaster preparedness committee meetings to plan for and practice community-wide disaster preparedness. Taking these sources into account, the Community Benefit Plan is aligned with the long-term strategic plan of the organization. Current focus areas include: . Care for uninsured, underinsured and indigent; . Partnering with community resources to provide access to inpatient and outpatient services; . Partnering with the University of Illinois in research, education and program offerings. . Further development of the Emergency Preparedness plan to meet Carle Foundation Hospital affiliates' and community needs. The Community Health and Wellness program, emergency preparedness efforts, and research initiatives are reviewed and confirmed through the annual budgeting process.NeedsAssessment
Patient Education AssistanceA generous Community Care Discount Program, consistently and diligently applied, has also resulted in our ability to reach more people. In FY10, charity care provided (at cost) was a remarkable $9,025,099, serving 3,569 individuals. There were even more people registered, having qualified for free or discounted care in advance, providing the comfort of knowing that their medical bills would be taken care of if they should need the care. We fully believe our ability to reach a significant number of people needing financial assistance is the cumulative benefit of our sustained commitment over the past seven years to work with local organizations and to communicate the availability of the program through a wide variety of channels. Carle Foundation Hospital has made a concerted, continuous effort to be sure that people have access to information that will help them with their hospital bills, even if they have not spoken with a member of the financial assistance team or a social worker. These include: Increased advertising expenditures for promoting awareness of the Community Care Discount Program. We continue the regular publication of newspaper ads in Champaign County, as well as in appropriate special event booklets. Other advertising includes bus poster ads, television and radio ads, web-based banner ads, and digital displays within the Hospital. Billing envelopes that carry the message: Need help with your hospital bill? Call 888-479-0008, prominently on the outside of the envelope. Community Care application forms available at all registration points, as well as in the Hospital's main lobby. These forms have also been translated into Spanish. In addition, at registration points, signs ask if patients need help with their medical bills and encourages them to let us know. Further more, they are informed about the Community Care Discount program on multiple occasions throughout the billing process. Every hospital patient-inpatient, outpatient and Emergency Department admission-is given the charity care/ discount information table and complete guidance on receiving help with their hospital bill in the Patient Registration & Admitting Information booklet (pg 16) upon registration. In this booklet, it also states that our financial assistance team can help the patient with alternative methods of coverage and setting up payment plans. This content reflects the hospital's Community Care Discount Program policy in consumer-friendly language. A simplified application form. Complete information and application forms on our Websites, www.carle.org and a newly launched website, mycarlebill.com. Information about the Illinois Hospital Uninsured Patient Discount Act is also included in the Patient Registration and Admitting Information booklet. Carles practice, however, is to look at each patients financial status vis--vis both the Carle Community Care Discount Program and the criteria of the Uninsured Patient Discount Act, and to provide patients with the deepest discount available to them. Patients who have difficulty applying for Medicaid are provided assistance through an outside agency whose sole purpose is to walk them through this process. This service is provided courtesy of the Hospital. Working with Land of Lincoln Legal Assistance Foundation, a pre-qualifying system is in place so that all area Section 8 residents and homeless people are able to qualify without filling out an application. Extended evening hours one night per week in Patient Accounting so that the working uninsured or underinsured have easier and more worker-friendly access to for financial assistance. Carle is acutely aware of the need for access to care, making that a mainstay of our community benefit program, with a strong charity care program that is based in a philosophy of doing the right thing for their community and patients and balanced by a careful stewardship of the community's resources.PatientEducationAssistance
Community InformationCarle Foundation Hospital serves Champaign-Urbana and rural communities reaching residents in 40 counties in east central Illinois and western Indiana. The total population of its service area is approximately 1.26 million. However, 55 percent of Carle Foundation Hospital patients reside in Champaign County. Carle Foundation Hospital serves as the region's only Level I Trauma Center and maintains a 42-bed Level III Neonatal Intensive Care Unit. As the provider of the region's perinatal services, Carle provides care to patients who live throughout the area extending from Kankakee to the southern-most tip of Illinois, and spanning from as far west as Decatur and east into western Indiana. Many of these patients receive financial assistance through Carle's Charity Care Discount Program, which is available to residents throughout Carle's service area. Qualifying trauma patients, regardless of their place of residency, are covered for their trauma care, hospitalization and follow-up care. There are two not-for-profit community hospitals, including Carle, both of which are located in Urbana. The population of Champaign County (2009 estimate) is recorded at 195,671. Champaign County has the third highest extreme poverty rate (8.8%) in Illinois, with 17% of the children living in poverty. At Carle, between FY 07 and FY 10, Medicaid cases have grown by 35%; and the uninsured by 37%. In FY 10, 8% of Carle Foundation Hospital patients were uninsured. According to research conducted by the United Way of Champaign County, slightly under 80% of people have a primary source of care. As expressed by one Champaign County community leader, "many have good access to care, but there are segments that lack access to care for one reason or another and who suffer." In a 2010 collaborative research study by the Robert Wood Johnson Foundation and University of Wisconsin Population Health Institute, Champaign County ranks at 35 out of 101 counties, with 1 indicating the highest access to care. As an indicator of how healthy local residents are, this same study ranked Champaign County at 31.CommunityInformation
Community Building ActivitiesThe greatest investment in Community Building Activities has been in Physical Improvements/Housing. In Urbana, where our headquarters and primary medical campus is located, Carle leads the community in providing affordable housing, with the City having identified this as a significant community need. Having ownership of several houses in one of the few remaining neighborhoods where homes are modest and affordable, we have provided rentals at below market prices at a cost to our organization. We have also committed to keeping our rentals in good condition and thus also provide the landscaping to keep the neighborhood tidy-contributing to stability in housing values for the adjacent owner-occupied properties. Furthermore, we maintain two transitional houses at essentially no rent to the City which operates a program to help people re-integrate into society in a private setting. We also contribute to the cost of public works in this neighborhood for projects that might not otherwise get done in a timely fashion because of budgetary issues. Other significant Community Building Activities included: . Providing leadership and grant dollars to prepare our hospital and those in the surrounding 21-county region to respond to any natural disaster or act of terrorism. This year that preparation included: preparing for H1N1; providing IV pumps to regional hospitals; and helping to develop an emergency health education facility. . Hosting a community coalition 10 times annually to not only get input on our charity care program, but to discuss solutions to access to care in our community. . Reducing environmental destruction through providing convenient access to drop off boxes for old medications, as well as needle disposal boxes-keeping thousands of pounds of wastes out of our environment.CommunityBuildingActivities
Other InformationCarle Foundation Hospital has an open medical staff and is governed by a community board. Surplus funds are reinvested into the organization for technology replacement and advancement, building renovation or construction, clinical service expansion, and quality improvement. Surplus funds are also invested into our communities to meet identified health needs which contribute to the overall well-being of the residents of the communities we serve. Because the needs are great and varied and it would be unreasonable to address each and every identified need, we set priorities and use our resources to focus primarily on those needs that correlate with our mission; such as access to healthcare, medical and health education and community health education, research and providing important medical services that operate at a loss. Based on a variety of community needs assessments, research and reports, the Hospital's Community Benefit Plan is organized into three categories: Initiatives, Grants, and Partnerships, with cross-over between these categories. Partnerships and Initiatives (not inclusive) . The following programs are designed to meet identified needs and to improve the health of the community primarily through education and prevention. These may be Carle-based initiatives or accomplished in partnership with community organizations. Programs are evaluated annually for effectiveness in meeting needs and community acceptance in conjunction with the budget process. The Community Parish Nurse Program-designed to train nurses from local churches to educate congregants and to advocate for their healthcare interests. This year we trained 22 additional nurses. This program is one of the largest in the nation, with 436 total nurses trained from 221 congregations in 31 counties and three states. Playing It Safe-This free family safety fair is co-sponsored with Safe Kids Champaign County. Planned for the 14th consecutive year, the participation from countywide and local public safety agencies as well as other community organizations concerned with our children's' safety was extraordinary. The event involved more than 40 interactive displays and stage presentations and more than 700 people of all ages participated. Our well-established child safety programs-Playing it Safe, Poison Prevention, Risk Watch, and farm safety education for children-are focused on reducing unintentional injury and accidental deaths. Unintentional injury is the number one killer of children between the ages of 1 and 17. According to recent statistics, programs like these have been credited for keeping Illinois' pediatric mortality rate well below the national average. Poison Prevention-Carle is a satellite education center for the Illinois Poison Control Center, serving schoolchildren, healthcare professionals, and consumers. Approximately 2,000 children were reached in Champaign and Urbana school districts. Poison prevention materials are available in multiple languages for our diverse population. Risk Watch-is a safety curriculum integrated into local schools by a Carle Foundation Hospital staff member and representatives from public safety agencies as expert resources for teachers. In school year 2009-2010, the program reached thousands of students through presentations of 11 different subjects. Carle trends current safety topics, and adapts curriculum to student needs. Center for Rural Health and Farm Safety-functions with partners at the University of Illinois and area farm bureaus and extension services. Free programming focuses on educating rural farmers, children, families and emergency responders on preventing injury. The 34 children's educational programs included Progressive Agriculture Farm Safety Days and rural safety presentations at schools reaching 2,623 youth. The 67 general programs included Farm Family Emergency Response, First Aid and CPR certification reaching 751 participants. The 19 emergency responder training courses included Agricultural Emergency Response classes as well as tractor rollover and grain entrapment sessions reaching 286 EMTs, firefighters and other responders. Interpersonal Violence-The Interpersonal Violence Prevention program provides community education with a focus on reducing domestic violence as well as training for Sexual Assault Nurse Examiners (SANE) and others who treat rape and abuse victims. Carle has nine SANE nurses in the Emergency Department, assisting approximately 124 adult/adolescent and pediatric sexual assault patients this year. Carle/Salvation Army Toy Drive-For the 25th year, Carle Foundation Hospital was a primary corporate sponsor of this annual holiday event. We shared this sponsorship honor with the legacy Carle Clinic and WHMS/WDWS radio. A total of 1,932 toys, 99 gift cards and $2,515 was collected at Carle drop off locations and through a one-day drive-through collection. www.HelpSource.org- This on-line directory of human services in east central Illinois helps agencies and caregivers and providers link people with community resources. Carle took a leadership role in its development 11 years ago and has continued to support HelpSource.org, providing guidance to its maintenance and promotion. Donations and Grants (not inclusive) Financial, in-kind, and leadership support of health and human service organizations with similar/compatible missions. Nearly 100 community organizations received financial support this year. Some of those are: A Woman's Fund American Diabetes Association American Red Cross Catholic Charities Central Illinois Down Syndrome Organization Champaign County Mental Health Board Champaign County YMCA Champaign-Urbana Autism Network Coles County Council on Aging Crisis Nursery Cunningham Children's Home Danville Area United Way HelpLine* Developmental Services Center Don Moyer Boys and Girls Club Eastern Illinois Food Bank Family Service of Champaign County Frances Nelson Health Center Operation Cool seat belt safety program PACE United Way of Champaign County** *A corporate campaign contribution was also made to Vermilion, Piatt and Edgar counties. **Carle Foundation Hospital annually matches a portion of our employees' contribution to United Way of Champaign County. This year, our corporate contribution was a 100% match plus an additional $21,477.16, bringing the reportable total corporate donation to $58,888.58. Access to Care and Service Multiple community organizations continue to advocate the need for access to health and dental care for everyone, keeping the awareness at a high level. Carle Foundation Hospital continues to respond to this need. Here are ways in which we are working with other local and area organizations to increase access to care: Dental Care Led by the United Way, a community group in Champaign County that includes Carle representation is successfully tackling the growing problem of inadequate dental care for adults and children. As a result, a dental clinic which will serve the uninsured adult, as well as children, is scheduled to open in 2011 at Frances Nelson Health Center (FNHC). Carle has made a $100,000 pledge towards the build-out of FNHC to accommodate a dental facility and has agreed to provide facility maintenance and utilities, as well as to only charge minimal rent on this additional space. $198,154 for Frances Nelson Health Center Frances Nelson Health Center is a Federally Qualified Community Health Center that provides primary care clinic services on a sliding fee scale to the underserved and underinsured. In 2005, in concert with a community effort, Carle Foundation Hospital purchased a former furniture store in Champaign, renovated it for $1.2 million, provided free rent for three years and continues to support the operations of the facility: - The Hospital continues to pay utilities and operating costs associated with the facility's maintenance, cleaning and security, which totaled $166,501 in FY10. - A donation of $10,000 was provided for asthma education. - Carle's All About Baby staff regularly provides prenatal, breastfeeding and newborn care education at the clinic. - Occasional provision of pharmaceutical drugs. Coles County Community Health Clinic Carle continued to provide leadership towards the establishment of a Federally Qualified Health Center to serve the indigent, uninsured and underinsured in Coles County in conjunction with other community partners. Officials estimated that more than 17% of the Coles County population is without health care. About 9,300 people use public aid, while more than 7,100 workers have no health insurance. This health center opened in September 2010. Free Clinics - Champaign County Christian Health Center Since 2004, Carle Foundation Hospital has been a significant funder of this free clinic, regularly contributing towards rent for the facility and operating expenses. In FY10, however, our contribution of $8,087 was in leadership and community service, with a contribution towards rent having beenOtherInformation
Affiliated Health Care SystemThe Hospital and its associated business units under The Carle Foundation umbrella all participate in carrying out the same annual community benefit plan, participating in programs that fit their specific missions. For instance, Chronic Disease Management Classes are provided at no cost at our retirement community; our directors are involved on human agency boards and committees and identify programs that we can financially support or participate in which meet a community need; and our ambulances provide complimentary stand-by services at community events.AffiliatedHealthCareSystem

ExplanationThe majority of the requests for funding are to The Carle Foundation or its affiliates located in the communities we serve. Any charitable organization wishing to request funds is asked to submit their request in writing. Emailed requests are acceptable. The request must include a description of the purpose of the contribution and other relevant information such as requestors name, amount of the request, etc. The requests are reviewed, evaluated, and administered by the Public Relations Director, seeking additional information from the requestor, as needed, and input from any appropriate Carle administrator or director. A set of established guidelines drive these decisions, and most are made to improve healthcare/access to healthcare or are in line with our role as a corporate citizen. Assessed need is a common criteria. In the case of most donations of $5000 or more, such as to colleges and universities or community clinics, written agreements are in place and we have regular, casual contact with those organizations, oftentimes through a championwho may or may not be the PR director--identified as the Carle contact person to assure that the donations are being used as agreed upon. Records are maintained either in Administration or Public Relations.Explanation
