This support requires balancing a steadfast commitment to our mission while remaining stewards of valuable community programs that keep Cape Cod healthy. Through collaborative partnerships with state and local community health and human service agencies, CCHC develops initiatives to meet the needs of special populations identified through ongoing and formal community health needs assessments, and provides programs which align with Massachusetts State-wide priorities. The following is a list the key accomplishments of the FY2010 Community Benefits Plan and the needs established by the Community Health Committee in June, 2009 for FY2010 - FY2012. Community Benefit Accomplishments FY2010: The year 2010 emerged as turbulent and challenging for the Cape Cod community. Despite these challenges CCHC remained dedicated to its core vision; "identify and respond to the needs of our community." And, with the transition of the Community Benefits Program under new management on October 1, 2009, it presented an opportunity to re-evaluate the objectives of the program which remained flexible throughout the year to meet the needs of Cape Cod, the ability to fund programs and to identify existing hospital-sponsored community initiatives that qualify as community benefit programs per the new Attorney General Guidelines. As always, CCHC's commitment through its Community Benefits Program, Cape-wide partnerships and through the contributions of CCHC's dedicated physicians, employees and volunteers, provided numerous programs that align with the Community Benefits Mission. The following is a list of short-term goals and accomplishments for FY2010: 1. Work with FY2009 Community Benefits contract holders to assess program needs and renew support for FY2010 as appropriate. After careful assessment of FY2009 funded programs, CCHC Community Benefits continued funding 9 programs through FY2010 to prevent gaps in services. This assessment provided an opportunity for the Senior Manager of Community Benefits to learn about the programs to write the FY2009 Attorney General report. 2. Renew the Cape Cod Healthcare Community Health Committee to carry out the objectives of the Community Benefits Plan. The Community Health Committee reviewed the Bylaws, Community Health Priorities for FY2010 and the current membership. The committee's first task was to fill vacant seats and ensure representation from all regions of Barnstable County. 3. Review hospital-sponsored community programs to determine alignment with identified target populations and priorities. A new internal Community Benefits Taskforce convened and identified a number of community-based programs sponsored by Cape Cod Hospital and Falmouth Hospital that align with the Community Benefits Guidelines. 4. Establish a mini-grant process for emerging health care needs by June 2010. Cape Cod Healthcare budgeted for short-term, "one-time" mini grants, to provide assistance for emerging community health issues. The Senior Manger, in collaboration the CHNA 27 RFP Sub-Committee, and several local community organizations developed a formal mini-grant process. The Community Health Committee reviewed and accepted 50% of the submitted proposals which resulted in a total award of $42,060. 5. Develop and improve a strategic RFP process for FY2012 funding by July 2010 to provide more transparency and greater community participation. The Senior Manager of Community Benefits, utilizing best practices, worked closely with local and regional organizations, the CHNA 27 and members of the community experienced in grant processing to revise the grant funding process. The Community Health Committee played an important role in the evaluation of the new RFP process for FY2011 and offered recommendations for improvements in the review and scoring of the proposals for future grant funding. Using the identified community health needs and supporting documentation and data, the Community Health Committee developed the following long-term Community Benefit Goals for FY 2010 - 2012: 1. Improve Access to Health Care and Reduce Health Disparities: In collaboration with the four community health centers and local health and human service agencies, Cape Cod Healthcare increased access to care for community members who have government-funded insurance programs or who are un/underinsured through our physician recruitment program and our collaborative insurance enrollment programs. In addition, CCHC provided programs to assist with cultural and language support for community members who may be unaware of how to access health care, may encounter language barriers, or are hesitant to seek care due to residency documentation. 2. Chronic Disease Management: Diabetes, cardiovascular disease and oral health related issues were identified by the latest formal needs assessment to be the top three preventable diseases based on the following four local indicators; health disparities, growth rates in obesity, decrease in access to specialty care and awareness of the implications of such health issues. 3. Mental Health and Substance Abuse Related Issues:
According to Beinecke, Worlock Moore, and Mandersheid, 2008; Archives ofCape Cod face more behavioral health problems and challenges than general regions of the state. This may result in increased mental health-related illnesses, substance abuse and suicide. County data shows that during the peak season on Cape Cod, housing becomes more expensive and unavailable. Families working in seasonal industries face fluctuating incomes, high rates of unemployment, bankruptcy and home foreclosures, which may result in feelings of despair, self-medication and suicide attempts and completions. State data shows the suicide rate for Cape Cod and the Islands is 1.5 times the state average creating a greater demand for mental health and substance abuse services. Through collaborations with the Community Health Center Network and local health and human services agencies, CCHC funded programs that increase access to programs that support community members with mental health and substance abuse issues. 4. Promoting Wellness of Vulnerable Populations: For the purposes of this report and per the completed needs assessment in 2009, "vulnerable people" are groups of people whose physical, cultural or mental condition or circumstances render them vulnerable to health-related issues and present significant barriers to access. These include but are not limited to: a. Low-income individuals and families. b. Persons living in poverty. c. The homeless. d. Immigrant populations. e. The elderly. f. Persons with mental retardation, mental health and/or substance abuse disorders. g. Persons lacking resources to meet basic needs (e.g., food, shelter). h. Persons with chronic health issues related to diabetes, cardiovascular disease and/or oral health. i. Persons with chronic diseases which, if left untreated, threaten the public's health (e.g. HIV, Viral Hepatitis, Hepatitis C, tuberculosis). Cape Cod Healthcare considers these populations vulnerable due to their circumstances, the geography of Cape Cod, limited public transportation, and restrictions to access to health care due to the limited number of health care providers accepting MassHealth. 5. Emerging Health Issues: Cape Cod Healthcare budgets for "one-time" donations to assist emerging health issues on a priority basis. Plans for Next Reporting Year: 1. Collaborate with the four community health centers and local health and human service agencies to address health disparities: Cape Cod Healthcare, through its collaborative partnerships with the community health centers and local health and human service agencies, will continue to develop and support programs that that meet the needs of our community. 2. Conduct Formal Cape-Wide Community Health Needs Assessment: Cape Cod Healthcare will work with community partners in the development of the framework and objectives for the community health needs assessment to be conducted in 2011. CCHC will work closely with the internal finance department to ensure all fiduciary and tax-related requirements are met. Internal hospital data, public health data, local and regional health status indicators and direct community and patient feedback will be used. In addition, Physician and hospital staff will provide clinical perspective on service line needs currently facing patients. Cape Cod Healthcare will engage an experienced professional research firm to assist with the technical aspects of the study, including the survey design, data collection, analysis and reporting for the study. CCHC, along with the Community Health Committee, will adopt an implementation strategy in collaboration with our community partners, and to meet the community health needs indentified by the assessment. The findings of this study will be made widely available to the public through a variety of communication mechanisms. 3. Re-evaluate contracted programs for multi-year funding: Cape Cod Healthcare, through its Community Benefits Program, funds programs through annual budgeted grant funds via a formal RFP process. In collaboration with the Community Health Committee, CCHC will re-evaluate three (3) of the current Community Benefits programs in FY2011 to determine if multi-year funding might be an option. These programs are: (1) REACH Collaborative program; (2) Specialty Network for the Uninsured Program; and (3) Community Based Interpreter Services Program. The Community Health Committee will prioritize needs in FY2011 through a formal Community Health Needs Assessment and identify more specific programs with defined measurable goals. 4. Enhance tracking of Community Benefits programs and initiatives: Cape Cod Healthcare will establish a uniform infrastructure for collection of community benefit data to structure community benefit plans, track programs, document activities and report the highlights of our hospitals' valuable contributions to the community. Community Benefits Process: Cape Cod Healthcare's Community Benefits program is a division of the Office of Service Line Development. This division with its Community Health Committee (CHC) reports to the Board of Trustees. The Community Health Committee is comprised of members who work in health care services on Cape Cod, community-based organizations, community advocacy groups and county government, as well as two members of the CCHC Board of Trustees. The committee develops and recommends policies to the Board regarding community health and Community Benefits programs. It also conducts formal Community Benefits planning to set priorities on grant funding, advises on community health issues and initiatives, and disperses funds through a formal RFP process. Cape Cod Healthcare addresses potential emergent health care trends. To support such events, CCHC budgets a small reserve for emergent "one-time" donations in support of our community's health and well being, and aligns with our Community Benefits Program. These funds may be used at the discretion of Cape Cod Healthcare's leadership, who make a timely effort to inform the CHC as circumstances arise. As always, the goal of the Community Health Committee is to work openly and collaboratively with our community partners. Community Partners: CCHC's Community Benefits programs are an integral part of Cape Cod Healthcare's mission and commitment to our community. Cape Cod Healthcare's community health needs assessment is fluid anddetermine the health care needs of the four regions of Barnstable County, (Upper, Mid, Lower, and Outer Cape Cod), CCHC seeks community opinion about the services provided, satisfaction with such services, and what particular services are needed. These opinions, together with secondary data from multiple sources, are used to build an agenda aimed at providing needed health care services and additional community-based programs. The latest formal Community Health Needs Assessment resulted in a two-tiered process. The first phase of this assessment was in 2008 and involved data gathering and analysis. The committee accessed multiple data sources and health status indicators in collaboration with community partners from Cape Cod and the Islands, which resulted in the creation of the Salient Health Issues Report June, 2008. The committee conducted the second component (the qualitative part of the needs assessment) in 2009, engaging residents and providers from the four regions of Cape Cod through the Community Solutions Forums. These forums explored how various populations on the Cape are specifically affected by the health issues identified in the Salient Health Issues Report. Communities were encouraged to explore viable and relevant solutions to reduce or eliminate the issues. This assessment was completed in June 2009 and continues to guide the Community Benefits program through FY2012. Cape Cod Healthcare plans to begin the next formal Community Health Needs Assessment in FY2011. The Salient Health Issues Report 2008 was the result of the first phase of the Needs Assessment. Goal Status: 1.0: Coast held a pilot Forum on September 23, 2010 at the Harwich Council on Aging. The number of requested registrations exceeded the capacity limit of 70. Goal Description: 2. Improve awareness of mental health and wellbeing in elders on Cape Cod experiencing isolation and depression. Goal Status: 2.0: Ten tools to build resiliency were introduced through a unique teaching design, banners, handouts and attendee tables decorated with references to each tool. The COAST Forum engaged ten community "experts." Each expert was assigned a topic (tool) and a table. Participants were seated at one of the ten tables for ten minutes. Participants rotated to each table until each attendee was exposed to all ten tools. The Table Topic Leaders led their small group of 7 in hands-on learning that went from table exercise, to meditation to animated conversation. -2.1: The COAST forum helped the attendees change the way they viewed wellness. Attendees learned that physical and mental wellness are linked and they have a right to expect mental wellness as they age. -2.2: COAST evaluated that outcome by using a simple card asking if the participant would now: 1. try this tool, 2. maybe try, 3. would not try. Approximately 80% said they would try 9 of the 10 tools. Pre-and post-program surveys were conducted resulting in the following responses: -1.1: Pre-Program Surveys: 70% of participants had previously participated in a class or program specifically designed for individuals with Parkinson's disease but most of these experiences were in formal rehabilitation settings. Participants took advantage of opportunities to learn about Parkinson's disease, and were receptive to new kinds of experiences. When queried about "what brings you to" this class, the majority of respondents (almost to the person) said it was the chance for physical activity and social interaction. -1.2: Post-Program Evaluations 100% of participants ranked the programs "excellent" and that all expectations were met. The facilities were also rated well. -The only suggestions for improvement were to extend the length of the program (the individual sessions and the length of the course). It was projected that 25 consultations would be completed during the grant period, some on the med-surg floors, and some in the Emergency Department. Goal Status: 1.0: Thirteen (13) consultations were done and all were done on the med-surg floors. A retrospective review of twenty-five (25) medical records was completed from the six-month period immediately preceding the pilot period. The records were randomly selected from a group of patients who were hospitalized at Falmouth Hospital for alcohol withdrawal treatment or for an alcohol-related condition (pancreatitis, cirrhosis, etc.). -1.1: The review sought to learn how many of these 25 patients required transfer to the Intensive Care Unit. Thirteen (13), or 52% of the sample, required ICU care. It is interesting to note that of the thirteen (13) patients who received consulting services provided by this grant, only four (4), or 31%, required transfer to the ICU. While this data is based on a very small sample, it is nonetheless encouraging for future consideration regarding alcoholism specialist services in the hospital and the potential it provides for fewer complications, earlier identification and better patient care. Goal Description: 2. Provide education and training to hospital medical/clinical staff re: patients at risk for alcohol withdrawal syndrome. Goal Status: 2.0: With the grants being awarded at the busiest time of the season on Cape Cod, [the Emergency Department population increases significantly on Cape Cod between June and September], CCHC's Community Benefit award funded the consultation component of the program, as data shows trainings during this critical time of the year are not well attended. Financial counselors are dedicated to improving access to care through eligibility screening, assessed affordability and income verification. They attend local community and MassHealth meetings and trainings to ensure compliance with up-to-date regulations, assist with advocacy efforts and troubleshooting to avoid coverage delays. PAT Team: The PAT team performs mental health assessments 24 hours a day, 7 days a week including holidays at Cape Cod Hospital and Falmouth Hospital emergency rooms. Patients are referred to the emergency rooms by concerned family members, schools, law enforcement, private physicians and concerned members of the community. The PAT team serves the psychiatric needs of the community every day of the year. Physician Recruitment: On Cape Cod access to primary care physicians and specialists is often limited for those patients covered under Medicaid and State subsidized health plans. Cape Cod Healthcare recognizes the importance of health care to this often medically challenging population. The healthcare system works closely with the area's four community health centers providing recruitment assistance. Qualified physicians, specialists and physician extenders are identified and referred to these important community resources so that the professional staff at the clinics can be adequately maintained and expanded to accommodate a growing need. REACH: A collaborative program providing community coordination, capacity building and leadership to enhance the safety and wellbeing of the elder population on Cape Cod. Working with the Councils on Aging, VNA, EMS and other community partners, elders are assessed in their homes and referred to appropriate services. REACH provides community training on elder issues and advocates for elder health and wellness. Community Based Interpreter Services: Cape Cod Healthcare, through its Community-Based Interpreter Services program, promotes excellence and professionalism in interpretative services to improve quality clinical outcomes to Cape Cod's Limited-English population. Interpreting services are provided in local community-based physician practices and in conjunction with the Specialty Network for the Uninsured. SNU: The Specialty Network for the Uninsured (SNU) program involves coordination of patient care with health care specialists on Cape Cod who have agreed to accept uninsured patients at a reduced rate and/or to provide specialty clinics on-site at the community health centers. Coordinated Insurance Enrollment: The Community Health Center of Cape Cod (CHCCC), Duffy Health Center, Mid-Upper Community Cape Health Center (MUCCHC) and Outer Cape Health Services (OCHS) provide outreach and enrollment and re-enrollment into MassHealth, Commonwealth Care and other insurance programs. This program also links clients to health and human services providers immediately and follows up with any post-enrollment and access to other health care issues. Although this is one program, each of the four community health centers received a separate grant and reported accomplishments individually. Infrastructure Coordinator: The administrative coordinator supports the executives of the five community health centers which comprise the Cape and Vineyard Community Health Center Network (CVCHCNet). This position ensures efficient communication, collaboration and reduces duplication. The CVCHCNet addresses the unmet needs of existing and new patients. CVCHCNet is a vehicle through which the organizations can engage in joint planning and contribute resources to carry out collaborative and integrative projects. The Prescription Assistance Program is an initiative of Cape Cod Hospitalcommunity benefit to help uninsured or underinsured patients receive required medications upon discharge from the ER if they have no other means and are struggling financially. Transportation Assistance Program: Cape Cod Hospital and Falmouth Hospital provided transportation through Taxi Vouchers to various locations on Cape Cod to help needy patients who are being discharged from CCH and FH Emergency Rooms. Brazilian Mental Health: Catholic Social Services offers mental health counseling in Portuguese to Brazilians living on Cape Cod by providing a diverse program designed to reach out to those facing socioeconomic, cultural and racial barriers. Cultural Competence: To ensure access and reduce health disparities, Cape Cod Healthcare offers translation of medical documents to community-based physician practices in Portuguese and Spanish at no cost to the patient or the physician practice. CCHC recognizes the fact that cultural competence goes beyond language barriers and provides programs to enhance the skills of CCHC's employees at every level. Cultural awareness is improved through educational venues, beginning with initial orientation and a Cross Cultural Awareness and Health Care program. Kick Butts: Cape Cod Healthcare offers free and reduced-fee personalized tobacco cessation classes to provide education, support and motivation to quit using tobacco. The smoking cessation program is a 6-week program held in local community sites between Bourne and Hyannis. Certified Smoking Cessation experts facilitate the programs. Sessions are interactive and group sizes vary from five to fifteen. Diabetes and Nutrition Education: Through community wellness events, Cape Cod Healthcare provides community members with identified issues back to their primary care physician or link them with appropriate care, if they do not have a health care home. In addition, CCHC holds Heart Healthy eating programs that follow the ADA guidelines to educate participants on how to make more thoughtful and healthy food choices. This program is free or reduced rate for the un/underinsured and those enrolled in state-subsidized health insurance programs. OBOT: Duffy Health Center, working with the CHC Network of Cape Cod & Islands, developed a coordinated model to provide the evidence-based practice of Office-Based Opioid Treatment (OBOT) across three Cape Cod Health Centers (Community Health Center Network) using Duffy Health Center's integrated model. This is a tiered process that started in FY2009 with the goal of including three Community Health Centers by year 3 in FY2011. Participants in FY2010 (Year 2) include: Duffy Health Center (DHC) and the Community Health Center of Cape Cod (CHCCC). The OBOT program addresses the high rates of identified addiction on Cape Cod. This program involves a specially trained medical team of physicians and nurse practitioners prescribing very low doses of Suboxone. Teaching of life skills, addressing mental health issues, and holding patients accountable for their actions is this program's approach to this very complex issue. Funded under Cape Cod Healthcare's Massachusetts Department of Public Health Determination of Need Program, the licensed OBOT program completed the second year of a four-year expansion plan. CHNA 27: Cape Cod Healthcare, through the Clark Cancer Center DoN, agreed with the Department of Public Health to disperse a total of $401,000 over a 5-year period commencing in late FY2010 to address issues that eliminate health disparities, promote wellness, and prevent/manage chronic disease for individuals who are elderly and/or persons with disabilities. This population was identified through a community health needs assessment by Cape Cod Healthcare and endorsed by the Community Health Network Area 27 (Cape Cod and the Islands). Suicide Coalition: State and local data shows that the suicide rate on Cape Cod and the Islands is 1.5 times the state average. In order to begin to address this problem, the Cape and Islands Regional Suicide Prevention Coalition was formed in 2009. The Coalition brings together a diverse group of community stakeholders to raise awareness around suicide in order to reduce the number of suicides. The Cape & Islands Suicide Prevention Coalition provided an aggressive, community-wide public education campaign throughout the region to increase awareness of and to promote discussions about the issue, reduce stigma and save lives. H.O.P.E.: The H.O.P.E. project, a program of the Community Action Committee of Cape Cod and the Islands, offers access to health care services through assistance with enrolling individuals and families in MassHealth, Commonwealth Care, and other state insurance plans. It also connects them to primary care and other needed services and provides educational information. Unfortunately, the wait times for assistance exceeded one to two weeks in many cases, due to the limited hours of the benefits coordinator prior to CCHC Community Benefits funding. Tri-CCOH: The Tri-County Collaborative for Oral Health Excellence (Tri-CCOHE) is an organization committed to reducing dental disease and providing affordable dental care to low-income children and families. Tri-CCOHE, a collaborative of 12 organizations, was awarded $5,000 to recruit and convene people with disabilities as well as agencies and groups serving people with disabilities to complete a needs assessment and produce a report related to the dental needs of the disabled. Airway Management: The Airway Management training program involved the purchase of an Emergency Simulation Advanced Life Support STAT Manikin, dubbed "Sim-Man", to enhance the quality of and access to pre-hospital emergency medicine for individuals in the community. Sim-Man provides emergency first responders with a simulated experience not previously available, in a safe, controlled training environment.
