DescriptionSt. John's Mercy Health System ("SJMHS") provides quality medical health care regardless of race, creed, sex, national origin, handicap, age or ability to pay. In active pursuit of this mission, SJMHS provided services to 48,031 inpatients, 111,669 emergency room visits, 966,309 outpatient visits, and 53,537 clinic visits.SJMHS is a Catholic health care corporation that, pursuant to the organizational core belief that health care services are a vital and integral part of the Church's healing mission, engages in a ministry which provides general acute care, ambulatory, long-term and home care health services to individuals and families in its service communities. Consistent with the organization's commitment to social accountability and the provision of community benefit, SJMHS offers other services and programs which further health promotion, maintenance and care. SJMHS's services include two clinics which were established specifically for individuals who are uninsured or underinsured and do not have a means to pay for healthcare services. These clinics provide a significant focus on prenatal care. Many clinic patients were also treated, on a charitable basis, as inpatients. In response to the increased needs of the underinsured in the St. Louis area, these clinics have continually expanded, offering new services to more patients. The institution continues to focus on the provision of primary health services and education to the community, regardless of their means to pay for these services. The organization also has a continuing commitment to health research and to health education at all levels.Although reimbursement for services rendered is critical to the continued ability to provide healthcare services to all patients, SJMHS recognizes that not all individuals possess the ability to purchase essential medical services and, further, that part of our mission is to provide healthcare services and healthcare education to the communities in which our facilities are located. In keeping with SJMHS's commitment to serve all members of the communities it serves, SJMHS provides (i) free care and/or subsidized care; (ii) care to persons covered by governmental programs at below cost; (iii) health activities and programs to support the community; (iv) health education programs; and, (v) a variety of broad community support activities. Among the community support activities offered by SJMHS are the following: Community Education Programs Support Group Meetings Health Fairs and Screenings Immunizations Provision of Free Health Information Home Delivered and Congregate Meals Indigent Care Clinics Pre-Natal and Well Baby Care for the Indigent Health Professions Education Programs Support of Community Programs, including active United Way participant, Provision of Scholarships, Healthcare for the Homeless Coalition, etc. Provider of Financial Counseling and Discharge Counseling Services for all Patients Provision of Chapels in each Hospital and Chaplains/Religious Lay Staff to Minister to the Religious Needs of any Patient and their Families. Volunteer AssociationSJMHS provides care to patients who lack financial resources and are deemed to be medically indigent. As defined by the institution's charity policy, SJMHS does not pursue collection of amounts determined to qualify for charity care. In addition, SJMHS provides services to other patients under the Medicare Program and various state Medicaid programs. Such programs pay providers amounts that are substantially less than billed charges, and frequently less than the cost, of the services provided to the recipients. Care is provided to those with limited or no ability to pay. The cost, net of any payment received (net cost), to SJMHS of providing care to persons qualifying for charity or Medicaid totaled $44,566,046 in the year ended 6/30/10. Additionally, the net cost for services provided to elderly and disabled patients under the Medicare program totaled $42,717,268. These amounts do not include the cost of health activities and programs to support the community, health education programs and other community support activities. The services provided by SJMHS, as well as the amount of charity care provided, demonstrate the ongoing commitment of the organization to serve in a leadership role as an advocate for responsive healthcare services for the community, with a deep commitment and sensitivity to meeting the needs of the poor. Each activity for which income is reported in column (B) of Part VIII is undertaken in furtherance of the organization's healthcare and community benefit mission described above.Description

Bad Debt FootnotePart III, Line 4: The text of the footnote that is included in the Sisters of Mercy Health System, Inc. and Subsidiaries audited financial statements that describes bad debt expense follows: "Patient accounts that are uncollected, including those placed with collection agencies, are initially charged against the allowance for uncollectible accounts in accordance with collections policies of the Health System and, in certain cases, are reclassified to charity care if deemed to otherwise meet the health system's charity care policy. The provision for uncollectible receivables is based on management's assessment of historical and expected net collections considering business and economic conditions, trends in healthcare coverage, and other collection indicators. Periodically throughout the year, management assess the adequacy of the allowance for uncollectible receivables based upon the payor composition and aging of receivables as of the reporting date with consideration of the historical payment and write-off experience by payor category. The results of these reviews are then used to make any modifications to the provision for uncollectible receivables to establish an appropriate allowance for uncollectible receivables. After satisfaction of amounts due from insurance, the health system follows established guidelines for placing past-due balances with collection agencies."To determine the amount of bad debt expense, at cost, bad debt expense attributable to patient accounts was multiplied by a ratio of cost to charges. The ratio of cost to charges used was based on detailed cost accounting, where available. Where cost accounting is not available, cost report cost to charge ratios were utilized. The filing organization determined that the estimated amount of bad debt expense (at cost) attributable to patients eligible under the organization's charity care policy is $0. Although the charity care policy requires the participation of the patient requesting assistance, we have a process under presumptive charity (refer to disclosure for Part III, #9b) to address accounts for patients who do not provide the information. We believe that our charity policy is comprehensive enough to capture almost all patients who qualify for charity care.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: The Medicare shortfall was computed using the annual Medicare cost report (CMS Form 2552-96). It is the position of St. John's Mercy Health System that 100% of the shortfall should be treated as community benefit. This amount represents the costs of providing services that remain uncompensated to the provider.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: St. John's Mercy Health System (SJMHS) will utilize the Search America tool to enhance our ability to determine charity qualification. Presumptive charity will be granted in situations where we have been unable to obtain information from our patients or the information provided is not complete enough to make a charity determination. SJMHS will utilize presumptive charity prior to bad debt placement for charity adjustments in excess of $6,500. In these cases, SJMHS will utilize the Search America report status of "probable charity" as the basis for the assigning the charity level.SJMHS will pursue appropriate means in the collection of delinquent accounts from patients with an established ability to pay or an unwillingness to cooperate in validating eligibility for financial assistance, including legal action consistent with SJMHS mission and values, which may include liens upon real property and reasonable wage garnishments. Legal actions will generally not include bank garnishment, repossession of assets or foreclosures to enforce satisfaction of lien. SJMHS has policies and procedures established to address the initiation of legal action. SJMHS annually reviews collection agency compliance with all policies.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: St. John's Mercy Health System (SJMHS) conducted its most recent community health needs assessment, March - August 2010.In an effort to include input from both community individuals served by SJMHS and also those with expertise in public health, community roundtable meetings were held in Washington (3/30) and St. Louis (4/22) Missouri. Participants shared their views on both the current and future state of health in their communities. Focus included not only traditional health care services, but also prevention, education and wellness. Ideas for next steps and partnerships were surfaced along with identifying any potential barriers to success. A second step in the needs assessment process included the collection and analysis of available community/public health data sources. SJMHS staff, supported by Saint Louis University Masters of Public Health students, focused on eleven counties in SJMHS's service area: Crawford, Franklin, Gasconade, Jefferson, Lincoln, Osage, St. Charles, St. Louis, St. Louis City, Warren and Washington. Data for the assessment was obtained from a variety of sources:-Progress Toward Building a Healthier St. Louis - 2009 Access to Care ReportSt. Louis Regional Health CommissionEach year SJMHS's JFK Clinic is sited throughout the report as a major provider of care for the uninsured and underinsured in our region.-Children of Metropolitan St. Louis - 2009-10 Report to the Community Vision for Children at Risk This report provides valuable health and social service information relating to children and is critical as SJMHS plans for programs and services for families. -The County Health Rankings for 2010 This is a new resource that was unveiled in March 2010. It is county level data that will be updated annually by the University of Wisconsin - Population Health Institute and the Robert Wood Johnson Foundation. -Community Data Profiles & Missouri Information for Community Assessment (MICA) Missouri Department of Health & Senior Services - Bureau of Health Informatics This resource provides some of the best community health assessment planning tools available including community data profiles on 115 Missouri counties, thirty health indicators, and the capability to create customized tables. -Issues in Missouri Health Care 2009Health Management Association, Inc., Missouri Foundation for Health, Healthcare Foundation for Greater Kansas City-Data from SJMHS Information Systems DepartmentData from SJMHS's own records was pulled and used to assess the needs of the community. -Local county health departmentsLocal county health departments were contacted for information and it was discovered that even the smaller counties in SJMHS's service area had performed an assessment. All had some level of analysis within the past three years. SJMHS also looked at some of the counties in community, into Illinois, that have not historically been considered SJMHS's primary service areas but may offer opportunities to serve as SJMHS grows. After careful analysis of the data, five Community Health Priorities were identified: Injuries, Obesity, Smoking, Access to Care and Mental Health/Substance Abuse. A review was then conducted of current community benefit activities/what SJMHS was currently doing to meet these identified priorities, identifying any opportunities that might exist. The final and ongoing step of the process was the formation of a Community Collaborative Workstream. The scope of this group's work is to identify and evaluate various collaboration opportunities, with particular focus on the economically poor and underserved. Six sub-groups were identified and continue to meet and develop programming. The first five focus on "Access to Care": JFK Clinic, McAuley Clinic, Inappropriate use of the Emergency Department, Mental Health/Substance Abuse, and Dental Health. The final group is focusing on Health Literacy/Healthy Living and includes maximizing health education/prevention strategies with particular emphasis on partnerships with local schools.Mercy Neighborhood Ministry, a department of St John's Mercy Medical Center, is responsible for overseeing the Community Benefit/Community Health Steering team. The community needs assessment is updated on an ongoing basis as annual community/public health data is released. A summary of the needs assessment is available to the community on the Medical Center and Hospital websites.NeedsAssessment
Patient Education AssistancePart VI, Line 3: The filing organization has printed material available at each point of registration in our facility, in addition to electronically availability at our web site. Specific information provided: Financial Assistance Information As a part of our healing ministry, the filing organization is committed to providing quality healthcare services to patients regardless of their financial situation. We offer payment assistance for those who do not have insurance or who are in financial need. Uninsured Patient Discounts A discount from the hospital's regular billed charges will be provided to patients who do not have insurance. This includes patients whose financial situation normally would not otherwise qualify them for charity care discounts. Patients without insurance whose financial situation qualifies them for charity care discounts (see Eligibility Guidelines below) must first find out if they are eligible for Medicaid before applying for financial assistance. Patients denied Medicaid eligibility then must provide certain information to show financial need. Applications for charity care discounts will be reviewed for eligibility based on the applicant's income, family size, amount of payment due and availability of other assets or resources. Please note that care must be medically necessary to be considered eligible for uninsured patient discounts. Eligibility Guidelines for Charity Care Discounts The Federal Poverty Guidelines for income are the basis for determining eligibility for charity care discounts. For example, individuals with incomes below 100% of the Federal Poverty Guidelines will be eligible for free care. Individuals with incomes greater than 100% of the Federal Poverty Guidelines may be eligible for care at discounted rates depending on their income level and/or the amount due to the hospital. Additional Financial Assistance After appropriate discounts have been applied, arrangements may be made for an interest-free monthly payment plan. Generally, no patient's financial responsibility will be greater than 10% of annual household income, adjusted to consider availability of other assets that could be used toward making payments.PatientEducationAssistance
Community InformationPart VI, Line 4: St. John's Mercy Health System (SJMHS) serves a large geographic area. St. John's Mercy Medical Center primarily serves those in the urban/suburban St. Louis City and County and St. Charles areas. St. John's Mercy Hospital serves the rural counties of Crawford, Franklin, Gasconade, Jefferson, Lincoln, Warren and Washington.According to the St. Louis Regional Health Commission, there has been an increase of 2.3% in the total safety net population between 2002 and 2008. The number of uninsured has increased while the number of Medicaid recipients has decreased. St. John's Mercy is seeing more uninsured both at the medical center and at the hospital. As a state, Missouri is doing better than most, ranking 21st in the number of uninsured individuals, and half of the counties examined in the needs assessment were under the Missouri rate of 14%. In St Louis City and County we see provider rates higher than the state average, but there is greater need and less access as you move out from the core region of the city/county. Because of this data, access to care for uninsured adults under 65 is one of the five health priorities identified by SJMHS in the needs assessment. There are 16 Federally Qualified Health Centers (FQHC) that serve as the primary safety net in St. Louis City and County. These health centers are all part of the Integrated Health Network and receive federal funds to primarily serve the uninsured and Medicaid populations. In addition to these 16 FQHCs there are several other health centers providing service to low income individuals, including St. John's Mercy JFK Clinic in St. Louis and the McAuley Clinic in Washington. The JFK Clinic receives no federal funds and is supported by SJMHS and charitable contributions/grants. Of special note, there is only one FQHC/community clinic in the SJMH-Washington region besides the St. John's Mercy McAuley Clinic. These two clinics serve the uninsured/Medicaid recipients of those in Crawford, Franklin, Gasconade, Jefferson, Lincoln, Warren and Washington counties. From the needs assessment, five health priorities have been identified. These priorities were chosen because of their high rates in the ten counties in the SJMHS service area in comparison to other health concerns.1. Unintended Injuries - Motor Vehicle Crash Deaths 6 of 10 counties analyzed ranked above the state average.Intended/Violent Crime - Family safety, abuse and neglect Health assessments from every county in St. John's Mercy's community mentioned safety as a concern affecting health. Although it appears that all counties are below the Missouri crime rate average, it is important to note that overall Missouri ranks in the lower quartile ranking 38th out of 50 states.2. Obesity St. John's Mercy counties rank poorly, at or above the Missouri average, and Missouri ranks 39th out of the 50 states.Health Literacy In Missouri, 1.6 million adults have limited health literacy. This attributes to $3.3 - $7.5 billion in unnecessary health care costs annually and this includes emergency department use for routine or delayed care. 3. Adult Smoking Five counties (St. Louis City, Franklin, Jefferson, Lincoln, Washington) have rates higher than the Missouri average and Missouri ranks 47th in prevalence of tobacco use.4. Access to Care - Uninsured Adults under 65Discussed at the beginning of Question 2.5. Mental Health & Substance AbuseMissouri ranks 35th, and seven of the counties in SJMHS's needs assessment reported higher levels of stress, emotional problems and depression than the state average.Binge Drinking (4 or more drinks for women and 5 or more for men)Missouri ranks in the middle at 25th with six St. John's Mercy counties ranking well above the state's 6% targeted value.Since 1871, St. John's Mercy Health System has been known for comprehensive family medical care, medical education, research, community services, and dedication to the Mercy values of compassion and caring for all members of the community. As one of the nation's largest Catholic hospitals and the second largest hospital in metropolitan St. Louis, SJMHS operates the only Level I Trauma Center and Level III Neonatal Intensive Care Unit in St. Louis County, and serves as the community hospital for patients with a variety of conditions and health needs. As a teaching and research hospital, St. John's Mercy utilizes innovative technology and draws health care professionals from across the nation who practice in nearly every clinical specialty.By providing comprehensive health services within the St. Louis community, St. John's Mercy has progressively evolved from a 25-bed infirmary for women and children in 1871 to our current Medical Center campus in west St. Louis County which comprises nearly 80 acres. The Medical Center campus includes: an 859-bed, fully accredited teaching hospital; a nine-level Heart and Vascular Hospital with 96 private patient rooms; a 100,000-square-foot Cancer Center with advanced diagnostic and treatment capabilities; dedicated inpatient women's health services, including a two-story addition exclusively for mothers and babies and featuring all private rooms; a comprehensive Children's Hospital, including a pediatric emergency department and the only pediatric intensive care unit in St. Louis County; a Surgery Center that provides inpatient and outpatient surgical services; and a 120-bed Skilled Nursing Center.St. John's Mercy Hospital - Washington is an acute care hospital offering comprehensive medical, surgical, obstetric and pediatric services. As the only Level III Trauma Center between St. Louis and Jefferson City, St. John's Mercy Hospital offers an experienced medical staff that is specially trained in trauma care. Located just 50 miles southwest of St. Louis, St. John's Mercy Hospital is committed to quality health care for its service area of more than 150,000 residents. Community Building ActivitiesPart VI, Line 5: St. John's Mercy Health System's (SJMHS) community building activities promote the health of the communities in which they serve. Through active participation in community boards, neighborhood/community meetings and involvement in community-based events, such as the annual town fair in Washington, SJMHS demonstrates its ongoing commitment to the community. Community building activities serve as a link to engage Mercy coworkers to look beyond the walls of the facilities in which they serve. Programs such as the annual Adopt-a-Family Program link Mercy co-workers with those most in need in our community during the holiday season. In addition to providing food, clothing and other basic human needs, mental and emotional health of these families and individuals are supported. The Supported Employment Program is another example of reaching out to those in need in the community, this time to those with developmental disabilities who might otherwise be unable to find employment. SJMHS believes in the value and mutual benefit of engaging coworkers in activities that promote the extension of their talents to all in the community. St. John's Mercy is committed to partnering with both individuals and other agencies to improve the lives of those in our community through a holistic approach that includes mind, body and spirit. Below are just a few of SJMHS's Community Building activities:Life Skills Video - The Audiovisual Department created and produced a video to be used by the Lifeskills Organization, a community organization that attempts to place people with disabilities in the workforce. The video that was produced will help to explain the benefits of hiring people with disabilities to the CEOs and managers of community corporations.Special Services (Pastoral Services) - Provide special services (blessings, rite, rituals, funerals, etc.) to the community at large.Involvement in the Midwest Association of Healthcare Volunteer Resource Professionals - Educational meetings regarding volunteers in non-profit healthcare facilities (volunteer administrators in the St. Louis and surrounding area).Supported Employment Coordinator - The Supported Employment Coordinator coordinates services for applicants and co-workers with developmental disabilities.Advance St. Louis - Funds donated to Metro expansion campaign. If the proposal was approved, access to public transportation (bus and Metrolink) would increase. The ballot did pass in April 2010.Adopt-A-Family Program - SJMHS departments adopted families from the JFK Clinic and the Neighborhood Ministry and provided them with gifts such as household items (dishes, pots and pans, linens, etc.), toys and clothing, and assistance with utility bills. 136 families consisting of 518 total people were recipients of this program. Washington Town & Country Fair - Annual parade kicking off start of Washington Town & Country Fair.CommunityBuildingActivities
Other InformationPart VI, Line 6: St. John's Mercy Health System (SJMHS) is a Catholic health care corporation that, pursuant to the organizational core belief that health care services are a vital and integral part of the Church's healing mission, engages in a ministry which provides general acute care, ambulatory, long-term and home care health services to individuals and families in its service communities. In keeping with SJMHS's commitment to serve all members of the communities it serves, SJMHS provides care to patients regardless of ability to pay. SJMHS is governed by a Board of Directors which includes representation from community leaders in the organization's primary service areas. All board members are required to complete an annual Conflict of Interest survey. Any potential conflicts of interest disclosed by board members are screened by general counsel and reviewed by the board chairperson and CEO. This process ensures that public, rather than private, interests are served by SJMHS. Medical staff privileges are extended to all qualified physicians in the community for the majority of SJMHS clinical services. The exceptions include Radiology, Pathology, Anesthesiology, Neonatology, and CV surgery. As a result, over 90% of the physicians on staff are in categories that are open to qualified physicians in the community.Surplus funds and unrestricted assets held by SJMHS are reinvested in patient care, medical education and research initiatives which support the organization's mission to deliver compassionate care and exceptional health care services to the communities it serves. Examples include the following:-New patient tower dedicated to Mercy Children's Hospital - offering a full scope of pediatric services to improve the health and well-being of children in the communities served, regardless of ability to pay-New outpatient ambulatory facility in Clarkson Valley, MO (west of St. Louis) providing comprehensive cancer care and other services in one location-Expansion of emergency room facilities at St. John's Mercy Hospital in Washington, MO to meet the health care needs of this community-SJMHS established the Mercy Research Institute which is dedicated to improving patient outcomes by supporting new and potentially life-changing health research, including over 100 active clinical trials at St. John's Mercy Medical Center -Continued funding of graduate medical education program (provides residency training in family medicine, internal medicine, OB/Gyn, dental medicine, orthopedics and critical care) and schools of radiology, respiratory and clinical laboratory science.OtherInformation
Affiliated Health Care SystemPart VI, Line 7: The filing organization is part of the Sisters of Mercy Health System ("Mercy").
