Income Based CriteriaPart I, Line 3c: Saint VINCENT HEALTH CENTER provides medically necessary services to all patients, regardless of race, color, creed, gender or country of national origin and without regard to the patient's ability to pay. Saint VINCENT HEALTH CENTER has a Charity Care Policy that is among the most generous in the region. This Policy applies to all Saint VINCENT HEALTH CENTER facilities, and the amount of care provided under the Policy is determined by need and is not limited or rationed by budgeted amounts under the policy. Saint VINCENT HEALTH CENTER will provide free care to individuals without insurance with incomes up to 350% of the Federal Poverty Level and discounted care on a sliding scale up to 350% of the Federal Poverty Level. Unlike the charity care policies of most hospitals, the Saint VINCENT HEALTH CENTER policy applies to hospital charges and professional fees for services provided by Saint Vincent Medical Education and Research Institute employed physicians.IncomeBasedCriteria
Costing Method UsedPart I, Line 7: The best available data was used to calculate the cost amounts reported in Item 7. For certain categories, the best available data was derived from a cost accounting system. In other categories, specific cost-to-charge ratios were calculated and applied to that category. Saint Vincent Health Center has used the Catholic Health Association methodology for reporting community benefit for many years.Saint Vincent Health Center incurs medical professional fees for physicians serving its programs, therefore the associated costs and charges relating to these physician services are included in all relevant categories of Part I.CostingMethodUsed
Subsidized Health Services ExplnPart I, Line 7g: SUBSIDIZED HEALTH SERVICES INCLUDED IN FISCAL YEAR 2010 WERE: NICU/OB CLINIC, BEHAVIORAL HEALTH, AND AMBULANCE SERVICES.SubsidizedHealthServicesExpln
Percent Of Total Expense ExplnPart I, Line 7f: The Bad debt expense of 20,167,388 included on Form 990, Part IX, line 25, column A, was subtracted for purposes of calculating the percentage of total expenses.PercentOfTotalExpenseExpln
Bad Debt FootnotePart III, Line 4: Patients who are uninsured are screened as close to time of service as possible by Financial Counselors. Uninsured In-Patients are usually contacted within a day of admission (or their families depending on the medical circumstances). As many uninsured Emergency Department patients as possible are seen following medical screen by Physicians. Diagnostic Out-Patients and those with scheduled procedures that are without coverage are often pre-admitted and part of the call includes conversation regarding payment expectations. In all cases, screening includes searching for payment options through insurance carriers or in liability situations. For those who remain uncovered, we attempt to establish potential qualification for State Medicaid coverage. Failing that patients are given information regarding our charity care program and are given the option to apply. All patients, whether uninsured or who owe balances after insurance payments are received, are billed at the same incremental times, i.e. we try to follow a 150 day timeline. Activities during this period include our sending regular statements which include references to our charity care and discount policies. Additionally, phone contact is attempted based on account age and amount due. Discounts are offered to the uninsured in the spirit of "equity" with those who are insured. We offer 50% off of charges to this group and also offer a 10% "prompt pay" discount for full payment within a ten day time frame from contact. Additional activity includes our utilizing a "pre-collect" agency who continues with statement sending and phone call reminders, followed by notice of final billing. At that point, generally around the end of the 150 day timeline, unpaid accounts are prepped for turn over to a collection agency. It is at that point that they are classified as "bad debt" by virtue of their being written off of the active AR and transferred to a bad debt file where they reside until such time as they are paid (through the collection agency) or returned to us as "uncollectible". Any payments received on account as well as any discounts taken over the life of the account, excluding those in the bad debt file, are recorded as such when they occur and would therefore not be included in the final amounts determined to be bad debt. However, payments made to the collection agencies subsequent to bad debt reporting are posted against the accounts and would offset the bad debt expense within that particular month as reported on the financial statements. As part of our charity care policy process, we try to determine if patients who are not paying and/or are not responding to any attempts for communication, might reasonably be considered as charity. A section of the policy is labeled "Presumptive Financial Assistance Policy" and allows us to screen for Homeless persons (as defined by the Health Resources and Services Administration (HRSA), patients who are deceased with no known immediate family members, patients who have previously been on Medicaid in the past 24 months, patients who are within the financial eligibility levels for state Medicaid but who refuse to comply with the application process, patients currently on Medicaid but with prior uninsured balances, patient who are proven eligible for food stamps and those who are non Pennsylvania residents who qualify based on income but not on residency. The cost of bad debt as reflected in Part III, line 2 was calculated using a cost accounting system. Any discounts provided or payments made to a particular patient account are applied to that patient account prior to any bad debt write-off and are thus, not included in bad debt expense.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: Medicare allowable costs are calculated using a cost-to-charge ratio. Medicare allowable costs of care came from Medicare Cost Report for Fiscal year 2010.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: In an uncertain economic environment, Saint Vincent financial counselors remain committed to assisting those in our community who are unable to afford their medical expenses, yet seek expert care. The financial counselors work with patients and their families to help them qualify for the community care programs, answering their questions and helping them through the enrollment process. Community Care information is available in the Saint Vincent Community Care brochure or by visiting the Web site at www.saintvincenthealth.com/about/saint-vincent-community-benefit. For patients who receive bills in the mail following their treatment, the Community Care program is introduced on the bottom of their bill with the appropriate contact information. Financial advisors are available weekdays and are conveniently located next to patient registration. It is our policy not to pursue collection practices against patients known to qualify for charity care or other financial assistance. In certain cases it may not be easily determined whether or not a patient qualifies for charity care or financial assistance, however, if after collection practices have begun it later becomes known that a patient qualifies, the collection efforts cease.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: Saint Vincent Health Center does not currently have a formalized method to assess the community for its health care needs. We currently use internal and external market studies to establish patterns of utilization of services and age of population. In the next few years, Saint Vincent Health Center plans to launch a more formalized process to assess our community needs.NeedsAssessment
Patient Education AssistancePart VI, Line 3: Informing the public that charity care is available is an important element of our charity care program and Saint Vincent Health Center continuously strives to improve its communications with patients on the availability of charity care information about the charity care policy. This policy is summarized on the Saint Vincent Health Center website. A summary description of the charity care policy is available in patient registration areas and from financial counselors who are present on-site to assist patients in qualifying for governmental assistance programs and charity care.PatientEducationAssistance
Community InformationPart VI, Line 4: Saint Vincent Health Center serves residents in Erie County, Pennsylvania. Erie County is approximently 802 square miles and has a popluation of 280,291. The Cost of living index is less than average and the percent of residents living in poverty was 15.8 in 2009.CommunityInformation
Other InformationPart VI, Line 6: One of the hallmarks of a charitable organization is that the organization serves a broad, indefinite charitable cause. One of the key indicators that an organization serves the broader community is control of the organization by independent community leaders. Saint Vincent Health Center's governing board is made up of two members from its sponsor, the Sisters of Saint Joseph of Northwestern Pennsylvania, and the balance from the community at large who direct and guide management in carrying out the mission of Saint Vincent Health Center. Trustees are selected on the basis of their expertise and experience in a variety of areas beneficial to Saint Vincent Health Center and are not compensated for their services.Saint Vincent Health Center was built on a foundation of caring for all people and that commitment has remained strong through the years. This mission has helped to make Saint Vincent one of the largest employers in Erie, Pennsylvania with the ability to make a significantly positive economic impact on Erie County in terms of salaries paid to Saint Vincent associates and dollars spent within the region. This economic impact in fiscal year 2010 amounts to more than $552 million, a 5.5% increase over the previous year. In addition, more than $23.7 million was spent caring for those most in need through the hospital's community service, free services and Medicaid patients. It is Saint Vincent's mission to provide medically necessary treatment and service to all persons, regardless of their ability to pay for care. In fiscal year 2010, Saint Vincent maintained its commitment to its Community Care Program in response to the economic and financial concerns affecting patients. Patients earning 350% above the national poverty level qualified for the Community Care Program, which is 150% above the national average. Realizing that many people forego medical care in economic hardships, Saint Vincent also continued to offer a process to allow patients to make arrangements to satisfy their insurance co-payments if they are facing financial difficulties. Just because the economy is struggling, the need for medical treatment does not stop, and Saint Vincent has answered that need. The community benefit ministry ensures Saint Vincent caregivers provide "God's healing love to all."Saint Vincent Health Center extends medical staff privileges to all qualified physicians in its community.OtherInformation
Affiliated Health Care SystemPart VI, Line 7: Saint Vincent Health Center is the primary hospital facility of the combined Saint Vincent Health System, an integrated health system consisting of Saint Vincent Health Center, Westfield Memorial Hospital (a rural hospital in New York State), Saint Vincent Foundation for Health and Human Services, physician practice sites, various ancillary services, over one hundred employed physicians and a variety of other health-related providers.AffiliatedHealthCareSystem
