THERE ARE ALSO FOUR IMPORTANT GOALS THAT DRIVE HOW THE ORGANIZATION CARES FOR PATIENTS AND THE COMMUNITY. THEY ARE TO PROVIDE THE BEST PATIENT EXPERIENCE, THE BEST COMMUNITY EXPERIENCE, THE BEST PHYSICIAN EXPERIENCE AND THE BEST EMPLOYEE EXPERIENCE.UNFORTUNATELY, FISCAL YEAR 2009 BROUGHT FINANCIAL HARDSHIP AND ECONOMIC CRISIS TO MICHIGAN AND MANY METRO HEALTH PATIENTS. WITH LOSS OF JOBS AND HEALTH INSURANCE, MANY PEOPLE IN THE METRO HEALTH COMMUNITIES SIMPLY COULD NOT AFFORD THE REGULAR AND EMERGENCY HEALTH CARE THEY NEEDED. KNOWING THAT EMERGENCIES CAN HAPPEN TO ANYONE AND THAT REGULAR, ROUTINE CARE IS THE BEST WAY TO AVOID HEALTH EMERGENCIES, METRO HEALTH LAUNCHED METRO CARE TO INCREASE ACCESS AND REDUCE COSTS FOR OUR PATIENTS AND COMMUNITY. THE METRO CARE PROGRAM PROVIDES WEST MICHIGAN SIGNIFICANT COMMUNITY BENEFIT. IT INCLUDES SIGNIFICANT DISCOUNTS FOR PATIENTS WITHOUT HEALTH INSURANCE AND A NO-INTEREST, 25-MONTH PLAN FOR ALL PATIENTS WITH COPAYMENTS GREATER THAN $25. THE DISCOUNT AND PAYMENT PLANS PREVENT COMMUNITY MEMBERS FROM FALLING THROUGH THE CRACKS IN OTHER PROGRAMS AND HELPS COMMUNITY MEMBERS RETAIN THEIR DIGNITY DURING THE CURRENT ECONOMIC CRISIS. ADDITIONALLY, METRO CARE INCLUDES LIVE HEALTH PROGRAMMING THROUGH OUR COMMUNITY OUTREACH DEPARTMENT. LIVE HEALTH PROVIDES FREE AND LOW COST HEALTH CLASSES AND HEALTH SCREENS, INCLUDING PROSTATE, PAD AND BLOOD PRESSURE SCREENINGS.Description

Bad Debt FootnotePart III, Line 4: The Organization's footnote on charity care reads in part, "In addition, under arrangements with various governmental insurance programs, the Hospital provides significant care to the local indigent population for which reimbursement for services rendered is generally less than the cost of providing such services. During the years ended June 30, 2010 and 2009, these costs exceeded reimbursement by approximately $21,400,000 and $20,800,000, respectively. As part of their obligation to the local communities, the Corporation and its subsidiaries also provide numerous other services that benefit the communities and are generally performed at no cost." The organization determines the costs of such unpaid services by applying a cost-to-charge ratio to the billed charges. It is the goal of the Organization to determine if the patient qualifies under its Charity Care Policy prior to billing for services. However, many factors contribute to the inability to accomplish this in many situations. If the Organization is unable to obtain information from the patient regarding their available means to pay or financial status, the patient will be billed for the services provided. Many times in such circumstances, only after billing is it determined that the patient qualifies under the Charity Care Policy. Under these circumstances the adjustment to the billed amount is recorded as bad debt, rather than charity care. The organization does not believe it can accurately estimate the amount of bad debt expense that would have qualified for charity care had documentation been turned in, therefore, it is excluded.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: All of the Medicare shortfall should be treated as community benefit as services this is the extent to which costs related to Medicare services go unpaid. The cost methodology is the same as in Part III line 4 of the form 990 as described above.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: Metro tries to inform the patient if they qualify for charity care as soon as possible because the patient benefits from knowing about their financial obligations and the hospital can avoid a costly collections process if the patient has the inability to pay. Every patient bill has the Federal poverty guidelines on the back and its on Metro's website. Both explains the procedures and limits for charity care and discounting programs. Patient Financial representatives are available to discuss filling out the paperwork for government medical assistance programs.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: Metro Health assesses the health care of the community in a number of different ways. In November 2008, we hired an outside agency, the Community Research Institute (CRI) from the Johnson Center at Grand Valley State University, to prepare a Community Needs Assessment. The assessment included data from Healthy Kent 2010, a community collaboration comprised of representatives from the three area hospitals, the county health department, the city of Grand Rapids Chamber of Commerce, and a wide variety of community health agencies. Additionally, we considered data from the State of Michigan web site, which hosts health data for every county in Michigan, and data from the annual Kent County Health Department Behavioral Risk Factor telephone survey. In 2010, planning began on an updated needs assessment. The Kent County Health Department will serve as the lead agency, bringing together Metro Health, Saint Marys Hospital, Spectrum Health, Michigan State University, Grand Valley State University and other health and human service organizations in a collaborative study to be completed by December 31, 2011.NeedsAssessment
Patient Education AssistancePart VI, Line 3: Metro Health recognizes that individuals and families who are facing financial instability or hardship often neglect healthcare needs until they become emergent conditions. This practice places stress on patients, families and the healthcare system. Metro Health seeks to inform patients of their eligibility for charity care as soon as possible in the healthcare process so that patients know there is help for them and so that the organization avoid costly collections processes. Metro Health posts information on financial assistance on the organization web site. Patients who report having no insurance or high deductible insurance at registration are provided information about our MetroCare program. This program provides a 40% discount to all patients without insurance and a two-year, no-interest payment plan for those patients with a balance of more than $25.00. In addition, every patient bill includes the federal poverty guidelines on the back and explains the procedures and limits for charity care and discounting programs. Patient financial representative are available to discuss filling out paperwork for government medical assistance programs.PatientEducationAssistance
Community InformationPart VI, Line 4: Metro Healths primary services area includes Kent, Allegan and Ottawa counties. With the 2007 hospital relocation to Wyoming, the hospital has begun to see patients from areas farther south and west than before. The organizations reach into the community is enhanced by the positioning healthcare services throughout the community, including12 neighborhood outpatient centers ringing the greater Grand Rapids area; a satellite cardiology office in Greenville, Michigan; an ambulatory surgery center in downtown Grand Rapids, Michigan; and the Metro Health Cancer Center located near the hospital offering medical oncology though Metro Health physicians and radiation oncology though a partnership with University of Michigan Department of Radiation Oncology.Kent County, where the majority of Metro Health patients reside, is the fourth largest county in Michigan. There is a diverse mix of demographics. According to the United States Census Bureau web site (2009 estimates), the majority of the population is white (86.3 percent). The Hispanic population is 9.5 percent of residents; the African American population is 9.2 percent of the community, and Asian Americans comprise 2.2 percent of the population. The census bureau 2008 median household income estimate in Kent County is $50,746, and 14.6 percent of the population is considered below the poverty level.CommunityInformation
Community Building ActivitiesPart VI, Line 5: Metro Health participates in numerous community building activities. As an Osteopathic teaching hospital, we have a significant presence with the Michigan Association of Osteopathic Directors and Medical Educators. This association helps train new physicians and develops the curriculum used for medical residents and student interns throughout our state. Several members of our staff are involved in this group at the state level and spend a large number of hours every year serving in this capacity. In 2010, Metro Health was the education site for 115 medical residents and students. In addition, Metro Health plays an active role in the education of students at other levels. Through a partnership with the Kent County Intermediate School District (KISD), Metro provides a place for disabled students who are learning job and life skills and developing a sense of self worth. Metro also provides KISD high school students with hands-on learning opportunities. These students graduate from high school with marketable healthcare skills and a background for continuing healthcare education.Metro launched a new education initiative with the hosting of the 2010 Heart and Vascular Winter Update for physicians. Over 30 physicians from around the state participated in education sessions that provided CMEs on heart and vascular topics. This new event focused on bringing specialists and primary care providers together to advance each others understanding of the disease process and how to help patients. Metro Health staff members play an active role on the boards and/or committees of several health and human service agencies in our community, such as YMCA, Grand Rapids Chamber of Commerce Health Care committee, Senior Neighbors, Kent Health Plan and more.Metro Healths Live Health community outreach program provides significant interaction with the community through free and low cost classes and screenings. Classes cover a wide variety of topics from smoking cessation to healthy cooking to diabetes management to cancer prevention and more. Screenings also cover a wide array of topics including memory screening, blood pressure screening, peripheral arterial disease (PAD) screening and more. These screenings provide opportunities for community members to interact directly with providers in an environment that encourages follow up, open conversation, relationship building and healthy lifestyles. At an October 2009 PAD screening, 164 people were screened. Of these, 66 were found to have high blood pressure and received related counseling, and 13 were found to have indicators for PAD and were provided consultation and follow up to prevent further disease progress. Our Sports Medicine department also plays and active role in the community, providing first aide at races and community events year round. In 2010, the group was available to over 50,000 event participants and spectators throughout the community.Metro Health is also committed to protecting the environment through a number of green initiatives. Our hospital building is one of the first hospitals in the nation designed and built to be LEED-certified (Leadership in Energy and Environmental Design), and it is located in the Metro Health Village which requires all buildings on the property to be designed and built for LEED certification.Already conscious of recycling and composting, Metro Health increased efforts in these areas throughout the year. The hospital kitchen participates in a robust composting program, and the organization set goals to focus every staff member on increasing the overall recycle rate at the hospital. Planning was initiated in 2010 to significantly increase the amount of composting done by cafeteria customers. The hospital continued its May to October farm market as a means to encourage healthy lifestyles and the connection between health and our earth. Over 6,000 staff members and community residents visited the farm market in 2010.CommunityBuildingActivities
Other InformationPart VI, Line 6: As a community hospital, Metro Health is committed placing community leaders, physicians and key hospital leaders on its board of directors. This allows the community to have a voice in how the hospital serves it and allows physicians to have a say in how the hospital is run. Metro Health supports an open medical staff, welcoming both employed and non-employed physicians to practice within the Metro Health system.Metro Health and the employees at Metro Health recognize that cost of health care reaches far beyond that of drugs, physician visits, laboratory test, etc. Often staff members see patients and their families who are struggling to balance medical costs, housing costs, food costs and other basic needs. To better serve these patients, the Metro Health Hospital Foundation provides community members and Metro Health employees opportunities to contribute to various patient care funds. These funds provide resources to meet needs ranging from immediate food needs, to bus transportation for tests, shoes to wear home from the hospital, rent or mortgage assistance and more.OtherInformation
