Income Based CriteriaIf applicable, describe the income-based criteria for determining eligibility for free or discounted care under the organization's charity care policy. Also describe whether the organization uses the asset test or other threshold regardless of income to determine eligibility for free or discounted care. When Catholic Health Initiatives (the parent organization to Carrington Health Center) established its charity care policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among the communities in 19 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines ("FPG"), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. Carrington Health Center ("Carrington") bases its charity care eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain charity care write-offs, based on a sliding scale, ranging from 25%-100% of charges. An individual's income under the HUD guidelines is a significant factor in determining eligibility for charity care. However, in determining whether to extend discounted or free care to a patient, the patient's assets may also be taken into consideration. For example, a patient suffering a catastrophic illness may have a reasonable level of income, but a low level of liquid assets such that the payment of medical bills would be seriously detrimental to the patient's basic financial (and ultimately physical) well-being and survival. Such a patient may be extended discounted or free care based upon the facts and circumstances.IncomeBasedCriteria
Annual Community Benefit ExplnIf the organization's community benefit report is contained in a report prepared by a related organization, rather than a separate report prepared by the organization, identify the related organization. Carrington prepares its own annual written community benefit report. Its community benefit report is not contained in that of a related organization.AnnualCommunityBenefitExpln
Costing Method UsedProvide an explanation of the costing methodology used to calculate the amounts reported in the table. If a cost accounting system was used, indicate whether the cost accounting system addresses all patient segments (for example, inpatient, outpatient, emergency room, private insurance, Medicaid, Medicare, uninsured or self pay). Also indicate whether a cost-to-charge ratio was used for any of the figures reported in the table. Describe whether this cost-to-charge ratio was derived from Worksheet 2, and, if not, what kind of cost-to-charge ratio was used and how it was derived. If some other costing methodology was used besides a cost accounting system, cost-to-charge ratio, or a combination of the two, describe the method used. A cost accounting system was not used to compute amounts in the table; rather costs in the table were computed using the organization's cost-to-charge ratio. The cost-to-charge ratio covers all patient segments. The cost-to-charge ratio for the year ended 6/30/10 was computed using the following formula: Operating expense (before restructuring, impairment and other losses) divided by gross patient revenue. Based on this formula, $12,854,974 / $22,839,584 results in a 56.28% cost-to-charge ratio. Worksheet 2 was not used to derive the cost-to-charge ratio.CostingMethodUsed
Subsidized Health Services ExplnTo the extent that an organization includes any costs associated with physician clinics as subsidized health services in Part I, line 7g it must describe that it has done so and report in Part VI such costs included in part I, line 7g, line 1 There are no physician clinics included in subsidized health servicesSubsidizedHealthServicesExpln
Percent Of Total Expense ExplnIf applicable, state the bad debt expense included on Form 990 Part IX, line 25, column (A) but subtracted for purposes of calculating the percentage in this column. Total bad debt expense reported on Form 990 Part IX, line 25, Column A was $346,195PercentOfTotalExpenseExpln
Bad Debt FootnoteProvide the rationale and the costing methodology used to determine the amount reported in Part III lines 2 and 3. Describe how the organization accounts for discounts and payments on patient accounts in determining bad debt expense. Also describe the method the organization uses to determine the amount that reasonably could be attributable to patients who likely would qualify for financial assistance under the hospital's charity care policy, if sufficient information has been available to make a determination as to their eligibility. Also, provide if applicable, the text of the footnote to the organization's financial statements that describes bad debt expense. If the organization's financial statements include a footnote on these issues that also includes other information, report only the relevant portions of the footnote. If the organization's financial statements do not contain such a footnote, state that the organization's financial statements do not include such a footnote and explain how the financial statements account for bad debt, if at all. Costing methodology for amounts reported on line 2 is determined using the organization's cost-to-charge ratio of 56.28% When discounts are extended to self-pay patients, these patient account discounts are recorded as a reduction in revenue, not as bad debt expense. Carrington does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care. Carrington does not issue separate company audited financial statements, however, this organization is included in the consolidated financial statements of Catholic Health Initiatives. The consolidated bad debt footnote reads as follows: "the provision for bad debts is based upon management's assessment of historical and expected net collections considering historical business and economic conditions, trends in health care coverage, and other collection indicators. Management periodically assesses the adequacy of the allowances for uncollectible accounts based upon historical write-off experience by payer category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility".BadDebtFootnote
Shortfall As Community Benefit(A) Describe the costing methodology used to determine the Medicare allowable costs reported in the organization's Medicare Cost Report, as reflected in the amount reported in Part III, line 6. (B) Describe, if applicable, the extent to which any shortfall reported in Part III, line 7 should be treated as a community benefit, and the rationale for the organization's position. (A) Carrington Health Center is designated as a Critical Access Hospital ("CAH"). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (CoP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, and Carrington Health Center, due to its designation as a CAH, received cost-based reimbursement for Medicare purposes, Carrington Health Center will not experience Medicare related shortfalls. (B) However Carrington Health Center, and its affiliates, believe that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays.ShortfallAsCommunityBenefit
Debt Collection PolicyIf the organization has a written debt collection policy and answered "yes" to Part III, line 9b, describe the collection practices set forth in the policy that apply to patients who it knows qualify for charity care or financial assistance, whether or not such practices apply specifically to such patients or more broadly to also cover other types of patients. Carrington's debt collection policy provides that Carrington will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through Carrington's community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet the Carrington community assistance policy is required to be returned immediately by the third-party collection agent to Carrington for appropriate follow-up. All of Catholic Health Initiatives' hospital's contracts with third party collection agencies include the following standards: * Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; * Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and * No Catholic Health Initiatives' collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect.DebtCollectionPolicy

Describe the community the organization serves, taking into account the geographic area and demographic constituents it serves. Describe how the organization's community building activities, as reported in Part II, promote the health of the communities the organization serves. Provide any other information important to describing how the organization's hospitals or other health care facilities further its exempt purpose by promoting health of the community (e.g. open medical staff, community board, use of surplus funds, etc). CARRINGTON HEALTH CENTER CARRINGTON, ND INTRODUCTION: I. ORGANIZATION'S MISSION, VISION, AND TAX-EXEMPT PURPOSE Over 2000 years ago, Jesus Christ issued a call to His followers to heal the sick and comfort the afflicted. This call has echoed down through the centuries and been answered by those who started the religious order that, in turn, founded the hospitals and nursing homes which comprise today's Catholic health care ministry. Each of the men and women who joined these orders also responded to that call to relieve the suffering of their fellow human beings. Today the call is still being answered by the thousands upon thousands of dedicated employees who work in Catholic health care facilities. Many of these committed people may never have even consciously recognized that call. But each, in his own way, has heard it - and responded. The Carrington Hospital Association charted on September 23, 1915 and built the original hospital in 1916. In 1941, the hospital was leased to the Sisters of the Presentation of the Blessed Virgin Mary of the Diocese of Fargo, ND. Full ownership was transferred to the Presentation Sisters in 1970. Carrington Health Center is comprised of a hospital, built in 1955, currently used for auxiliary and gift shop, physical therapy, and administration; a Rural Health Clinic, a long term care (now basic care) facility built in 1964 and a new hospital complex built in 1986 complete the campus. The Presentation Sisters joined the Catholic Health Corporation of Omaha in 1980 and in 1996, the Catholic Health Corporation consolidated with two other Catholic health systems, the Sisters of Charity Health Care System, Inc., Cincinnati, and the Franciscan Health System, Aston, PA to form a new corporation, Catholic Health Initiatives. The mission of Carrington Health Center and Catholic Health Initiatives is "to nurture the healing ministry of the church by bringing it new life, energy and viability in the 21st Century. Fidelity to the Gospel urges us to emphasize human dignity and social justice as we move toward the creation of healthier communities." Catholic Health Initiative's vision is to live up to our name as one CHI. Catholic: Living our Mission and Core Values. Health: Improving the health of the people and communities we serve Initiatives: Pioneering models and systems of care to enhance care delivery Carrington Health Center's vision is to be recognized as one of the best Critical Access Hospitals in North Dakota, and to be known as a hospital that gives the highest quality patient care. The primary exempt purpose of Carrington Health Center is to provide healthcare services to residents in the community and surrounding area regardless of their ability to pay. II. COMMUNITY BENEFIT APPROACH Carrington Health Center ("CHC") provides a wide array of health services for our area including: a Critical Access Hospital; Emergency Care; Trauma Level 5; Acute Care; Swing Bed; General Surgery; Patient Education; Pharmacy; Radiology; Laboratory Services; Physical Therapy; Occupation Therapy; Speech Therapy; Monitored Coronary Care; Respiratory Therapy; Cancer treatments; Same-Day Surgery; Ultrasound; DEXA Scans; CT Scanning; Electrocardiography; Dietetic Counseling; Mammography; a Basic Care Facility; Hospice; and a full Clinic with 3 Physicians, and 3 Physician Assistants. CHC also has a rural health clinic in Eddy County and had a rural health clinic in Wells County through October of 2010. CHC also assists in addressing the spiritual and emotional needs of Carrington and portions of five surrounding counties. CHC's Mission department, Spiritual Care and Social Services department provide assistance for those needing guidance and help. A clinical psychologist is one of the specialists based at Carrington Health Center's campus. CHC is located in Carrington, ND, Foster County. Surrounding counties served by CHC include Eddy County, and portions of Wells County and Nelson County. According to the U.S. Census Bureau these 3 counties total 2,582 square miles with a population of approximately 11,600 people. CHC has a governing Board of Directors whose membership includes three representatives from Carrington and the immediate surrounding area, two representatives from New Rockford, where a satellite clinic is located, and one representative from Fessenden. The Board also includes two women religious from our founding order, the Sisters of the Presentation of the Blessed Virgin Mary. COMMUNITY ASSESSMENT On April 28, 2009, a Planning Committee met to identify and discuss the top issues facing older adults and persons with disabilities residing in Foster County. This group identified 104 issues important to Foster County elderly and persons with disabilities. The committee narrowed the list to the five most critical issues and a plan was developed to address these issues. In addition, the Healthy Community Initiatives Committee meets once a month under the direction of CHC's Spiritual Care Coordinator working with the school, public health, county social services, churches, community members and other health care professionals. Healthy Community is comprised of community residents and organizations collaborating on projects such as: providing supplemental food for individuals or families in need; educational opportunities for residents to learn new crafts and skills; and bringing awareness to healthier behaviors, including discouraging underage drinking and binge drinking. DESCRIPTIONS: Our best-known, but often least-utilized, Community Benefit program is the financial assistance we provide to patients who cannot afford to pay for their care. Financial assistance applications are available on our website (www.carringtonhealthcenter.org) and at our facilities. The process is extremely confidential and our goal is to award financial assistance rather than turn patients over to collection for healthcare bills. Charity Care is free or discounted health and health-related services provided to persons who cannot afford to pay for medically necessary health care. Eligibility is based on household income as well as other assets. Consideration is given to a patient who has significant or catastrophic medical bills and does not meet the income guidelines. Charity Care is given to uninsured, low-income patients who are not expected to pay all or part of a bill, or who are able to pay only a portion using an income-related fee schedule. The income benchmark is typically considered to be 130% of the federal poverty level. Community benefit programs for the poor are geared to reducing morbidity and mortality in beneficiaries of Medicaid or state or local indigent programs. COMMUNITY BENEFIT Community Benefit is the name given to the programs we create in response to health needs in our service areas. An important part of our mission is building healthier communities through programs designed to serve the needs of people who are poor and disadvantaged in our community. This type of Community Benefit has the added, positive side-effect of delivering an improved quality of life for everyone. Community Benefit most often takes place outside the walls of our facilities and involves partner organizations in the community. Our Community Benefit activities are designed to: *Identify and increase awareness of health needs in Carrington. *Provide resources to meet those needs. *Create collaboration between community organizations that focus on health and well being. *Tackle problems such as poverty, homelessness, or behaviors (such as domestic violence or underage drinking) that can contribute to poor health. Carrington Health Center's Human Investments and Statistics: Number of activities: 66 Staff hours: 4399.1 Volunteer hours: 295 A. COMMUNITY HEALTH SERVICES Community Health Education is offered at no charge to the broader community and encompasses a wide span of patient types. Carrington Health Center provides diabetic education, self monitoring of blood-glucose, and preparation of advanced directives. Education includes lectures, presentations, and other programs and activities provided to groups without providing clinical or diagnostic services. Support Groups typically are established to adNeedsAssessment
Patient Education AssistanceDescribe how the organization informs and educates patients and persons who may be billed for patient care about their eligibility for assistance under federal, state, or local government programs or under the organization's charity care policy. Carrington Health Center includes information concerning its financial assistance policy on its website. In addition, Carrington Health Center prominently displays its charity care policy in obvious locations throughout the hospital, including the emergency room and other patient intake areas, as well as in the three rural health clinics. In addition, Carrington Health Center registration clerks are trained to provide consultation to those who have no insurance or potentially inadequate insurance concerning their financial options including application for Medicaid and for financial assistance under Carrington Health Center's charity care policy. Upon registration (and once all EMTALA requirements are met), patients who are identified as uninsured (and not covered by Medicare or Medicaid) are provided with a packet of information that addresses the financial assistance policy and procedures including an application for assistance. Carrington Health Center registration clerks read the organization's medical assistance policy to those who appear to be incapable of reading. Carrington Health Center staff will also assist the patient/guarantor with applying for other available coverage (such as Medicare or Medicaid), if necessary.PatientEducationAssistance

Affiliated Health Care SystemIf the organization is part of an affiliated health care system, describe the respective roles of the organization and its affiliates in promoting the health of the communities served. Carrington Health Center is part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Denver, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 78 hospitals; 40 long-term care, assisted- and residential-living facilities; and two community health-services organizations. Together, these facilities provided $590 million in charity care and community benefit in the 2010 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain - provides economies of scale and purchasing power to the MBOs. Cost savings associated with centralized services for the fiscal year ended June 30, 2010 include more than $105 million in supply chain expense, approximately $30 million for insurance costs and more than $3.9 million as the result of a system-wide effort to coordinate and negotiate technology and equipment purchases. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society.AffiliatedHealthCareSystem

non fixed paymentsSch J Part I, Q7Carrington Health Center maintains a variable pay plan for Senior Executive Management that puts a certain amount of compensation at risk. Awards of incentive compensation under the variable pay plan are made based upon achievement of organizational objectives including performance in relation to budget, and achievement of individual goals. All compensation, including any incentive compensation, is capped to ensure that compensation remains at Fair Market Value. Physician incentive compensation is based on the Medicare methodology (CMS RBRVS method) for determining physician wRVUs for personally performed physician services. The value of a wRVU is determined with reference to the most current National Physician Fee Schedule Relative Value File. Physicians shall be compensated based on a 2-tiered conversion factor: $44 per wRVU in excess of 3636 and $52.85 per wRVU in excess of 5600.

A copy of the return will be provided to both the finance and audit compliance committee and the board prior to filing with the IRS. Subsequently, the CHI tax department files the return with the appropriate Federal agency, making any non-substantive changes necessary to effect e-filing. Any such changes are not re-submitted to the board.
Procedures for monitoring and enforcing the COI policyForm 990, Part VI, Q. 12cDISCLOSURE, REVIEW AND INITIAL DETERMINATION: Each director must promptly and fully report to the Board Chair situations that may create a conflict of interest when he or she becomes aware of such situations. In the case of an officer, disclosure must be made to the Corporation's President and Chief Executive Officer who will report such disclosure to the Board Chair. In any situation where a director or officer is in doubt, full disclosure should be made so as to permit an impartial and objective determination. A written record of the disclosure will be made. Annual Disclosure Statement In addition to the ongoing disclosure obligation, the Corporation's President and Chief Executive Officer shall annually send to all directors and officers a copy of this Policy statement and the Conflict of Interest Disclosure Statement. The directors and officers must promptly complete, sign, and return the statement to the Corporation's President and Chief Executive Officer. The completed statements will be reviewed by the President and Chief Executive Officer and the Board Chair. Review, Evaluation and Initial Determination The Board Chair or designee shall make such further investigation of conflicts of interest disclosures as he or she may deem appropriate. If the conflict involves the Board Chair, the Vice Chair will assume the Chair's role outlined in this Policy. Based on review and evaluation of the relevant facts and circumstances, the Board Chair will make an initial determination as to whether a conflict of interest exists and whether, pursuant to this Policy statement, review and approval or other action by the Board of Stewardship Directors is required. A written record of the Board Chair's determination, including relevant facts and circumstances, will be made. The Board Chair shall then make an appropriate report to the Executive Committee of the Board concerning such review, evaluation and determination. If there is a difference of opinion between the Board Chair and another director or officer as to whether the facts and circumstances of a given situation constitute a conflict of interest or whether Board of Directors review and approval or other action is required within this Policy statement, the matter shall be submitted to the Board's Executive Committee, which shall make a final determination as to the matter presented. Such determination, including relevant facts and circumstances, will be reflected in the Committee minutes and will be reported to the Board of Directors. BOARD REVIEW Transactional Conflicts of Interest The Board of Directors shall carefully scrutinize and must in good faith approve or disapprove any transaction in which the Corporation and/or any of its Affiliates is a party and in which one or more of the Corporation's directors or officers either: *Has a material financial interest; or *Is a director or officer of the other party (other than the Corporation's own Affiliates). The Board of Directors must approve the transaction by a majority of the directors on the Board, without counting the vote of any director who has an interest in the transaction. In reviewing such transactions between the Corporation and vendors or other contractors who are, or are affiliated with, directors or officers, the Board shall act no more or less favorably than it would in reviewing transactions with unrelated third parties. The transaction will not be approved unless the Board determines that the transaction is fair to the Corporation. Other Conflicts of Interest The Board shall carefully review and scrutinize non-transactional conflicts of interests (e.g., disclosure of nonpublic information, competition with the Corporation, failure to disclose a corporate opportunity, excessive gifts or entertainment, etc.). By a majority vote of the disinterested directors, the Board shall take whatever action is deemed appropriate under the circumstances with respect to the director or officer in order to best protect the interests of the Corporation including possible disciplinary or corrective action. The Board should consult with the General Counsel of the Corporation when considering disciplinary or corrective action. Disclosure by Interested Director When conflicts of interest are considered by the Board, the director or officer must disclose all of the material facts to the Board. The director or officer shall not vote or use his or her personal influence on the matter. However, if requested, such director or officer is not prevented from briefly stating his or her position in the matter, nor from answering pertinent questions from Board members, as his or her knowledge may be of significant importance. The director or officer shall be excused from the meeting during discussion and vote on the conflict of interest. Record of Proceedings Minutes of the Board of Directors shall reflect the following: the individual making the disclosure, the nature of the disclosure, discussion regarding any proposed transaction, the decision made by the Board, and that the interested director abstained from voting. Implementation and Interpretation Questions regarding the implementation and interpretation of this Policy shall be referred to the General Counsel of the Corporation. POLICY VIOLATIONS If the Board reasonably believes that a director or officer has failed to disclose either an actual or potential conflict of interest, or all material facts surrounding an actual or possible conflict as required by this Policy, the director or officer will be given an opportunity to explain such alleged failure to disclose. After hearing the response of the director or officer, the Board will conduct such additional investigation as may be appropriate. If the Board determines that the director or officer has in fact failed to disclose as required by this Policy, the Board shall take appropriate disciplinary or corrective action.

This independent review is delivered by Hay Group to the HR Committee of the CHI Board of Stewardship Trustees annually at their September meeting and minutes are shared with the full board at the December meeting. The last review was September, 2010.
