| Description | St. Luke's Episcopal Hospital (SLEH), as an integral part of the Health System, contributes to enhancing community health by delivering superior value in high-quality, cost-effective acute care. As a community teaching hospital and a tertiary referral center, SLEH serves both the Greater Houston area and the global community. In collaboration with its medical staff, SLEH is dedicated to excellence and compassion in caring for the whole person-body, mind and spirit. STATS Admissions 32,734 Emergency Room Visits 31,196 Surgeries 23,985 Outpatient Visits 107,277 St. Luke's Community Emergency Centers Pearland 10,774 Holcombe 27,013 San Felipe 14,207 Patient Care In 2010, SLEH maintained its commitment to providing compassionate, high-quality health services. With more than 700 beds in service (including 157 ICU beds), SLEH medical staff and 4,449 employees cared for an average daily census of more than 550 inpatients. The high acuity of its patients is reflected in an overall case mix index (CMI) of 1.95 and a Medicare CMI of 2.29-one of the highest Medicare CMIs in the country. SLEH ranks at 94.62 for overall hospital and 99.14 for Medicare in SLEH's compare group, consisting of 23 hospitals. At SLEH, approximately 80 percent of the active medical staff, comprised of more than 500 physicians, has attained board certification in an area of medical specialty or subspecialty. Board certification indicates that physicians have been highly trained in their field. Education & Research SLEH serves as the primary private adult teaching hospital for Baylor College of Medicine and also is affiliated with The University of Texas Medical School at Houston and The University of Texas Medical Branch. As a result of these affiliations, SLEH participated in the training of more than 200 medical residents and fellows in 2010. SLEH has nursing education affiliations with Texas Woman's University, The University of Texas, The University of Texas Medical Branch-Galveston, Houston Baptist University, Houston Community College, Prairie View A&M University, Alvin Community College and Walden University. One of America's Best In its 2010 annual survey of "America's Best Hospitals," U.S.News & World Report ranked St. Luke's among the top 50 healthcare institutions in the nation in eight specialties. For 2010, the Texas Heart Institute at St. Luke's Episcopal Hospital placed in the top 10 in Heart and Heart Surgery for the 20th consecutive year. Number four overall, the Texas Heart Institute is the only heart center in Houston, and the Southwestern United States, to be listed among the top five in its category. St. Luke's also ranked among the nation's elite hospitals in Gastroenterology; Geriatrics; Kidney Disorders and Urology. Quality of Care Center for Liver Disease In 2010, SLEH completed a comprehensive Liver Center Agreement with Baylor College of Medicine (BCM). This agreement secures a relationship with the BCM team of liver transplant surgeons and hepatologists. The SLEH/BCM Liver Transplant Program is recognized as one of the top-performing liver transplant services in the U.S., based on observed patient outcomes and annual volume. The agreement intends to foster further development and success of the liver transplant program and hepatology service line and collaboratively improve the quality of services to SLEH patients and their families. Home to the largest liver transplant program in the Texas Medical Center, SLEH performs approximately 75 liver transplants annually. SLEH liver transplant success is the best in the nation, with a one-year adult patient survival rate of 96.5 percent, compared to the national expected survival rate of 89.38 percent. In 2010, SLEH celebrated with BCM its 1,000th liver transplantation procedure. Heart Failure Institute SLEH became the first hospital in Texas accredited as a Heart Failure Institute by the Healthcare Accreditation Colloquium. The accreditation is based on proficiency in managing heart failure while placing emphasis on quantity and quality of life. Telemedicine SLEH is one of only four sites in the nation selected to evaluate an advanced telemedicine system that will allow physicians to learn innovative techniques via the Internet. Zvonimir Krajcer, MD, Interventional Cardiologist and Co-Director, SLEH Peripheral Vascular Intervention and Research Program, tested the remote proctoring robot prototype while performing an endovascular abdominal aortic aneurysm repair. Cardiac Catheterization Advances SLEH is among select hospitals to offer radial artery "wrist" cardiac catheterization-used only one percent (1%) of the time in the U.S. The newer method results in reduced bruising and bleeding, anywhere between 58 to 73 percent, compared to groin access. Excellence in Patient Care SLEH was presented an HCAHPS Excellence in Patient Care Award in the "Patients Who Gave a Rating of 9 or 10 (high),"greater than 500 beds category. Nursing Excellence The Texas Nurses District 9 Foundation named four SLEH nurses as Outstanding Nurses of 2010. Twenty Houston nurses were honored with the foundation's President's Award, which recognizes leading advocates for nurses and the nursing profession. SLEH 7 South 3 ICU received the Beacon Award for Critical Care Excellence for 2009-2010 by the American Association of Critical Care Nurses. The unit is one of only 30 in the U.S. to have received the award twice. Only eight units have received the award three times, including SLEH's 7 South 1&2 ICU. The nation's top hospital intensive care units, from among an estimated 6,000 units, are recognized for excellence in the care of acutely and critically ill patients, SLEH is one of only five Texas hospitals to earn the award. Lung Certification The Texas Department of State Health Services surveyed the Lung Transplant Program on behalf of the Centers for Medicare & Medicaid Services (CMS) in January 2010. The surveyors were impressed with the program's quality of care and commitment to patients. In March 2010, a second review was performed and certification was recommended. On April 15, 2010, CMS Certification was granted-the first time in the SLEH program's history. DNV Accreditation SLEH's annual DNV Survey was conducted in May 2010 and the hospital received full NIAHO accreditation. Several "noteworthy efforts" were cited in the final survey report, including: Medical staff and board leadership's involvement in the SLEH quality management system STEMI program's rapid delivery of care 24 hours a day Central Sterile Processing Department for use of LEAN performance improvement Overall appearance and operations of the Pearland and Holcombe Community Emergency Centers The DNV lead surveyor commended SLEH leadership for its "commitment to excellence" and thanked all of the hospital's caregivers for the "tireless and compassionate care" witnessed during the visit. SLEH Diagnostic & Treatment Center-Kirby Glen premiered a new panoramic, wide-open, high-field MRI in October 2010 - the first location in the Texas Medical Center to offer kinematic joint studies. Patient benefits include more space and comfort; accommodation of any patient size at any position; unlimited joint positioning; and improved image quality. New-Generation MRI SLEH is one of only two hospitals in the U.S. to have the new-generation 3.0T MRI scanner technology, which provides state-of-the art imaging capabilities, such as functional magnetic resonance imaging (fMRI) and diffusion tensor imaging, to support SLEH's neuroscience/neurosurgery technology. St. Luke's Medical Clinics Two Houston area St. Luke's Medical Clinics offer adult primary care provided by board-certified physicians in Internal Medicine. Services include: Treatment of acute and chronic medical conditions Annual physicals, immunizations and preventive care EKGs and labs on-site SLEH's Community Emergency Centers (CEC) SLEH's brand of care has become a neighborhood fixture with the SLEH Community Emergency Centers-freestanding emergency centers offering comprehensive adult and pediatric care. SLEHS is the largest hospital sponsored provider of freestanding emergency services in Greater Houston. With almost 73,000 visits annually, the CECs feature onsite laboratory testing and diagnostic imaging services and provide consultation by specialists in all major medical disciplines readily available 24 hours a day, seven days a week. One of the most visited emergency centers in Houston; SLEH's CEC-Holcombe celebrated its 10th year of providing high-quality emergency care. CEC-Pearland celebrated its one-year anniversary since joining one of Houston's fastest-growing suburban communities in 2009. | Description |

|---|---|---|
| charity care | SCHEDULE H, PART I, LINE 7 | The cost to charge ratio method was utilized to determine financial assistance at cost; however, st. Luke's internal cost accounting system was used to determine unreimbursed Medicaid costs. Subsidized health services amounts represent cash paid to other healthcare providers for indigient healthcare services. If the percentage reported in part i, line 7, financial assistance and certain other community benefits at cost, were to be calculated on a system-wide basis, the combined percentage for all of the hospital facilities of the st. luke's episcopal health system would be approximately 8.29%. |
| FOOTNOTE FOR BAD DEBT EXPENSE | SCHEDULE H, PART III, SECTION A, LINE 4 | TEXT IN FOOTNOTES THAT DESCRIBES BAD DEBT EXPENSE: "UNPAID ACCOUNTS ARE WRITTEN OFF AS BAD DEBT ONCE THEY REACH DELINQUENT STATUS." |
| COSTING METHODOLOGY | SCHEDULE H, PART III, SECTION A, LINE 4 | BAD DEBT AMOUNTS ARE CALCULATED USING THE COST TO CHARGE RATIO METHOD. |
| RATIONALE FOR INCLUDING BAD DEBT AS COMMUNITY BENEFIT | SCHEDULE H, PART III, SECTION A, LINE 4 | ALTHOUGH ST. LUKE'S EPISCOPAL HOSPITAL STRIVES TO IDENTIFY ELIGIBLE CANDIDATES FOR CHARITY CARE OR DISCOUNTED CARE, THERE ARE CIRCUMSTANCES IN WHICH THE CANDIDATE DOES NOT ASSIST IN THE DETERMINATION PROCESS, AND CHARITY CARE/DISCOUNTED CARE GUIDELINeS CAN NOT BE MET, therefore THE ACCOUNTS BECOME a BAD DEBT EXPENSE FOR THE HOSPITAL'S RECORDS. IT IS VERY DIFFICULT TO ACCURATELY DETERMINE HOW MANY OF THESE ACCOUNTS WOULD HAVE MET THE CHARITY CARE/DISCOUNTED CARE GUIDELINES HAD THE CANDIDATES COOPERATED WITH THE APPLICATION PROCESS. |
| RATIONALE FOR INCLUDING MEDICARE SHORTFALL AS COMMUNITY BENEFIT | SCHEDULE H, PART III, LINE 8 | ST. LUKE'S EPISCOPAL HOSPITAL HAS ADOPTED THE COMMUNITY BENEFIT AND CHARITY CARE POLICIES OF THE ST. LUKE'S EPISCOPAL HEALTH SYSTEM, WHICH REQUIRE IT TO PROVIDE CARE TO ALL PATIENTS WHO PRESENT THEMSELVES FOR CARE. MEDICARE PATIENTS RECEIVE THE SAME HIGH QUALITY CARE AS PATIENTS THAT HAVE OTHER FORMS OF PAYMENT. AS MEDICARE REIMBURSEMENTS DO NOT FULLY COVER THE COST OF THE HEALTHCARE PROVIDED, ST. LUKE'S EPISCOPAL HOSPITAL CONSIDERS THE SHORTFALL AMOUNT AS AN ADITIONAL COMMUNITY BENEFIT TO THE ELDERLY MEMBERS OF THE COMMUNITY IT SERVES, AND THE COMMUNITY ENJOYS HIGH QUALITY HEALTHCARE FROM ST. LUKE'S EPISCOPAL HOSPITAL FOR ALL ITS CITIZENS, REGARDLESS OF METHOD OF PAYMENT. the cost to charge ratio is a costing methodology utilized in healthcare to estimate costs. the overall ratio of total costs to total charges is computed and the ratio applied to medicare patient charges to compute medicare costs. |
| COLLECTION POLICY | SCHEDULE H, PART III, LINE 9B | ST. LUKE'S EPISCOPAL HOSPITAL HAS ADOPTED THE DEBT COLLECTION POLICY OF THE ST. LUKE'S EPISCOPAL HEALTH SYSTEM ("SYSTEM"). THE SYSTEM'S POLICY STATES THAT ST. LUKE'S EPISCOPAL HOSPITAL WILL NOT PURSUE COLLECTION PRACTICES AGAINST PATIENTS WHO HAVE QUALIFIED FOR PARTIAL ASSISTANCE. AN ADJUSTMENT IS POSTED TO THE CHARITY ALLOWANCE AND THE REMAINING BALANCE MAY BE PURSUED AS DIRECTED BY OUR COLLECTION POLICY. IN CERTAIN CASES, A PATIENT'S QUALIFICATION FOR CHARITY CARE OR FINANCIAL ASSISTANCE MAY NOT BE READILY DETERMINED; HOWEVER, IF AFTER COLLECTION PRACTICES HAVE BEGUN IT LATER BECOMES KNOWN THAT A PERSON QUALIFIES, THE COLLECTION EFFORTS WILL IMMEDIATELY CEASE FOR THE CHARITY CARE PATIENT OR BE PURSUED AT THE LOWER PARTIALLY ADJUSTED AMOUNT. |
| NEEDS ASSESSMENT | SCHEDULE H, PART VI, LINE 2 | ST. LUKE'S EPISCOPAL HOSPITAL MEETS THE NEEDS OF THE COMMUNITY THROUGH ITS EDUCATION, OUTREACH PROGRAMS AND SERVICES IN THE COMMUNITIES IT SERVES. ST. LUKE'S EPISCOPAL HOSPITAL SPONSORS A VARIETY OF PROGRAMS IDENTIFIED THROUGH COMMUNITY NEEDS ASSESSMENT, INCLUDING HEALTH AND WELLNESS FAIRS, "SAFETY" DAYS, EDUCATIONAL LECTURE SERIES, ECONOMIC OUTLOOK CONFERENCES, AND LOCAL CHAPTER FUNDRAISERS FOR NATIONAL CAUSES. |
| PATIENT EDUCAtiON OF ELIGIBILITY FOR ASSISTANCE | SCHEDULE H, PART VI, LINE 3 | AS A PART OF ITS MISSION, ST. LUKE'S EPISCOPAL HEALTH SYSTEM PROVIDES CARE TO PATIENTS WITHOUT FINANCIAL MEANS TO PAY FOR HOSPITAL SERVICES. CHARITY CARE WILL Be PROVIDED TO ALL PATIENTS WHO PRESENT THEMSELVES FOR CARE AT ST. LUKE'S EPISCOPAL HOSPITAL OR RELATED ENTITIES WITHOUT REGARD TO RACE, CREED, COLOR, OR NATIONAL ORIGIN AND WHO ARE CLASSIFIED AS FINANCIALLY OR MEDICALLY INDIGENT. ST. LUKE'S WILL PROVIDE PARTIAL OR FULL FINANCIAL ASSISTANCE FOR MEDICALLY NECESSARY CARE IN ACCORDANCE WITH TEXAS LAW AND HOSPITAL ELIGIBILITY GUIDELINES TO PATIENTS WHO REQUIRE HOSPITAL CARE OR MEDICAL SERVICES AND CANNOT AFFORD SUCH CARE OR SERVICES BECAUSE OF LIMITATIONS IN THEIR HEALTH INSURANCE OR PERSONAL FINANCES. ST. LUKE'S WORKs COLLABORATIVELY WITH OUR PARTNERS AT CARDON HEALTHCARE NETWORK TO ENSURE ALL SELF PAY AND UNDERINSURED PATIENTS ARE SCREENED FOR THE APPROPRIATE GOVERNMENTAL ASSISTANCE PRIOR TO QUALIFYING THEM FOR CHARITY. ONCE CARDON SCREENS A PATIENT AND DETERMINES HE/SHE DOES NOT QUALIFY FOR ANY THIRD PARTY FUNDING, THE PATIENT IS VERBALLY EDUCATED AND ADDITIONAL SCREENING TAKES PLACE BY A ST. LUKE'S EMPLOYEE TO DETERMINE IF THE PATIENT IS ELIGIBLE FOR CHARITY SERVICE PRIOR TO DISCHARGE. PATIENTS THAT HAVE BEEN DISCHARGED PRIOR TO CHARITY SCREENING, SUCH AS EMERGENCY ROOM PATIENTS, RECEIVE A NOTIFICATION OF CHARITY SERVICES. ONCE A PATIENT HAS BEEN SCREENED FOR CHARITY SERVICES, A NOTICE IS SENT OUT INFORMING THE PATIENT IF THEY QUALIFY FOR 100% CHARITY, OR DENIED CHARITY CARE SERVICES. |
| COMMUNITY INFORMATION | SCHEDULE H, PART VI, LINE 4 | ST. LUKE'S EPISCOPAL HOSPITAL DEFINES THE COMMUNITY IT SERVES BROADLY TO INCLUDE THE LOCAL HOUSTON METROPOLITAN AREA, THE COUNTRY, AND THE INTERNATIONAL COMMUNITY. |
| PROMOTION OF COMMUNITY HEALTH | SCHEDULE H, PART VI, LINE 5 | ST. LUKE'S EPISCOPAL HOSPITAL'S COMMUNITY BUILDING ACTIVITIES PROVIDE TREATMENT, EDUCATION, AND AWARENESS TO PROMOTE HEALTH AND HEALING IN THE COMMUNITY. MANY OF THESE ACTIVITIES PROVIDE EDucATION FOR THE COMMUNITY ABOUT SPECIFIC DISEASES, HEALTHY HABITS, AND CAREERS IN HEALTH CARE. AS PART OF ST. LUKE'S MISSION, IT PROVIDES THE UNDERSERVED WITH VITAL HEALTH TESTS TO EDucATE INDIVIDUALS ABOUT THEIR CURRENT HEALTH NEEDS. ST. LUKE'S RESPONDS TO PUBLIC HEALTH NEEDS IN NUMEROUS WAYS INCLUDING HEALTH EDUCATION, SUPPORT GROUPS, DISEASE MANAGEMENT, SCREENINGS, CLINICS, WELLNESS AND AWARENESS. |
